Sarcoidosis, which is a chronic inflammatory granulomatous disease, can mimic different rheumatologic diseases including connective tissue diseases. Antinuclear antibodies are the markers used for connective tissue diseases. Aim. To determine antinuclear antibody frequency and any possible correlation with clinical and laboratory data in sarcoidosis patients. Material and Method. Forty-two sarcoidosis patients, 45 rheumatoid arthritis patients, and 45 healthy volunteers who were followed up in rheumatology outpatient clinic were included in this study. Demographic, clinical, serological, and radiological data of all patients were recorded. Antinuclear antibodies were determined with indirect immunofluorescent method and 1/100 titration was accepted as positive. The cases that were ANA positive were evaluated with immunoblot method. Results. Average age of the 42 patients (10 males) with sarcoidosis was 45.2 (20-70 years), and average disease duration was 3.5 years. ANA positivity was detected in 12 (28.5%) patients with sarcoidosis (1/100 in 10 patients, 1/320 in two patients), in 19 of RA patients (42.2%), and in two of healthy volunteers in low titer (< 0.001). In the subgroup analysis made by immunblot test, one patient had anticentromere antibody, one had anti-Ro antibody, one had anti-Scl-70 antibody, one had anti-dsDNA antibody, and eight patients were negative. The two patients who had anticentromere and anti-Scl-70 antibodies had also Sjögren's syndrome and scleroderma diagnosis, respectively. Discussion. The prevalence of ANA in patients with sarcoidosis was found to be significantly higher than healthy control group and lower than RA patients. This result shows that ANA may have an important role in the pathogenesis of sarcoidosis and also could be important in revealing the overlap syndromes of sarcoidosis-connective tissue diseases. Further studies with larger series are necessary in this subject.

Nucleotide binding and oligomerization domains/caspase recruitment domain-containing protein 15 (... more Nucleotide binding and oligomerization domains/caspase recruitment domain-containing protein 15 (NOD2/CARD15) is a cytoplasmic molecule controlling apoptosis and inflammatory processes by recognizing some microbial components. We aimed to identify the frequencies of NOD2/CARD15 gene mutations in patients with gouty arthritis and to determine their possible correlation with the disease phenotype. The study included 93 patients with gouty arthritis and 51 healthy controls matched for age, gender, and ethnicity. The NOD2/CARD15 R702W and G908R gene mutations were explored by the polymerase chain reaction restriction fragment length polymorphism method while the 3020insC mutation was analyzed by DNA sequencing. The mean patient age was 54.2 ± 14.2 years and mean duration of the disease was 3.1 ± 2.9 years. The first metatarsophalangeal and finger joint involvements were detected in 72 (77.4%) and 18 (19.5%) patients, respectively. Ankle arthritis and knee arthritis were detected in 43 (46.2%) and 20 (21.5%) patients, respectively. In total, 4 (9%) heterozygous mutations were detected in the G908R and R702W genes, while no mutation was detected in the 3020insC gene. Compared to the control group, there were no significant differences in all three DNA regions (G908R, R702W, and 3020insC; p = 0.452, p = 0.583, and p = 0.350, respectively). No correlation between the NOD2/CARD15 variants and clinical or laboratory findings (p > 0.05) was found. The frequencies of the NOD2/CARD15 gene mutations in the patients were similar to healthy control group. No association between clinical or laboratory findings and the NOD2/CARD15 gene mutations was observed.

Ocular involvement has been reported in 25-60% of the patients. Seven percent of the patients with sarcoidosis may first see an ophthalmologist due to ocular complaints. This report aims to present our diagnostic and treatment approach to a female patient with significant unilateral lacrimal gland swelling and musculoskeletal involvement, who was diagnosed with sarcoidosis on the basis of tests and histological studies.

A 26-year-old male patient presented to our rheumatology clinic with pain, swelling and limitatio... more A 26-year-old male patient presented to our rheumatology clinic with pain, swelling and limitation of movement in his right ankle, and also purpuric skin lesions in the lower extremity pretibial region. He was asked questions, and he said that he had been having chronic low back pain and morning stiffness for the last few years. His physical examination revealed that he had arthritis in his right ankle, purpuric skin lesions in pretibial regions of both legs, and bilateral FABERE/FADIR positivity. The sacroiliac joint imaging and MRI revealed bilateral sacroiliitis findings, and the lateral heel imaging revealed enthesitis. HLA-B27 was positive. Skin biopsy from lower skin lesions was reported to be consistent with leukocytoclastic vasculitis. Based on clinical, laboratory, radiological, and pathological examinations, the patient was diagnosed with ankylosing spondylitis and leukocytoclastic vasculitis. Administration of corticosteroid, salazopyrin, and nonsteroid anti-inflammatory medications was started. Notable clinical and laboratory regression was observed during his checks 3 months later.

The aim is to present our new method of compression, a compression tube instead of conventional c... more The aim is to present our new method of compression, a compression tube instead of conventional compression screw and to investigate the difference of proximal locking screw bending resistance between compression screw application (6 mm wide contact) and compression tube (two contact points with 13 mm gap) application. We formed six groups each consisting of 10 proximal locking screws. On metal cylinder representing lesser trochanter level, we performed 3-point bending tests with compression screw and with compression tube. We determined the yield points of the screws in 3-point bending tests using an axial compression testing machine. We determined the yield point of 5 mm screws as 1963±53 N (mean±SD) with compression screw, and as 2929±140 N with compression tubes. We found 51% more locking screw bending resistance with compression tube than with compression screw (p=0,000). Therefore compression tubes instead of compression screw must be preferred at femur compression nails.

Mutations of this gene are the cause of familial Mediterranean fever (FMF). A connection between MEFV gene mutations and rheumatic diseases has been suggested. The aim of this study was to explore the frequency and clinical significance of MEFV gene mutations in a cohort of Turkish patients with rheumatoid arthritis (RA). Methods: The study included 103 patients with RA and 103 age-, sex-and origin-matched healthy controls (HC). In all participants, genomic DNA was isolated and genotyped using amplification refractory mutation system or restriction fragment length polymorphism for the eight MEFV gene mutations (E148Q, M694V, M694I, M680I, V726A, A744S, R761H, and P369S). In the RA group, disease activity was determined using the disease activity score-28 (DAS-28), and radiological damage was evaluated by the modified Larsen scoring method. Results: Carrier rates of MEFV gene mutations were 26/103 (25.2%) and 24/103 (23.3%) in the RA and HC groups, respectively (p > 0.05, OR: 0.9, 95% CI: 0.48-1.71). In the RA group, while deformed joint count was significantly higher in the mutation carrier group than those of the non-carrier group (p < 0.05), the level of C-reactive protein, DAS-28 and modified-Larsen scores were slightly but not significantly higher in the carrier group. Conclusion: The results of this study suggest that MEFV gene mutations appear to be an aggravating factor for the severity of RA, and consequently, patients with RA might be screened for MEFV gene mutations in countries where FMF is frequent. Whether the searching of MEFV gene mutations in RA patients is cost-effective deserves further investigations.

Many radiological measures have been introduced to help radiologists and orthopaedic surgeons to evaluate surgeries, diagnose diseases and plan for surgical interventions. The main purpose of this study was to assess the validity of a novel radiological land marks and angles to help in Dynamic Hip screw (DHS) application surgery in the proximal femur. Method: 200 true hip anteroposterior radiographs were obtained for patients between ages 50 to 80 years. We have developed the lesser trochanteric fovea capitis angle (LTFCA) and the lesser trochanteric lateral cortex angle (LTLFCA). Patients were divided into two groups, the male group (n=100) and the female group (n=100). The LTFCA + LTLFCA were assessed in the left and right hips in both genders. Results: In male population, the average LTFCA + LTLFCA values for the right and left hip joints were (133.2°±5.27, 134.5°±4.92) respectively. There was no statistical difference between both sides, p= 0.74. Whereas in female population, the average LTFCA + LTLFCA values for the right and left hip joints were (134.8°±6.13, 135.6°±5.45) respectively. No statistical significance was detected between the two sides, p=0.37. Conclusions: The LTFCA is an easy method that can facilitate trustable radiological land marks for surgeons for proper DHS insertion. Further studies should be conducted to evaluate the sensitivity and specificity of the lesser trochanteric fovea capitis angle (LTFCA) and its validity in proximal femoral surgery. Hastalıklara tanı koymak, yapılacak cerrahiyi planlamak, yapılan ameliyatları değerlendirmek ve radyologlara ve ortopedistlere yardımcı olmak gayesi ile birçok radyolojik ölçüm yöntemi bildirildi. Proksimal femurda dinamik kalça vidası (DHS) cerrahi uygulamasına yardımcı yeni radyolojik işaret ve açıların geçerliliğini değerlendirmek bu çalışmanın ana amacıdır. Yöntem: Yaşları 50 ile 80 yaş arası hastalardan çekilen 200 true kalça AP radyografisi çalışmaya alındı. Küçük trochanter fovea kapitis açısını (LTFCA) ve küçük trochanter lateral korteks açısını (LTLFCA) geliştirdik. Erkekler (n=100) ve kadınlar (n=100) olarak hastalar iki guruba ayrıldı. LTFCA + LTLFCA her iki cinste sağ ve sol kalçada ayrı ayrı değerlendirildi. Bulgular: Erkek topluluğunda ortalama LTFCA + LTLFCA değerleri sağ kalçada 133.2°±5.27, sol kalçada 134.5°±4.92 idi ve her iki taraf arasında istatistiki fark bulunmadı (p= 0.74). Kadın topluluğunda ortalama LTFCA + LTLFCA değerleri sağ kalçada 134.8°±6.13, sol kalçada 135.6°±5.45 idi ve her iki taraf arasında istatistiki fark bulunmadı (p= 0.37). It rarely affects patients older than 65 years old.The purpose of this study is to compare and evaluate the demographic, clinical and laboratory features of elderly-onset psoriatic arthritis (EOPsA) and young-onset (YOPsA) patients.A total of 180 patients diagnosed with PsA according to CASPAR criteria and followed-up in single center were included in this study. The patients with initial symptoms started after age 65 were accepted as EOPsA. Demographic, clinic, and laboratory data and the medications which the patients received were recorded and retrospectively evaluated.Nineteen (10.5%) of 180 patients were diagnosed as EOPsA, and 161 (89.5%) patients were evaluated as YOPsA. Bu çalışmanın amacı tibia intramedüller çivileme sisteminde beş farklı proksimal kilitleme vidasının bükülme dirençlerini karşılaştırmaktır. Gereç ve Yöntemler: 50 adet vida üç farklı çapta(4.5 mm, 5 mm ,5.5 mm) ve 2 farklı yiv şeklinde(yivli ve yivsiz)olacak şekilde beş farklı gruba ayrıldı. 34 mm iç çapa sahip bir paslanmaz çelik tüp tibiayı temsil etmek üzere hazırlandı. Tüm vidalara uygulama sonrası üç nokta bükülme testi yapıldı. Bulgular: Yivsiz 4.5 mm ve yivli 5mm vida gruplarının ortalama değerleri yivsiz 5 mm, yivli 5.5 mm ve yivsiz 5.5 mm gruplarına gore anlamlı derecede düşük bulundu(p=0.000) Sonuç: Proksimal kilitleme vidasının deformasyonundan kaçınmak için yivsiz 4.5 mm ve yivli 5 mm vidaların kullanılması uygun olmayabilir. Yivsiz 5 mm, yivli 5.5 mm ve yivsiz 5.5 mm vidaların kullanılması daha güvenli olabilir.

Background: Many studies in the literature focused on the proximal geometry of the femur using ca... more Background: Many studies in the literature focused on the proximal geometry of the femur using cadaveric samples, X-rays or computed tomography, demonstrated substantial differences in the proximal femoral geometry in different patient groups. Many radiological parameters have been introduced in the literature to aid in diagnosis and surgical interventions. Objectives: We aimed to assess the diameter of the femoral head fovea capitis in both genders with different age groups and to record their variant configurations. Patients and Methods: Six-hundred true pelvis anteroposterior radiographs of patients were divided into three different age groups. Results: In group 1, the general average values of fovea capitis index (FCI) in both female and male genders were 25.35% ± 4.12% and 26.82 ± 4.81%, respectively (P = 0.08). The general average FCI value of both sides was 26.08% ± 4.46%. In group 2, the general average values of FCI values in both female and male genders were 26.69% ± 5.27% and 26.88% ± 4.77%, respectively (P = 0.43). The general average FCI value of both sides was 26.78% ± 4.93%. In group 3, the general average values of FCI value in both female and male genders were 28.86% ± 4.98% and 29.16% ± 3.82%, respectively (P = 0.27). The general average value of both sides was 28.93% ± 4.40%. Conclusion: Fovea capitis size increases with aging process. It has four different configurations. Further clinical studies should be conducted to correlate the different configurations with other femoral head pathologies and anatomical variations.

ANA positivity was detected in 12 (28.5%) patients with sarcoidosis (1/100 in 10 patients, 1/320 in two patients), in 19 of RA patients (42.2%), and in two of healthy volunteers in low titer (< 0.001). In the subgroup analysis made by immunblot test, one patient had anticentromere antibody, one had anti-Ro antibody, one had anti-Scl-70 antibody, one had anti-dsDNA antibody, and eight patients were negative. The two patients who had anticentromere and anti-Scl-70 antibodies had also Sjögren's syndrome and scleroderma diagnosis, respectively. Discussion. The prevalence of ANA in patients with sarcoidosis was found to be significantly higher than healthy control group and lower than RA patients. This result shows that ANA may have an important role in the pathogenesis of sarcoidosis and also could be important in revealing the overlap syndromes of sarcoidosis-connective tissue diseases. Further studies with larger series are necessary in this subject.

We aimed to identify the frequencies of NOD2/CARD15 gene mutations in patients with gouty arthritis and to determine their possible correlation with the disease phenotype. The study included 93 patients with gouty arthritis and 51 healthy controls matched for age, gender, and ethnicity. The NOD2/CARD15 R702W and G908R gene mutations were explored by the polymerase chain reaction restriction fragment length polymorphism method while the 3020insC mutation was analyzed by DNA sequencing. The mean patient age was 54.2 ± 14.2 years and mean duration of the disease was 3.1 ± 2.9 years. The first metatarsophalangeal and finger joint involvements were detected in 72 (77.4%) and 18 (19.5%) patients, respectively. Ankle arthritis and knee arthritis were detected in 43 (46.2%) and 20 (21.5%) patients, respectively. In total, 4 (9%) heterozygous mutations were detected in the G908R and R702W genes, while no mutation was detected in the 3020insC gene. Compared to the control group, there were no significant differences in all three DNA regions (G908R, R702W, and 3020insC; p = 0.452, p = 0.583, and p = 0.350, respectively). No correlation between the NOD2/CARD15 variants and clinical or laboratory findings (p > 0.05) was found. The frequencies of the NOD2/CARD15 gene mutations in the patients were similar to healthy control group. No association between clinical or laboratory findings and the NOD2/CARD15 gene mutations was observed.

He was asked questions, and he said that he had been having chronic low back pain and morning stiffness for the last few years. His physical examination revealed that he had arthritis in his right ankle, purpuric skin lesions in pretibial regions of both legs, and bilateral FABERE/FADIR positivity. The sacroiliac joint imaging and MRI revealed bilateral sacroiliitis findings, and the lateral heel imaging revealed enthesitis. HLA-B27 was positive. Skin biopsy from lower skin lesions was reported to be consistent with leukocytoclastic vasculitis. Based on clinical, laboratory, radiological, and pathological examinations, the patient was diagnosed with ankylosing spondylitis and leukocytoclastic vasculitis. Administration of corticosteroid, salazopyrin, and nonsteroid anti-inflammatory medications was started. Notable clinical and laboratory regression was observed during his checks 3 months later.

The aim is to present our new method of compression, a compression tube instead of conventional c... more The aim is to present our new method of compression, a compression tube instead of conventional compression screw and to investigate the difference of proximal locking screw bending resistance between compression screw application (6 mm wide contact) and compression tube (two contact points with 13 mm gap) application. We formed six groups each consisting of 10 proximal locking screws. On metal cylinder representing lesser trochanter level, we performed 3-point bending tests with compression screw and with compression tube. We determined the yield points of the screws in 3-point bending tests using an axial compression testing machine. We determined the yield point of 5 mm screws as 1963±53 N (mean±SD) with compression screw, and as 2929±140 N with compression tubes. We found 51% more locking screw bending resistance with compression tube than with compression screw (p=0,000). Therefore compression tubes instead of compression screw must be preferred at femur compression nails.

Mutations of this gene are the cause of familial Mediterranean fever (FMF). A connection between MEFV gene mutations and rheumatic diseases has been suggested. The aim of this study was to explore the frequency and clinical significance of MEFV gene mutations in a cohort of Turkish patients with rheumatoid arthritis (RA). Methods: The study included 103 patients with RA and 103 age-, sex-and origin-matched healthy controls (HC). In all participants, genomic DNA was isolated and genotyped using amplification refractory mutation system or restriction fragment length polymorphism for the eight MEFV gene mutations (E148Q, M694V, M694I, M680I, V726A, A744S, R761H, and P369S). In the RA group, disease activity was determined using the disease activity score-28 (DAS-28), and radiological damage was evaluated by the modified Larsen scoring method. Results: Carrier rates of MEFV gene mutations were 26/103 (25.2%) and 24/103 (23.3%) in the RA and HC groups, respectively (p > 0.05, OR: 0.9, 95% CI: 0.48-1.71). In the RA group, while deformed joint count was significantly higher in the mutation carrier group than those of the non-carrier group (p < 0.05), the level of C-reactive protein, DAS-28 and modified-Larsen scores were slightly but not significantly higher in the carrier group. Conclusion: The results of this study suggest that MEFV gene mutations appear to be an aggravating factor for the severity of RA, and consequently, patients with RA might be screened for MEFV gene mutations in countries where FMF is frequent. Whether the searching of MEFV gene mutations in RA patients is cost-effective deserves further investigations.

Many radiological measures have been introduced to help radiologists and orthopaedic surgeons to evaluate surgeries, diagnose diseases and plan for surgical interventions. The main purpose of this study was to assess the validity of a novel radiological land marks and angles to help in Dynamic Hip screw (DHS) application surgery in the proximal femur. Method: 200 true hip anteroposterior radiographs were obtained for patients between ages 50 to 80 years. We have developed the lesser trochanteric fovea capitis angle (LTFCA) and the lesser trochanteric lateral cortex angle (LTLFCA). Patients were divided into two groups, the male group (n=100) and the female group (n=100). The LTFCA + LTLFCA were assessed in the left and right hips in both genders. Results: In male population, the average LTFCA + LTLFCA values for the right and left hip joints were (133.2°±5.27, 134.5°±4.92) respectively. There was no statistical difference between both sides, p= 0.74. Whereas in female population, the average LTFCA + LTLFCA values for the right and left hip joints were (134.8°±6.13, 135.6°±5.45) respectively. No statistical significance was detected between the two sides, p=0.37. Conclusions: The LTFCA is an easy method that can facilitate trustable radiological land marks for surgeons for proper DHS insertion. Further studies should be conducted to evaluate the sensitivity and specificity of the lesser trochanteric fovea capitis angle (LTFCA) and its validity in proximal femoral surgery. Bu çalışmanın amacı tibia intramedüller çivileme sisteminde beş farklı proksimal kilitleme vidasının bükülme dirençlerini karşılaştırmaktır. Gereç ve Yöntemler: 50 adet vida üç farklı çapta(4.5 mm, 5 mm ,5.5 mm) ve 2 farklı yiv şeklinde(yivli ve yivsiz)olacak şekilde beş farklı gruba ayrıldı. 34 mm iç çapa sahip bir paslanmaz çelik tüp tibiayı temsil etmek üzere hazırlandı. Tüm vidalara uygulama sonrası üç nokta bükülme testi yapıldı. Bulgular: Yivsiz 4.5 mm ve yivli 5mm vida gruplarının ortalama değerleri yivsiz 5 mm, yivli 5.5 mm ve yivsiz 5.5 mm gruplarına gore anlamlı derecede düşük bulundu(p=0.000) Sonuç: Proksimal kilitleme vidasının deformasyonundan kaçınmak için yivsiz 4.5 mm ve yivli 5 mm vidaların kullanılması uygun olmayabilir. Yivsiz 5 mm, yivli 5.5 mm ve yivsiz 5.5 mm vidaların kullanılması daha güvenli olabilir.

Many radiological parameters have been introduced in the literature to aid in diagnosis and surgical interventions. Objectives: We aimed to assess the diameter of the femoral head fovea capitis in both genders with different age groups and to record their variant configurations. Patients and Methods: Six-hundred true pelvis anteroposterior radiographs of patients were divided into three different age groups. Results: In group 1, the general average values of fovea capitis index (FCI) in both female and male genders were 25.35% ± 4.12% and 26.82 ± 4.81%, respectively (P = 0.08). The general average FCI value of both sides was 26.08% ± 4.46%. In group 2, the general average values of FCI values in both female and male genders were 26.69% ± 5.27% and 26.88% ± 4.77%, respectively (P = 0.43). The general average FCI value of both sides was 26.78% ± 4.93%. In group 3, the general average values of FCI value in both female and male genders were 28.86% ± 4.98% and 29.16% ± 3.82%, respectively (P = 0.27). The general average value of both sides was 28.93% ± 4.40%. Conclusion: Fovea capitis size increases with aging process. It has four different configurations. Further clinical studies should be conducted to correlate the different configurations with other femoral head pathologies and anatomical variations.
