Many state policymakers are looking to apply the concepts of consumer-directed care to the Medica... more Many state policymakers are looking to apply the concepts of consumer-directed care to the Medicaid program, believing that a more engaged and at-risk consumer will use health care services more appropriately. This issue brief examines four critical success factors states should take into account when launching consumer-directed Medicaid reforms such as those proposed in Florida, South Carolina, and West Virginia. An executive summary and update was made to this report in August 2006.

Evidence from clinical trials and systematic reviews demonstrates the potential of long-chain Omega-3 supplementation for learning and behavior. However, findings are inconclusive and in need of robust replication studies since such work is lacking. Replication of the 2012 DOLAB 1 study findings that a dietary supplementation with the long-chain omega-3 docosahexaenoic acid (DHA) had beneficial effects on the reading, working memory, and behavior of healthy schoolchildren. Parallel group, fixed-dose, randomized (minimization, 30% random element), double-blind, placebo-controlled trial (RCT). Mainstream primary schools (n = 84) from five counties in the UK in 2012-2015. Background: Assessment of severe maternal morbidity using World Health Organization (WHO) 'near-m... more Background: Assessment of severe maternal morbidity using World Health Organization (WHO) 'near-miss' criteria is gaining in importance as a valuable tool in the assessment of maternity care of women. Identification of cases allows an understanding of aetiology of severe morbidity and factors contributing to poor maternal outcomes. Aim: The aim of this study is to determine the rate of maternal 'near miss' at Royal Darwin Hospital (RDH) and the utility of the WHO near-miss criteria as a tool for data collection in a regional Australian context. Methods and Materials: Cases of maternal 'near miss' and deaths were prospectively identified over a period of twelve months using the WHO criteria. Results: During the audit period, there were 2,080 live births at Royal Darwin Hospital (RDH): 10 women presented with a 'near miss' and there was one maternal death. The maternal mortality ratio for the hospital was 48/100,000 live births, the maternal 'near-miss' index ratio was 4.8/1,000 live births, and the combination of maternal deaths and near misses gave a severe maternal outcome (SMO) ratio of 5.3/1,000 live births. The main cause of obstetric 'near miss' was obstetric haemorrhage. Indigenous women and women from remote areas comprised a significant portion of 'near-miss' cases. Conclusion: The rates of maternal 'near miss' at RDH are consistent with other studies in the developed world. The WHO maternal 'near-miss' audit tool helps health professionals understand and anticipate severe maternal morbidities, with the aim of improving maternal and perinatal outcomes.

The report is presented in three parts. Part 1 describes the statewide effort to provide professional development rewards for existing and future education and training. This section describes the origins of the initiative in new federal funding sources to states, child care advocacy efforts, statewide demand for K-3 teachers, and the existence of an extensive early childhood training network. Legislative efforts are also described. Part 2 of the document presents information on how California counties have developed CARE-style plans of their own. Proponents of the laparoscopic approach to myomectomy propose that the advantages include shorter length of hospital stay and recovery time. Others suggest longer operative time, greater blood loss, increased risk of recurrence, risk of uterine rupture in future pregnancies, and potential dissemination of cells with use of morcellation. This review outlines techniques for performance of laparoscopic myomectomy and critically appraises the available evidence for operative data, short-term and long-term complications, and reproductive outcomes.

Single-stranded progeny DNA is not formed when host cells are infected at the restrictive tempera... more Single-stranded progeny DNA is not formed when host cells are infected at the restrictive temperature with a temperature-sensitlve coat protein mutant of ~X174. Once begun, the synthesis of single-stranaed DNA is interrupted when an infected culture is shifted from the permissive to the restrictive temperature. The synthesis of double-stranded "replicative form" DI~A is observed under restrictive conditions. Upon shifting an infected culture from restrictive to permissive conditions after 30 minutes of infection, single-stranded DNA synthesis starts almost immediately. In this instance, the initial singlestranded viral DNA is formed out of the pre-formed double-stranded replicative form DNA by degradation o£ the complementary strand. The attachment of coat protein to the viral strand and the release of the viral strand from the doublestranded precursor appear to take place simultaneously.
