Atraumatic splenic rupture (ASR) is a rare complication of acute pancreatitis with high mortality... more Atraumatic splenic rupture (ASR) is a rare complication of acute pancreatitis with high mortality and morbidity rates. We present a case of a 63-year-old woman with a history of hypertension, presenting with acute pancreatitis who subsequently developed a splenic rupture requiring a laparotomy and splenectomy. ASR is a rare but life-threatening complication requiring prompt recognition and management and should be considered in patient with pancreatitis who develops sudden haemodynamic compromise and worsening anaemia.

To observe the effect of basic fibroblast growth factor (bFGF) and alpha-melanocyte stimulating h... more To observe the effect of basic fibroblast growth factor (bFGF) and alpha-melanocyte stimulating hormone (alpha-MSH) on adhesion and migration of melanocytes in vitro. Human melanocytes were obtained from normal human foreskins. Culture dishes covered with fibronectin were used to perform melanocytes adhesion assay, and cell motility was assessed using the Transwell micropore filter method. bFGF and alpha-MSH increased melanocytes adhesion on culture dishes covered with fibronectin. bFGF stimulated melanocytes migration through micropore filter while alpha-MSH had no significant effects. bFGF and alpha-MSH could promote the adhesion and migration of melanocytes, which suggests that two agents may play a role in the repigmentation of vitiligo.

Results showed: diabetic nephropathy in 28.2%, hypertensive nephrosclerosis 25.5%, UTI, cystitis and pyelonephritis in 6.8%, renal stones in 5.9%, obstructive uropathy in 7.6%, simple cysts in 4.5%, CRF of unknown origin in 13.1%, and others in 26.4%. DM and HTN were S related to kidney function tests and increase in elderly. Other cardiovascular risk factors and smoking are reported by previous workers to be HS related to renal diseases. Age was significantly related to GFR, BUN and Cr. but sex difference was not significantly related to renal diseases. Multiple myeloma, lupus nephritis, vasculitis and hepatitis B were all recorded in few numbers of elderly Egyptians. HCV was more common and more likely to cause renal diseases. Abdomino-pelvic ultrasound was confirmatory to clinical renal diseases diagnosis. Among patients (n=20) biopsies showed focal necrotizing GN in 20%,...

Sleeve gastrectomy is increasing in popularity for the treatment of morbid obesity. The most sign... more Sleeve gastrectomy is increasing in popularity for the treatment of morbid obesity. The most significant complication posed is leak from the resection line. Typically, this occurs at the proximal end of the sleeve, adjacent the gastroesophageal junction. Such leaks can be notoriously persistent resulting in a chronic gastric fistula. Even when low in output, these fistulas rarely spontaneously resolve. It may mean months of poor health, multiple interventions and prolonged hospitalisation. Various strategies have been employed to aid closure of a sleeve leak, including “drain and wait”, stenting, biological glues, mesh plugs and mucosal clips, all with variable and unreliable success. Many cases ultimately come to complex revisional surgery employing resection or diversion as a solution, but sometimes yielding a less than ideal bariatric result. A simple endoscopic solution would be ideal to this difficult problem and potentially shift the paradigm of leak management. We report two consecutive cases of managing sleeve leak using an over-the-scope endoscopic clip (OTSC) device which differs from standard mucosal endoscopic clips by attempting full thickness or near full thickness circumferential closure of the area of perforation and discuss the technical aspects of this procedure.

Cervical burning and regurgitation, often more pronounced when supine, can be troublesome and even disabling, interfering substantially with quality of life. The aim of this study was to identify the factors contributing to reflux after oesophagectomy and evaluate measures to prevent or control it. Methods A Medline search using the terms ‘gastro-oesophageal reflux’, ‘oesophagectomy’ and ‘antireflux surgery’ was conducted. Additional references and search pathways were sourced from the bibliographies of articles located. Both osteryoung square wave voltammetry and cyclic voltammetry have been utilized to elucidate an... more Both osteryoung square wave voltammetry and cyclic voltammetry have been utilized to elucidate and confirm the possible complexation reaction that occur between the various cephalosporin antibiotics and either the toxic, non-essential metal ion, viz. Cd (II), or the essential but toxic (when their concentration exceeds certain level in serum) metal ions, viz. Cu (II) and Zn (II). Voltammetric measurements indicated the existence of 1:1 metal-to-ligand ratio (as in cephalexin and cephapirin complexes), 1:2 ratio (such as in cefamandole, cefuroxime and cefotaxime complexes) and 2:1 ratio in case of ceftazidime complexes. Adsorption behavior was evidenced for Cu (II)-cefuroxime or ceftazidime complexes as well as for those for Zn (II)-cephalexin or cephapirin. This phenomenon could be used for the determination of either the antibiotic or the metal ion using adsorptive stripping voltammetry. Detection limits down to 7x10(-10) M have been easily achieved.
