Research paper thumbnail of Burns Demography and Management during COVID-19 Pandemic- A Descriptive Study

This has put a huge strain on hospitals and healthcare workers. Burn patients usually require hospitalisation and surgery, which adds to the burden of care provided by healthcare workers. Modifications in management protocol during a pandemic are of utmost importance to prevent transmission of Coronavirus Disease-2019 (COVID-19) while simultaneously treating burn patients. Aim: To study burn demography and describe changes in burn management protocol during COVID-19 pandemic. Materials and Methods: This descriptive study was done from March to July 2020, during the COVID-19 pandemic’s initial stages in India, at a tertiary care hospital in Southern India. During these months, screening protocol and guidelines for admission and testing of patients for COVID-19 were followed as given by Indian Council of Medical Research (ICMR). All burn patients who visited ...

Research paper thumbnail of Comparison of Three Methods of Dressings for Split Thickness Skin Graft Donor Site - Non Woven Dressing Impregnated with Amorphous Hydrogel, Amorphous Hydrogel with Colloidal Silver and Conventional Paraffin Gauze Dressing

Management of split skin graft donor sites are targeted at promoting faster healing, minimizing pain and complications. A vast number of dressing options for donor sites are available. Search for a better dressing method continues. Aim of this study was to determine the best method of dressing the donor site among three different methods with respect to the rate of healing, pain, exudates and infection. Material and methods: Institutional ethical clearance and patient consent was obtained prior to start of the study. Based on investigator's previous clinical experience, sample size of 75 was arrived at with a power of 80%. Patients were randomly divided into 3 groups by Graph Pad quick calc computing provided by statistician. Group A-dressings with conventional paraffin gauze. Group B-dressings with non woven dressings impregnated with amorphous hydrogel. Group C-dressings with amorphous hydrogel with colloidal silver and paraffin gauze. Statistical analysis was done by Anova and Kruskal Wallis Test. Results: Median healing percentage was 98%, 96% and 99% in groups A, B, C respectively on 8th post operative day. This difference was statistically significant (P=0.033). Pain score on post op day 3 was lower in group C (P=0.08). Two patients had infection one in group b and one in group c, which was not statistically significant. Conclusion: Patients undergoing dressings with amorphous hydrogel with colloidal silver and paraffin gauze had faster healing and experienced lower pain.

Inappropriate resuscitation impacts morbidity and mortality. Urine output (UO), the current gold standard, alone, is not an adequate end point of fluid resuscitation in burns. Hence, we studied the utility of serum lactate levels as a reliable marker and end point of resuscitation. Aims: (a) To understand the role of serum lactate levels in burns as end point in acute burn resuscitation and (b) To assess its sensitivity and specificity. Settings and Design: Tertiary care hospital, urban setting, and cross-sectional study. Subjects and Methods: The study was done from September 2015 to July 2017. Sixty-four patients with thermal burns were included. Serum lactate levels were recorded at arrival, 8 h, 16 h, 24 h, and 48 h interval postburn incident. Hourly UO heart rate and mean arterial pressure were recorded for the outcome measures. Statistical Analysis Used: Chi-square test and nonparametric Mann–Whitney U-test. P ≤ 0.05 was considered statistically significant. Results: (a) The initial serum lactate levels at presentation were directly proportional to: The total body surface area and percent deep component of burns sustained Delay in starting resuscitation. b. Time taken for serum lactate levels to normalize is directly proportional to initial serum lactate level. At 16 h, serum lactate showed (94%) specificity, (53%) sensitivity, and at 48 h (85%) sensitivity, (44%) specificity. Conclusions: Serum lactate levels can help to monitor the adequacy of fluid resuscitation in burns. Serum lactate and UO, both, should be taken into consideration to determine the end point of resuscitation.

The goals of clitoroplasty are to achieve normal genital anatomy and to preserve tactile sensation with a satisfactory sexual response. We present a rare case of idiopathic clitoromegaly managed by reduction clitoroplasty, preserving the dorsal neurovascular bundle and extensive network of nerves around the corpora to the glans and the creation of labia minora.

It requires intermittent intra operative dental occlusion which precludes oral or nasal intubation. In such cases submental intubation (SMI) is a recognized technique in practice. We describe a modified technique for smooth exteriorization of the endotracheal tube (ETT) during SMI. As the SMI technique is unusual for the performer, emphasis is laid on the applied aspects to minimize probable complications during the procedure. With the modified technique we performed SMI uneventfully on five patients.

With the sheer number of applications available and more being created everyday, it is easy to miss out on apps which could be of great relevance. Moreover, the range of android applications available has not been extensively discussed in the literature. To this end, we have compiled an exhaustive list of android smartphone applications, which we feel can help our day to day functioning. The apps have been extensively reviewed and neatly described along with all their potential uses. In addition, we have made an effort to highlight ‘non-medical’ or efficiency apps which can improve departmental functioning. These apps have not been described in prior articles, and their functionality might not be known to all. We believe that the technology savvy plastic surgeon can make maximum use of these apps to his benefit.
