Since 25years the EUROCARE study monitors the survival of cancer patients in Europe through centr... more Since 25years the EUROCARE study monitors the survival of cancer patients in Europe through centralised collection, quality check and statistical analysis of population-based cancer registries (CRs) data. The European population covered by the study increased remarkably in the latest round. The study design and statistical methods were also changed to improve timeliness and comparability of survival estimates. To interpret the EUROCARE-5 results on adult cancer patients better here we assess the impact of these changes on data quality and on survival comparisons. In EUROCARE-5 the survival differences by area were studied applying the complete cohort approach to data on nearly nine million cancer patients diagnosed in 2000-2007 and followed up to 2008. Survival time trends were analysed applying the period approach to data on about 10 million cancer cases diagnosed from 1995 to 2007 and followed up to 2008. I Lignani costituiscono una particolare classe di composti fenolici caratterizzati da attivit\ue0... more I Lignani costituiscono una particolare classe di composti fenolici caratterizzati da attivit\ue0 antiestrogenica e antitumorale. Sono contenuti principalmente nei semi di lino e di sesamo, nell\u2019olio extra vergine di oliva, nei cereali a chicco intero, in varie verdure e frutta. In letteratura si ritrovano dati relativi al contenuto in lignani di vari cereali tra cui il grano tenero, ma quelli del grano duro sono frammentari o nulli. Per questo motivo abbiamo condotto uno studio preliminare per la determinazione del contenuto in lignani in campioni di grano duro, semole, pasta e pane prodotti in Sicilia raccolti nel corso del 2010. Dopo estrazione, i campioni sono stati sottoposti ad analisi in RP-HPLC con rivelatori UV-fotodiodi e coulometrico in linea per la determinazione dei principali lignani: Pinoresinolo (PINO), Lariciresinolo (LARI), Secoisolariciresinolo (SECO). I risultati, seppure preliminari e caratterizzati da elevata variabilit\ue0, mostrano che nel grano duro siciliano e nei suoi derivati (pane e pasta) prevale il LARI mentre il SECO e il PINO sono presenti in quantit\ue0 10 volte inferiori. Il contenuto totale dei lignani nei chicchi interi di grano duro va da 3957,0 a 5524,0 \ub5g/100g s.s.. Dopo molinatura, le semole utilizzate per la produzione della pasta, presentano dal 23.5% a circa il 40% del contenuto originario di lignani, con una ulteriore, seppure irrisoria, riduzione durante la trasformazione in pasta. Al contrario, le semole integrali utilizzate per produrre il pane, mantengono dal 70 al 90% del rispettivo contenuto in lignani ma dopo lievitazione ed in conseguenza del calore per la trasformazione a pane il contenuto scende a circa il 10%. I dati relativi al contenuto totale di lignani nei grani e derivati analizzati in questo studio mostrano livelli elevati se confrontati con i dati presenti in letteratura (Lucy et al., 2000 riportano in particolare per il SECO un contenuto in 8,1 \ub5g/100 g s.s.)

Nodal involvement is accepted as the best single marker of prognosis in breast cancer. However, t... more Nodal involvement is accepted as the best single marker of prognosis in breast cancer. However, there is little information on the subdivision of node-positive patients according to the oestrogen receptor status of the nodal tissue. We have previously reported (Eur. J. Ca. 1987, 23, 31) that, in almost all cases, involved nodes are only oestrogen receptor positive (ER+) in patients whose primary tumours are uniformly ER+. This paper presents clinical follow-up on a larger group of patients with node positive breast cancer. For each patient, both soluble and nuclear receptor concentrations were determined in three separate parts of the primary tumour and in at least one involved node (we have previously defined tumours which contained ER in all six fractions of the primary as HS+ +, those lacking receptor in some fractions as HS+-and wholly receptor negative tumours as HS-). Median follow-up time was 71.5 months. As expected, patients whose tumours were HS + + had a significant (P < 0.008) survival advantage. More importantly, patients with ER in both the soluble and nuclear fractions of their involved nodes survived significantly (P<0.003) longer than those with ER-nodes. Thus, full oestrogen receptor status of involved nodes will give sufficient prognostic information when adequate primary tissue is not available.

The urinary excretion patterns of oestrogen metabolites, including unusual metabolites, were dete... more The urinary excretion patterns of oestrogen metabolites, including unusual metabolites, were determined by gas chromatography and mass spectrometry for 63 women with advanced breast cancer and 39 normal postmenopausal women. The concentration of total unusual metabolites excreted was found to be an excellent discriminant between breast-cancer patients and controls (P <0.0001). Discrimination between responders and non-responders to endocrine therapy was attempted, using several different indices. Of these, the ratio of Classical Oestrogens to Unusual Metabolites (CE/UM) proved a fair discriminant, but the product of this ratio and the oestriol ratio (CE/UM x E3R) was much the best discriminant. This product, termed a Pattern Index, has considerable potential, not only as a discriminant for selecting therapy, but also as a rapid index of patient response to that therapy.

In this randomized dietary intervention study (DI) we analyzed levels of androgens, phytoestrogen... more In this randomized dietary intervention study (DI) we analyzed levels of androgens, phytoestrogens, and estrogens in 12-h urine samples of 69 healthy postmenopausal women, 37 of whom followed a traditional Mediterranean diet for 6 months (intervention group) as compared to 32 women who followed their regular diet (control group). Circulating levels of both insulin and testosterone (T) were also assayed. Overall, enterolactone (ENL) was the most prominent phytoestrogen in urines of both control and intervention women, and its levels increased by a 20% after DI. At the baseline the ENL levels were seen to be significantly associated with both the total androgens (TOT-A) (r = 0.371, P = 0.002) and the TOT-A/total estrogen (TOT-E) ratio (r = 0.351, P = 0.005) in all 69 postmenopausal women. Furthermore, the DI resulted in a more pronounced negative association of both ENL with insulin (r = −0.321, P = 0.05) and insulin with TOT-A (r = −0.421, P = 0.01). Regarding urinary androgens, ENL associated with both 3α-androsterone (5α-androgen, r = 0.363, P = 0.002) and 3α-etiocolanolone (5β-androgen, r = 0.295, P = 0.01) at baseline, while after DI, circulating insulin and T exhibited a significant negative association with the 5β-androgen metabolite etiocolanolone (r = −0.487, P = 0.002; and r = −0.336, P = 0.042, respectively). We conclude that lignan components of the Mediterranean diet, notably ENL, are associated with urinary levels of products of androgen metabolism, including both 5α-and 5β-reductase enzymes, in healthy postmenopausal women. Further studies are necessary to better understand the interplay of sex hormones with dietary phytoestrogens.

In this retrospective study we assessed the expression of the HER2/ neu oncogene product in a ser... more In this retrospective study we assessed the expression of the HER2/ neu oncogene product in a series of 574 consecutive breast cancer cases, all recruited at the Maurizio Ascoli Cancer Center of Civico Hospital, in Palermo, between January 1998 and June 2003. The HER2/neu expression was evaluated using immunohistochemistry and scored from 0 to +3 as per FDA recommendations. The HER2/neu expression levels were related to the clinicalpathological features of the disease, including tumor size, nodal and menopausal status, estrogen and progesterone receptors, and hormonal or chemotherapeutic treatment. In 108 patients with a follow-up period of 3 years or more, the HER2/neu expression was also related to their survival characteristics. A significant correlation (P = 0.011) between HER2/neu +3 and estrogen receptor-negative cases was observed in the 487 M0 patients. In addition, HER2/neu +3 cases were associated with a positive nodal status (57.4%), although this association was not quite significant (P = 0.06). More importantly, follow-up data revealed that, in the 91 M0 patients with an average follow-up period of 37 months, the percentage of HER2/neu +3 patients who relapsed was remarkably greater (54.8%) than that observed for the HER2/neu +1/0 cases when combined (34.2%). Furthermore, the disease-free interval (DFI) was 47 months in the HER2/neu +1/0 group, while it dropped to 45 months in c-HER2/neu +3 cases. Although the limited number of cases does not allow us to draw any definitive conclusions, our data suggest that high expression levels of HER2/neu +3 are associated with an early relapse and a shorter disease-free interval in M0 breast cancer patients.

Metabolic syndrome (MS), conventionally defined by the presence of at least three out of five dys... more Metabolic syndrome (MS), conventionally defined by the presence of at least three out of five dysmetabolic traits (abdominal obesity, hypertension, low plasma HDL-cholesterol, high plasma glucose and high triglycerides), has been associated with an increased risk of several age-related chronic diseases, including breast cancer (BC). This may have prognostic implications for BC survivors. 2,092 early stage BC survivors aged 35-70, recruited in eleven Italian centres 0-5 years after surgical treatment (1.74 years on average), were followed-up over 2.8 years on average for additional BC-related events, including BC-specific mortality, distant metastasis, local recurrences and contralateral BC. At recruitment, 20 % of the patients had MS. Logistic regression models were carried out to generate OR and 95 % confidence intervals (CI) for new BC events associated with MS, adjusting for baseline pathological prognostic factors. New BC events occurred in 164 patients, including 89 distant metastases. The adjusted ORs for women with MS versus women without any MS traits were 2.17 (CI 1.31-3.60) overall, and 2.45 (CI 1.24-4.82) for distant metastasis. The OR of new BC events for women with only one or two MS traits was 1.40 (CI 0.91-2.16). All MS traits were positively associated with new BC events, and significantly so for low HDL and high triglycerides. MS is an important prognostic factor in BC. As MS is reversible through lifestyle changes, interventions to decrease MS traits in BC patients should be implemented in BC clinics.

Regarding urinary androgens, ENL associated with both 3α-androsterone (5α-androgen, r = 0.363, P = 0.002) and 3α-etiocolanolone (5β-androgen, r = 0.295, P = 0.01) at baseline, while after DI, circulating insulin and T exhibited a significant negative association with the 5β-androgen metabolite etiocolanolone (r = −0.487, P = 0.002; and r = −0.336, P = 0.042, respectively). We conclude that lignan components of the Mediterranean diet, notably ENL, are associated with urinary levels of products of androgen metabolism, including both 5α-and 5β-reductase enzymes, in healthy postmenopausal women. Further studies are necessary to better understand the interplay of sex hormones with dietary phytoestrogens.

In this retrospective study we assessed the expression of the HER2/ neu oncogene product in a ser... more In this retrospective study we assessed the expression of the HER2/ neu oncogene product in a series of 574 consecutive breast cancer cases, all recruited at the Maurizio Ascoli Cancer Center of Civico Hospital, in Palermo, between January 1998 and June 2003. The HER2/neu expression was evaluated using immunohistochemistry and scored from 0 to +3 as per FDA recommendations. The HER2/neu expression levels were related to the clinicalpathological features of the disease, including tumor size, nodal and menopausal status, estrogen and progesterone receptors, and hormonal or chemotherapeutic treatment. In 108 patients with a follow-up period of 3 years or more, the HER2/neu expression was also related to their survival characteristics. A significant correlation (P = 0.011) between HER2/neu +3 and estrogen receptor-negative cases was observed in the 487 M0 patients. In addition, HER2/neu +3 cases were associated with a positive nodal status (57.4%), although this association was not quite significant (P = 0.06). More importantly, follow-up data revealed that, in the 91 M0 patients with an average follow-up period of 37 months, the percentage of HER2/neu +3 patients who relapsed was remarkably greater (54.8%) than that observed for the HER2/neu +1/0 cases when combined (34.2%). Furthermore, the disease-free interval (DFI) was 47 months in the HER2/neu +1/0 group, while it dropped to 45 months in c-HER2/neu +3 cases. Although the limited number of cases does not allow us to draw any definitive conclusions, our data suggest that high expression levels of HER2/neu +3 are associated with an early relapse and a shorter disease-free interval in M0 breast cancer patients.

Metabolic syndrome (MS), conventionally defined by the presence of at least three out of five dys... more Metabolic syndrome (MS), conventionally defined by the presence of at least three out of five dysmetabolic traits (abdominal obesity, hypertension, low plasma HDL-cholesterol, high plasma glucose and high triglycerides), has been associated with an increased risk of several age-related chronic diseases, including breast cancer (BC). This may have prognostic implications for BC survivors. 2,092 early stage BC survivors aged 35-70, recruited in eleven Italian centres 0-5 years after surgical treatment (1.74 years on average), were followed-up over 2.8 years on average for additional BC-related events, including BC-specific mortality, distant metastasis, local recurrences and contralateral BC. At recruitment, 20 % of the patients had MS. Logistic regression models were carried out to generate OR and 95 % confidence intervals (CI) for new BC events associated with MS, adjusting for baseline pathological prognostic factors. New BC events occurred in 164 patients, including 89 distant metastases. The adjusted ORs for women with MS versus women without any MS traits were 2.17 (CI 1.31-3.60) overall, and 2.45 (CI 1.24-4.82) for distant metastasis. The OR of new BC events for women with only one or two MS traits was 1.40 (CI 0.91-2.16). All MS traits were positively associated with new BC events, and significantly so for low HDL and high triglycerides. MS is an important prognostic factor in BC. As MS is reversible through lifestyle changes, interventions to decrease MS traits in BC patients should be implemented in BC clinics.
