Changes in left ventricular (LV) diastolic indices after left ventriculography (LVG) with iopamid... more Changes in left ventricular (LV) diastolic indices after left ventriculography (LVG) with iopamidol or urografin were studied in 42 subjects. Increase in heart rate and decrease in LV systolic pressure were more significant with urografin than with iopamidol (p less than 0.05 to 0.001). LV end-diastolic pressure was elevated more with urografin than with iopamidol (p less than 0.005 to 0.05) 1 to 3 minutes after LVG. LV peak negative dP/dt decreased significantly with urografin immediately (10 to 15 seconds, -511 and 30 seconds, -376 mm Hg/sec; p less than 0.0005 to 0.02), but with iopamidol it did not decrease significantly after LVG. Time constant, T, was elongated with iopamidol (10 to 15 seconds, +13 and 30 seconds, +6 msec; p less than 0.0005), but this elongation was significantly less than urografin (10 to 15 seconds, +34; 30 seconds, +25; 1 minute, +15; and 2 minutes, +10 msec; p less than 0.05 to 0.0005). We conclude that iopamidol disturbed LV diastolic function to a lesser degree than did urografin.

The clinical value of biplane transesophageal echocardiography (TEE) was assessed on basis of the... more The clinical value of biplane transesophageal echocardiography (TEE) was assessed on basis of the experience of the first 100 cases. All studies were performed in awake patients, there was no complication or failure. There wee 43 males and 57 females, aged between 13 and 82 years. The indication of TEE was infective endocarditis in 33, congenital heart disease in 25, suspected tumor or thrombus in 18, search for source of embolism in 9, thoracic aortic pathology in 9, prosthetic valve dysfunction in 3 and other problems in 3 cases. Biplane technique provided additional information as compared to single plane TEE in the diagnosis of infective endocarditis (by the visualization of small vegetations and of mitral valve fenestration, and by the differentiation of mitral vegetation from prolapse or aneurysm), in the identification of source of embolism (by easier visualization of patent foramen ovale and of the left atrial appendage thrombus), in the imaging of ascending aorta, of right ...

Femoral artery pseudoaneurysm was found by combined 2D, pulsed and continuous wave Doppler echogr... more Femoral artery pseudoaneurysm was found by combined 2D, pulsed and continuous wave Doppler echography in 10 out of 16 patients with hematoma formation following cardiac catheterization. The typical features included an echofree area communicating with the femoral artery through a narrow neck. There was a low velocity systolic flow in the pseudoaneurysm and a high velocity systolic and reverse diastolic flow in the neck.

Introduction: Contact force measurement allows feedback on tissue contact during lesion formation... more Introduction: Contact force measurement allows feedback on tissue contact during lesion formation. An ablation catheter with fibre-optic contact force sensing was introduced in 2014. With the introduction of new technology comes a need for real-world outcomes data in multiple geographies. Purpose: This analysis compares user-reported complication rates for a contact force sensing catheter by country and year. Methods: Data used were unit volumes of the ablation catheter by country and year from January 2014-July 2017, and a company database of reports on any complications attributed to the catheter. Events recorded included atrioesophageal fistula and cardiac perforation or tamponade. Results are presented for individual countries in Europe and North America, combined as necessary to ensure no groups of fewer than 10 events. Multivariable logistic regression was used to determine the effect of time and region on event rates. Results: Of 99,357 units in Europe, the U.S. and Canada, there were 217 perforations or tamponades (0.22%). There were 11 fistulas observed in North America (0.02%) and fewer than 10 in Europe. Event rates declined over time (P<0.001 for both types). In multivariable regression, there was a significant association of year of ablation with perforations (0.26% in 2014-5 vs. 0.17% in 2016-7). The difference between geographic regions (0.30% in Europe vs. 0.16% in North America) became non-significant when year of ablation was included as a covariate. Rates by country are shown in the table. Conclusion: Fistula and perforation rates for the contact force sensing catheter are lower than historical complication rates. A more complete analysis could consider additional types of procedural complications, as well as those attributed to other tools used during ablation. Abstract P897 Table. Event rates by country. Methods and Results: 46 patients were selected for catheter ablation (CA) because of nonvalvular PAF. Transthoracic, tissue Doppler and transoesophageal echocardiography was performed before CA. LA volumes and volume index (LAVI) were calculated. LA function was assessed by LA filling fraction (LAFF), LA emptying fraction (LAEF), systolic fraction of pulmonary venous flow (PVSF) and late diastolic velocities of mitral annulus (Aa sept , Aa lat) LAA function was assessed by peak LAA emptying flow velocity (PLAAEFV). Diastolic dysfunction (DD) was also assessed. Dilated LAVI in 32, LA dysfunction in 20, DD with elevated LV filling pressure in 19 patients was found. Aa lat and Aa sept correlated with LAFF

In patients with patent foramen ovale and cryptogenic stroke, the risk of recurrent event is abou... more In patients with patent foramen ovale and cryptogenic stroke, the risk of recurrent event is about 4-5% yearly, despite of the antithrombotic treatment. Transcatheter closure of patent foramen ovale has been shown to decrease risk of recurrence. This report summarizes the first experiences in Hungary and the medium term follow up of our patients. Thirty-three patients were selected for closure. All the patients had at least one ischemic stroke, transitory ischemic attack or peripheral embolus. In 31 of them the stroke was confirmed by computer tomographic or magnetic resonance imaging. Patent foramen ovale with right-to-left shunt was diagnosed by transesophageal echocardiography. Transcatheter closure was performed under fluoroscopic and transesophageal echocardiographic control. Closure was successful in 31 patients. Passage of atrial septum did not succeed in two cases. An AV fistula at puncture site was the only early complication. There was no late complication. No device dislo...

UNLABELLED Due to successful surgical treatment of congenital heart defects in infants and childr... more UNLABELLED Due to successful surgical treatment of congenital heart defects in infants and children, the number of patients who reach the adolescent/adult age is continuously increasing. AIMS The authors sought to identify the short- and medium-term outcomes of reconstruction of right ventricular outflow tract in adolescents and adults who underwent surgical intervention for congenital heart defect in infancy or early childhood. METHODS Between 2001 and 2012, 48 patients (age: 15-39, mean 21 years) (30 tetralogy of Fallot, 11 pulmonary atresia + ventricular septal defect, 6 transposition of great arteries + ventricular septal defect + left ventricular outflow tract obstruction, and 1 truncus arteriosus) had repeat operation because of right ventricular dysfunction. With the presented case it could be concluded that coronary angiography is recommended before LAA closure.
