Case presentation: We are describing a case of 49-years old female diabetic patient who came to the rheumatology clinic of King Khalid Hospital in Al-Kharj with a two-year history rheumatoid arthritis and treated with methotrexate. In addition, she had a right knee arthroscopy and synovectomy for “pigmented villonodular synovitis”. Thereafter, the patient came in the outpatient clinic with a swollen knee. A systematic review of exercise guidelines was undertaken to synthesise themes and patterns. Methods: The review was completed according to the PRISMA statement. Guideline-specific databases were searched for worldwide clinical practice guidelines (CPGs). All included papers underwent quality assessment using the AGREE II protocol. The results are compared with the results of the heat exchanger without porous. Experiments were conducted on a double-core heat exchanger with dimensions of inner and outer diameter 20.7 and 22.2 mm and a length of 1.94 m of copper inner tube, inserted into the galvanized pipe with an inner diameter of 42.7 mm, wall thickness 2.15 mm, length 1.8 m, its outer surface insulated to prevent heat loss to the ambient. Novel sensor-based continuous biomedical monitoring technologies have a major role in chronic dis... more Novel sensor-based continuous biomedical monitoring technologies have a major role in chronic disease management for early detection and prevention of known adverse trends. In the future, a diversity of physiological, biochemical and mechanical sensing principles will be available through sensor device 'ecosystems'. In anticipation of these sensor-based ecosystems, we have developed Healthcare@Home (HH)-a research-phase generic intervention-outcome monitoring framework. HH incorporates a closedloop intervention effect analysis engine to evaluate the relevance of measured (sensor) input variables to system-defined outcomes. HH offers real-world sensor type validation by evaluating the degree to which sensor-derived variables are relevant to the predicted outcome. This 'index of relevance' is essential where clinical decision support applications depend on sensor inputs. HH can help determine system-integrated cost-utility ratios of bespoke sensor families within defined applications-taking into account critical factors like device robustness / reliability / reproducibility, mobility / interoperability, authentication / security and scalability / usability. Through examples of hardware / software technologies incorporated in the HH end-to-end monitoring system, this paper discusses aspects of novel sensor technology integration for outcome-based risk analysis in diabetes.

This syndrome can be complicated by multiple life-threatening complications like hepatitis, pancreatitis, macrophage activation syndrome and Acute Respiratory Distress syndrome. We report a 22-year-old Hispanic male with no remarkable past medical history who presented to the hospital with abdominal pain, myalgia, arthralgia, and persistent fever for two weeks and was found to have tachycardia, pancytopenia, transaminitis and an elevated lipase. The patient was initially evaluated for acute pancreatitis and hepatitis due to binge drinking and presumptive viral infection. Infectious work up was unrevealing, and the patient continued to deteriorate. This method was used for the preparation of thin films on the important semiconductors ІІ-VІ. The effect of deposition parameters on the structural, optical and electrical properties of the ZnO thin film was investigated. Zinc acetate dehydrate, 2- methoxethanol and monoethanolamine (MEA) were used as a starting material, solvent and stabilizer, respectively. XRD of the dried gel showed that weight loss continued until 300°C, with smallest particle size at 400 rpm spin coating speed.

The people of Hmong descent are one of the largest resettled communities in the United States (US... more The people of Hmong descent are one of the largest resettled communities in the United States (US). The Central Valley of California is well known to be the home to the largest Hmong population in the US. However, despite the presence of such a large Hmong community in the Central Valley, our knowledge of their cultural perceptions of medicine is limited. Based on local Central Valley health providers' experiences and observations, the Hmong people have a number of health-related challenges that differ from those of the general population, and this should be considered when dealing with their healthcare needs. In this report, we present a quick guide about the Hmong community and their health-related issues. We hope this will help clinicians and researchers better understand the Hmong community, which in turn would help provide a better quality of healthcare to the Hmong people and stimulate intellectual curiosity among healthcare providers towards this unique Asian ethnicity.

A 29-year-old African American woman with lupus presented with 2 days of malaise and right-sided ... more A 29-year-old African American woman with lupus presented with 2 days of malaise and right-sided weakness. Her symptoms started 4 days before admission with nausea, vomiting, diarrhea, generalized weakness, and recurrent falls. Two days later, she developed right-sided weakness and sensory impairment. Her home regimen for lupus includes prednisone 20 mg daily, methotrexate 15 mg weekly, and hydroxychloroquine 400 mg daily. Shewas febrile (39.3°C); otherwise, her vital signs were stable. Neurological examination showed reduced sensation, pronounced weakness, and diminished reflexes in the right upper and lower extremities. Twomonths earlier, the patient had finished a 7-day course of valacyclovir for herpes zoster. Laboratory tests revealed pancytopenia (hemoglobin [9.7 g/dL], white blood cells [800 cell/mL], and platelets [107,000 cell/L]), mild transaminitis (alanine aminotransferase 56 IU/L and aspartate aminotransferase 54 IU/L), hypertriglyceridemia (358 mg/dL), low fibrinogen level (173mg/dL), significant elevation of ferritin (10,824 ng/mL), high antinuclear antibody titer (1:2560 mg/dL), high antiphospholipid antibody (10 IU/mL), and decreased natural killer cell activity. Tests for interleukin 2 receptor (sCD25), anti–aquaporin-4 antibody, and cytogenetic analysis for hemophagocytic lymphohistiocytosis (HLH)– associated mutations showed negative results. Cerebrospinal fluid (CSF) analysis showed high white blood cells (16 cells/m with 66% lymphocytes and 5% neutrophils), high protein (54 mg/dL), high glucose (104 mg/dL), high immunoglobulin G (7.9 mg/dL), and normal albumin (17.1mg/dL). Blood and CSF cultures aswell as CSF viral panel and CSF cryptococcal antigen were all unremarkable. The smears from the CSF and bone marrow biopsy were consistent with hemophagocytosis (Figs. 1A, B and 2A, B). Ultrasound of the abdomen showed no hepatosplenomegaly; head magnetic resonance imaging and computed tomography with and without contrast were unrevealing. Magnetic resonance imaging of the spine (cervical, thoracic, and lumbar) showed extensive continuous signal abnormality involving both cervical and thoracic spine. Based on the findings above, a diagnosis of HLH was made, and the patient was treated with methylprednisolone 1 g daily for 7 days, intravenous immunoglobulin 2 g/kg daily divided over 5 days and continued home methotrexate

Genetic analysis was performed using the joint scaling test based on three and six parameter mode... more Genetic analysis was performed using the joint scaling test based on three and six parameter models. The results revealed that inheritance of the traits could not be explained by additive-dominance model. Additive and dominance gene effects were significant for most traits in both crosses due to different origins of the parents. The values of (dominance × dominance) were greater than (additive × additive) and (additive × dominance) of most studied traits in both crosses in addition to greater values of dominance gene effect than the additive in most traits. It was suggested that selection all of them should be effective in advanced segregating generations due to epistatic gene effects. High narrow and broad sense heritability (0.94) was obtained for flag leaf area in Mexipak × Acsad-8 cross, while high narrow sense heritability (0.86) and high broad sense heritability (0.98) were obtained for spike length and flag leaf area in Ipa-99 × Attla, so the early generation testing and selection might be recommended for these traits. Heterosis was found to be positive for most traits in both crosses.

In light of evidence of waning of vaccineinduced immunity to pertussis, this study aimed to evaluate the waning of immunity in vaccinated school-entry age children, as measured by susceptibility to infection at population level. For this purpose, IgG and IgA levels were measured as markers of recent (infection in the last 6 months) and non-recent infections (infection in the last 12 months). Material and Methods: This was a cross sectional study undertaken in Tripoli, in February 2015. Children of school-entry age (> 5 to ≤ 7 years) were recruited on convenience basis at vaccination centers. Sera were tested for antibodies to pertussis. Results: Samples from 791 children (421 males and 370 females, mean age 6.50 were tested). All of the participants had received 4 doses of Whole cell pertussis containing vaccine in the first 2 years of life. The prevalence of recent and non-recent pertussis infection were 4.8% and 2.5%, respectively. The proportion of children with undetectable level of IgG was 76.1. Conclusion: The findings of this study showed significant circulation of Bordotella pertussis among vaccinated children by school-entry age. The circulation of B. pertussis in this population may be an indirect sign of waned immunity, which is simply corroborated by the absence of detectable antibodies in 76.1% of the children.

To determine the frequency of symptoms of anxiety and depression among the young caregivers of fa... more To determine the frequency of symptoms of anxiety and depression among the young caregivers of family members with cancer and their correlation with role of gender, age and socio-economic status. A total of 87.8% of caregivers were between 11 and 16 years of age, with 94.6% reported having support from another caregiver. At least 95% of caregivers reported symptoms of anxiety with a higher predisposition among females. Around 73% of caregivers had low monthly incomes followed by (22.9%) middle and (4.1%) high monthly incomes. Care givers belonging to low income groups were more likely to report anxiety and depressive symptoms (70%). Young adults 17-18 years of age reported fewer symptoms of anxiety (10.9%) than their younger counterparts. Reported symptoms of anxiety and depression decreased when the number of care givers increased-2 (67.5%), 3 (16.2%), 4 (5.4%). Increased hospital stay was associated with increased frequency of symptoms, but not beyond 5 weeks.

Genetic selection for these traits is difficult because they are polygenic and result from multifactorial interactions. To study the genetic architecture of these traits, phenotypic characterization of whole body weight (WBW), muscle yield, fat content, shear force and whiteness were measured in ~500 fish representing 98 families from a growth-selected line. RNA-Seq was used to sequence the muscle transcriptome of different families exhibiting divergent phenotypes for each trait. We have identified 240 and 1,280 differentially expressed (DE) protein-coding genes and long noncoding RNAs (lncRNAs), respectively, in fish families exhibiting contrasting phenotypes. This method was used for the preparation of thin films on the important semiconductors ІІ-VІ. The effect of deposition parameters on the structural, optical and electrical properties of the ZnO thin film was investigated. Zinc acetate dehydrate, 2- methoxethanol and monoethanolamine (MEA) were used as a starting material, solvent and stabilizer, respectively. XRD of the dried gel showed that weight loss continued until 300°C, with smallest particle size at 400 rpm spin coating speed.

The Central Valley of California is well known to be the home to the largest Hmong population in the US. However, despite the presence of such a large Hmong community in the Central Valley, our knowledge of their cultural perceptions of medicine is limited. Based on local Central Valley health providers' experiences and observations, the Hmong people have a number of health-related challenges that differ from those of the general population, and this should be considered when dealing with their healthcare needs. In this report, we present a quick guide about the Hmong community and their health-related issues. We hope this will help clinicians and researchers better understand the Hmong community, which in turn would help provide a better quality of healthcare to the Hmong people and stimulate intellectual curiosity among healthcare providers towards this unique Asian ethnicity.

A 29-year-old African American woman with lupus presented with 2 days of malaise and right-sided ... more A 29-year-old African American woman with lupus presented with 2 days of malaise and right-sided weakness. Her symptoms started 4 days before admission with nausea, vomiting, diarrhea, generalized weakness, and recurrent falls. Two days later, she developed right-sided weakness and sensory impairment. Her home regimen for lupus includes prednisone 20 mg daily, methotrexate 15 mg weekly, and hydroxychloroquine 400 mg daily. Shewas febrile (39.3°C); otherwise, her vital signs were stable. Neurological examination showed reduced sensation, pronounced weakness, and diminished reflexes in the right upper and lower extremities. Twomonths earlier, the patient had finished a 7-day course of valacyclovir for herpes zoster. Laboratory tests revealed pancytopenia (hemoglobin [9.7 g/dL], white blood cells [800 cell/mL], and platelets [107,000 cell/L]), mild transaminitis (alanine aminotransferase 56 IU/L and aspartate aminotransferase 54 IU/L), hypertriglyceridemia (358 mg/dL), low fibrinogen level (173mg/dL), significant elevation of ferritin (10,824 ng/mL), high antinuclear antibody titer (1:2560 mg/dL), high antiphospholipid antibody (10 IU/mL), and decreased natural killer cell activity. Tests for interleukin 2 receptor (sCD25), anti–aquaporin-4 antibody, and cytogenetic analysis for hemophagocytic lymphohistiocytosis (HLH)– associated mutations showed negative results. Cerebrospinal fluid (CSF) analysis showed high white blood cells (16 cells/m with 66% lymphocytes and 5% neutrophils), high protein (54 mg/dL), high glucose (104 mg/dL), high immunoglobulin G (7.9 mg/dL), and normal albumin (17.1mg/dL). Blood and CSF cultures aswell as CSF viral panel and CSF cryptococcal antigen were all unremarkable. The smears from the CSF and bone marrow biopsy were consistent with hemophagocytosis (Figs. 1A, B and 2A, B). Ultrasound of the abdomen showed no hepatosplenomegaly; head magnetic resonance imaging and computed tomography with and without contrast were unrevealing. Magnetic resonance imaging of the spine (cervical, thoracic, and lumbar) showed extensive continuous signal abnormality involving both cervical and thoracic spine. Based on the findings above, a diagnosis of HLH was made, and the patient was treated with methylprednisolone 1 g daily for 7 days, intravenous immunoglobulin 2 g/kg daily divided over 5 days and continued home methotrexate

The results revealed that inheritance of the traits could not be explained by additive-dominance model. Additive and dominance gene effects were significant for most traits in both crosses due to different origins of the parents. The values of (dominance × dominance) were greater than (additive × additive) and (additive × dominance) of most studied traits in both crosses in addition to greater values of dominance gene effect than the additive in most traits. It was suggested that selection all of them should be effective in advanced segregating generations due to epistatic gene effects. High narrow and broad sense heritability (0.94) was obtained for flag leaf area in Mexipak × Acsad-8 cross, while high narrow sense heritability (0.86) and high broad sense heritability (0.98) were obtained for spike length and flag leaf area in Ipa-99 × Attla, so the early generation testing and selection might be recommended for these traits. Heterosis was found to be positive for most traits in both crosses.

In light of evidence of waning of vaccineinduced immunity to pertussis, this study aimed to evaluate the waning of immunity in vaccinated school-entry age children, as measured by susceptibility to infection at population level. For this purpose, IgG and IgA levels were measured as markers of recent (infection in the last 6 months) and non-recent infections (infection in the last 12 months). Material and Methods: This was a cross sectional study undertaken in Tripoli, in February 2015. Children of school-entry age (> 5 to ≤ 7 years) were recruited on convenience basis at vaccination centers. Sera were tested for antibodies to pertussis. Results: Samples from 791 children (421 males and 370 females, mean age 6.50 were tested). All of the participants had received 4 doses of Whole cell pertussis containing vaccine in the first 2 years of life. The prevalence of recent and non-recent pertussis infection were 4.8% and 2.5%, respectively. The proportion of children with undetectable level of IgG was 76.1. Conclusion: The findings of this study showed significant circulation of Bordotella pertussis among vaccinated children by school-entry age. The circulation of B. pertussis in this population may be an indirect sign of waned immunity, which is simply corroborated by the absence of detectable antibodies in 76.1% of the children.

To determine the frequency of symptoms of anxiety and depression among the young caregivers of fa... more To determine the frequency of symptoms of anxiety and depression among the young caregivers of family members with cancer and their correlation with role of gender, age and socio-economic status. A total of 87.8% of caregivers were between 11 and 16 years of age, with 94.6% reported having support from another caregiver. At least 95% of caregivers reported symptoms of anxiety with a higher predisposition among females. Around 73% of caregivers had low monthly incomes followed by (22.9%) middle and (4.1%) high monthly incomes. Care givers belonging to low income groups were more likely to report anxiety and depressive symptoms (70%). Young adults 17-18 years of age reported fewer symptoms of anxiety (10.9%) than their younger counterparts. Reported symptoms of anxiety and depression decreased when the number of care givers increased-2 (67.5%), 3 (16.2%), 4 (5.4%). Increased hospital stay was associated with increased frequency of symptoms, but not beyond 5 weeks.

Genetic selection for these traits is difficult because they are polygenic and result from multifactorial interactions. To study the genetic architecture of these traits, phenotypic characterization of whole body weight (WBW), muscle yield, fat content, shear force and whiteness were measured in ~500 fish representing 98 families from a growth-selected line. RNA-Seq was used to sequence the muscle transcriptome of different families exhibiting divergent phenotypes for each trait. We have identified 240 and 1,280 differentially expressed (DE) protein-coding genes and long noncoding RNAs (lncRNAs), respectively, in fish families exhibiting contrasting phenotypes.
