Case Report Introduction: Alkaptonuria (AKU) is a rare autosomal recessive disorder result from a... more Case Report Introduction: Alkaptonuria (AKU) is a rare autosomal recessive disorder result from a deficiency of homogentisate 1, 2 dioxygenase (HGD), and leading to the accumulation of homogentisic acid (HGA) in the tissue. The deposition of excess oxidized HGA in the connective tissues causes pigmentation (ochronosis) and degeneration of the joint (ochronotic arthropathy) and tendons (ochronotic tendinopathy). There is no effective therapy for this disorder a side of symptomatic treatment. Case: we report our 43-year-old male farmer, who presents with bilateral asynchronous spontaneous Achilles tendon rupture (5 months apart) with no history of trauma or drug use, nonspecific low back pain, and apparent black pigments in the sclera, with a positive family history of similar condition. Diagnosis: laboratory investigations revealed black urine upon long-standing. A blackish degenerative tendon found during the operation. In addition, the Intraoperative Histopathological study shows dark stains and fibrosis with presence of inflammatory cells consistent with Alkaptonuria. Interventions: debridement of the distal end of the tendon was done; repair was done with augmentation of repair by anchor suture to calcaneus. Outcomes: The patient recovered very well after the operation with the restoration of full activity.

Despite the apparent spread of Shisha smoking phenomenon among Malaysian population, little is known about the safety concerns and determinants of Shisha smokers in this community. Objectives: This study aimed to determine the social determinants of Shisha smoking among Malaysian population in Penang Island, Malaysia and to explore their health related knowledge and perception toward Shisha smoking. Methods: The survey was conducted using simple random sampling by randomly distributing self-administered questionnaires to consumers in Shisha lounges located in Penang Island. Statistical analyses were performed using the Statistical Package for Social Science (SPSS) version 20. Results: A total number of 171 (42.75%) of respondents participated in this study. It is a chronic, progressive, and destructive disease primarily caused by fungi or bacteria characterized by formation of dark pale grains commonly involve skin, soft tissue and rarely bone. Case presentation A 19 year old male patient with chronic right ankle pain, swelling and abscess formation for more than 1 year, patient was treated repeatedly with incision and drainage without any success. No X-ray, biopsy or swab for culture and sensitivity had been considered through the course of presentation. Patient was referred to Omdurman hospital where osteomyelitis secondary euomycetoma infection has been confirmed based on radiological and pathological assessment. Patient was treated surgically with aggressive debridement and bone curettage plus postoperative Itraconazole for 1 year. Steganography is a much better way to convey a secret message compared to cryptography. This is because cryptography is outlawed while steganography can avoid such policies to pass message covertly. Steganography is one of the most common techniques of securing information, which insists on concealing the clandestine data within the cover image in a way that nobody suspects to its existence. The challenge of steganography techniques is to make a reasonable balance between the file and the size of data that can be exchanged. Furthermore, the strength of the strategy and the security of the mysterious data are the fact that can't be hidden.

It begins by providing with the background of the development of this methodology. From then on, it expands into describing the principles that governs the techniques such as the 40 Inventive Principles, the Contradiction principal and its' matrix, and the 39 engineering parameters. Some suggestions are provided for applying TRIZ in manufacturing and how TRIZ could be integrated with other selected problem solving tools.
