It is safe, feasible, and cost-effective. Case presentation A 29-year-old G3P2 + 0 woman had two previous cesarean sections. She was pregnant at 32 weeks. The fetus had anencephaly. She had acute cholecystitis. Laparoscopic cholecystectomy done at the time of termination of pregnancy by cesarean section. Conclusions In a critical period, such as acute cholecystitis, the combination of laparoscopic cholecystectomy immediately post cesarean section is effective if the surgeon is highly qualified and experienced.

There are suggestions to wait for spontaneous resolution than to operate these cases without harmful adverse events. Herein, we evaluated the outcome of the watchfulness of these cases over 18 months. Methods: The study included 93 infants with communicating hydrocele for the Pediatric Surgery Department, Faculty of Medicine (Assiut, Egypt). They were planned to be followed up for 18 months, and indications for intervention included hernia, increasing in size, being tense, and completion of 18 months of follow-up without improvement. Results: The gestational age of the included patients was 38.5 ± 2.2 weeks and the age at the time of presentation was 50 (7, 495) days. Most cases were bilateral, reducible, and had an intermittent course. After 18 months of followup, 60.2% of the patients resolved spontaneously and 39.8% were surgically treated. Age at the time of presentation was higher among operated patients. Patients with reducibility criteria on clinical examination and lack of intermittent course had higher frequency among operated patients (89.2%). Conclusions: It is safe to wait and not to operate on infants with hydrocele up to 18 months as long as there was no hernia. Higher age at presentation and reducibility on examination are indicators that favor the need for surgery.

Conclusion: As long as the surgeon is highly quali ed and experienced, the combination of laparoscopic cholecystectomy immediately post cesarean section in a critical period as acute cholecystitis is effective.

The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Vestibular anus means congenital presence of the anus in the vestibule behind the vaginal orifice. It shares a common wall between the vagina and the rectum. In addition to an abnormal cosmetic appearance, it leads to recurrent attacks of urinary tract and vaginal infections. In adult female patients, it leads to dyspareunia. Several techniques have been described for the treatment of this type of anomaly, however; the postoperative continence is not always satisfactory. Optimal surgical repair would be easy to assess minimum dissection to spare pelvic nerves and guide the rectal pouch through the muscles of continence and intact sphincter [1]. The surgeon must cautiously during surgical separation of the posterior vaginal wall from the anterior rectal wall to avert rectal wall tear.

This prospective, randomized, comparative study included female children with rectovestibular fistulae who were selected from patients with Anorectal Malformations (ARMs) treated between January 2016 and July 2020. The patients were randomly divided into four groups based on the operative technique: Trans-Sphincter Anorectoplasty (TSARP), Posterior Sagittal Anorectoplasty (PSARP), Classic Anterior Sagittal Anorectoplasty (ASARP), and modified ASARP. Those with CF-mediated vasal agenesis probably suffered some insult to the vas deferens only. Those with renal and vassal agenesis, however, may have an intrinsic Wolffian defect, which caused complete developmental failure of the duct [5]. Here we report 3 cases of different anomalies of the vas deferens discovered accidentally during routine orchidopexy and herniotomy procedure. We used a definition of congenital vanished of the vas deferens that reflects maldevelopment of any unreconstructable segment of the vas deferens.

They may affect fertility following orchiopexy. Herein, we describe the cases of abnormalities of the vas deferens and epididymis in children undergoing orchidopexy for cryptorchidism. Data were collected retrospectively from six centers providing care for patients with undescended testicles from September 2017 to February 2019. All patients whose congenital anomalies of the epididymis or vas deferens were discovered incidentally during operative intervention for cryptorchidism and where photographs of the anomalies were taken were included. The total incidence of congenital anomalies on the 467 testicles which had operations was 13.2%. Congenital epididymal and vas deferens anomalies were more common on the left side than the right side in patients with cryptorchidism (P = 0.038). Accumulating evidence has been reported regarding the effect of curcumin as a dietary antiviral o... more Accumulating evidence has been reported regarding the effect of curcumin as a dietary antiviral on patients with COVID-19; however, findings are controversial. Our systematic review aimed to evaluate the effects of curcumin in patients with COVID-19. Electronic databases (PubMed, EMBASE, Scopus, Web of Science, Cochrane Central, and Google Scholar) were systematically searched to identify only randomized clinical trials (RCTs) that assessed curcumin in patients with COVID-19 from inception to September 23, 2021 relevant keywords. The Cochrane risk-of-bias tool for randomized trials was used to evaluate the risk of bias. After a critical review of 1,098 search hits, only six RCTs were selected for discussion. A total of 480 patients were included, with 240 amongst the curcumin groups and 240 in the control group. The lymphocyte count was significantly higher in the curcumin group compared to the placebo group. Curcumin was found to decrease the number of T-helper 17 cells, downregulate T-helper-17 cell-related factors, reduce levels of T-helper-17 cell-related cytokines, yet increase the gene expression of Treg transcription factor forkhead box P3 (FOXP3), and decrease T-Box transcription factor 21 (TBX21). Our review revealed that curcumin might have a positive effect on relieving COVID-19 related inflammatory response due to its powerful immune-modulatory effects on cytokines production, T-cell responses, and gene expression. These findings suggest that curcumin confers clinical benefits in patients with COVID-19. However, due to the limited number of the included studies, further high-quality studies are needed to establish the clinical efficacy of the curcumin.

For Diagnosed cases, excision of the duplicated part is indispensable, due to reported risk of malignancy. Case presentation We report two cases of colonic duplication in female children with a double anus (vestibular anus and normal site anus); one of them had total double-barreled large intestinal duplication with a double appendix without any communication of both lumens, which is a rare finding. The other case had Y-shaped Rectosigmiod duplication. No genitourinary and other organ duplication detected. Conclusion When the female infant presented with the double anus, predictably colonic duplication should be considered. The precise and relevant technique should be tailored during management. Informed consent has been taken for publishing the article.

Generally, patients with vestibular fistulas have good functional results when treated by fistula transposition (8). Materials and methods: This was a prospective study did from January 2016 to January 2017. We included all female with age range from (25 days-12years old) diagnosed as a rectovestibular fistula (RVF) operated by single stage TSARP, at pediatric surgery units of Assiut and Aswan university hospitals. The functional results, cosmetic appearance and postoperative complication for each case after treatment were evaluated in patients for 3-6 months. Results: The total numbers of cases are 22 cases from rural areas of Upper Egypt, (36.36%) of patient's fathers and mothers are relatives and have associated anomalies. (50%) of cases are complaining of recurrent attack of constipation. (45.5%) of cases developed wound infection at the neoanal site. 7 cases are now more than 3years old, 6 of those 7 cases achieved voluntary bowel control and toilet trained and have good (4-5 points) wing spread score. One case less than 3 years old also achieved voluntary bowel control and toilet trained and has good (5 points) wing spread score. Conclusion: Trans-sphincter anorectoplasty (TSARP) is easy to perform with reasonable operative time, good cosmetic and functional outcome. Separation of the rectum from the posterior wall of the vagina is the most delicate step of the operation, takes place under direct vision. Constipation is an essential problem in patients that can be managed by laxatives. Meticulous postoperative wound care is essential.

PRESENTATION OF CASE: We describe a case in which a 4-year-old boy presented with recurrent attacks of colicky abdominal pain and non-bilious vomiting. Abdominal examination was irrelevant. Per rectum (PR) examination indicated an empty rectum without red currant jelly stool and non-palpable mass. Abdominal ultrasonography revealed a target sign of intussusception. After a period of conservative treatment, the child passed normal coloured and well-formed stool. Exploratory laparotomy referred to a long segment ileo-ileal intussusception with a polypoid lipoma as a lead point. Microscopic examination showed a benign non-capsulated intramuscular ileal lipoma. DISCUSSION: Small bowel lipoma produces symptoms of intermittent bowel obstruction. Herein, the child presented with non-bilious vomiting, we attribute this to reflex sympathetic stimulation of the pylorus leading to pylorospasm. CONCLUSION: Small intestinal lipoma is a rare finding in children that may cause intussusception which does not resolve spontaneously.

The author below describes a simple, easy and safe method for the repair of the umbilicus in cases of omphalocele minor. Methods During the period from January 2015 to May 2017, 16 patients (11 males, 5 females) with omphalocele minor were operated by purse string technique. It is a prospective observational study with a level of evidence type 4. One case presented by intestinal obstruction due to twisted Meckel`s diverticulum. Other case presented by intestinal obstruction due to intestinal malrotation. One case the contents were the cecum and appendix. In two cases the sac contained part of the left lobe of the liver. Results Only six cases were diagnosed antenatally. Thirteen patients presented the same day of birth and 3 patients presented at the 3rd birthday. The sac was intact in 14 cases and ruptured in two. Two cases had hypospadias. One case had absent radius and ulna on the right hand and polydactyl on the left hand. Size of the fascial defects ranged from 2cm to 5cm (mean 3.5 cm). Good outcomes occurred in 14 patients. Conclusion: A good aesthetic appearance obtained when using purse string umbilicoplasty for repair of omphalocele minor. Type of study: prospective observational study Level of Evidence: Therapeutic type 4.

Case report We had a unique case of multiple ileal atresia, atresia associated with cardiac anomaly and dysmorphic features. Our case had genetic predispositions as she is the 3rd offspring of a consanguineous parent. The 2nd offspring was also a female and died due to neonatal intestinal obstruction. Histopathological examination of our case showed total colon mucosal atrophy associated with absent ganglion cells and disappearance of myenteric plexus. So, we suggest a presence of association for colonic atresia and absent ganglion cells. Conclusion Is there a need to modify the usual classification of intestinal atresia? We recommend that early diagnosis and treatment is important to improve the prognosis in colonic atresia.

We compared four different operative techniques in terms of postoperative complications, continence, and cosmetic appearance. This prospective, randomized, comparative study included female children with rectovestibular fistulae who were selected from patients with Anorectal Malformations (ARMs) treated between January 2016 and July 2020. The patients were randomly divided into four groups based on the operative technique: Trans-Sphincter Anorectoplasty (TSARP), Posterior Sagittal Anorectoplasty (PSARP), Classic Anterior Sagittal Anorectoplasty (ASARP), and modified ASARP. Those with CF-mediated vasal agenesis probably suffered some insult to the vas deferens only. Those with renal and vassal agenesis, however, may have an intrinsic Wolffian defect, which caused complete developmental failure of the duct [5]. Here we report 3 cases of different anomalies of the vas deferens discovered accidentally during routine orchidopexy and herniotomy procedure. Our systematic review aimed to evaluate the effects of curcumin in patients with COVID-19. Electronic databases (PubMed, EMBASE, Scopus, Web of Science, Cochrane Central, and Google Scholar) were systematically searched to identify only randomized clinical trials (RCTs) that assessed curcumin in patients with COVID-19 from inception to September 23, 2021 relevant keywords. The Cochrane risk-of-bias tool for randomized trials was used to evaluate the risk of bias. After a critical review of 1,098 search hits, only six RCTs were selected for discussion. A total of 480 patients were included, with 240 amongst the curcumin groups and 240 in the control group. The lymphocyte count was significantly higher in the curcumin group compared to the placebo group. Curcumin was found to decrease the number of T-helper 17 cells, downregulate T-helper-17 cell-related factors, reduce levels of T-helper-17 cell-related cytokines, yet increase the gene expression of Treg transcription factor forkhead box P3 (FOXP3), and decrease T-Box transcription factor 21 (TBX21). Our review revealed that curcumin might have a positive effect on relieving COVID-19 related inflammatory response due to its powerful immune-modulatory effects on cytokines production, T-cell responses, and gene expression. These findings suggest that curcumin confers clinical benefits in patients with COVID-19. However, due to the limited number of the included studies, further high-quality studies are needed to establish the clinical efficacy of the curcumin.

For Diagnosed cases, excision of the duplicated part is indispensable, due to reported risk of malignancy. Case presentation We report two cases of colonic duplication in female children with a double anus (vestibular anus and normal site anus); one of them had total double-barreled large intestinal duplication with a double appendix without any communication of both lumens, which is a rare finding. The other case had Y-shaped Rectosigmiod duplication. No genitourinary and other organ duplication detected. Conclusion When the female infant presented with the double anus, predictably colonic duplication should be considered. The precise and relevant technique should be tailored during management. Informed consent has been taken for publishing the article.

Generally, patients with vestibular fistulas have good functional results when treated by fistula transposition (8). Materials and methods: This was a prospective study did from January 2016 to January 2017. We included all female with age range from (25 days-12years old) diagnosed as a rectovestibular fistula (RVF) operated by single stage TSARP, at pediatric surgery units of Assiut and Aswan university hospitals. The functional results, cosmetic appearance and postoperative complication for each case after treatment were evaluated in patients for 3-6 months. Results: The total numbers of cases are 22 cases from rural areas of Upper Egypt, (36.36%) of patient's fathers and mothers are relatives and have associated anomalies. (50%) of cases are complaining of recurrent attack of constipation. (45.5%) of cases developed wound infection at the neoanal site. 7 cases are now more than 3years old, 6 of those 7 cases achieved voluntary bowel control and toilet trained and have good (4-5 points) wing spread score. One case less than 3 years old also achieved voluntary bowel control and toilet trained and has good (5 points) wing spread score. Conclusion: Trans-sphincter anorectoplasty (TSARP) is easy to perform with reasonable operative time, good cosmetic and functional outcome. Separation of the rectum from the posterior wall of the vagina is the most delicate step of the operation, takes place under direct vision. Constipation is an essential problem in patients that can be managed by laxatives. Meticulous postoperative wound care is essential.

PRESENTATION OF CASE: We describe a case in which a 4-year-old boy presented with recurrent attacks of colicky abdominal pain and non-bilious vomiting. Abdominal examination was irrelevant. Per rectum (PR) examination indicated an empty rectum without red currant jelly stool and non-palpable mass. Abdominal ultrasonography revealed a target sign of intussusception. After a period of conservative treatment, the child passed normal coloured and well-formed stool. Exploratory laparotomy referred to a long segment ileo-ileal intussusception with a polypoid lipoma as a lead point. Microscopic examination showed a benign non-capsulated intramuscular ileal lipoma. DISCUSSION: Small bowel lipoma produces symptoms of intermittent bowel obstruction. Herein, the child presented with non-bilious vomiting, we attribute this to reflex sympathetic stimulation of the pylorus leading to pylorospasm. CONCLUSION: Small intestinal lipoma is a rare finding in children that may cause intussusception which does not resolve spontaneously.

The author below describes a simple, easy and safe method for the repair of the umbilicus in cases of omphalocele minor. Methods During the period from January 2015 to May 2017, 16 patients (11 males, 5 females) with omphalocele minor were operated by purse string technique. It is a prospective observational study with a level of evidence type 4. One case presented by intestinal obstruction due to twisted Meckel`s diverticulum. Other case presented by intestinal obstruction due to intestinal malrotation. One case the contents were the cecum and appendix. In two cases the sac contained part of the left lobe of the liver. Results Only six cases were diagnosed antenatally. Thirteen patients presented the same day of birth and 3 patients presented at the 3rd birthday. The sac was intact in 14 cases and ruptured in two. Two cases had hypospadias. One case had absent radius and ulna on the right hand and polydactyl on the left hand. Size of the fascial defects ranged from 2cm to 5cm (mean 3.5 cm). Good outcomes occurred in 14 patients. Conclusion: A good aesthetic appearance obtained when using purse string umbilicoplasty for repair of omphalocele minor. Case report We had a unique case of multiple ileal atresia, atresia associated with cardiac anomaly and dysmorphic features. Our case had genetic predispositions as she is the 3rd offspring of a consanguineous parent. The 2nd offspring was also a female and died due to neonatal intestinal obstruction. Histopathological examination of our case showed total colon mucosal atrophy associated with absent ganglion cells and disappearance of myenteric plexus. So, we suggest a presence of association for colonic atresia and absent ganglion cells. Conclusion Is there a need to modify the usual classification of intestinal atresia? We recommend that early diagnosis and treatment is important to improve the prognosis in colonic atresia.
