Discretion that occur at the lowest level of implementation of a policy indicates there is still ... more Discretion that occur at the lowest level of implementation of a policy indicates there is still a gap between the policy strategy with the diversity of the existing problems. BPJS-Kesehatan Policy as a top-down policy, health center position as the spearhead of health services, especially in health centers Kepanjen poor districts. issues raised in this research is how the pattern of discretion occurs and its impact on policy implementation in the clinic BPJS-Health Kepanjen. This study used descriptive qualitative method by focusing on 1) the restrictions on access and request 2) administrative injustice 3) the service user settings and work situation, and 4) an effort to influence the mentality of users of the service. The study revealed that there was no discretion committed by lower level bureaucrats Kepanjen health center, it was because the norm of the profession are upheld by lower level bureaucrats and simple design of health services. Discretion occur at managerial levels, the changing patterns of communication and coordination by the Office of BPJS-Kesehatan, Public Health Service of Malang Regency and Puskesmas Kepanjen. theoretically, the pattern of discretion by Michael Lipsky did not occur in the implementation of BPJS-Kesehatan policies at the Puskesmas Kepanjen. There are several steps that can be taken to overcome such discretion. First the capitation system changes, strengthening and centralizing the BPJS-Kesehatan database, the regulations that health centers can manage a budget independently.
