A total of 2070 participants, including 775 diabetic patients and 1295 healthy individuals, from five studies were included in this meta-analysis. True positive, true negative, false positive, and false negative data were extracted to calculate pooled sensitivity, specificity, positive and negative likelihood ratios, diagnostic odds ratio, and area under the receiver operating characteristics curve. One hundred and eleven consecutive emergency department patients age 65 or older who had undergone MRI to evaluate for clinically suspected hip fracture after negative radiographs were included in the study. The large field-of-view coronal T1 and STIR sequences were isolated from the complete six-sequence MRI protocol and reviewed independently in a blinded fashion by two musculoskeletal fellowship-trained radiologists who recorded presence or absence of fractures of the proximal femora or pelvis, fracture type, and presence or absence of soft tissue injury. Results: Overall, 1755 subjects (2554 knees) met inclusion criteria (mean age 62, 64% female, 86.5% White, mean WOMAC pain 2.7). 113 knees underwent KR overall (4.4%), whereas of those knees that were JSN¼2 at baseline, 19.8% (41/258) underwent KR and of those knees with WOMAC 8-18 at baseline, 10.2% (25/244) underwent KR. Overall, both severity and extent of baseline cartilage lesions as well as change in cartilage lesions over 2.5 years were significantly associated with risk of KR (Table). In contrast, while cartilage lesions were significantly associated with WOMAC pain severity, the relationship was weak (Table). Conclusions: These findings support the clinical relevance of cartilage lesions to the clinically important endpoint of KR. Further, relatively short-term changes in cartilage lesions over 2.5 years may be potentially useful as a prognostic marker for later clinical endpoints such as KR. However, the magnitude of these effects estimates may not be sufficient for validation as an imaging biomarker. In contrast, cartilage lesions were not strongly associated with WOMAC pain. This may partly reflect the uncertainty in the utility of WOMAC pain as an instrument to detect long-term change in pain over years since WOMAC has primarily been used and validated in RCTs to date that are of 3-6 months rather than years-long studies.

Introduction: SARS-CoV-2 causes more severe symptoms in most chronic diseases, and rheumatic dise... more Introduction: SARS-CoV-2 causes more severe symptoms in most chronic diseases, and rheumatic disease is no exception. This study aims to investigate whether there is an association between the use of immunomodulatory medications, including conventional disease-modifying agents (csDMARDs), glucocorticoids, and biologic DMARDs, and outcomes such as hospitalization and lung involvement in patients with rheumatic disease with COVID-19. Methods: We performed a cross-sectional study on 177 COVID-19 cases with rheumatologic diseases using immunomodulatory drugs as their regular treatment. All patients were evaluated regarding their initial chest computed tomography (CT) scan, COVID-19 symptoms, and comorbidities. We ran predictive models to find variables associated with chest CT-scan involvement and hospitalization status. Results: CT findings showed lung involvement in 87 patients with chest CT-scan severity score (C-ss) of less than 8 in 59 (33%) and more than 8 in 28 (16%) of our patients. Of all patients, 76 (43%) were hospitalized. Hospitalized patients were significantly older and had more comorbidities (P = 0.02). On multivariate analysis, older age [odds ratio (OR) 1.90, 95% confidence interval (CI) 1.31-3.08] and comorbidity (OR 2.75, 95% CI 1.06-3.66) were significantly associated with higher odds of hospitalization (P = 0.03). On multivariate analysis, older age (OR 1.15, 95% CI 0.94-2.01), pulmonary diseases (OR 2.05, 95% CI 1.18-3.32), and treatment with csDMARDs (OR 1.88, 95% CI 0.37-1.93) were associated with higher C-ss (P = 0.039). Conclusions: This study found that advanced age and comorbidities, similar to the general population, are risk factors for hospitalization in patients with COVID-19 with rheumatic disorders. Administration of csDMARDs, older age, and pulmonary disorders were linked to increased risk of COVID-19 pneumonia in these individuals.

S... more Introduction: The Sinopharm BBIBP-CorV vaccine produces a variety of cutaneous adverse effects. Scleromyxedema is a mucinous connective tissue disorder that causes skin thickness and sclerodermoid changes. According to our findings, this is the first case of scleromyxedema induced by the Sinopharm immunization. Case description: We discuss the case of a 75-year-old woman who acquired progressive thickening of the skin in her limbs and trunk after getting the Sinopharm vaccination. Examination, laboratory testing, and a biopsy were used to verify scleromyxedema diagnosis. Intravenous immunoglobulins, mycophenolate mofetil, and prednisolone were used in the treatment of the patient. The outcomes from the 4-month follow-up were reassuring. Conclusion: This study emphasizes the need of considering scleromyxedema as a connective tissue pathology in patients who have recently received Sinopharm vaccine and have similar cutaneous signs.

All medical students at the university entrance (first semester), beginning of the clerkship, beginning of the internship, and end of the internship (graduation time) were enrolled. Maslach`s Burnout Inventory for Students (MBI-SS) was used to assess the prevalence of burnout. Chi-square test and Kendal tau_c test were utilized to compare subscales and curricular phases. Results: 668 students participated in the study. The prevalence of burnout was 24.1% which was significantly higher at the end of their internship. 65% of the students had high emotional exhaustion, 75.4% had high cynicism, and 37% had low academic efficacy. Results: Overall, 1755 subjects (2554 knees) met inclusion criteria (mean age 62, 64% female, 86.5% White, mean WOMAC pain 2.7). 113 knees underwent KR overall (4.4%), whereas of those knees that were JSN¼2 at baseline, 19.8% (41/258) underwent KR and of those knees with WOMAC 8-18 at baseline, 10.2% (25/244) underwent KR. Overall, both severity and extent of baseline cartilage lesions as well as change in cartilage lesions over 2.5 years were significantly associated with risk of KR (Table). In contrast, while cartilage lesions were significantly associated with WOMAC pain severity, the relationship was weak (Table). Conclusions: These findings support the clinical relevance of cartilage lesions to the clinically important endpoint of KR. Further, relatively short-term changes in cartilage lesions over 2.5 years may be potentially useful as a prognostic marker for later clinical endpoints such as KR. However, the magnitude of these effects estimates may not be sufficient for validation as an imaging biomarker. In contrast, cartilage lesions were not strongly associated with WOMAC pain. This may partly reflect the uncertainty in the utility of WOMAC pain as an instrument to detect long-term change in pain over years since WOMAC has primarily been used and validated in RCTs to date that are of 3-6 months rather than years-long studies.

This study aims to investigate whether there is an association between the use of immunomodulatory medications, including conventional disease-modifying agents (csDMARDs), glucocorticoids, and biologic DMARDs, and outcomes such as hospitalization and lung involvement in patients with rheumatic disease with COVID-19. Methods: We performed a cross-sectional study on 177 COVID-19 cases with rheumatologic diseases using immunomodulatory drugs as their regular treatment. All patients were evaluated regarding their initial chest computed tomography (CT) scan, COVID-19 symptoms, and comorbidities. We ran predictive models to find variables associated with chest CT-scan involvement and hospitalization status. Results: CT findings showed lung involvement in 87 patients with chest CT-scan severity score (C-ss) of less than 8 in 59 (33%) and more than 8 in 28 (16%) of our patients. Of all patients, 76 (43%) were hospitalized. Hospitalized patients were significantly older and had more comorbidities (P = 0.02). On multivariate analysis, older age [odds ratio (OR) 1.90, 95% confidence interval (CI) 1.31-3.08] and comorbidity (OR 2.75, 95% CI 1.06-3.66) were significantly associated with higher odds of hospitalization (P = 0.03). On multivariate analysis, older age (OR 1.15, 95% CI 0.94-2.01), pulmonary diseases (OR 2.05, 95% CI 1.18-3.32), and treatment with csDMARDs (OR 1.88, 95% CI 0.37-1.93) were associated with higher C-ss (P = 0.039). Conclusions: This study found that advanced age and comorbidities, similar to the general population, are risk factors for hospitalization in patients with COVID-19 with rheumatic disorders. Administration of csDMARDs, older age, and pulmonary disorders were linked to increased risk of COVID-19 pneumonia in these individuals.

S... more Introduction: The Sinopharm BBIBP-CorV vaccine produces a variety of cutaneous adverse effects. Scleromyxedema is a mucinous connective tissue disorder that causes skin thickness and sclerodermoid changes. According to our findings, this is the first case of scleromyxedema induced by the Sinopharm immunization. Case description: We discuss the case of a 75-year-old woman who acquired progressive thickening of the skin in her limbs and trunk after getting the Sinopharm vaccination. Examination, laboratory testing, and a biopsy were used to verify scleromyxedema diagnosis. Intravenous immunoglobulins, mycophenolate mofetil, and prednisolone were used in the treatment of the patient. The outcomes from the 4-month follow-up were reassuring. Conclusion: This study emphasizes the need of considering scleromyxedema as a connective tissue pathology in patients who have recently received Sinopharm vaccine and have similar cutaneous signs.

Maslach`s Burnout Inventory for Students (MBI-SS) was used to assess the prevalence of burnout. Chi-square test and Kendal tau_c test were utilized to compare subscales and curricular phases. Results: 668 students participated in the study. The prevalence of burnout was 24.1% which was significantly higher at the end of their internship. 65% of the students had high emotional exhaustion, 75.4% had high cynicism, and 37% had low academic efficacy.
