Kaposi's sarcoma is a multifocal angiogenic tumor disease whose principal causal agent is human h... more Kaposi's sarcoma is a multifocal angiogenic tumor disease whose principal causal agent is human herpes virus 8 (HHV-8). Herein, we report a rare case of iatrogenic Kaposi's sarcoma developing during oral corticotherapy. A 76-year-old, HIV-negative male presented with papulous, angiomatous lesions on the trunk and limbs, which appeared three months after the beginning of oral corticotherapy for bullous pemphigoid. We suspected iatrogenic Kaposi's sarcoma given the time to lesion onset in relation to the immunosuppressive treatment, together with histological and virological confirmation of HHV-8. The lesions began to subside when corticosteroids were tapered down to 10 mg/day. This was the first case reported in our setting and it emphasized the need for the rigorous monitoring of patients receiving immunosuppressants to avoid overlooking the side effects or rare complications of these treatments.

Vitiligo, a chronic autoimmune dermatosis characterized by asymptomatic, achromic macules, affects the integrity of the skin. Methodology: This was a descriptive, cross-sectional study conducted from March to June 2019 in six public hospitals in the city of Ouagadougou, Burkina Faso. The patients included in the study were followed in the dermatology departments of these structures for vitiligo, aged at least eighteen years and consenting. The Dermatology Life Quality Index (DLQI) and the vitiligo-specific health-related quality of life scale (VitiQoL) were employed to measure QoL (quality of life). Results: A total of fifty patients agreed to participate in the study. The mean age was 40.56 years, ranging from 18 to 79 years. There were as many females as males (25), and 23 married patients out of the fifty. The majority of the patients (43/50) resided in urban areas. Twenty-six patients had at least secondary education and eighteen patients worked in the informal sector. The average duration of vitiligo progression was 10.56 years, ranging from three months to 49 years. The evolution of vitiligo was stationary for 17/50 patients. The lesions were mainly located on the head and neck. The average body surface area (BSA) affected was 12.04%, ranging from 1% to 82%. The treatment was mainly local and general corticosteroid therapy. The evaluation of the patients' quality of life (QoL) by the DLQI (Dermatology Life Quality Index) yielded a mean score of 5.44/30. Vitiligo had a small effect on the QoL of eighteen patients and a moderate effect on sixteen patients. VitiQoL assessment yielded a mean total score of 32.32/96 and stigma had the highest score of 18.04/36. The patients' QoL was influenced by age and body surface area affected by vitiligo. Restriction of participation in activities and changes in the patient's behavior were significantly correlated with the duration of vitiligo progression, followed by stigma, which was related to vitiligo progression. Conclusion: The alteration of the QoL of the patients with vitiligo was low to moderate. This alteration was related to the stigmatization by one's environment.

Materials and Methods: The aim of this study was to document the clinical and epidemiological profiles of dermatitis herpetiformis and to detail its treatment in a hospital setting in order to increase our knowledge about this disease in our context and so to improve its management. We conducted a retrospective, cross-sectional study of the records of all patients seen in consultation or hospitalized at the dermatology department of Yalgado Ouédraogo University Hospital Centre, Ouagadougou, a public hospital in Burkina Faso, from January 2016 to December 2020. Human herpesvirus type 8 (HHV-8) is the main etiological agent of Kaposi's sarcoma. This virus is... more Human herpesvirus type 8 (HHV-8) is the main etiological agent of Kaposi's sarcoma. This virus is frequently associated with immunocompromision. This study aimed to detect HHV-8 in people with compromised immune system. Patients and Methods: This is a cross-sectional study that included 180 subjects: 179 HIV-infected patients and 1 patient with bullous pemphigoid. Blood samples were taken from all subjects, and swabs of lesions were then taken from individuals with symptoms of Kaposi's sarcoma. Viral load and CD4 + T lymphocytes count were performed for persons living with HIV and real-time PCR detection of HHV-8 DNA was performed in all subjects in the study. Results: Among HIV-infected persons, 13.41% had a viral load of more than 10,000 copies/mL, and 22.91% had a CD4 + T lymphocytes count of fewer than 350 cells/µL. A total of four (three HIV-1 infected patients and one patient with bullous pemphigoid) patients (2.22%) had apparent lesions of Kaposi's sarcoma. In the plasmas and swabs from associated lesions, HHV-8 DNAwas found in only two individuals, with an HHV-8 prevalence of 1.11% (2/180) with 0.55% (1/179) in an HIV-infected patient on antiretroviral therapy. Conclusion: These results exposing low prevalence levels of HHV-8 in HIV-infected patients could be due to the beneficial effect of antiretroviral drugs.

The evaluation of the psychosocial impact of chronic dermatoses on the quality of life of patients may help to orientate the objectives of management in order to improve their daily life, hence the interest of our study with the objective to evaluate the impact of psoriasis on the quality of life of patients followed in the dermatology, venereology, and rheumatology departments of the city of Ouagadougou, Burkina Faso. Patients and Method: This was a descriptive, cross-sectional study that took place from March 1 to June 28, 2019, in six public hospitals in the city of Ouagadougou. The Dermatology Life Quality Index (DLQI) and Psoriasis Disability Index (PDI) were the quality of life tools employed for this study. Results: Forty-eight (48) patients with psoriasis met the inclusion criteria. The mean age of the patients was 46.20 years, ranging from 22 to 79 years. There were 18 females and 30 males, with a sex ratio of 1.6. The measurement of the patients' QoL by the DLQI reported a mean score of 9.14 out of 30. There was a low impact of the disease on the QoL for seventeen patients. The evaluation of the QOL with the PDI noted an alteration of daily activities (3.95/15) and alteration of the patients' psychosocial relationships (2.66/18). The analysis of the QoL according to sociodemographic and clinical variables noted an alteration significantly related to age, level of education, and severity of the disease. An alteration in professional relationships was significant in female patients. An alteration in the different dimensions of the QoL was more significant in patients with a low level of education. The duration of the disease seemed to have no impact on the patients' quality of life. Daily activities were significantly altered for patients with a PASI between 7 and 10. The DLQI did not correlate with disease severity (PASI) (r = 0.228; p = 0.120), unlike the PDI (r = 0.371; p = 0.009). Conclusion: The QoL of psoriatic patients in Ouagadougou seemed to be slightly altered. This alteration was more significant for females. Professional relationships were altered for young subjects while daily activities were altered for those older.

Peu d’etudes sur les allergenes sensibilisants ont ete publiees au Burkina Faso. L’objectif de notre etude etait de decrire le profil de sensibilisation des patients symptomatiques ayant realise un prick-test. Nous avons realise une etude retrospective, dans une structure sanitaire privee de Ouagadougou de mars 2015 a avril 2016. La population d’etude etait les patients, ayant des manifestations d’allure allergique, chez qui un prick-test a ete realise. Quatorze allergenes ont ete testes. Nous avons colliges 93 patients dont 60,22 % (56) de sexe masculin et 39,78 % (37) de sexe feminin. L’âge median etait de 19 ans +/- 18. Les enfants representaient 52,22 % (49). La filariose lymphatique maladie tropicale negligee, debilitante a transmission vectorielle existe en Afrique, Asie et Amerique du Sud. Pres de 119 millions de personnes en sont infectees. L’une des strategies de lutte consiste en l’interruption de la transmission du parasite par l’administration annuelle d’une dose unique de diethylcarbamazine ou d’ivermectine-albendazole aux personnes exposees en zone d’endemie (prevalence microfilaremie > 1%). Apres onze tours de traitement de masse (TDM), nous avons voulu evaluer l’impact du TDM sur la transmission du parasite. Methodologie. Une etude prospective transversale menee dans 6 sites de fevrier a mars 2014. Ont ete inclus toutes personnes residentes consentantes âges de plus de 5 ans. Nous rapportons deux cas, issus d’un mariage consanguin au Burkina. Observation. Il s’agit d’un frere et de sa sœur âges de 7 ans et 12 ans, recus pour des bulles generalisees prurigineuses evoluant depuis la naissance. L’examen notait des bulles hemorragiques, des placards exulcero-crouteux, sur le tronc, les membres, une atteinte ungueale, evoquant tant une EBH jonctionnelle qu’une EBH simple. L’histopathologie etait en faveur d’une EBJ montrant un clivage a la jonction dermo epidermique. Conclusion. Loa loa filariasis is usually found in the forest areas of Central and West Africa. We report a c... more Loa loa filariasis is usually found in the forest areas of Central and West Africa. We report a case that was diagnosed in Ouagadougou (Burkina Faso), a savanna area. The patient lived in Gabon but was visiting his family in Ouagadougou. He complained of fatigue, fever, itchy legs with scratch marks, and intermittent edema of the legs. A blood smear was first examined for malaria parasites, but Loa loa microfilariae were observed. Laboratory tests showed hypereosinophilia (30%). Transient angioedema (Calabar edema) was observed. Loa loa filariasis was diagnosed based on these findings. There were no other laboratory test abnormalities, and ophthalmological examination was normal. The patient received a single dose of ivermectin at 200 µg/kg. After 1 month, the patient's course was favorable and a control blood smear was negative.

Epidermoid carcinomas account for one third of cutaneous carcinomas. These are the most common ca... more Epidermoid carcinomas account for one third of cutaneous carcinomas. These are the most common cancers in adults. On dark skin, epidermoid carcinomas do not mainly occur on photo-exposed areas but rather complicate chronic ulcers. We are reporting the case of a 39 years old patient, HIV 1 positive being consulted for a budding tumor of the upper lip occurred on chronic erosion that appeared three years before. The histopathological examination of the bioptic specimen and of the resection specimen objectified a differentiated and mature epidermoid carcinoma. The patient was classified as Stage II: T3 N0 M 0. A complete tumor resection was performed followed by a double labial reconstruction by local advancement flap. Chronic ulceration and HIV infection have been identified as factors that favored malignant transformation. Limited access to health facilities has reportedly favored the evolution of lesions towards a complicated form and difficult medical care. The therapeutic challenge was to remove the tumor while preserving the function and aesthetics of the mouth.

We report a case that was diagnosed in Ouagadougou (Burkina Faso), a savanna area. The patient lived in Gabon but was visiting his family in Ouagadougou. He complained of fatigue, fever, itchy legs with scratch marks, and intermittent edema of the legs. A blood smear was first examined for malaria parasites, but Loa loa microfilariae were observed. Laboratory tests showed hypereosinophilia (30%). Transient angioedema (Calabar edema) was observed. Loa loa filariasis was diagnosed based on these findings. There were no other laboratory test abnormalities, and ophthalmological examination was normal. The patient received a single dose of ivermectin at 200 µg/kg. After 1 month, the patient's course was favorable and a control blood smear was negative.

Epidermoid carcinomas account for one third of cutaneous carcinomas. These are the most common ca... more Epidermoid carcinomas account for one third of cutaneous carcinomas. These are the most common cancers in adults. On dark skin, epidermoid carcinomas do not mainly occur on photo-exposed areas but rather complicate chronic ulcers. We are reporting the case of a 39 years old patient, HIV 1 positive being consulted for a budding tumor of the upper lip occurred on chronic erosion that appeared three years before. The histopathological examination of the bioptic specimen and of the resection specimen objectified a differentiated and mature epidermoid carcinoma. The patient was classified as Stage II: T3 N0 M 0. A complete tumor resection was performed followed by a double labial reconstruction by local advancement flap. Chronic ulceration and HIV infection have been identified as factors that favored malignant transformation. Limited access to health facilities has reportedly favored the evolution of lesions towards a complicated form and difficult medical care. The therapeutic challenge was to remove the tumor while preserving the function and aesthetics of the mouth.
