increasing amount of evidence suggests that chronic subclinical inflammation is one of the triggering factors in the onset
of type 2 diabetes mellitus (T2DM). Objective: The current study was designed to determine the association of C-reactive protein (CRP) and glycated hemoglobin (HbA1c) in T2DM patients. Methods: This cross-sectional study was
carried out at Department of Laboratory Medicine, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka,
Bangladesh between March 2017 to February 2018. Total 80 adult patients of newly diagnosed T2DM were included in
this study. A detailed clinical history was recorded and relevant physical examinations were done. The complete blood
count (CBC), C-reactive protein (CRP), fasting blood glucose (FBG) and HbA1c were done accordingly. Data were
analyzed and compared by statistical tests. Results: The mean age of the study patients was 45.06±11.08 years with
female predominance. More than half (53.7%) of the study patients were in age group 41-59 years. Majority (65%) of
the study patients was overweight/obese. It was observed that, mean(±SD) FBG, HbA1c and CRP levels were 8.97±2.41
mmol/L 8.54±2.08% and 8.98±4.85 mg/L respectively. Pearson correlation analysis revealed that CRP has a significant
positive correlation with HbA1c (r= 0.457, p< 0.001). Conclusion: This study concluded that CRP has a significant
positive association with HbA1c in adult T2DM patients. CRP can be used as a valuable and effective tool for monitoring
T2DM.

Background: Diabetic nephropathy (DN) is a specific micro vascular complication of diabetes melli... more Background: Diabetic nephropathy (DN) is a specific micro vascular complication of diabetes mellitus (DM), approximately one third of affected diabetic patients eventually develop DN. The level of albuminuria is highly correlated with the intraglomerular pressure which is the key determinant of DN. A number of risk factors were suggested for the development of DN. Objective: To evaluate the common risk factors of diabetic nephropathy and to determine the association of different risk factors with diabetic nephropathy among adult diabetic patients. Methods: This cross-sectional study was conducted at the Department of Nephrology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh. A total of one hundred twenty-eight (128) adult diabetic patients were enrolled. We identified nephropathy among them by doing albumin creatinine ratio (ACR) on spot urine and carried out relevant laboratory investigations. Common risk factors of diabetic nephropathy like-hypertension (HTN), overweight/obesity, family history of HTN/DM/DN, duration of DM, glycated hemoglobin (HbA1C) and dyslipidemia were evaluated accordingly. Result: Among 128 study population, 52 (40.6%) patients were found to have diabetic nephropathy. Of them, 27.3% had microalbuminuria, 13.3% had macroalbuminuria. Hypertension, overweight/obesity, family history of HTN/DM and dyslipidemia were observed to be significantly higher in nephropathy group than those in the non-nephropathy group (p<0.05). Hypertension, dyslipidemia and duration of diabetes mellitus were the independent predictors for diabetic nephropathy. Conclusion: The prevalence of diabetic nephropathy is high (40.6%) among diabetic adults. Several risk factors are significantly associated with diabetic nephropathy. Hypertension, dyslipidemia and duration of diabetes mellitus are the independent predictors for diabetic nephropathy.

Original Research Article Background: It is essential to uncover musculoskeletal symptom prevalen... more Original Research Article Background: It is essential to uncover musculoskeletal symptom prevalence and associated risk factors among the working population to establish effective prevention and intervention strategies. While studies worldwide report significant risk among patient care attendants (PCAs), evidence is lacking in the context of Bangladesh. Objectives: The study aimed to assess musculoskeletal symptom prevalence in the last 7 days and 12 months, and explore links to socio-demographic and physical risk factors. Methods: A cross-sectional study involving 50 participants selected via convenient sampling from BRB hospitals in Dhaka, conducted between July 2019 and December 2019. The Modified Dutch Musculoskeletal Questionnaire assessed musculoskeletal symptoms, their associations with socio-demographic factors, and physical risk factors among patient care attendants (PCAs). Result: The findings revealed significant musculoskeletal symptom prevalence among patient care attendants (PCAs). Neck pain was prominent at 44%, mean 1.660 (SD = ± 0.478), followed by lower back and ankle/foot at 36%, mean 1.640 (SD = ± 0.484), and knee at 26%, mean 1.740 (SD = ± 0.443). Wrist and shoulder reported 18%, mean 1.820 (SD = ± 0.388), while upper back and hip reported 8%, mean 1.920 (SD = ± 0.274). Elbow symptoms were at 4%, mean 1.960 (SD = ± 0.197). In the past 12 months, 34% reported neck pain, mean 1.660 (SD = ± 0.478), and 32% reported lower back symptoms, mean 1.680 (SD = ± 0.471). The most affected body part was the neck. Socio-demographic factors were significantly linked to musculoskeletal symptoms. Identified work-related physical risk factors included prolonged standing (90%) and slight trunk bending (54%). Frequent arm, hand, or finger movements constituted the second-highest risk at 70%. Conclusion: Work-related musculoskeletal disorders are a significant global concern, affecting patient care attendants. Effective ergonomic management is crucial to mitigate risks, necessitating further research on prevalence and jobrelated factors.

In pregnancy, obesity is associated with increased risk of maternal death and of significant complications. Several papers have also reported an increased risk of major anomalies in the offspring of obese pregnant women. At the same time, carrying out an ultrasound examination on an obese pregnant woman is a difficult task, due to the impaired acoustic window. This study was discussing the clinical and technical problems associated with ultrasound examination in obese and overweight women. The study was done among 601 pregnant women with BMI 25-30 and >30 with 424, mean age 29.5 years and mean gestation age was 30.7 weeks, positive clinical history in 353 (58.7%). Visibility of fetal anatomy in ultrasound good in 344 (57.2%), moderate in (38.3%) and poor in (4.5%), the study found that there was strong significant correlation between BMI and visibility of fetal ultrasound anatomy (p <0.01).

The frequency of rotator cuff diseases makes shoulder ultrasound (US) one of the most prevalent m... more The frequency of rotator cuff diseases makes shoulder ultrasound (US) one of the most prevalent musculoskeletal ultrasound (MSK US) applications. Sudanese children's supraspinatus tendon thickness and width were measured as part of this study to help with the early detection of supraspinatus tendon disease. Between August and December 2020, researchers conducted this cross-sectional investigation. There were 114 participants in the study, ranging in age from 4 to 14 years, with 81 males (71.1%) and 33 females (28.9%) participating. Study variables were incorporated into a customized data collection sheet used to gather the information. The supraspinatus tendons were found to be 5.31 ± 0.69 mm in thickness and 26.91 ± 2.06 mm in width. While the supraspinatus tendon thickness was correlated significantly (p < 0.01) to age (r = 0.280), weight (r = 0.254), and height (r = 0.335) in terms of their p-values, it was not correlated significantly to body mass index (BMI) (p > 0.05). A significant correlation (p < 0.01) was found between the supraspinatus tendon's width and age (r = 0.519), weight (r = 0.507), height (r = 0.489), and BMI (r = 0.309). Standard reference values for supraspinatus tendon measures were found in Sudanese children. US musculoskeletal assessments of the supraspinatus tendons are proposed as a method for detecting anomalies in these tendons.

Objective: To assess the spleen and lymph nodes patterns Ultrasonographically for AIDS patients. Materials & Methods: Ultrasonographic of the spleen and lymph nodes were performed on 200 patients with known AIDS with concerning on spleen's and lymph nodes echogenicity. The effect of age, gender, onset, spleen parenchymal disease and lymph adenopathy was statistically analyzed. Results: The data was discussed and presented in percentage and frequencies between patients age, sex, status, spleen and lymph nodes disease. Then statistically the results was analyzed. Conclusion: The study proved that AIDS were subject to multiple abnormalities that can be diagnosed by ultrasound, this supports the use of ultrasound in AIDS units. Recommendation: using ultrasound routinely in the treatment and diagnosis of AIDS units and follow up patients with AIDS, also researches could constitute a base for further research in this field.

The aim of the study was to measure the width and thickness of normal supraspinatus by ultrasound among Sudanese population. The study was conducted on a seventy-seven Sudanese population from both gender, 49 females and 28 males scanned by ultrasound machine with the participant sitting on a rotating stool and Place the participant's arm posteriorly, placing the palm side of the hand on the superior aspect of the iliac wing with the elbow flexed and directed posteriorly. There was a correlation between supraspinatus tendon thickness and volunteers body weight, height and age. The best correlation was between the supraspinatus tendon and volunteer's body weight followed by the supraspinatus tendon and volunteer's height and finally the correlation between the supraspinatus tendon and volunteer's age. The mean standard of supraspinatus tendon thickness was 4.8 ± 0.76mm, the mean standard of supraspinatus tendon width was 26.06 ± 3.11mm, and therefore we should take the supraspinatus tendon measurements in addition to the essential Rotator Cuff measurements.

The aim of the study was to measure the normal Acromio-clavicular Joint width sonographically among Sudanese population. The study was conducted on a seventy seven Sudanese population includes 63.6% of males and 36.4% of females of the sample, scanned by ultrasound machine with the participant sitting on a rotating stool and the forearm or hand resting in a supinated position on the thigh, palm up, Begin the examination by facing the participant then place the transducer in the coronal plane over the shoulder to examine the AC J.. There was a correlation between AC J space and volunteer's sex and age; where there was no significant correlation between AC J and the volunteer's body weight and height. The best correlation was between the ACJ and volunteer's age followed by the ACJ and volunteer's sex. The mean standard of AC J was 4.9, therefore we should take the AC J measurement in addition to the essential Rotator cuff measurements.

Ultrasound (US) of the pancreas is sensitive for evaluating the gland and provides the accurate diagnosis of pancreatic diseases. Aims: The aim of this study is to determine whether there were alterations in pancreatic size and echogenicity in type 1 diabetes mellitus (DM). Materials and Methods: A total of 100 patients with Type-1 DM (insulin dependent) were examined using the ultrasonography. They were compared to 22 healthy controls. Measurements of the pancreas were performed on type 1 diabetic patients and controls. Results: Pancreatic diameters of the head, body, and tail of the pancreas were reduced in Type 1 DM versus controls (1.78 vs. 1.80 cm, 95% confidence interval [CI]: −0.147 −0.099, P = 0.699 for the head of the pancreas; and 1.093 vs. 1.13 cm: 95% CI: −2.97-1.196, P = 0.484 for the body of the pancreas; 1.12 vs. 1.46 cm, 95% CI: −0.491-0.194, P < 0.001 for the tail of the pancreas). The echogenicity of the pancreas in patients with Type 1 DM increased significantly as the duration of the disease increased (P = 0.015). Conclusions: Individuals with Type 1 DM have reduced pancreatic size in comparison with controls. Patients with Type 1 DM have increased echogenicity of the pancreas as the duration of the disease increased.

While studies worldwide report significant risk among patient care attendants (PCAs), evidence is lacking in the context of Bangladesh. Objectives: The study aimed to assess musculoskeletal symptom prevalence in the last 7 days and 12 months, and explore links to socio-demographic and physical risk factors. Methods: A cross-sectional study involving 50 participants selected via convenient sampling from BRB hospitals in Dhaka, conducted between July 2019 and December 2019. The Modified Dutch Musculoskeletal Questionnaire assessed musculoskeletal symptoms, their associations with socio-demographic factors, and physical risk factors among patient care attendants (PCAs). Result: The findings revealed significant musculoskeletal symptom prevalence among patient care attendants (PCAs). Neck pain was prominent at 44%, mean 1.660 (SD = ± 0.478), followed by lower back and ankle/foot at 36%, mean 1.640 (SD = ± 0.484), and knee at 26%, mean 1.740 (SD = ± 0.443). Wrist and shoulder reported 18%, mean 1.820 (SD = ± 0.388), while upper back and hip reported 8%, mean 1.920 (SD = ± 0.274). Elbow symptoms were at 4%, mean 1.960 (SD = ± 0.197). In the past 12 months, 34% reported neck pain, mean 1.660 (SD = ± 0.478), and 32% reported lower back symptoms, mean 1.680 (SD = ± 0.471). The most affected body part was the neck. Socio-demographic factors were significantly linked to musculoskeletal symptoms. Identified work-related physical risk factors included prolonged standing (90%) and slight trunk bending (54%). Frequent arm, hand, or finger movements constituted the second-highest risk at 70%. Conclusion: Work-related musculoskeletal disorders are a significant global concern, affecting patient care attendants. Effective ergonomic management is crucial to mitigate risks, necessitating further research on prevalence and jobrelated factors.

In pregnancy, obesity is associated with increased risk of maternal death and of significant complications. Several papers have also reported an increased risk of major anomalies in the offspring of obese pregnant women. At the same time, carrying out an ultrasound examination on an obese pregnant woman is a difficult task, due to the impaired acoustic window. This study was discussing the clinical and technical problems associated with ultrasound examination in obese and overweight women. The study was done among 601 pregnant women with BMI 25-30 and >30 with 424, mean age 29.5 years and mean gestation age was 30.7 weeks, positive clinical history in 353 (58.7%). Visibility of fetal anatomy in ultrasound good in 344 (57.2%), moderate in (38.3%) and poor in (4.5%), the study found that there was strong significant correlation between BMI and visibility of fetal ultrasound anatomy (p <0.01).

Sudanese children's supraspinatus tendon thickness and width were measured as part of this study to help with the early detection of supraspinatus tendon disease. Between August and December 2020, researchers conducted this cross-sectional investigation. There were 114 participants in the study, ranging in age from 4 to 14 years, with 81 males (71.1%) and 33 females (28.9%) participating. Study variables were incorporated into a customized data collection sheet used to gather the information. The supraspinatus tendons were found to be 5.31 ± 0.69 mm in thickness and 26.91 ± 2.06 mm in width. While the supraspinatus tendon thickness was correlated significantly (p < 0.01) to age (r = 0.280), weight (r = 0.254), and height (r = 0.335) in terms of their p-values, it was not correlated significantly to body mass index (BMI) (p > 0.05). A significant correlation (p < 0.01) was found between the supraspinatus tendon's width and age (r = 0.519), weight (r = 0.507), height (r = 0.489), and BMI (r = 0.309). Standard reference values for supraspinatus tendon measures were found in Sudanese children. US musculoskeletal assessments of the supraspinatus tendons are proposed as a method for detecting anomalies in these tendons.

Objective: To assess the spleen and lymph nodes patterns Ultrasonographically for AIDS patients. Materials & Methods: Ultrasonographic of the spleen and lymph nodes were performed on 200 patients with known AIDS with concerning on spleen's and lymph nodes echogenicity. The effect of age, gender, onset, spleen parenchymal disease and lymph adenopathy was statistically analyzed. Results: The data was discussed and presented in percentage and frequencies between patients age, sex, status, spleen and lymph nodes disease. Then statistically the results was analyzed. Conclusion: The study proved that AIDS were subject to multiple abnormalities that can be diagnosed by ultrasound, this supports the use of ultrasound in AIDS units. Recommendation: using ultrasound routinely in the treatment and diagnosis of AIDS units and follow up patients with AIDS, also researches could constitute a base for further research in this field.

The aim of the study was to measure the width and thickness of normal supraspinatus by ultrasound among Sudanese population. The study was conducted on a seventy-seven Sudanese population from both gender, 49 females and 28 males scanned by ultrasound machine with the participant sitting on a rotating stool and Place the participant's arm posteriorly, placing the palm side of the hand on the superior aspect of the iliac wing with the elbow flexed and directed posteriorly. There was a correlation between supraspinatus tendon thickness and volunteers body weight, height and age. The best correlation was between the supraspinatus tendon and volunteer's body weight followed by the supraspinatus tendon and volunteer's height and finally the correlation between the supraspinatus tendon and volunteer's age. The mean standard of supraspinatus tendon thickness was 4.8 ± 0.76mm, the mean standard of supraspinatus tendon width was 26.06 ± 3.11mm, and therefore we should take the supraspinatus tendon measurements in addition to the essential Rotator Cuff measurements.

The aim of the study was to measure the normal Acromio-clavicular Joint width sonographically among Sudanese population. The study was conducted on a seventy seven Sudanese population includes 63.6% of males and 36.4% of females of the sample, scanned by ultrasound machine with the participant sitting on a rotating stool and the forearm or hand resting in a supinated position on the thigh, palm up, Begin the examination by facing the participant then place the transducer in the coronal plane over the shoulder to examine the AC J.. There was a correlation between AC J space and volunteer's sex and age; where there was no significant correlation between AC J and the volunteer's body weight and height. The best correlation was between the ACJ and volunteer's age followed by the ACJ and volunteer's sex. The mean standard of AC J was 4.9, therefore we should take the AC J measurement in addition to the essential Rotator cuff measurements.

Ultrasound (US) of the pancreas is sensitive for evaluating the gland and provides the accurate diagnosis of pancreatic diseases. Aims: The aim of this study is to determine whether there were alterations in pancreatic size and echogenicity in type 1 diabetes mellitus (DM). Materials and Methods: A total of 100 patients with Type-1 DM (insulin dependent) were examined using the ultrasonography. They were compared to 22 healthy controls. Measurements of the pancreas were performed on type 1 diabetic patients and controls. Results: Pancreatic diameters of the head, body, and tail of the pancreas were reduced in Type 1 DM versus controls (1.78 vs. 1.80 cm, 95% confidence interval [CI]: −0.147 −0.099, P = 0.699 for the head of the pancreas; and 1.093 vs. 1.13 cm: 95% CI: −2.97-1.196, P = 0.484 for the body of the pancreas; 1.12 vs. 1.46 cm, 95% CI: −0.491-0.194, P < 0.001 for the tail of the pancreas). The echogenicity of the pancreas in patients with Type 1 DM increased significantly as the duration of the disease increased (P = 0.015). Conclusions: Individuals with Type 1 DM have reduced pancreatic size in comparison with controls. Patients with Type 1 DM have increased echogenicity of the pancreas as the duration of the disease increased.
