However, the high mortality associated with thoracoscopic lung biopsy using general anesthesia (GA) in patients with ILD has deterred physicians from offering this procedure and adopt a diagnostic approach based on high-resolution CT. Here we report that thoracoscopy under regional anesthesia could be a safer alternative for lung biopsy and effectively guide ILD treatment.MethodsThis was a single-center retrospective review of prospectively maintained database and consisted of patients who underwent thoracoscopic lung biopsy between March 2016 and March 2018. Patients were divided into two groups: (A) GA, and (B) regional anesthesia using monitored anesthesia care (MAC) and thoracic epidural anesthesia (TEA).ResultsDuring the study period, 44 patients underwent thoracoscopic lung biopsy. Research being conducted to identify beneficial strategies for integrating displays and signals u... more Research being conducted to identify beneficial strategies for integrating displays and signals used in anesthesia monitoring is reported. It is motivated by the fact that the anesthesiologist's equipment is typically procured over time from a variety of vendors, so that the anesthesiologist's task is made more difficult by a diversity of instrument behaviors and display formats. An experiment that compared intervention decisions made viewing parameters presented in a separated strip chart format and in the form of an integrated geometric object is described. The study investigated the detection of idealized physiological events, each presenting its classic symptoms. Anesthesiologists using unfamiliar displays rapidly recognized these emergencies under either display format, usually within 30 s. However, the object display reduced response latencies and led to intervention at less extreme parameter values without appreciable increase in false alarm rates

Advanced coronary artery disease (CAD) is increasingly common in patients awaiting orthotopic liv... more Advanced coronary artery disease (CAD) is increasingly common in patients awaiting orthotopic liver transplantation (OLT). Unfortunately, in patients whose coronary artery anatomy is not amenable to angioplasty, coronary artery bypass grafting (CABG) alone may precipitate hepatic decompensation. Thus, combined liver transplant and coronary artery bypass grafting (CABG-OLT) may be required to effectively treat both conditions. Clinical records were analyzed for 5 CABG-OLT procedures at a single institution. Operative indications, technical details, and postoperative course were determined for each patient. Patients undergoing CABG-OLT had a mean age of 57.8 years (range, 54-66) and were predominantly male (80%). All patients had significant 3-vessel coronary atherosclerotic disease with preserved left ventricular function. There were no intraoperative deaths. At mean 25 months of follow-up (range, 8.0-25) there was an 80% graft and patient survival. Overall average length of stay was 21 days (range, 7-59 days). In conclusion, CABG-OLT procedure appears to be safe and effective in the population of patients with advanced CAD and liver disease. In this series, patients appear to benefit from multidisciplinary preoperative evaluation, coordination between cardiac and transplant surgeons, careful graft selection, and use of sapheno-atrial veno-veno bypass.

Thirty-two patients with coronary artery disease who underwent liver transplantation between 1990... more Thirty-two patients with coronary artery disease who underwent liver transplantation between 1990 and 1994 were identified. Coronary artery disease was managed medically (n = 9), by angioplasty (n = l), or surgically (n = 22) prior to liver transplantation. Two patients underwent simultaneous coronary artery bypass gralting and liver transplantation. Complete preoperative cardiac evaluation was performed in all patients. Perioperative and postoperative morbidity and mortality were retrospectively determined. Overall mortality was 50%, whereas morbidity was 81 %. Follow-up was between 1 and 3 years after liver transplantation. Subgroup analysis revealed that medically managed patients had a 56% mortality and a 100% morbidity. The patient who underwent angioplasty survived without morbidity. One patient who underwent simultaneous coronary artery bypass grafting and liver transplantation died intraoperatively. The ultrastructural distribution of calcium was studied in human lens fibres with the oxalate pyr... more The ultrastructural distribution of calcium was studied in human lens fibres with the oxalate pyroantimonate technique. In the intermediate cortex precipitates were found perimembranous and occasionally in the fibre cytoplasm. The improved resolution of electron tomography revealed (a) that the perimembranous precipitates as described earlier by Vrensen et al. (1995) are restricted to the intercellular space ; no indications were found of an increased intracellular submembranous calcium level, and (b) the existence of an intracellular pool of small protein attached precipitates in the fibre cytoplasm not observed with conventional electron microscopy. It is concluded that in the intermediate and deep cortex, where in the mature fibres all cellular calcium pools have disappeared, two calcium pools exist : (a) a pool of free calcium ionically bound to the negatively charged phospholipids of the external face of the fibre membrane and (b) a cytoplasmic pool of protein associated calcium. Possible candidates for this cytoplasmic calcium binding are discussed.
