Purpose: To evaluate fungal sinusitis on computed tomography and its correlation with endoscopy a... more Purpose: To evaluate fungal sinusitis on computed tomography and its correlation with endoscopy and histopathology findings.Methodology: A comparative cross sectional analytical study was conducted at a tertiary care in Lahore. During four months of research period 138 patients were observed. CT scan was performed routinely on 128 slice CT machine by Siemens Germany Ltd having slice thickness of 3 mm, reconstructions at 0.75 mm in coronal and sagittal planes. Scanning parameters included 190 mA , 120 KV and tube rotation time of 0.5 s. Preoperative evaluation by CT was mandatory for all patients undergoing functional endoscopic sinus surgery (FESS).The findings of endoscopy and histopathology were also collected in all those cases when the investigation was done. Symptoms of Obsessive Compulsive Disorder (OCD) not only affect the individual, but also impact the individual’s environment. Family members are easily drawn into compulsive rituals or avoidance behaviors in order to prevent or reduce the distress related to the illness. Such behaviors of family members may serve the same function as compulsions which are contradictory to the principles of exposure-based interventions used in the treatment of OCD. Given this bidirectional relationship between family reactions and symptomatology in the affected person, family context plays an important role in the maintenance and treatment of OCD. Thus, it is imperative to understand the role family variables that can have an effect on the course/outcome of illness, which might also help in management of this complex psychosocial condition. We designed a study to determine the frequency and prognostic utility of TILs and MMR in advanced unresectable non-colorectal GI (NCGI) cancers. Methods: This is a retrospective cohort study of patients who were diagnosed with metastatic or unresectable NCGI cancers between 2009 and 2015 at St. John Hospital and Medical Center. Immunohistochemistry panels were performed on representative tissue sections for MMR testing. TILs were assessed on the hematoxylin and eosin stained slide of the same tissue section. Objectives Due to inadequate sampling concerns, pathologists frequently request additional tissue... more Objectives Due to inadequate sampling concerns, pathologists frequently request additional tissue sections (ie, more tissue sections in addition to initial representative sections) in breast specimens. This study explores the effectiveness of the additional tissue sections in the breast specimens. Methods Breast specimens received in our institution on which additional tissue sections were requested were included in the study. Pathology reports and glass slides for these specimens were reviewed. Additional tissue sections were considered effective if they changed the final diagnosis altogether, changed a benign entity to borderline or malignant category, changed pathologic tumor stage, changed margin status, changed tumor focality, or if lymphovascular invasion was identified. Results A total of 325 specimens were identified. Of these, 98 (30.1%) were benign while 224 (68.9%) were malignant. Among malignant cases, 53 (23.7%) had neoadjuvant therapy. Sinus histiocytosis with massive lymphadenopathy, generally known by the name of Rosai-Dorfman di... more Sinus histiocytosis with massive lymphadenopathy, generally known by the name of Rosai-Dorfman disease is a rare benign condition principally affecting cervical lymph nodes. Concurrent extra-nodal disease frequently occurs, however, solitary extra-nodal disease involving the mandible is exceedingly rare with less than five reported cases in the English literature. We describe a case of primary involvement of the mandible in a 27-year-old female, and discuss the differential diagnosis of this disease with other histiocytic lesions.

The number of involved lymph nodes is directly related to decreased 5-year overall survival for all pT stages according to United States Surveillance, Epidemiology, and End Results (SEER) cancer registry database. The National Quality Forum specifies that the presence of at least 12 lymph nodes in a surgical resection is one of the key quality measures for the evaluation of colorectal cancer. Therefore, the harvesting of a minimum of twelve lymph nodes is the most widely accepted standard for evaluating colorectal cancer. Since this is an accepted quality standard, a second attempt at lymph node dissection in the gross specimen is often performed when the initial lymph node count is less than 12, incurring a delay in reporting and additional expense. Advancement in genetic and molecular biology techniques has greatly helped our understanding of v... more Advancement in genetic and molecular biology techniques has greatly helped our understanding of various diseases, especially hematological disorders. We describe a case of primary myelofibrosis (PMF) that transformed into acute myeloid leukemia with a very rare and unusual genetic translocation of (1;21). There are only five reported cases of this translocation in acute myeloid leukemia (AML) or myelodysplastic syndrome but none of them transformed from PMF. This case not only highlights the importance of rare genetic translocations but also provides the natural history of the disease and its poor prognosis. To the best of our knowledge our patient is the first reported case of AML transformed from PMF to have this unique translocation of (1;21).

This study assessed frequency, diagnostic yield, distribution, and cost of AS. Among 1542 AS cases, we calculated mean AS per case; fraction of AS that altered diagnosis or stage; AS variation by tissue, malignant versus benign lesions, presence or absence of neoadjuvant therapy, mass, margin, lymph nodes, or other source, resident versus pathologist assistant (PA) dissector; and AS cost per case. Overall 9.2 ± 8.8 AS were collected per case. In only 3.8% (58/1542) of cases AS altered diagnosis or stage. Urinary bladder cases provoked the most AS: 19.5 ± 15.1 per case. Significantly more AS came from malignant versus benign lesions (10.8 ± 9.7 vs 7.6 ± 7.5, P = <.0001) and from specimens treated with neoadjuvant therapy versus malignant lesions not so treated (12.3 ± 9.4 vs 10.3 ± 9.8, P = .02). Background: Occupational stress and associated physical and mental health related issues are not ... more Background: Occupational stress and associated physical and mental health related issues are not addressed in Indian police personnel with adequate importance. Methods: Cross-sectional survey was conducted among police personnel (both male and female) in Calicut urban police district, Kerala state, India. Police personnel from all designations (ranks), except from the all India services (Indian Police Service) were included in the study. Data were collected using a specifically designed datasheet covering socio-demographic profile, physical and mental health related details which was prepared by researchers. Occupational stress was measured using Operational Police Stress Questionnaire (PSQ-OP) and Organisational Police Stress Questionnaire (PSQ-ORG). Result: The study found that both operational and organisational stress was significant among the police officers. Organisational stress was experienced in moderate level by 68% and in high level by 14%. Operational stress scores were in the moderate range in 67% and in high range in 16.5%. The younger age group (21-35 years) and lower level rank police personnel had higher stress. Stress was higher among female police personnel compared to males. While 23% of them had been diagnosed with physical illnesses, a significant four per cent of them with mental illness, and 29% of them reported substance abuse. Conclusion: The results point to the high level of stress among Indian police personnel and the need for urgent interventions from the government to address the occupational stress.
