In this study, a new surgical technique for correction of lid retraction secondary to Graves’ orbitopathy is described. Sixteen eyelids of patients older than 18 years old underwent surgical correction for moderate to severe lid retraction secondary to Graves’ orbitopathy. In this procedure, levator aponeurectomy was performed via a transconjunctival approach. Upper marginal reflex distance (MRD1) was measured before the surgery and at 1 week, 3 months, and 6 months after the surgery. MRD1 was reduced significantly from preoperatively (mean: 7.84 mm) to 1 week after the surgery (mean: 3.59 mm) (P < 0.001). Three and six months after surgery, mean MRD1 was 5.09 mm and 5.10 mm, respectively, showing that lid retraction was improved significantly (P < 0.001). Lateral levator aponeurectomy via the transconjunctival approach is a simple, scar-less, quick procedure that has opt...

Design This is a prospective study on 32 female patients with aged between 35 and 75 years and underwent standard blepharoplasty. Method Measurement of mid brow to hairline distance (MBHLD) and inter brow distance (IBD) were performed within 3 days before, 1 and 6 months after the blepharoplasty surgery. The surgeon and patients reported the extent of their satisfaction about the beneficial effect of upper lid blepharoplasty on decreasing forehead and glabellar lines. Results Compared to MBHLD at baseline time before the surgery, significant increase was observed in MDHLD after 1 and 6 months follow up in both eyes (p-values were less than 0.05). IBD reached the significant increase after 1 and 6 month(s) after upper lid blepharoplasty (p-values were less than 0.05). On the other hand, compared to mean of IB...

There are several surgical approaches to treat the narrowing of the palpebral fissure. The purpose of the present study was to evaluate the efficacy of unilateral medial rectus recession (MRR) muscle combined lateral rectus (LR) muscle marginal myotomy (MM) with unilateral MRR alone in the management of narrowing of the palpebral fissure of patients with Type 1 Duane's retraction syndrome (DRS). Materials and Methods: Twenty-eight patients with unilateral DRS Type 1 were randomly divided into two groups (14 eyes of 14 patients in each group). Age ≥5 years with DRS Type 1 with <20 prism diopters in primary position who were candidates for surgery were consecutively enrolled in this randomized controlled trial. Patients were divided into treatment groups to receive unilateral MR recession with simultaneous MM group...

Objective: To compare the results of internal browpexy (IBP) and temporal brow lift (TBL) in patients with dermatochalasis undergoing simultaneous upper blepharoplasty. Methods: This study was a single-center, parallel-group randomized controlled trial conducted on 32 female patients suitable for upper blepharoplasty. Patients were divided into two groups: the IBP group and the TBL group. The brow lift was measured using change in the distance between the ala nasi and lateral tail of the eyebrow as nasal ala to lateral brow (NALB) in millimeter (mm), and the vertical line between the lateral tail of eyebrow and horizontal line extending the lateral cantus as lateral brow plump line (LBPL) in mm before and after the surgery. The follow-up time was 6 months. Results: The mean AE SD age of patients was 55.93 AE 7.1 years and 53.94 AE 7.7 years in the TBL and IBP groups, respectively (P > .05). No significant change in mean LBPL at 6 months compared to baseline was observed in the TBL group (baseline: 15.7 AE 1.6 mm vs 6 month: 15.8 AE 1.3 mm; P = .602). In the IBP group, a significant increase in mean LBPL at 6 months compared to baseline was observed (baseline: 15.09 AE 2.13 mm vs 6 months: 17. 43 AE 2.68 mm; P < .001). Conclusions: Internal browpexy combined with blepharoplasty could be considered the better procedure in patients with upper eyelid dermatochalasis in terms of longlasting stability and lateral brow elevation.

In this prospective interventional paired-eye controlled study, a total of 100 eyes of 50 participants with myopia with or without astigmatism underwent thin-flap Femto-LASIK, using a Femto LDV femtosecond laser (Ziemer Ophthalmic Systems AG, Port, Switzerland). For each patient, one eye was randomly treated with tissue-saving algorithm (TS group) by the Technolas 217z100 excimer laser and the other eye was treated with optimized smart ablation profile (ASA group) by the MEL 80 excimer laser. Outcome measures were uncorrected distance visual acuities (UDVAs), corrected distance visual acuities (CDVAs), manifest refraction, higher-order aberrations (HOAs), contrast sensitivity, and patient satisfaction 12 months...

To report the efficacy, safety, predictability and complications of photorefractive keratectomy (... more To report the efficacy, safety, predictability and complications of photorefractive keratectomy (PRK) with intraoperative application of mitomycin-C (MMC). This historical cohort study was performed on 1,250 eyes of 625 patients who underwent PRK using the Technolas 217 excimer laser machine by a single surgeon with intraoperative use of MMC 0.02% up to 2 minutes, depending on depth of ablation. A complete ophthalmologic examination was performed which included refraction uncorrected visual acuity (UCVA), best-corrected visual acuity (BCVA) and slitlamp biomicroscopy. Outcomes were analyzed after one year of follow-up. The mean preoperative spherical equivalent refractive error was -4.85±2.27 (range, -2.50 to -13.5) diopters (D). Mean depth of ablation was 89±22 microns and mean time to reepithelialization was 4.5±1.7 days. At final follow-up, UCVA of 20/20 and 20/40 or more was achieved in 92.1% and 99.2% of eyes, respectively. One year postoperatively, 69.4% and 91% of eyes were w...

This randomized controlled trial included 1,250 eyes of 625 patients undergoing PRK for correction of myopia and myopic astigmatism. Each patient was randomly assigned to alcohol-assisted or mechanical epithelial removal. A total of 658 eyes underwent alcohol-assisted epithelial removal while the epithelium was removed mechanically in 592 eyes. Mean spherical equivalent was -4.37±2.3 D in the alcohol group and -3.8±1.3 D in the mechanical group (P = 0.78). There was no significant difference in postoperative pain between the study groups (P = 0.22). Uncorrected visual acuity ≥ 20/20 and ≥ 20/40 was achieved in 90.9% versus 93.4% (P = 0.08), and 98.9% versus 99.5% (P = 0.36) of eyes in the alcohol and mechanical groups, respectively. Final refractive error within 1D of emmetropia was achieved in 90% versus 92.2% of eyes in the alcohol a...

In this study we compare the effect of two phakic intraocular lenses, Artiflex versus Implantable Contact Lens (ICL). Methods: In a prospective clinical trial study, 40 eyes of 20 patients who came to ophthalmology clinic to correct their refractive error were examined. In 20 eyes artiflex was inserted randomly and in the other 20 eyes, ICL was inserted. After 6 months uncorrected visual acuity (UCVA), best corrected visual acuity (BCVA), contrast sensitivity, intraocular pressure (IOP), specular microscopy and anterior uveitis was assessed in both groups. Findings: In this study, there is no statistical difference between UCVA, BCVA, IOP, cataract incidence, contrast sensitivity, specular microscopy 6 months after surgery in both groups. 40% of eyes in Artiflex group developed anterior chamber reaction 6 months post-operatively but no patient in ICL group. So there is obvious difference in the rate of anterior chamber reaction between these two groups. Conclusion: These two lenses have similar outcomes except in the incidence rate of anterior chamber reaction that is greater in the artiflex group. So these two lenses are safe with predictable outcome in treating high myopia.
