Amaç: Çalışmamızda, Gazi Üniversitesi Tıp Fakültesi Hastanesi Kök Hücre Nakil Birimi (KHNB)'nde, ... more Amaç: Çalışmamızda, Gazi Üniversitesi Tıp Fakültesi Hastanesi Kök Hücre Nakil Birimi (KHNB)'nde, eğitimi takiben klorheksidinli swap-stick ile pansuman ve klorheksidin emdirilmiş transparan örtü kullanımı gibi önlemlerin kan dolaşımı enfeksiyonlarının (KDE) insidansına olan etkisinin gösterilmesi amaçlanmıştır. Gereç ve Metod: Kateter ilişkili kan dolaşımı enfeksiyonları (KİKDE) sürveyansı enfeksiyon kontrol hemşireleri tarafından aktif prospektif olarak gerçekleştirilmiştir. 1 Mart 2011 ile 31 Aralık 2011 tarihleri birinci dönem olarak kabul edilmiştir. Birinci dönemde yüksek enfeksiyon oranları görülmesi üzerine personel eğitimi, klorheksidinli swap-stick ve klorheksidin emdirilmiş transparan örtü kullanımı gibi enfeksiyon kontrol önlemleri uygulamaya konulmuştur. Uygulama sonrası, 1 Ocak 2012 ile 31 Ekim 2012 tarihleri ise 2. dönem olarak kabul edilmiştir. İki dönemdeki KİKDE oranları ve insidans dansitesi ki-kare testi kullanılarak karşılaştırılmıştır. Mikroorganizmalar da ayrıca kayıt edilmiştir. Bulgular: Birinci dönemde 329 hastada toplam 51 KDE tanısı konmuştur. Bunların % 62.7'sinin kateter kaynaklı olduğu tespit edilmiştir. İkinci dönemde ise 371 hastada toplam 29 KDE tanısı konmuş, enfeksiyonların % 27.3'ü kateter ilişkili olarak bulunmuştur. Uygulama öncesi dönemde KDE için insidans dansitesi 1000 günde 17.24 iken sonraki dönemde 1000 günde 8.3 olarak bulunmuştur (p=0.001). Her iki dönemde de en sık izole edilen etken KNS'ler olmuştur. Üreyen mikroorganizmalar karşılaştırıldığında toplamda gram pozitif mikroorganizmalarda anlamlı azalma saptanmıştır (p=0.006). Sonuç: Çalışmamızın sonucunda, eğitim, klorheksidinli swap-stick uygulaması ve klorheksidin emdirilmiş transparan örtü kullanımı gibi enfeksiyon kontrol önlemlerinin KDE ve özellikle KİKDE insidansında belirgin azalma sağladığı tespit edilmiştir. Bu azalma, başlıca gram pozitif mikroorganizmalarla gelişen infeksiyonlardaki azalmaya bağlı olarak ortaya çıkmıştır.

Coagulation and fibrinolytic abnormalities are common in patients with acute myeloblastic leukaem... more Coagulation and fibrinolytic abnormalities are common in patients with acute myeloblastic leukaemia (AML) like other forms of leukaemias. In this study, we investigated if total plasminogen activator inhibitor (PAI) activity, which is believed to increase in initial diagnosis and relapse in AML patients could be accepted as a relapse criterion or not. Total of 34 AML patients and 18 healthy volunteers were included in this study. The patients' diagnosis were based on clinical criteria as well as morphological, cytochemical, immunuphenotypic examinations of peripheral blood and bone marrow specimens. Total PAI activity was measured with Dade Behring Bericrom PAI reagent in BCS system. Total PAI activity was higher than 3.5 U/ml in 11 AML patients while it was normal (0.3-3.5 U/ml) in control group (P < 0.01). There was no significant difference in total PAI activity between AML subgroups (P > 0.05). We found significant difference in total PAI activity between patients who have active disease and remission. In conclusion, the total PAI activity could be accepted as a relapse and an initial diagnosis criterion of AML patients during follow up.

These errors encompass a spectrum of events ranging from incorrect transfusion to administration of infected blood products. To overcome these complications, we developed a new blood management software system - the BT-Online system - to decrease erroneous transfusions that are mostly human based. Like previously developed transfusion safety systems, this program uses handheld devices to improve the safety of bedside practices. The BT-online system consists of 3 main structures: (1) the personal digital assistant, (2) the equipment and software (user interface), and (3) the relational database and a web service providing communication of a security-layered database with other personal digital assistants. Twenty three year old man with Hodgkin lymphoma had a resistant fever although treatment with bro... more Twenty three year old man with Hodgkin lymphoma had a resistant fever although treatment with broad spectrum antibiotics. Transthoracic echocardiography was performed to explain the origin of the fever and a huge mass was detected in the left atrium. Cardiac magnetic resonance imaging was performed and reported as intracardiac thrombus. Patient was referred to surgery. Culture of the surgical material favored to Aspergillus fumigates. Voriconazole treatment was started and patient recovered. The authors report overlapping two different clinical situations, intracardiac thrombus and fungal infective endocarditis.

Intravenous bisphosphonates-the potent inhibitors of osteoclast-mediated bone resorption are amon... more Intravenous bisphosphonates-the potent inhibitors of osteoclast-mediated bone resorption are among the most commonly prescribed drugs in the management of multiple myeloma (MM). Zoledronic acid (ZA) is a new generation potent intravenous bisphosphonate that has been approved for the treatment and prevention of bone lesions, and/or hypercalcemia associated with MM. Osteonecrosis of the jaw (ONJ) is an emerging serious side effect of the new generation bisphosphonates with a growing number of reports related to this pathological entity. ONJ usually appears following oral surgical and dental procedures but sometimes occur spontaneously. These cases are mostly seen and treated by dentists and oral surgeons. The aim of this study was to discuss the frequency, characteristics, risk factors, management and histopathological features of ZA induced ONJ based on the literature and illustrated with five own cases. Thirty-two patients with MM who received ZA for a median period of 26.5 ± 18.7 months (min: 5 months, max: 76 months) were evaluated. ONJ was detected in five patients and mean drug duration time was 34 months. The frequency was 15% and the patients were usually symptomatic. There was no significant difference in terms of the duration of ZA in patients with and without ONJ. Management of these established cases were performed with medical treatment, minor debridement, sequestrectomy, and combining bone resection with autologous platelet rich plasma. Our data indicate that ZA therapy has a major role in the development of ONJ a fact that should be considered by physicians treating MM patients.
