This trial aimed to explore if the proposed multidisciplinary program could improve quality of life and disability of the patients suffering from chronic low back up to 36 months. Methods and Material: In this Clinical Randomized trial seventy percent (139 of 197) of the participants who had taken part in the original study including 66 patients in intervention group and 73 patients in control group were followed up to 36 months after intervention. The intervention group continued receiving monthly motivational consultation and booster classes plus oral medication but the other group received just medication. Data on measures of Short Form 36 (SF-36) Quebec Disability Scale (QDS) and Ronald Morris Disability (RDQ) were collected at 3-6-12-18-24-30-and 36-month follow ups and analyzed through RMANOVA. Rheumatoid arthritis (RA) and migraine are both common disorders which are caused by a faulty imm... more Rheumatoid arthritis (RA) and migraine are both common disorders which are caused by a faulty immune system and autonomic nervous system dysfunction, respectively. Although current treatment outlook has been shown a great improvement in these two diseases, however many side effects have been reported. We report a case of 43-years-female that has been suffered from rheumatoid arthritis for 3 years with a 6 years history of migraine. She had used different types of medication for both rheumatoid arthritis and migraine but during these 6 years no improvement was observed and even migraine progression in this patient became worse. She was admitted to the hospital for 12 weeks follow up to evaluate the effect of β-D-mannuronic acid (M2000) on her RA disease. During 12 weeks of M2000 therapy RA indices including Disease Activity Score (DAS28), simple disease activity index(SDAI) and C-reactive protein (CRP),erythrocyte sedimentation rate (ESR), rheumatoid factor (RF)), anti-cyclic citrullinated peptide antibodies(anti-CCP) and blood determinants were measured. A significant improvement was seen after 12 weeks of M2000 therapy in both rheumatoid arthritis and migraine which are considered as inflammatory diseases. Therefore, it might be concluded that M2000 as a novel non-steroidal anti-inflammatory drugs (NSAIDs) with immunosuppressive property is able to treat migraine in addition to its potent efficacy on treatment of rheumatoid arthritis.

It seems that non-HLA genes, as well as HLA-B27 gene, are linked to the etiology of the disease. Recently, it has been documented that KIRs and their HLA ligands are contributed to the Ankylosing Spondylitis. The aim of this study was to evaluate the KIR genes and their HLA ligands in Iranian AS patients and healthy individuals. The present study includes 200 AS patient samples and 200 healthy control samples. KIR genotyping was performed using the polymerase chain reaction sequence-specific primer (PCR-SSP) method to type the presence or absence of the 16 KIR genes, 6 known specific HLA class I ligands and also, two pseudogenes. The aim of this study was to assess the role of HLA-B*27 and it's subtypes in determining sev... more The aim of this study was to assess the role of HLA-B*27 and it's subtypes in determining severity and clinical manifestations of ankylosing spondylitis (AS).A total of 163 AS patients were assessed for clinical manifestations and severity using structured questionnaires. HLA-B*27 screening and B*27 sub-typing were performed by PCR.One hundred twenty two patients (74.8%) were B*27 positive. The male to female ratio, peripheral arthritis, steroid use, intense dorsal kyphosis and decrease of cervical slope had a significantly higher frequency in B*27 positive patients compared to B*27 negative ones (p=0.01, 0.001, 0.01, 0.04 and 0.04, respectively). However, the age of diagnosis was significantly lower in B*27 positive patients (p=0.005). Tehran, with one-ninth of the population of Iran and of mixed ethnic origins, was selected as the field. Subjects were randomly selected from the 22 districts. Interviews were conducted once a week, on the weekend. The 3 phases of stage 1 were done on the same day, in parallel, like the fast-track Community Oriented Program for Control of Rheumatic Diseases (COPCORD). Four thousand ninety-six houses were visited and 10,291 persons were interviewed. Musculoskeletal complaints during the past 7 days were detected in 41.9% of the interviewed subjects. The distribution was: shoulder 14.5%, wrist 10%, hands and fingers 9.4%, hip 7.1%, knee 25.5%, ankle 9.8%, toes 6.1%, cervical spine 13.4%, and dorsal and lumbar spine 21.7%. The mortality rate and causes of death are different in various countries. The present study is done to find out the causes of mortality in the systemic lupus erythematosus (SLE) patients in lupus unit of Rheumatology Research Center and other wards of Shariati Hospital. This study was retrospective according to medical records of patients who were referred to Lupus Unit of Rheumatology Research center, ICU, Gynecology and Nephrology wards of Shariati hospital during 10 years, from 1991 till 2001. Of 2021-recorded patient’s files, 165 (7.8%) deaths were already recorded. 84.8% were female and 15.2% male. The duration of disease was 36±12 months. The mean duration of follow up from entering the study till time of death was 25.9±16.8 months. Firstly, the authors interviewed 10,000 inhabitants with a refusal rate of 21.5%. These 2150 persons are not seen in Figure 1. The last point is that if you survey 10,000 inhabitants and find 13 patients with RA the prevalence of RA will be simply 0.13% [10/5515 (0.18%) in women; 3/4485 (0.06%) in men] instead of 0.37% (0.48% and 0.23% in women and men, respectively) even if those who have refused are excluded.

Background: To estimate the prevalence and characteristics of Rheumatoid Arthritis (RA) in an urb... more Background: To estimate the prevalence and characteristics of Rheumatoid Arthritis (RA) in an urban area of Tehran. Methods: A total of 50 clusters were randomly selected in Tehran and 10291 subjects completed the COPCORD Core Questionnaire during 2004 and 2005. Patients with rheumatic complaints were examined and diagnosed by subspecialty fellows in rheumatology. Laboratory and radiology tests were also performed if required. Results: A total of 35 subjects (5 men and 30 women) were diagnosed with RA, with a prevalence of 0.33% (95% CI: 0.22-0.46). Our results demonstrated that RA was six times more common in women than men. The mean age (± SD) of patients was 52.3 (± 17.6) years. Morning stiffness > 1 hour was reported in 37.1% of patients. Rheumatic signs were commonly found in wrist (60%), knee (60%), metacarpophalangeal (48.6%) and proximal interphalangeals of hand (40%). There is some data in the literature on the association of HLA-B5 and some manifestations of Behc... more There is some data in the literature on the association of HLA-B5 and some manifestations of Behcet's disease (BD), especially ocular lesions. We studied 433 patients to see if there was any relationship between B51 and the manifestations of the disease. Clinical manifestations of BD were compared in patients having HLA-B51 (155 patients) and those lacking HLA-B51 (278 patients). Oral aphthosis, genital aphthosis, skin manifestations, joint manifestations, gastrointestinal manifestations, phlebitis and neurological manifestations showed no significant difference in patients with and without HLA-B51. Ophthalmologic manifestations were seen in 52% of patients having B51 and in 42% of patients lacking it (χ 2 : 4.451, P = 0.035). Considering different lesions of ocular manifestations separately, no significant difference was found regarding the presence or absence of B51. The aim of this study was to assess the role of HLA-B*27 and it's subtypes in determining sev... more The aim of this study was to assess the role of HLA-B*27 and it's subtypes in determining severity and clinical manifestations of ankylosing spondylitis (AS).A total of 163 AS patients were assessed for clinical manifestations and severity using structured questionnaires. HLA-B*27 screening and B*27 sub-typing were performed by PCR.One hundred twenty two patients (74.8%) were B*27 positive. The male to female ratio, peripheral arthritis, steroid use, intense dorsal kyphosis and decrease of cervical slope had a significantly higher frequency in B*27 positive patients compared to B*27 negative ones (p=0.01, 0.001, 0.01, 0.04 and 0.04, respectively). However, the age of diagnosis was significantly lower in B*27 positive patients (p=0.005). Tehran, with one-ninth of the population of Iran and of mixed ethnic origins, was selected as the field. Subjects were randomly selected from the 22 districts. Interviews were conducted once a week, on the weekend. The 3 phases of stage 1 were done on the same day, in parallel, like the fast-track Community Oriented Program for Control of Rheumatic Diseases (COPCORD). Four thousand ninety-six houses were visited and 10,291 persons were interviewed. Musculoskeletal complaints during the past 7 days were detected in 41.9% of the interviewed subjects. The distribution was: shoulder 14.5%, wrist 10%, hands and fingers 9.4%, hip 7.1%, knee 25.5%, ankle 9.8%, toes 6.1%, cervical spine 13.4%, and dorsal and lumbar spine 21.7%. The mortality rate and causes of death are different in various countries. The present study is done to find out the causes of mortality in the systemic lupus erythematosus (SLE) patients in lupus unit of Rheumatology Research Center and other wards of Shariati Hospital. This study was retrospective according to medical records of patients who were referred to Lupus Unit of Rheumatology Research center, ICU, Gynecology and Nephrology wards of Shariati hospital during 10 years, from 1991 till 2001. Of 2021-recorded patient’s files, 165 (7.8%) deaths were already recorded. 84.8% were female and 15.2% male. The duration of disease was 36±12 months. The mean duration of follow up from entering the study till time of death was 25.9±16.8 months. Firstly, the authors interviewed 10,000 inhabitants with a refusal rate of 21.5%. These 2150 persons are not seen in Figure 1. The last point is that if you survey 10,000 inhabitants and find 13 patients with RA the prevalence of RA will be simply 0.13% [10/5515 (0.18%) in women; 3/4485 (0.06%) in men] instead of 0.37% (0.48% and 0.23% in women and men, respectively) even if those who have refused are excluded.

Background: To estimate the prevalence and characteristics of Rheumatoid Arthritis (RA) in an urb... more Background: To estimate the prevalence and characteristics of Rheumatoid Arthritis (RA) in an urban area of Tehran. Methods: A total of 50 clusters were randomly selected in Tehran and 10291 subjects completed the COPCORD Core Questionnaire during 2004 and 2005. Patients with rheumatic complaints were examined and diagnosed by subspecialty fellows in rheumatology. Laboratory and radiology tests were also performed if required. Results: A total of 35 subjects (5 men and 30 women) were diagnosed with RA, with a prevalence of 0.33% (95% CI: 0.22-0.46). Our results demonstrated that RA was six times more common in women than men. The mean age (± SD) of patients was 52.3 (± 17.6) years. Morning stiffness > 1 hour was reported in 37.1% of patients. Rheumatic signs were commonly found in wrist (60%), knee (60%), metacarpophalangeal (48.6%) and proximal interphalangeals of hand (40%). There is some data in the literature on the association of HLA-B5 and some manifestations of Behc... more There is some data in the literature on the association of HLA-B5 and some manifestations of Behcet's disease (BD), especially ocular lesions. We studied 433 patients to see if there was any relationship between B51 and the manifestations of the disease. Clinical manifestations of BD were compared in patients having HLA-B51 (155 patients) and those lacking HLA-B51 (278 patients). Oral aphthosis, genital aphthosis, skin manifestations, joint manifestations, gastrointestinal manifestations, phlebitis and neurological manifestations showed no significant difference in patients with and without HLA-B51. Ophthalmologic manifestations were seen in 52% of patients having B51 and in 42% of patients lacking it (χ 2 : 4.451, P = 0.035). Considering different lesions of ocular manifestations separately, no significant difference was found regarding the presence or absence of B51.
