Clinicians may encounter a variety of skin conditions that present with vesiculobullous lesions i... more Clinicians may encounter a variety of skin conditions that present with vesiculobullous lesions in their everyday practice. Pemphigus vulgaris, pemphigus foliaceus, IgA pemphigus, and paraneoplastic pemphigus represent the spectrum of autoimmune bullous dermatoses of the pemphigus family. The pemphigus family of diseases is characterized by significant morbidity and mortality. Considering the risks associated with a delayed diagnosis or misdiagnosis and the potential for overlap in clinical features and treatment, evaluation for suspected pemphigus disease often requires thorough clinical assessment and laboratory testing. Diagnosis is focused on individual biopsies for histopathology and direct immunofluorescence. Additional laboratory methods used for diagnosis include indirect immunofluorescence and enzyme-linked immunosorbent assay. As the discipline has failed to live up to the promise now strategic management accounting has been presented as a messiah for the discipline of accounting. New promises have been made that while the traditional management accounting failed to make use of strategic thinking and other qualitative aspects of management the new discipline is likely to make accounting more relevant and important for managers. The empirical evidence on successful diffusion of strategic management accounting is still not overwhelming. It is therefore yet to be seen if strategic management accounting can live up to its promise in future or not.

Background: Negative symptoms of schizophrenia have been demonstrated to be due to decreased dopa... more Background: Negative symptoms of schizophrenia have been demonstrated to be due to decreased dopamine in the mesocortical pathways. Stimulant medications are a class of medications that can increase dopamine activity in the mesocortical pathway. Case Presentation: We present the case of a patient whose negative symptoms improved from a Positive and Negative Syndrome (PANSS) score of 39 to 11 on the negative symptoms subscale during a three-week trial of dopamine agonist augmentation of antipsychotic therapy. The score on the positive symptoms subscale on PANSS remained low with a two-point reduction at the end of the three-week period. Conclusion: The potential benefits of using stimulant medications in treating negative symptoms of schizophrenia are discussed.

Transfemoral transcatheter aortic valve replacement (TF-TAVR) has enjoyed a rapid diffusion in th... more Transfemoral transcatheter aortic valve replacement (TF-TAVR) has enjoyed a rapid diffusion in the US and Europe, but the procedure is resource intensive. We sought to identify differences in resource utilization between a US and European center to ascertain opportunities for improvement in the performance of TF-TAVR. All patients undergoing TF-TAVR using the Edwards SAPIEN valve series at the San Raffaele Hospital (Milan, Italy) and the Cleveland Clinic Foundation (Cleveland, OH) between January 2011 and December 2012 were included. Resource utilization was defined as preprocedural imaging, intraprocedural imaging, and personnel present during the procedure. A total of 158 patients from Cleveland and 113 from Milan were analyzed. Median logistic EuroScore (22.1 vs. 15.5, P = 0.013) and STS-PROM (8.9 vs. 5.0%, P < 0.001) were higher in Cleveland. Thirty-day outcomes were similar. Majority of procedures in Milan were conducted without a cardiac surgeon (79%) or an imaging specialist (99%) present during the case. There was greater reliance on endotracheal intubation (94.3 vs. 3.5%), general anesthesia (GA: 94.3 vs. 4.4%), and intraprocedural TEE (93 vs. 12.4%) in Cleveland, but shorter length of stay (mean: 6.42 vs. 11.16 days). We characterized resources used for patients undergoing TF-TAVR in Cleveland and Milan. Important differences included less reliance on GA and TEE and streamlining of procedural personnel in Milan, and reduced length-of-stay in Cleveland. Evolution of TF-TAVR to involve fewer operators must be balanced with the need for ready access to a comprehensive group of specialists in case of emergencies. © 2015 Wiley Periodicals, Inc.

Requirements at higher level of detail are gradually elaborated into requirements at lower level of detail as more and more information becomes available. This paper presents the design and results of an empirical study conducted to quantitatively measure this process of requirements elaboration in projects developed using the object-oriented paradigm. This study focuses on the post-design-stage requirements elaboration i.e. the elaboration of classes into source lines of code (SLOC). Each class is elaborated into attributes and operations and in turn those attributes and operations are implemented by SLOC. Requirements of over 100 professional projects of various types and domains are analyzed and the elaboration factors between different consecutive levels of requirements are obtained. ABSTRACT BACKGROUND This study aimed at investigating opportunities to manipulate development rhy... more ABSTRACT BACKGROUND This study aimed at investigating opportunities to manipulate development rhythms of barley (Hordeum vulgare L.) and to relate such rhythms to protein concentration, composition and breakdown during malting. The spring malting barley cultivar Tipple was grown in daylight chambers at various sowing dates, nitrogen application timings and temperature levels to govern the development rhythms.RESULTSExtensive options were shown to be available for manipulating the plant development rhythm of spring malting barley. Individually, sowing times, N application timings and temperature levels were negatively correlated with the development time. The combination of early sowing and 100% N application at sowing/low temperature during the grain maturation period (GMP) resulted in a long development time before and after anthesis. The combination of late sowing and 50% N at sowing/high temperature during GMP led to short development time before and after anthesis. The highest protein concentration and lowest percentage of sodium dodecyl sulfate-unextractable polymeric proteins in total polymeric proteins were found to be associated with short development time. Extensive degradation of proteins during malting was the result of short development time as compared to long development time.CONCLUSION The development rhythm of barley affects protein concentration, composition and breakdown and might thus not only influence the end-use quality of barley but also the next generation of plants. © 2013 Society of Chemical Industry

Background: Significant seroma rate are quoted in literature for ELD flap donor site. These rates... more Background: Significant seroma rate are quoted in literature for ELD flap donor site. These rates are considerably reduced with the use of quilting technique. Nevertheless the drains are still required to be left in flap donor site for number of days. Drains are potential source of infection, cause pain, and requires patient to be kept in hospital. Material & Method: Comparison of drainage and duration of drains in ELD donor site in 11 consecutive prospective patients in which flap donor site was closed using fibrin glue and quilting vs. 24 consecutive retrospective patients, in which only quilting was used. Result: Total amount of drainage in control group 645 mls (330-973) vs. Fibrin group 330 mls (210-430) (p ¼ 0.018). Number of days drain left in situ in control group 5 (4.25-6) vs. Fibrin group 4 (4-5) (p ¼ 0.022). Conclusion: Fibrin glue and quilting significantly reduce the amount of drainage and the numbers of days the drains are left in situ following ELD flap based breast reconstruction. Fibrin glue can potentially reduce the number of days drains are left in situ or reduce the drainage to such an extent that drains are no longer required.

It has received a warm welcome from all over the world and these banks operating on Islamic principles have been able to get a sizeable business not only in Islamic countries but in non-Islamic countries too. Despite exemplary advancements and achievements, there remains number of controversies over various underlying concepts and practices. This paper basically explores and highlights all those controversies and challenges which are in minds of different school of thoughts and are needed to be addressed and overcome if Islamic banking continues flourishing the way it is at present. The authors have also tried to suggest suitable remedies to overcome these challenges where appropriate.
