The traumatic dissection of the internal carotid artery (TDICA) is a highly underdiagnosed diseas... more The traumatic dissection of the internal carotid artery (TDICA) is a highly underdiagnosed disease. Seventeen percent may remain asymptomatic for periods longer than one day, and the occurrence of clinical symptoms after two weeks or more is not frequent. We report the case of a 35-year-old patient who had been hit by a car two weeks before admission. The same patient was hemodynamically stable, Glasgow 10, with right hemiparesis and motor aphasia. The CT scan showed image of hypodensity in temporal, parietal and left thalamus. Arteriography of the carotid-vertebral system showed bilateral dissection of internal carotid arteries. This report tries to get the attention to a disease that can lead to severe sequelae. Further studies are needed to address the creation of clinical protocols to diagnose and follow patients with strong suspicion of TDICA.

Antiphospholipid syndrome (APS), more common in females, manifests clinically as thrombosis and/o... more Antiphospholipid syndrome (APS), more common in females, manifests clinically as thrombosis and/or recurrent fetal loss. Hemolytic autoimmune anemia and neurological, cardiac and cutaneous manifestations are common. This is the case report of a male patient whose fi rst manifestation of the disease was gangrene of the auricle. The diagnosis of APS was established by biopsy of the lower limb skin, which showed thrombotic vasculopathy with no evidence of vasculitis. This is one of the two major criteria, which, along with a minor criterion, establishes the diagnosis of APS. Possible differential diagnoses are discussed. The importance of the biopsy in the APS diagnosis of this male patient is emphasized.

However, low adherence to prevention programs is cited in the literature as a recurring problem, questioning the effectiveness and sustainability of these programs. The aim of this study is to compare subgroups of fallers with different levels of adherence to a multifactorial fall prevention program in terms of a broad range of sociodemographic, medical, disability, physical functioning, and psychological measures in a multifactorial geriatric assessment, and identify which of these measures explain optimal adherence.MethodsA prospective study conducted with two hundred and fifty-seven community-dwelling people aged ≥ 60 years who participated in a multifactorial fall prevention program. INTRODUÇÃO RELATO DE CASO A dissecção traumática da artéria carótida interna (DTACI) é uma doença... more INTRODUÇÃO RELATO DE CASO A dissecção traumática da artéria carótida interna (DTACI) é uma doença altamente subdiagnosticada. Dezessete por cento podem permanecer assintomáticos por períodos maiores que um dia; e as manifestações clínicas após duas semanas são raras. Relata-se o caso de um paciente de 35 anos que fora vítima de atropelamento quinze dias antes da admissão. O mesmo apresentava-se hemodinamicamente estável, Glasgow 10, com hemiparesia à direita e afasia motora. A tomografia computadorizada de crânio evidenciou imagem hipodensa temporal superficial e profunda à esquerda. A arteriografia do sistema carotídeo-vertebral mostrou dissecção bilateral de artérias carótidas internas. Através deste caso, chama-se a atenção para uma doença que pode acarretar graves sequelas. São necessários mais estudos que visem à criação de protocolos de atendimento e seguimento dos pacientes com forte suspeição de DTACI.
