The objectives of this study are to describe the antibiotic resistance pattern of ESBL producing Enterobacteriaceae responsible for bacteraemia and identify factors associated with these infections in a University Hospital in Madagascar.Methodology: This is a descriptive cross-sectional study of 300 randomly selected patients with clinical features of bacteraemia whose blood cultures were processed for isolation and identification of bacterial pathogens over a period of six months (October 2019 to March 2020) at the laboratory of the University Hospital of Befelatanana. Blood culture samples were processed by conventional microbiological method for isolation of Enterobacteriaceae, which were identified to species level using Analytical Profile Index (API) 20E® test system. Madagascar is the country that reports the most cases of plague in the world. This is an evaluati... more Madagascar is the country that reports the most cases of plague in the world. This is an evaluation study of plague diagnosis over a 2-year period from January 2017 to December 2018. For all suspected plague cases, peripheral RDT (performed locally), central RDT (performed in the laboratory) and PCR were performed. Parameters of the diagnostic tests used, year of study, performance and study of concordance between tests were studied. 2981 cases were collected, 21.6% were confirmed, 28.4% probable and 50% suspected. The sensitivities of peripheral RDT, central RDT and PCR were 96.55, 100 and 97.41%, respectively; the specificities were 41.43, 51.17 and 89.24%. Cohen's kappa was 0.12 between peripheral RDT and culture; 0.17 between culture and central RDT and 0.64 between CRP and culture. For pneumonic plague (PP) patient samples, sensitivities were 80.00, 66.66 and 93.33%, Cohen's kappa was 0.017 between peripheral RDT and culture; 0.013 between central RDT and culture and 0.371 between PCR and culture; sensitivities of peripheral RDT and central RDT were 75.00 and 62.50% for 2017, respectively. The null hypothesis between diagnostic tests was rejected, discordance between tests was found. Sensitivity is lowered during lung sampling and during 2017.

Des enquetes de seroprevalence du VIH et de la syphilis ont ete realisees en mi-1995 dans trois g... more Des enquetes de seroprevalence du VIH et de la syphilis ont ete realisees en mi-1995 dans trois grandes villes de Madagascar pour completer les donnees de sero-surveillance recueillies annuellement par le Laboratoire national de reference VIH/sida, afin d'estimer le nombre d'individus infectes par le VIH a Madagascar On a teste trois groupes d'individus : femmes enceintes, clients des cliniques MST et prostitue(e)s. Les resultats illustrent non seulement la forte prevalence de la syphilis, mais indiquent aussi que l'on a actuellement environ 5.000 personnes seropositives dans le pays. En outre, a partir du nombre des personnes atteintes de MST, on estime qu'un million de malgaches risquent d'etre infectes par le VIH. Since late 1985, the Institut Pasteur de Madagascar has studied the problem of the distribution o... more Since late 1985, the Institut Pasteur de Madagascar has studied the problem of the distribution of HIV in the island. With the approval and cooperation of the Malagasy Ministry of Health, a number of different surveys have been undertaken, particularly of those sections of the population diagnosed as being of "high-risk". In 1987, a Malagasy prostitute from the country's second largest port was identified as seropositive. One case of clinical AIDS was diagnosed in 1988 in a Franco-Malagasy subject living in NOSY BE. Systematic tests for seropositivity among women with multiple partners in nine "high-risk" areas of the country are applied. The aim of this survey during 1988 is to make such groups aware of the vital necessity of overcoming the problem by preventing its growth by taking appropriate measures. The IPM has obtained government approval and has established the facilities to confirm findings of seropositivity among the population of Madagascar.

The objective was to evaluate the level of transmission of P. falciparum by entomological, parasitological and serological indices. All indices were found consistent with the description of a focus of hyperendemic transmission dependent on the presence of An. funestus at high density, corresponding to more than 50 females per house collected by hand catch. The positivity rate for the circumsporozoite protein (CS) of P. falciparum in blood-fed or gravid An. funestus was 2.0% (11/562) in the house resting sample. The parasite rate in the village population was 47.9% reaching 68.4% in the 0-14 age group and the gametocyte rate in the total sample was 25.0%. In a cross-sectional study, sera from all adult inpatients at the Tulear hospital were screened using rapid plasma reagin; reactive sera were confirmed using Treponema pallidum hemagglutination. All sera were also tested for HIV antibodies by enzyme immunosorbent assay and reactive specimens were confirmed by Western blot. Age, sex, and clinical information were collected from patient charts. Overall, 36 (24.5%) of 147 hospitalized patients were found to be syphilis seroreactive, one patient (0.7%) had HIV antibodies. Syphilis was mentioned on the patient charts of only three (8.3%) of the 36 syphilis seroreactive individuals; they were all three diagnosed with neurosyphilis. Ear infections are common in poor countries and one of the most common reasons for health consultations and prescriptions for antibiotics. Very few studies of chronic otitis media were performed in Malagasy people. The aim of the present study was to identify the bacteria responsible for chronic otitis media in order to assess the susceptibility of bacteria to antibiotics. This is a prospective, multicentric, descriptive and analytical study of unilateral atrial flow from patients with chronic otitis media seen in external consultation in 3 sanitary formations in Antananarivo. Le purpura thrombopenique idiopathique (PTI), hemopathie acquise touchant electivement les plaque... more Le purpura thrombopenique idiopathique (PTI), hemopathie acquise touchant electivement les plaquettes, se manifeste cliniquement par des hemorragies cutaneo-muqueuses precedees d'un syndrome febrile. Bien que rare, la possibilite de survenue d'une hemorragie cerebrale existe. Le PTI est une maladie mal connue sur le continent africain et a Madagascar. Pour combler cette lacune, les auteurs ont effectue des etudes tant retrospectives que prospectives dans deux services pediatriques de la ville d'Antananarivo. 4 cas ont ete releves pendant la periode d'etude (janvier 1996 - aout 1998). La symptomatologie des cas rapportes est moderee. Les corticoides se sont reveles efficaces dans le traitement de ces cas de PTI. Un programme de surveillance hematologique des enfants connus comme etant malades ou suspects d'etre malade est preconise.

Of 4131 individuals studied 8 were HIV seropositive and 732 were seropositive for syphilis infection. However it is estimated that more like 5000 people are HIV seropositive countrywide and that about 1 million out of a total population of 13.8 million are at risk of infection. This rate of infection places Madagascar in the same situation as that of 1983-85 in most other countries which are now heavily burdened with HIV infection and AIDS. Madagascar is at the beginning of an HIV/AIDS epidemic while seropositivity for syphilis infection appears to be significantly associated with illiteracy and prostitution in the country. Screening for HIV is recommended for all TB patients but it has not yet been initiated in some re... more Screening for HIV is recommended for all TB patients but it has not yet been initiated in some regions of Madagascar. In this study, our main objective was to determine the seroprevalence of HIV infection in TB patients and secondarily to determine the risk factors favoring TB infection in these patients. This prospective cross-sectional study took place in the pneumology department of the University Hospital of Tulear from January to April, 2017. We diagnosed 75 patients with tuberculosis, 96.05% of them new cases. The sex-ratio was 1.81. Patients' mean age was 35 years. Two patients (2.66%) were coinfected with HIV. Among the new cases diagnosed, 62.66% had pulmonary tuberculosis with positive microscopy (TPM +). Two patients reported a person with tuberculosis among their family or close friends. Living with someone in poor hygiene conditions was reported by 82.66% of the cases. Overall, 24% were unemployed. These patients had quite varied ethnic origins. The prevalence of HI...
Le test de la C-Reactive Protein (CRP) est utilise pour orienter le diagnostic d'un processus... more Le test de la C-Reactive Protein (CRP) est utilise pour orienter le diagnostic d'un processus inflammatoire, notamment dans les affections febriles de l'enfant. Une etude retrospective a ete effectuee au service de Pediatrie de l'Hopital general de Befelatanana, a Antananarivo, durant les annees 1997-1998. Sur 714 cas d'affections febriles apparies selon des caracteres anthropometriques et cliniques, la population d'etude comprenait 361 enfants. Au total, 384 dosages de CRP ont ete effectues: 305 dans un but diagnostique (CRP initiale), et 79 en tant que CRP de controle (en cours ou enfin de traitement). La CRP initiale a permis de distinguer d'une part, 152 affections probablement de nature bacterienne: 49 en pathologie respiratoire, 62 en neurologique, 10 en digestive, 19 en ORL, 12 en urinaire, et d'autre part 153 affections probablement de nature virale: 86 en pathologie respiratoire, 29 en neurologie, 12 en digestive, 26 en ORL.
