Aim: To determine the normal carpal height ratio for the Trinidadian population. Design & method:... more Aim: To determine the normal carpal height ratio for the Trinidadian population. Design & method: A retrospective review of wrist radiographs performed during the period January 2022 to June 2022 at San Fernando General Hospital was conducted. A total of 200 radiographs were independently reviewed by two blinded assessors utilizing the PACS system. The carpal height ratio (CHR) and revised carpal height (RCH) ratio was calculated using two methods. A p-value < 0.05 was assumed to be significant. Results: The radiographs reviewed consisted of 94 female and 106 male patients with an age range of 19 to 86 years. The mean CHR was 0.473 and the revised CHR 1.469. The ratio for carpal height as well as the revised CHR was observed to vary amongst the different age groups studied. The CHR and revised CHR for the population was observed to vary based on gender with gender significantly
influencing the CHR value (p = 0.001). Conclusions: The CHR of the Trinidadian population as calculated by the data collected falls below the average CHR documented worldwide. However, the interpretation of this value is based on the baseline reference ratio for a population. Therefore, according to our average CHR for our population previous comparison of a CHR index to international figures would have led to erroneous interpretation of results. A decrease in CHR is indicative of carpal collapse. The comparison of CHR to the established normal CHR for the population is believed to allow for early detection of carpal collapse

Percepção dos Pacientes a Respeito da Utilidade do Ambulatório de Avaliação Pré-Anestésica em um ... more Percepção dos Pacientes a Respeito da Utilidade do Ambulatório de Avaliação Pré-Anestésica em um País Caribenho em Desenvolvimento. JUSTIFICATIVA E OBJETIVOS: A percepção dos pacientes a respeito da utilidade do Ambulatório de Avaliação Pré-anestésica e o fluxo de pacientes não foram extensamente estudados no mundo em desenvolvimento. O objetivo deste estudo foi analisar esse aspecto. MÉTODO: Foi realizada pesquisa aplicando-se questionário entre pacientes atendidos no Ambulatório de Avaliação Pré-anestésica de um hospital terciário de ensino em Trinidad. A título de comparação, esse questionário também foi distribuído aos pacientes atendidos no Ambulatório de Cirurgia Geral. Os parâmetros demográficos, incluindo idade, sexo, nível escolar e estado físico ASA foram anotados. Outros dados registrados incluíam o fluxo de pacientes e detalhes a respeito da equipe médica. RESULTADOS: Dos 220 pacientes atendidos no Ambulatório de Avaliação Pré-anestésica, 92,7% participaram do estudo. A validade do questionário foi apoiada pelo coeficiente alfa de Cronbach (0,67). O tempo médio para o encaminhamento do Ambulatório de Cirurgia para a Clínica Pré-anestésica foi de 50 dias; a espera média na clínica foi de 2,7 horas e da espera pela intervenção cirúrgica após a aceitação pelo ambulatório foi de 13 dias. As opiniões dos pacientes a respeito dos benefícios da clínica e tempo de espera não foram influenciadas pela idade nem pelo nível educacional. Na opinião dos pacientes, o atendimento no Ambulatório de Avaliação Pré-anestésica foi satisfatório, mas os tempos de espera foram considerados longos. CONCLUSÕES: Na opinião dos pacientes, a consulta no Ambulatório de Avaliação Pré-anestésica antes do procedimento cirúrgico foi útil e o atendimento foi considerado satisfatório.

Perilunate dislocations and perilunate fracture dislocations are rare injuries that occur as a re... more Perilunate dislocations and perilunate fracture dislocations are rare injuries that occur as a result of high energy trauma. We describe a case of a volar fracture dislocation of the proximal pole of the scaphoid with an associated scapholunate and lunotriquetral ligament disruption as well as a lunate fossa fracture of the distal radius. These injuries are serious injuries that require a high degree of clinical acumen and radiographic scrutiny to allow for prompt treatment in order to avoid the sequelae of long-term complications that can arise. This case serves as a reminder of the complexity of these injuries and their associated mechanics.

Introduction: Distal radius fractures are one of the commonest fractures seen worldwide with a st... more Introduction: Distal radius fractures are one of the commonest fractures seen worldwide with a steadily rising incidence. There are numerous treatment options available for their management with the most recent advancement being the use of volar locking plates. These locking plates despite advancement in their design and biomechanical construct have been shown to result in flexor tendon rupture and tenosynovitis in up to 15% of patients. Case Report: A 70-year-old female underwent open reduction and internal fixation with a 3.5 mm volar locking plate for the treatment of a right intra-articular distal radius fracture with volar displacement. Twenty-six (26) months after the initial procedure she represented with the inability to flex her thumb after reaching for an object one month prior. The patient subsequently underwent surgical exploration to address her FPL tendon rupture with reconstruction utilising ipsilateral palmaris longus graft with a Pulvertaft weave for both proximal and distal anastomoses. Conclusion: The reconstruction technique employed in this case yielded a good functional outcome for this patient. The pulvertaft weave as a proximal and distal anastomosis suture provides a good option for reconstruction once there is sufficient graft length to permit its use on both ends.

Design & Method: A cross-sectional survey of health care professionals within two public hospitals was conducted using a questionnaire distributed to individuals working in Cardiology, General Surgery, Internal medicine, Orthopaedics, Radiology and Urology who require the use of ionizing radiation machines. The questionnaire comprised of 30 questions divided into five subcategories including demographic data, usage of the ionizing radiation machines, basic knowledge, and attitude towards personal protective equipment as well as radiation exposure measurement. Questionnaire results were tabulated and analysed using Microsoft Excel and Stata v11 with comparative statistical analysis being done using the one-way analysis of variance test. Introduction: The term "Lisfranc injury" commonly describes injuries which occur at the bases of ... more Introduction: The term "Lisfranc injury" commonly describes injuries which occur at the bases of the five metatarsals, their articulations with the distal four tarsal bones, and the Lisfranc ligament. The injury is rare and accounts for 0.2% of all fractures with up to 20% of injuries being missed in the acute setting. The treatment of these injuries has been the subject of much debate over the years. Primary arthrodesis has increased as a treatment option for acute injuries in the recent literature. Case Report: A 25-year-old female with Type 1 diabetes mellitus presented with a two-week history of a swollen right foot after falling while descending a flight of stairs. Radiographic imaging revealed a partial incongruity of her right Lis franc joint with a comminuted fracture of the second tarsometatarsal joint. She underwent a primary arthrodesis of the Lis Franc joint with no functional impairment or midfoot deformity progression at one year follow up. Conclusion: This construct has shown to better withstand the abnormal biomechanical forces which are placed on the midfoot during ambulation in patients with peripheral neuropathy such as diabetics. The case presented highlights the benefits of this treatment modality in the young, diabetic patient.

Design & Method: A cross-sectional survey of health care professionals within two public hospitals was conducted using a questionnaire distributed to individuals working in Cardiology, General Surgery, Internal medicine, Orthopaedics, Radiology and Urology who require the use of ionizing radiation machines. The questionnaire comprised of 30 questions divided into five subcategories including demographic data, usage of the ionizing radiation machines, basic knowledge, and attitude towards personal protective equipment as well as radiation exposure measurement. Questionnaire results were tabulated and analysed using Microsoft Excel and Stata v11 with comparative statistical analysis being done using the one-way analysis of variance test. Results: A total of 118 health care professionals participated in this study. The majority (85/118 (72%)) of individuals revealed that they had no formal training regarding safe practices when working with ionizing radiation despite the daily use of the fluoroscopy machines by at least 25% of participants. Individuals who had formal training in the use of ionizing radiation were found to score significantly higher than those without training (p < 0.0001). Participants' knowledge regarding the proper positioning of the C-arm image intensifier to reduce radiation exposure was low with 46% of individuals providing a correct response. Conclusions: The level of knowledge among the individuals who participate in the operation of ionizing radiation equipment throughout the country is low. The benefit of training in the use of the C-arm image intensifier is highlighted by the revelation that the individuals who had formal training in the use of these machines performed better than those individuals without training. Annual recertification courses should be implemented such that individuals are kept abreast with current changes and reminded of commonly neglected safety practices.

We describe a case of a volar fracture dislocation of the proximal pole of the scaphoid with an associated scapholunate and lunotriquetral ligament disruption as well as a lunate fossa fracture of the distal radius. These injuries are serious injuries that require a high degree of clinical acumen and radiographic scrutiny to allow for prompt treatment in order to avoid the sequelae of long-term complications that can arise. This case serves as a reminder of the complexity of these injuries and their associated mechanics.

Introduction: Distal radius fractures are one of the commonest fractures seen worldwide with a st... more Introduction: Distal radius fractures are one of the commonest fractures seen worldwide with a steadily rising incidence. There are numerous treatment options available for their management with the most recent advancement being the use of volar locking plates. These locking plates despite advancement in their design and biomechanical construct have been shown to result in flexor tendon rupture and tenosynovitis in up to 15% of patients. Case Report: A 70-year-old female underwent open reduction and internal fixation with a 3.5 mm volar locking plate for the treatment of a right intra-articular distal radius fracture with volar displacement. Twenty-six (26) months after the initial procedure she represented with the inability to flex her thumb after reaching for an object one month prior. The patient subsequently underwent surgical exploration to address her FPL tendon rupture with reconstruction utilising ipsilateral palmaris longus graft with a Pulvertaft weave for both proximal and distal anastomoses. Conclusion: The reconstruction technique employed in this case yielded a good functional outcome for this patient. The pulvertaft weave as a proximal and distal anastomosis suture provides a good option for reconstruction once there is sufficient graft length to permit its use on both ends.

JUSTIFICATIVA E OBJETIVOS: A percepção dos pacientes a respeito da utilidade do Ambulatório de Avaliação Pré-anestésica e o fluxo de pacientes não foram extensamente estudados no mundo em desenvolvimento. O objetivo deste estudo foi analisar esse aspecto. MÉTODO: Foi realizada pesquisa aplicando-se questionário entre pacientes atendidos no Ambulatório de Avaliação Pré-anestésica de um hospital terciário de ensino em Trinidad. A título de comparação, esse questionário também foi distribuído aos pacientes atendidos no Ambulatório de Cirurgia Geral. Os parâmetros demográficos, incluindo idade, sexo, nível escolar e estado físico ASA foram anotados. Outros dados registrados incluíam o fluxo de pacientes e detalhes a respeito da equipe médica. RESULTADOS: Dos 220 pacientes atendidos no Ambulatório de Avaliação Pré-anestésica, 92,7% participaram do estudo. A validade do questionário foi apoiada pelo coeficiente alfa de Cronbach (0,67). O tempo médio para o encaminhamento do Ambulatório de Cirurgia para a Clínica Pré-anestésica foi de 50 dias; a espera média na clínica foi de 2,7 horas e da espera pela intervenção cirúrgica após a aceitação pelo ambulatório foi de 13 dias. As opiniões dos pacientes a respeito dos benefícios da clínica e tempo de espera não foram influenciadas pela idade nem pelo nível educacional. Na opinião dos pacientes, o atendimento no Ambulatório de Avaliação Pré-anestésica foi satisfatório, mas os tempos de espera foram considerados longos. Introduction: The term "Lisfranc injury" commonly describes injuries which occur at the bases of ... more Introduction: The term "Lisfranc injury" commonly describes injuries which occur at the bases of the five metatarsals, their articulations with the distal four tarsal bones, and the Lisfranc ligament. The injury is rare and accounts for 0.2% of all fractures with up to 20% of injuries being missed in the acute setting. The treatment of these injuries has been the subject of much debate over the years. Primary arthrodesis has increased as a treatment option for acute injuries in the recent literature. Case Report: A 25-year-old female with Type 1 diabetes mellitus presented with a two-week history of a swollen right foot after falling while descending a flight of stairs. Radiographic imaging revealed a partial incongruity of her right Lis franc joint with a comminuted fracture of the second tarsometatarsal joint. She underwent a primary arthrodesis of the Lis Franc joint with no functional impairment or midfoot deformity progression at one year follow up. Conclusion: This construct has shown to better withstand the abnormal biomechanical forces which are placed on the midfoot during ambulation in patients with peripheral neuropathy such as diabetics. The case presented highlights the benefits of this treatment modality in the young, diabetic patient.

Results: A total of 118 health care professionals participated in this study. The majority (85/118 (72%)) of individuals revealed that they had no formal training regarding safe practices when working with ionizing radiation despite the daily use of the fluoroscopy machines by at least 25% of participants. Individuals who had formal training in the use of ionizing radiation were found to score significantly higher than those without training (p < 0.0001). Participants' knowledge regarding the proper positioning of the C-arm image intensifier to reduce radiation exposure was low with 46% of individuals providing a correct response. Conclusions: The level of knowledge among the individuals who participate in the operation of ionizing radiation equipment throughout the country is low. The benefit of training in the use of the C-arm image intensifier is highlighted by the revelation that the individuals who had formal training in the use of these machines performed better than those individuals without training. Annual recertification courses should be implemented such that individuals are kept abreast with current changes and reminded of commonly neglected safety practices.
