Chordomas are rare tumors of dysembryoplastic origin that differentiate from remnants of the noto... more Chordomas are rare tumors of dysembryoplastic origin that differentiate from remnants of the notochord, an embryonic structure that typically disappears before birth. These tumors particularly localize to the sacrum and coccyx. The clinical evolution is slow, explaining the late diagnosis. Both computed tomography and magnetic resonance imaging are useful for diagnosis and staging. There are several anatomopathological forms. Treatment relies on surgery, with radiotherapy potentially being associated, particularly in infiltrating and inoperable forms. Recurrences can occur following optimal surgical treatment. Prognosis varies and depends on several factors.

This scientific report presents a retrospective analysis of 31 cases undergoing Transjugular Intr... more This scientific report presents a retrospective analysis of 31 cases undergoing Transjugular Intrahepatic Portosystemic Shunt (TIPSS) placement. The study, conducted at a single center, aimed to evaluate the efficacy and safety of TIPSS in managing complications of portal hypertension. The main indications for TIPSS placement included variceal bleeding, refractory ascites, and Budd-Chiari syndrome. The procedural success rate was high, with only a small number of cases requiring re-intervention due to shunt occlusion. Significant improvements were observed in controlling ascites and variceal bleeding, although complications such as hepatic encephalopathy were noted. Despite these challenges, TIPSS emerged as a valuable intervention for patients with advanced liver disease and its associated complications. This study underscores the importance of TIPSS in the management of portal hypertension and highlights the need for further research to optimize patient selection and refine procedural techniques.

Pelvic congestion syndrome (PCS) is a chronic condition characterized by chronic pelvic pain, oft... more Pelvic congestion syndrome (PCS) is a chronic condition characterized by chronic pelvic pain, often associated with pelvic varicose veins. Endovascular embolization has emerged as a minimally invasive treatment option for PCS, but there is limited data on its long-term efficacy and safety. In this retrospective study, we analyze the outcomes of embolization procedures performed for PCS at our institution over a specified period. Patient selection, procedural techniques, and clinical outcomes are evaluated, with a focus on pain relief, quality of life improvements, and incidence of complications. Our findings provide valuable insights into the role of embolization in the management of PCS and contribute to the existing literature on this topic.

A case of technetium-99m MDP uptake in hepatic metastases from medullary carcinoma of the thyroid is presented and the literature is reviewed.

The incidence of diabetes mellitus was significantly higher in the fem-pop group and appears to significantly affect graft patency and the need for another intervention, as 83% of patients with graft stenosis needing angioplasty were diabetic. Diabetes mellitus also seem to affect stent patency as 40% of patients who needed repeat angioplasty in the long SFA stent group had diabetes, however more significantly in the fem-pop population. There appears to be no obvious correlation between the type of blood thinner used and the maintenance of patency in the group with long SFA stents. Overall, 50% of patients with long SFA stents needed a second intervention to maintain primary patency while only 30% of patients who had fem-pop bypass needed another intervention to maintain patency of the conduit. It also apparent that majority of SFA occlusions in this audit occurred within the first 6 months of deployment.

Introduction Le Sarcome d’Ewing ou Tumeur neuroectodermique primitive (PNET) est une tumeur malig... more Introduction Le Sarcome d’Ewing ou Tumeur neuroectodermique primitive (PNET) est une tumeur maligne a petites cellules rondes bleues d’origine neuroectodermique presumee, qui touche les os et les tissus mous chez les enfants et les jeunes adultes. Les SE/PNET dans les organes parenchymateux sont rares, et ceux qui apparaissent dans la glande surrenale sont exceptionnels : moins d’une trentaine de cas ont ete rapportes. Cas clinique Nous presentons l’observation d’une patiente âgee de 48 ans qui s’est presentee avec une douleur constante flanc droit, l’echographie a revele une masse surrenalienne droite de 99 × 75 × 97 mm. La tomodensitometrie a montre une masse heterogene par la presence de plages de necrose, presentant une extension a la veine cave inferieure retro-hepatique sur 52 mm de hauteur, avec des limites du bourgeon se projetant a l’abouchement des veines sus hepatique, associes a de multiples adenopathies inter-aortico-caves. L’exploration hormonale est revenue en faveur d’une masse non secretante. La TDM thoracique a objective une localisation pulmonaire de 52 × 32 mm siege de multiples calcifications. La patiente a ete jugee inoperable par l’equipe de chirurgie. Nous avons donc complete l’exploration par une biopsie scanoguidee qui est revenue en faveur d’une tumeur neuro-ectodermique primitive, fortement positive pour le CD99 a l’immunohistochimie. Le patiente est en cours de traitement recevant des cycles de chimiotherapie avec Vincristine, doxorubicin, et cyclophosphamide toutes les 2 semaines. Conclusion Le SE/PNET surrenalien est une tumeur rare avec un mauvais pronostic. Elle doit etre consideree dans le diagnostic differentiel des tumeurs surrenaliennes non-secretantes.

Reninoma (juxtaglomerular cell tumor) is a rare cause of renin-mediated hypertension. We reported... more Reninoma (juxtaglomerular cell tumor) is a rare cause of renin-mediated hypertension. We reported a 18 year old woman with history of hypertension for 3 years. Laboratory findings showed severe hypokalemia and markedly increased levels of renin and aldosterone. Kidney ultrasonography, abdominal computed tomography and magnetic resonance imaging revealed a small mass in the middle region of the right kidney. The patient underwent nephron-sparing surgery; immunohistochemical results demonstrated typical features of reninoma. Postoperatively, blood pressure and potassium levels were normal at 1 month follow-up.

The purpose of this study was to assess the feasibility and diagnostic performance of an unenhanc... more The purpose of this study was to assess the feasibility and diagnostic performance of an unenhanced MR angiography sequence (Syngo Native Space, Siemens Healthcare) to detect and quantify lower-limb peripheral arterial disease (PAD), with gadolinium-enhanced MR angiography (CE-MRA) as the reference standard. Fifty-one patients known to have lower-limb arteriopathy were included in this prospective study. For every patient, we performed Native sequence and CE-MRA on a 1.5-T system. We evaluated examination duration, image quality, and location, number, and severity of lesions. Examination duration was longer for Native sequence (mean, 39.6 min, vs 10 min for CE-MRA). Image quality was significantly better for CE-MRA, with 92% of images listed as good to excellent for CE-MRA, compared to 53% for Native. Sensitivity, specificity, negative predictive value (NPV), and accuracy of Native were respectively 75%, 95%, 89%, and 88% for all mixed levels; 52%, 97%, 88%, and 87% for aortoiliac level; 87%, 99%, 95%, and 92% for femoropopliteal level; and 82%, 87%, 87%, and 85% for subpopliteal level. If we considered only patients with Leriche and Fontaine stage II arteriopathy, Native results were slightly better, with respective specificities and NPVs of 96% and 91% for all mixed levels; 98% and 90% for aortoiliac level; 98% and 93% for femoropopliteal level; and 91% and 90% for subpopliteal level. Unenhanced MR angiography, cheaper than CE-MRA, showed in our study a good NPV, which suggests its utility as first-line test to screen for PAD, especially in patients at risk of nephrogenic systemic fibrosis.

This vascular achievement justifies first and foremost the hospitalizations of these patients in internal medical and constitutes the first cause of morbi-mortality.
Objectives: To describe the topography and the profile of the vascular Behçet‘s disease.
Patients and Methods: Retrospective study from 1996- 20012, excluding the thrombotic vasculitides localized at the level of the small vessels (retina, cochleovestibular, digital).
Results : We have collected 397 patients, which sex ratio is .3.3 and average age is about 31.9 years We observed 97 vascular events (24.4%) characterized by 69 deep venous thrombosis ‘DVT’ events (17.4%), and 28 arterial events (7%). The average delay to diagnosis of the vascular manifestations from the date of the BD diagnosis is about 4.4 years (1-9), and 5 cases (7%) revealed the BD. The DVT are divides up essentially in lower limbs (25), on cerebral topography (4%), on abdominal area (2%), on superior vein cava (2%). We noted in the others cases some unusual localization of thrombosis (6 cases of intra-cardiac, upper limbs and jugular veins…). The arterial infringements are located as followed: cerebral aneurism (5), pulmonary (10), abdominal (5), heart (4), inferior limb (3), and upper limb (1). The surgical treatment is required in 11 cases. The fatal course is deplored (10) linked in aneurysms of pulmonary arterial ruptur and/or to the massive pulmonary embolisms (7), Budd-Chiari's syndrome (4) and to the intestinal ischemia (1)
Conclusion: As reported in the most studies BD affects the veins more than the arteries. The vascular involvement constitutes poor factor prognosis and the major cause of increased mortality. So it’s must be prevented, detected and treated prematurely (embolization, correct anticoagulation, and reduction of thrombogenous’s risks factors….).
