ACE-2 receptors have been identified in many tissues including testes. The aim of the study has been to investigate the long-term effects of SARS-CoV-2 infection (COVID-19) and its relative treatment on male reproductive health. Methods: Cross-sectional analysis has been performed on 49 recovered COVID-19 patients who had semen analysis prior to the COVID-19 pandemic. Those who had a recovery time lag of at least 3 months have been re-examined, and 29 eligible patients with no andrological problems have been enrolled in the study. The links of these two spirals is constituted by the extensive and freely available information on the new detectors development and research and experimentation with microorganisms. In a near future, this phenomenon could lead to a race between detection devices designers and proliferators or terrorist groups that may access to the information and technology required to counter them. This paper highlights the relevance of protecting critical know-how and intangible technology in order to deter or limit the capacity of proliferators in the biological arena.

Antibiogram results are needed to maintain treatment in patients with suspected UTI. However, empirical antibiotic treatment is initiated in patients since it takes time to obtain the results of antibiograms. The aim of this study was to evaluate the urine culture and antibiogram results of patients who were admitted to our hospital with suspected UTI and compare the results with other studies. Methods: Urine cultures requested from the hospital information system database between January of 2018 and 2019 were analyzed. Microorganism-positive urine samples and antibiogram results were evaluated and included in the study. Results: Of the patients, 748 (61.8%) were female and 463 (38.2%) were male. Prostate sarcomas originate from the mesenchymal tissues including smooth muscle, fibromuscular s... more Prostate sarcomas originate from the mesenchymal tissues including smooth muscle, fibromuscular stroma, paraganglia, nerves, and blood vessels. They account for less than 0.1% of all prostate tumors and often present with obstructive symptoms. Patients with leiomyosarcoma of the prostate commonly have poor prognosis and their life expectancy varies between 0% and 60% depending on the stage of the disease at the initial diagnosis. Histopathological examination is essential for definitive diagnosis, and can be performed at an early stage using guided transrectal prostate biopsy. Since prostate specific antigen levels are generally normal, digital rectal examinations are extremely important. We report on a 50-year-old man who was diagnosed with leiomyosarcoma based on the histopathology of his transurethral prostatectomy specimen.

Methods: Twenty-four Sprague–Dawley male rats were allocated to three groups as control (group C), dutasteride (group D) and dutasteride plus tadalafil (group D + T). After a month of treatment, serum samples were obtained from rats to measure dihydrotestosterone and total testosterone. Nitric oxide (NO) synthase (NOS) immunoreactivity and levels of NOS enzyme isoforms, NO and cyclic guanosine monophosphate (cGMP) were evaluated in the harvested penile tissues. Also, corporal smooth muscle and collagen were examined. Results: Staining intensities of neuronal NOS and endothelial NOS were significantly lower in group D (p < .05). They were similar between group C and group D + T. Immunoreactivity of inducible NOS was observed higher in group D than group C (p = .01) whereas group D + T had the highest iNOS (p<.001). Klinefelter syndrome (KS) is one of the most common congenital chromosomal abnormalities and one ... more Klinefelter syndrome (KS) is one of the most common congenital chromosomal abnormalities and one of the most common causes of infertility in men. As there are mosaic forms (eg. 47,XXY/46,XY; 47,XXY/46,XX), there are also non-mosaic forms (eg. 47,XXY; 48,XXY; 49,XXXXY; 48,XXYY) of the disease. Although the clinical findings are variable, the mosaic forms have milder phenotype than non-mosaic forms. In addition to common features, some rare findings such as Persistent Mullerian Duct Syndrome, micropenis, cryptorchidism, and hypospadias may accompany the disease. We report here a 27-year-old male 47,XXY/46,XY (80%/20% respectively) mosaic type KS patient with unusual abnormalities like scrotal hyperpigmentation and evident intellectual disability as well as known ones and discuss it in the context of the literature.

Hyperhomocysteinemia is one of the hereditary hypercoagulability reasons. In order to elicit ethiology in the cases of arterial thrombosis, necessity of hypercoagulopathy screening tests is still controversial. Diagnosis of renal artery thromboembolism should be remembered in the patients who develop postoperative anuria and renal angiographic intervention must be performed immediately for thrombolytic therapy after renovascular imaging. We report a case of a 54-year-old woman with renal artery embolism on the contralateral kidney due to hyperhomocysteinemia after an open left radical nephrectomy.

Aromatic amines and polycyclic aromatic hydrocarbons are chemical carcinogens that stimulate urot... more Aromatic amines and polycyclic aromatic hydrocarbons are chemical carcinogens that stimulate urothelial cell dysplasia and result in bladder cancer. Bladder outlet obstruction due to BPH may increase the contact time of any potential carcinogens in the urine to the bladder urothelium. We aimed to compare the post-void residual (PVR) urine volume between recurrent and nonrecurrent nonmuscle invasive bladder cancer (NMIBC) patients with concomitant lower urinary tract symptoms (LUTS). A total of 480 patients with NMIBC were operated at our institution between May 2014 and May 2018. Of these patients, 93 men with concomitant LUTS were identified. After excluding 40 men who quitted smoking, 53 smoker men were included in study. Patient age, smoking habits, grade and stage of tumor, risk group of the tumor were recorded along with presence of recurrence. PVR urine volume was compared between patients with recurrent and nonrecurrent NMIBC. The mean age was similar between patients with recurrent (n=17) and nonrecurrent (n=36) NMIBC (p = 0.475). The amount of smoking (pack year) was not different between the two groups (p = 0.407). The grade and stage of tumors were also statistically insignificant in both groups. Mean PVR urine volume was higher in patients with recurrent (88 mL) compared to nonrecurrent(62 mL) group. However, these results were not statistically significant (p = 0.548). We believe that the lower PVR urine volume, the lower risk for bladder cancer recurrence, although we couldn't prove this hypothesis statistically. Further clinical studies are needed to confirm the relationship between those.

Dermoid cyst is usually known to be of primordial germ cell origin; however, there are some rare cases in which they have been noticed in other midline and paraxial organs. Although they generally settle in ovaries, they might be found in other tissues such as urinary bladder. Dermoid cyst should be kept in mind as a differential diagnosis for unusual presentation and histopathological appearances in bladder mass and should also be known that there is no need for further following up. We report a case of a 47-year-old woman with bladder dermoid cyst being the first case report from Turkey as a bladder mass resected endoscopically.

Aim: There are several treatment modalities for testosterone replacement therapy (TRT), including... more Aim: There are several treatment modalities for testosterone replacement therapy (TRT), including topical gels, subcutaneous testosterone pellets, transdermal patches, intramuscular injectables and oral forms. Despite the increasing usage of testosterone, there is limited information concerning patient adherence and compatibility of TRT. The aim of this study is to evaluate the factors which may have an impact on patients' adherence to TRT with topical gel. Methods: Between January 2013 and September 2013, 60 men from a tertiary urology clinic, who were prescribed 50 mg testosterone topical gel, were telephonically contacted to know if they continued or discontinued TRT, and when discontinued, the reasons for the same. Results: The mean age of the patients was 40.9 ± 9.9 (range: 21–59) years. The participation rate for the study through telephone was 51.6% (31/60). The most common reason for discontinued TRT was lack of perceived efficacy [n = 11 (35.5%)]. Factors, including age, weight, height, relationship status, and presence of comorbidity, were not associated with TRT adherence. The mean (standard error) time to TRT withdrawal was 5.9 (0.9) months. Conclusion: Most men voluntarily decided to discontinue testosterone and thus a close monitoring of patients by clinicians is essential to increase TRT adherence rate, with testosterone topical gel.

Various analgesic applications can be used during extracorporeal shock wave lithotripsy (SWL) for... more Various analgesic applications can be used during extracorporeal shock wave lithotripsy (SWL) for pain relief and maximal success rate. Intracutaneous sterile water injection (ISWI) has been shown to be effective in several types of pain, but a gap exists about its use during SWL. In this paper, we aimed to evaluate the effect of ISWI during SWL and compare that with diclofenac sodium injection used commonly to provide ideal patient contentment. Patients with kidney stone were randomized to have either ISWI therapy or intramuscular non-steroid anti-inflammatory drug (diclofenac sodium) injection. Using a syringe, 2-3 ml of sterile water was administered to the triangle area bounded by the 12th costal margin, the iliac crest and the vertebral spine in prone position. Visual analog scale (VAS) was employed to record pain scores of patients. Other parameters including stone size, SWL duration, total shock waves given, used energy and the necessity of rescue analgesia were also noted. A total of 524 patients were recruited, of those 216 patients were treated with ISWI and 308 patients had diclofenac sodium injections. The characteristics of the patients and shockwave therapy did not differ significantly between the two groups. Although the mean VAS scores prior to SWL and at every voltage increment during the procedure did not differ, more patients in the diclofenac sodium injection group required rescue analgesia with significantly greater side effects. ISWI is found to be as effective as the diclofenac sodium injection for pain management during SWL with lower adverse event rates.

Introduction and objectives: Although nocturnal emission (NE) is an integral part of normal sexua... more Introduction and objectives: Although nocturnal emission (NE) is an integral part of normal sexual function of males, little information has been documented in the literature. We aimed to assess masturbation habits and frequency of NE among virgin male religious Muslim teenagers. Materials and methods: A questionnaire was administered to students staying in a religious dormitory or attending a religion course. Data of 113 male-respondents including demographic characteristics, masturbation habits and frequency of NE were collected. Results: The mean age of students was 15.88 ± 1.47 (range: 13-20) years. Of the students, 46(41.4%) reported that they never masturbated and 19 (17.3%) never experienced NEs. NE frequency was not correlated with age (p = 0.092). Having NEs was not related to the time since last masturbation either (p = 0.479). Subjects watching TV more than 3 h/day had more NEs than the ones watching less (p = 0.006). Of the subjects 13.6%, 12.6% and 67% believed that masturbation is halal, permissible and forbidden by religion, respectively whereas these rates were 59.1%, 20.4% and 7.5% for NE. Conclusions: Although masturbation and NE are frequent among virgin male religious Muslim teenagers, a significant proportion of young men believe that only masturbation, but not NE, is forbidden by religion. Watching TV seems to be associated with the frequency of NEs. Further studies are required to elicit the factors affecting frequency of NEs.

[1,2] PSA seviyesi, PCa'ya spesifik olmayıp ek olarak benign prostat hiperplazisi (BPH) ve akut veya kronik prostatit gibi benign durumlar ile üretral enstrümantasyon ve ABSTRACT OBJECTIVE: Inflammation is one of the most common histological evidence when prostate biopsy specimens of patients presenting with high PSA (Prostatespecific antigen) levels are examined, but little is known about its effect on PSA levels. This study was conducted to evaluate whether C-reactive protein (CRP) and sterile pyuria has clinical significance in histologically-detected asymptomatic prostatitis cases. MATERIAL and METHODS: In this article, the data were obtained by a crosssectional and prospective clinical study. One hundred eleven consecutive patients without clinical prostatitis had normal digital rectal examination findings, PSA levels ranging from 3 to 20 ng/mL, and sterile urine culture results. All of them underwent transrectal ultrasound-guided 12-core prostatic biopsy. Preoperatively, patients who had urinary leukocyte count 3 or less than 3/high power field (h.p.f) were classified as non-pyuria, while those with urinary leukocyte count more than 3/high power field (h.p.f) were classified as pyuria. The serum CRP level was also used to categorize patients before the biopsy. The subgroups were compared regarding a number of clinical variables. RESULTS: Histological examination revealed inflammation in 69% of pyuria patients and 38.9% of non-pyuria patients. Histologically-detected inflammation rate was significantly different in the groups (p=0.008). The pyuria group exhibited significantly higher total PSA levels compared to the non-pyuria group (p=0.041), as well as significantly higher serum CRP levels (p=0.001). CRP positive and negative groups were similar regarding clinical variables and histologically-detected inflammation rates. CONCLUSION: It has been shown that prostatic inflammation can be detected more reliably with urinary leukocyte count rather than serum CRP. Patients with pyuria exhibited high levels of serum PSA. Sterile pyuria should be kept in mind as a cause of elevated PSA in patients without clinical prostatitis findings. Urinary leukocyte count should be incorporated in routine urological evaluation thanks to its simplicity, convenience, and non-invasiveness.

Methods: Twenty-four Sprague–Dawley male rats were allocated to three groups as control (group C), dutasteride (group D) and dutasteride plus tadalafil (group D + T). After a month of treatment, serum samples were obtained from rats to measure dihydrotestosterone and total testosterone. Nitric oxide (NO) synthase (NOS) immunoreactivity and levels of NOS enzyme isoforms, NO and cyclic guanosine monophosphate (cGMP) were evaluated in the harvested penile tissues. Also, corporal smooth muscle and collagen were examined. Results: Staining intensities of neuronal NOS and endothelial NOS were significantly lower in group D (p < .05). They were similar between group C and group D + T. Immunoreactivity of inducible NOS was observed higher in group D than group C (p = .01) whereas group D + T had the highest iNOS (p<.001). As there are mosaic forms (eg. 47,XXY/46,XY; 47,XXY/46,XX), there are also non-mosaic forms (eg. 47,XXY; 48,XXY; 49,XXXXY; 48,XXYY) of the disease. Although the clinical findings are variable, the mosaic forms have milder phenotype than non-mosaic forms. In addition to common features, some rare findings such as Persistent Mullerian Duct Syndrome, micropenis, cryptorchidism, and hypospadias may accompany the disease. We report here a 27-year-old male 47,XXY/46,XY (80%/20% respectively) mosaic type KS patient with unusual abnormalities like scrotal hyperpigmentation and evident intellectual disability as well as known ones and discuss it in the context of the literature.

Diagnosis of renal artery thromboembolism should be remembered in the patients who develop postoperative anuria and renal angiographic intervention must be performed immediately for thrombolytic therapy after renovascular imaging. We report a case of a 54-year-old woman with renal artery embolism on the contralateral kidney due to hyperhomocysteinemia after an open left radical nephrectomy.

Bladder outlet obstruction due to BPH may increase the contact time of any potential carcinogens in the urine to the bladder urothelium. We aimed to compare the post-void residual (PVR) urine volume between recurrent and nonrecurrent nonmuscle invasive bladder cancer (NMIBC) patients with concomitant lower urinary tract symptoms (LUTS). A total of 480 patients with NMIBC were operated at our institution between May 2014 and May 2018. Of these patients, 93 men with concomitant LUTS were identified. After excluding 40 men who quitted smoking, 53 smoker men were included in study. Patient age, smoking habits, grade and stage of tumor, risk group of the tumor were recorded along with presence of recurrence. PVR urine volume was compared between patients with recurrent and nonrecurrent NMIBC. The mean age was similar between patients with recurrent (n=17) and nonrecurrent (n=36) NMIBC (p = 0.475). The amount of smoking (pack year) was not different between the two groups (p = 0.407). The grade and stage of tumors were also statistically insignificant in both groups. Mean PVR urine volume was higher in patients with recurrent (88 mL) compared to nonrecurrent(62 mL) group. However, these results were not statistically significant (p = 0.548). We believe that the lower PVR urine volume, the lower risk for bladder cancer recurrence, although we couldn't prove this hypothesis statistically. Further clinical studies are needed to confirm the relationship between those.

Dermoid cyst is usually known to be of primordial germ cell origin; however, there are some rare cases in which they have been noticed in other midline and paraxial organs. Although they generally settle in ovaries, they might be found in other tissues such as urinary bladder. Dermoid cyst should be kept in mind as a differential diagnosis for unusual presentation and histopathological appearances in bladder mass and should also be known that there is no need for further following up. We report a case of a 47-year-old woman with bladder dermoid cyst being the first case report from Turkey as a bladder mass resected endoscopically.

Aim: There are several treatment modalities for testosterone replacement therapy (TRT), including... more Aim: There are several treatment modalities for testosterone replacement therapy (TRT), including topical gels, subcutaneous testosterone pellets, transdermal patches, intramuscular injectables and oral forms. Despite the increasing usage of testosterone, there is limited information concerning patient adherence and compatibility of TRT. The aim of this study is to evaluate the factors which may have an impact on patients' adherence to TRT with topical gel. Methods: Between January 2013 and September 2013, 60 men from a tertiary urology clinic, who were prescribed 50 mg testosterone topical gel, were telephonically contacted to know if they continued or discontinued TRT, and when discontinued, the reasons for the same. Results: The mean age of the patients was 40.9 ± 9.9 (range: 21–59) years. The participation rate for the study through telephone was 51.6% (31/60). The most common reason for discontinued TRT was lack of perceived efficacy [n = 11 (35.5%)]. Factors, including age, weight, height, relationship status, and presence of comorbidity, were not associated with TRT adherence. The mean (standard error) time to TRT withdrawal was 5.9 (0.9) months. Conclusion: Most men voluntarily decided to discontinue testosterone and thus a close monitoring of patients by clinicians is essential to increase TRT adherence rate, with testosterone topical gel.

Various analgesic applications can be used during extracorporeal shock wave lithotripsy (SWL) for... more Various analgesic applications can be used during extracorporeal shock wave lithotripsy (SWL) for pain relief and maximal success rate. Intracutaneous sterile water injection (ISWI) has been shown to be effective in several types of pain, but a gap exists about its use during SWL. In this paper, we aimed to evaluate the effect of ISWI during SWL and compare that with diclofenac sodium injection used commonly to provide ideal patient contentment. Patients with kidney stone were randomized to have either ISWI therapy or intramuscular non-steroid anti-inflammatory drug (diclofenac sodium) injection. Using a syringe, 2-3 ml of sterile water was administered to the triangle area bounded by the 12th costal margin, the iliac crest and the vertebral spine in prone position. Visual analog scale (VAS) was employed to record pain scores of patients. Other parameters including stone size, SWL duration, total shock waves given, used energy and the necessity of rescue analgesia were also noted. A total of 524 patients were recruited, of those 216 patients were treated with ISWI and 308 patients had diclofenac sodium injections. The characteristics of the patients and shockwave therapy did not differ significantly between the two groups. Although the mean VAS scores prior to SWL and at every voltage increment during the procedure did not differ, more patients in the diclofenac sodium injection group required rescue analgesia with significantly greater side effects. ISWI is found to be as effective as the diclofenac sodium injection for pain management during SWL with lower adverse event rates.

Introduction and objectives: Although nocturnal emission (NE) is an integral part of normal sexua... more Introduction and objectives: Although nocturnal emission (NE) is an integral part of normal sexual function of males, little information has been documented in the literature. We aimed to assess masturbation habits and frequency of NE among virgin male religious Muslim teenagers. Materials and methods: A questionnaire was administered to students staying in a religious dormitory or attending a religion course. Data of 113 male-respondents including demographic characteristics, masturbation habits and frequency of NE were collected. Results: The mean age of students was 15.88 ± 1.47 (range: 13-20) years. Of the students, 46(41.4%) reported that they never masturbated and 19 (17.3%) never experienced NEs. NE frequency was not correlated with age (p = 0.092). Having NEs was not related to the time since last masturbation either (p = 0.479). Subjects watching TV more than 3 h/day had more NEs than the ones watching less (p = 0.006). Of the subjects 13.6%, 12.6% and 67% believed that masturbation is halal, permissible and forbidden by religion, respectively whereas these rates were 59.1%, 20.4% and 7.5% for NE. Conclusions: Although masturbation and NE are frequent among virgin male religious Muslim teenagers, a significant proportion of young men believe that only masturbation, but not NE, is forbidden by religion. Watching TV seems to be associated with the frequency of NEs. Further studies are required to elicit the factors affecting frequency of NEs.

[1,2] PSA seviyesi, PCa'ya spesifik olmayıp ek olarak benign prostat hiperplazisi (BPH) ve akut veya kronik prostatit gibi benign durumlar ile üretral enstrümantasyon ve ABSTRACT OBJECTIVE: Inflammation is one of the most common histological evidence when prostate biopsy specimens of patients presenting with high PSA (Prostatespecific antigen) levels are examined, but little is known about its effect on PSA levels. This study was conducted to evaluate whether C-reactive protein (CRP) and sterile pyuria has clinical significance in histologically-detected asymptomatic prostatitis cases. MATERIAL and METHODS: In this article, the data were obtained by a crosssectional and prospective clinical study. One hundred eleven consecutive patients without clinical prostatitis had normal digital rectal examination findings, PSA levels ranging from 3 to 20 ng/mL, and sterile urine culture results. All of them underwent transrectal ultrasound-guided 12-core prostatic biopsy. Preoperatively, patients who had urinary leukocyte count 3 or less than 3/high power field (h.p.f) were classified as non-pyuria, while those with urinary leukocyte count more than 3/high power field (h.p.f) were classified as pyuria. The serum CRP level was also used to categorize patients before the biopsy. The subgroups were compared regarding a number of clinical variables. RESULTS: Histological examination revealed inflammation in 69% of pyuria patients and 38.9% of non-pyuria patients. Histologically-detected inflammation rate was significantly different in the groups (p=0.008). The pyuria group exhibited significantly higher total PSA levels compared to the non-pyuria group (p=0.041), as well as significantly higher serum CRP levels (p=0.001). CRP positive and negative groups were similar regarding clinical variables and histologically-detected inflammation rates. CONCLUSION: It has been shown that prostatic inflammation can be detected more reliably with urinary leukocyte count rather than serum CRP. Patients with pyuria exhibited high levels of serum PSA. Sterile pyuria should be kept in mind as a cause of elevated PSA in patients without clinical prostatitis findings. Urinary leukocyte count should be incorporated in routine urological evaluation thanks to its simplicity, convenience, and non-invasiveness.
