As a result, learning methods have been shifted to be delivered through online learning. Online learning has increased reliance on computer screens, which can cause visual discomfort and may cause or exacerbate headaches due to prolonged screen exposure. However, time spent using electronic devices has not yet been examined in relation to the online learning experience. Purpose: This study assessed the online learning experiences and reported headaches associated with screen exposure time among health sciences university students. Methods: A cross-sectional study was conducted among a convenience sample of 353 students at Saudi University. Online learning experiences, screen time exposure, and reported headache questionnaires were used to collect the data. Results: Students were moderately satisfied with the online learning experience. Nevertheless, they faced many challenges with online learning that affected their communication efficacy, and they preferred that blended learning be continued. In addition, this study found a high prevalence of headache (65.72%) and a high screen exposure time among the studied students (52.69%). Increased screen time exposure is linked with increased headache and migraine reporting among students (p < 0.05). Conclusion: Headache is a common health issue among health professional students, and it can harm their academic performance and quality of life, especially related to online learning. Greater awareness of headaches, stress reduction and prevention programs, and ergonomic practices to deal with headaches are essential. Blended learning approaches can improve student learning and performance in health science courses.

Postoperative fistulae are one of the most challenging complications after palate repair. In this clinical study, we reviewed the records of patients who underwent palatoplasty using acellular dermal matrix (ADM) as an addition to facilitate difficult cleft palate and palatal fistula closure. It was a retrospective, comparative, single-centre study, in which records of patients who underwent cleft palate surgeries between 2015 and 2018 were reviewed. Patients who underwent cleft palate or palatal fistula repair with and without ADM were included. Fischer's exact test was used to compare the two groups (primary cleft palate repair with and without ADM) in relation to the rate of fistula occurrence postoperatively. Charts of a total of 31 patients were reviewed. ADM was used in 13 patients; 8(61.5%) were primary repairs and 5(38.5%) were fistula repairs. Eighteen patients were repaired without ADM, of whom 16(88.9%) were primary cleft palate repairs and 2(11.1%) were fistula repairs. The statistical analysis showed no significant difference in fistula formation rate or recurrence in both the groups. ADM is a simple, safe, and helpful tool for augmenting cleft palate repair, mainly in relatively wide and high-tension cleft palate repairs. In our study, a trend showing decreased complications with ADM was observed. Therefore, we recommend a multi-centre study with a larger sample to assess the significance of ADM in cleft palate and palatal fistulae repair.

Seymour first identified a specific pattern of digit injury as an open fracture affecting the distal phalangeal physis associated with nail avulsion from the eponychial fold. In this article, we report a series of Seymour fractures associated with misuse of the newly emerging recreational/transportation vehicle, the hoverboard. METHOD: The study includes a retrospective chart review of patients who presented to a tertiary hospital between December 2015 and October 2016 with a Seymour fracture caused while operating a hoverboard. RESULTS: A total of 13 patients were treated for Seymour fractures. The majority were male patients, and the group had a mean age of 10.3 years (±2.21). The affected digits were primarily the middle and ring fingers of the non-dominant hand. The injury mechanism reported by patients or families indicated that the child was operating the hoverboard in a sitting position with the hands grasping the wheel rim parts. CONCLUSION: We report a case series of a fracture pattern resulting from the improper/unsafe use of a hoverboard. This type of fracture occurs with a tremendous load of energy. Although improper use was a factor, design fault also plays a role in causing the injury. Parent awareness and supervision are important to prevent such injuries.

It is usually seen with the use of mono-polar electro-cautery for circumcision resulting in penile necrosis. We report on a newborn treated by release of the subcutaneous corporal remnant and explain why this option should be considered of choice in cautery-related Grade V injuries. REPORT OF A CASE: A 25-day old Saudi Arabian newborn with Grade V penile injury underwent reconstruction at our tertiary-care center. Upon exploration, the remnant part of the penis under the suprapubic skin was 2.6 cm. This included the root of the penis (estimated to be 1.8 cm long in the newborn) as well as an extra 0.8 cm of corporal length from the pendulous part of the penis which has retracted under the skin. Full release of the suspensory ligament was done. The result at 6 months was satisfactory both functionally and cosmetically. DISCUSSION: Options of management of Grade V injuries include sex-reassignment, phallic reconstruction using flaps, and release of the subcutaneous corporal remnant. We demonstrate that the latter option should be considered of choice in cautery-related Grade V injuries because there is usually preservation of the most proximal part of the corpora of the shaft which becomes retracted under the skin. Hence, the released remnant is of adequate length. CONCLUSION: Post-circumcision Grade V penile injuries of the newborn are best reconstructed with release of the subcutaneous corporal remnant. The neophallus is erectile and has an acceptable length and appearance.

Junior trainees need adequate exposure to gain confidence and expertise in such procedures. This article systematically reviews the literature's different local skin advancement flap training models and describes a new, easy-to-use training model. A systematic review was performed following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. PubMed, Cochrane, Web of Science, and Google Scholar databases were searched from their inception until August 2022 for articles about local skin advancement flap training models. The metaanalysis results were pooled across the studies using a random-effects model and presented as a weighted mean difference with a 95% confidence interval (95% CI). Out of 773 reviewed articles, 18 were included in the systematic review, and four reported enough data to be included in the meta-analysis. Rhomboid and Zplasty flaps were the most commonly taught flaps by training models. The most commonly used training models were synthetic-based, followed by animal-based models. The training models significantly increased the trainees' confidence and expertise regarding local skin flap procedures (p<0.00001) for both domains. Training models, per our reported data, significantly improve the trainees' confidence and expertise in performing local skin advancement flap procedures; continuous efforts in developing and establishing new, simple-to-use, and effective training models are strongly encouraged to further improvement of surgical education and enhance the trainees' surgical skills.

An antihelix deficiency or absence is a common cause of ear protrusion. Although there are over 200 techniques to manage prominent ears, there is no evidence to support using dermabraders in this situation. To the author’s knowledge, this is one of the first articles to assess the clinical outcome of dermabrader use on patients with prominent ear deformities. Methods: An analysis of retrospective medical records was conducted by the senior author on patients with prominent ears treated with our novel dermabrasion technique between 2018 and 2021. The demographics, clinical assessment, and outcomes of the patients were assessed. Furthermore, the article provides a detailed description of the technique. Results: A total of 18 ears were operated on. Eight of the included patients had bilateral ear deformities, and two had unilateral deformities. The mean distance from the ear to the head at the superior helix was 29 mm (range, 25–35 mm). In our series, no complications were observed. During the most recent clinical evaluation, all patients had achieved acceptable ear positioning and excellent aesthetic results. Conclusions: Dermabrasion is a useful technique for otoplasty because no sutures are required and symmetry can be achieved in bilateral ears. Furthermore, this technology is safe, simple, reproducible, reliable, and versatile. We recommend that future studies be conducted in a prospective manner with larger sample sizes and longer follow-up periods to ensure the validity of the findings.

"Simple excision" is usually done if one of the duplicates is floating or severely hypoplastic. Most cases are treated with "reconstruction": the larger, more-functional duplicate is retained and reconstructed using techniques such as collateral ligament reconstruction, intrinsic muscle reattachment, intrinsic tendon balancing/reinsertion and soft-tissue augmentation from the excised smaller, less-functional duplicate. The third option is the "on-top plasty," which is best suited for duplications with one thumb being adequate proximally and the other thumb containing a superior nail and pulp distally. Finally, the Bilhaut-Cloquet (B-C) procedure is occasionally done when both thumbs are equal in length and size but neither is thought to be sufficient for reconstruction on its own. In the B-C procedure, the central parts of the duplicates are discarded, and the outer parts are combined to from the new thumb. 1 The use of the B-C procedure in thumb duplication has well-known drawbacks: the new thumb tends to be wider than normal, the new nail will be ridged in the center, and there is a variable degree of stiffness at the involved joints. Some hand surgeons clearly stated that the B-C procedure should be abandoned. 2,3 Others recommended the B-C procedure for Wassel types I and II duplications and not for more proximal duplication types. 4 Restricting the B-C procedure for Wassel types I and II results in stiffness at the interphalangeal joint (IPJ) only, which is well tolerated. In contrast, if the procedure is done for Wassel types III and IV, the resulting stiffness at both the IPJ and metacarpophalangeal joint (MPJ) is considered unacceptable. However, several authors still advocate the B-C procedure for types III, IV, and VII duplications. 5-14 Except for the stiffness, these authors noted several advantages of the B-C procedure including better joint stability, alignment, and overall function. The senior author (MMA) has used a modified B-C procedure in 4 cases of Wassel types III and IV zigzag thumb

The substantial number of people who are non-compliant makes this an imperative issue to address. Our objectives were to quantify the percentage of patient compliance for physiotherapy after their surgery, to measure the association between compliance and the status of health, mobility, and pain, and to identify the causes of non-compliance. Methods: A cross-sectional study was conducted on post-orthopedic surgery patients attending physical therapy sessions at King Khalid University Hospital in Riyadh, Kingdom of Saudi Arabia, over a one-year period. The sample size of 359 was calculated and selected using simple random sampling. Our questionnaire was developed by adopting questions from two previously validated studies. Results: The majority of the participants (n=194; 54%) were male. One hundred and ninety-three (53.8%) participants had a diploma or higher. The age group 18-35 was found to be significantly associated with skipping physiotherapy sessions when they started to feel well (P= 0.016) and skipping due to other responsibilities (P=0.002). Single people skip physiotherapy when they start to feel well (P=0.023), due to other responsibilities (P=0.028), and due to poor timing (P=0.049). Self-reported compliance to physical therapy after surgery was 231 (64.3%). Patient status showed overall improvement. Conclusion: There is a significant percentage of non-compliance and the patient's age, gender, marital status, and level of education play a role in the causes of non-compliance. In addition, the patient's status (health, pain, and mobility) is better in those who are compliant than in those who are not.

Which is considered a psychological disorder, were people suffering from this issue might seek help from cosmetic procedure providers. Such as dermatologists or plastic surgeons. OBJECTIVE Nowadays, there is a huge increase in cosmetic procedures done worldwide and in Saudi Arabia. Yet there are no enough studies conducted in our region to assess the awareness of BDD among physicians who provide any kind of cosmetic procedures and treatment. In regard to their attitude toward such cases, and how would they manage it. METHODS Our study is a quantitative (observational) cross sectional study, conducted among all cosmetic procedure providers, such as Dermatologist, Otorhinolaryngologists who Provide cosmetic treatments, and Plastic Surgeons. A four paged questionnaire has been distributed among physicians, in governmental hospitals and also among physicians working in cosmetic clinics. The questionnaire was a close-ended one, which had four different sections. Then data were analyzed using Statistical Package for the Social Sciences (SPSS) version 22.0. RESULTS Total number of participants was 155, majority were males, by a number of 113 (72.9%), while 42 (27.1%) were female. 52 (33.5%) reported being totally familiar with the clinical picture of BDD. More than half of physicians reported that they have familiar with the diagnostic criteria of BDD during their practice by a number of 82 (52.9%). A number of physicians 63 (40.6%), estimated the prevalence of BDD in cosmetic practice to range from (1%-5%). Physicians also reported BDD to be more common among females. 76 (49%) of physicians sometimes share the knowledge about BDD with their patients if they are suspecting them to have it CONCLUSIONS Recently, cosmetic treatments are more available to everyone, which lead to an influx of undiagnosed BDD patients electing to perform unnecessary cosmetic treatments. Therefore, physicians should have the clinical knowledge about BDD, on how to diagnose and how to manage. To avoid unnecessary costly procedures.

Body-contouring surgery offers a solution. This study assessed the psychosocial impact of body-contouring surgery on patients after weight loss. Methods (Settings, Design): In this cross-sectional study, a specifically designed questionnaire developed in collaboration with psychiatric department for our research was used for 43 patients who underwent body-contouring surgery. Data were collected during single visit to the plastic surgery clinic. All the patients had lost 20 kg or less before the surgery and were interviewed at least 6 months after the surgery. The questionnaire was used to compare the psychosocial status of the patients before and after surgery. Data were analyzed appropriately using Statistical Package for the Social Sciences. Results: The participants' mean age was 34 ± 10 years; the sample included 24 (55.8%) women and 19 (44.2%) men (total N = 43). The patients' quality of life improved significantly in the areas of social life (P < .001), job performance (P < .002), and sexual activity (P < .001). Moreover, while 17 (39.5%) patients suffered symptoms of depression before surgery, only 1 (2.3%) patient suffered symptoms of depression after surgery. The overall satisfaction was found to be 62.8%, with mammoplasty being the procedure with the highest satisfaction (66.6%). Conclusion: Body-contouring surgery after weight loss has shown to improve both psychological and social aspects of the patients' lives. Recall bias is the main limitation in our study.

Extensive deep chemical burns of the face are usually reconstructed with flaps. In males, the use of the hair-bearing superficial temporal artery flap has the advantage of achieving a satisfactory outcome with regards to both function and aesthetics. PRESENTATION OF CASE: A 49-year-old man sustained a deep chemical burn to the right side of the face and neck. The resulting right peri-oral and neck contractures were initially reconstructed with a latissimus dorsi myo-cutaneous free flap. The functional outcome was satisfactory. The cosmetic outcome was poor and caused the patient to go into depression. Salvage was done using a hair-bearing superficial temporal artery flap. DISCUSSION: In males, the use of the hair-bearing superficial temporal artery flap in facial reconstruction respects the aesthetic units and is also able to achieve a satisfactory functional outcome. CONCLUSION: We stress that aesthetic and functional considerations are equally important in facial reconstruction. In males, the hair-bearing superficial temporal artery flap is the flap of choice if the beard/moustache areas are involved.
