People with disabilities are a health disparity population that face many barriers to health prom... more People with disabilities are a health disparity population that face many barriers to health promotion opportunities in their communities. Inclusion in public health initiatives is a critical approach to address the health disparities that people with disabilities experience. The National Center on Health, Physical Activity and Disability (NCHPAD) is tackling health disparities in the areas of physical activity, healthy nutrition, and healthy weight management. Using the NCHPAD Knowledge Adaptation, Translation, and Scale-up Framework, NCHPAD is systematically facilitating, monitoring, and evaluating inclusive programmatic, policy, systems, and environmental (PPSE) changes in communities and organizations at a local and national level. Through examples we will highlight the importance of adapting knowledge, facilitating uptake, developing strategic partnerships and building community capacity that ultimately creates sustainable, inclusive change.

Intrinsic motivational indicators are critical in predicting successful health behavior change. T... more Intrinsic motivational indicators are critical in predicting successful health behavior change. The purpose of this randomized controlled trial was to develop a 12-month telephone-based exercise and nutrition coaching intervention using motivational interviewing. Sixty-six overweight/obese adults (M age =47 yrs. + 13.6) with mobility disabilities (multiple sclerosis, spinal cord injury, spina bifida, cerebral palsy) were enrolled. Participants were primarily female (80%) and were randomized into one of three groups: (a) control group; (b) physical activity group (PEP); (c) physical activity plus nutrition group (PEP Plus). Based on level of engagement in goal setting and readiness to lose weight, participants were classified as high and low responders. High responders wore their pedometer at least 70% of the contact weeks and responded to at least 90% of telephone calls from their wellness coach. We compared changes in body weight in adults with physical disabilities after participating in a ... more We compared changes in body weight in adults with physical disabilities after participating in a 12-month telephone intervention targeting either physical activity or physical activity and nutrition. A total of 101 participants (age: 46.512.7 years; BMI 32.06.1 kg/m2) were randomized to one of three conditions: physical activity only (PEP), physical activity and nutrition (PEP+), and control. PEP received a physical activity toolkit and regular coaching telephone calls (weekly during months 1-4, monthly during months 5-6, bi-monthly during months 7-8, and none during months 9-12). PEP+ received an identical intervention as PEP; however, they also received information and coaching on nutrition in addition to physical activity. The control group received the toolkit, information, and resources following the 12-month intervention. This randomized controlled trial tested a tailored, telephone-based physical activity coaching in... more This randomized controlled trial tested a tailored, telephone-based physical activity coaching intervention for a predominantly African American group of women with severe obesity and mobility disability. We recruited 92 clinic patients from the University of Illinois at Chicago Medical Center referred by their physicians during 2004-2007 and randomized participants to one of three groups--awareness(informational brochure, no coaching), lower support (phone coaching only) and higher support (phone coaching plus monthly exercise support group)--to determine the efficacy of a tailored coaching intervention on key health outcomes, which included body weight and body mass index, blood pressure, cholesterol, physical activity (barriers and self-reported activity), movement and mobility, general health, and social support.
