Lumbar disk herniation (LDH) is a common condition affecting millions worldwide. The management o... more Lumbar disk herniation (LDH) is a common condition affecting millions worldwide. The management of LDH has evolved over the years, with the development of newer surgical techniques that aim to provide better outcomes with minimal invasiveness. One promising emerging technique is biportal endoscopic spinal surgery (BESS), which utilizes specialized endoscopic equipment to treat LDH through two small incisions. This review aims to assess the effectiveness of BESS as a management option for LDH by analyzing the available literature on surgical outcomes and potential complications associated with the technique. Our review shows that BESS is associated with favorable postoperative results as judged by clinical scoring systems, such as visual analog scale, Oswestry disability index, and MacNab criteria. Harassment in academic medicine is an accepted and widely recognized problem. As a common pattern, the protagonists are always someone with power against people with a lower hierarchy. Abuse in any of its forms, be it physical, moral, or academic, ends up affecting the student's performance and behavior. It should be considered an unacceptable and punishable pattern in any academic setting.

The extent of the injury to the neuronal tissue following a head impact defines the primary injury. The acceleration energy delivered at the time of impact provokes this injury. There are numerous secondary responses to the injury that mostly intensify the primary injury. When a head impact causes a contusion, the hemorrhagic injury frequently progresses over the first few hours after impact, either growing or developing new, non-contiguous hemorrhagic lesions, a condition known as the hemorrhagic progression of a contusion 1,2,4,7,9-14. ILLUSTRATED CASE An 88-year-old woman was admitted with a severe headache, multiple emetic episodes, and a brief loss of consciousness after falling while doing housework. Her past medical history included anticoagulation with warfarin for atrial fibrillation (international normalized ratio (INR): 3.75). She had normal vital signs and a grossly intact neurological examination on admission. A cranial computed tomography (CT) scan was performed 45 minutes after arrival to the emergency department, which revealed small bifrontal contusions (Fig 1). It was decided to transfer her to the intermediate care unit with non-invasive hemodynamic monitoring, analgesia, continuous neurological evaluation, and neurosurgical evaluation. Methods: A search for RCT, not RCT, prospective and retrospective cohort studies will be carried out through electronic databases. The systemic inflammatory response syndrome (SIRS) consists of an inflammatory phenomenon as a re... more The systemic inflammatory response syndrome (SIRS) consists of an inflammatory phenomenon as a response of the immune system against infections, as well as non-infectious injuries, which includes manifestations that affect multiple organs, among which hyperthermia or hypothermia, leukopenia or leukocytosis, tachycardia, and tachypnea. SIRS accompanies different acute brain and spinal cord injuries, including subarachnoid hemorrhage, intracerebral hemorrhage, spinal cord trauma, traumatic brain injury, and status epilepticus. We suggest a new term for this condition neurogenically originated systemic inflammatory response syndrome (NoSIRS). NIRS can be considered a new syndrome associated with pathological neurological conditions. However, more research is needed to figure out the true severity of this clinical picture and also figure out the best way to treat this condition.

Background: Li-Fraumeni syndrome (LFS) is a hereditary, autosomal dominant malignancy predisposit... more Background: Li-Fraumeni syndrome (LFS) is a hereditary, autosomal dominant malignancy predisposition induced by a mutant TP53. Here, we describe a gliosarcoma (GS) case with a clinical family history impressive for an LFS case in the absence of inherited germline TP53 mutations. Case description: We present a 44-year-old female with a right high parietal mass. The mass proved pathologically to be GS with an isocitrate dehydrogenase-1 (IDH1) mutation. Pedigree analysis identified five first-degree and second-degree relatives with LFS spectrum malignancies. The patient tested positive for TP53 mutation; however, her family tested negative. Conclusion: Of all the tumours, GSs are the least described entity with LFS. We are highlighting the need to do a genetic survey in family members of the patient who has been diagnosed to have gliosarcoma and other tumours consistent with LFS.

BACKGROUND AO Spine RECODE-DCM (Research objectives and common data elements for degenerative cer... more BACKGROUND AO Spine RECODE-DCM (Research objectives and common data elements for degenerative cervical myelopathy) has highlighted that the subjective disability reported by people living with DCM is much broader than routinely considered today by most professionals. This includes a description of tremor. The objective of this review was to study the incidence and possible aetiology of tremor in degenerative cervical myelopathy (DCM). METHODS A systematic review registered in PROSPERO (CRD42020176905) was conducted in Embase and MEDLINE for papers studying tremor and DCM published on or before the 20th of July 2020. All manuscripts describing an association between tremor and DCM in humans were included. Articles relating to non-human animals, and those not available in English were excluded. An analysis was conducted in accordance with PRISMA and SWiM guidelines for systematic reviews. RESULTS Out of a total of 4402 screened abstracts, we identified 7 case reports and series describing tremor in 9 DCM patients. Papers were divided into three groups for the discussion. The first group includes DCM correctly identified on presentation, with tremor as a described symptom. The second group includes cases where DCM was misdiagnosed, often as Parkinson's disease. The third group includes a single case with a previous history of DCM, presenting with an otherwise unexplained tremor. This grouping allows for the clustering of cases supporting various arguments for the association between tremor and DCM. CONCLUSION DCM can be associated with tremor. The current evidence is restricted to case series. Further study is warranted to establish tremor prevalence, and its significance to assessment and management.
