Host-related factors, such as systemic inflammatory response and its interplay with the immune system, remain underexplored. We hypothesized that the prognostic impact of this response could vary between patients undergoing primary debulking surgery (PDS) and those undergoing interval debulking surgery (IDS). Therefore, we evaluated the outcomes of two surgical groups of newly diagnosed AEOC patients according to the neutrophil, monocyte and platelet to lymphocyte ratios (NLR, MLR, PLR), taking median ratio values as cutoffs. In the PDS group (n = 61), low NLR and PLR subgroups showed significantly better overall survival (not reached (NR) vs. 72.7 months, 95% confidence interval [CI]: 40.9–95.2, p = 0.019; and NR vs. 56.1 months, 95% CI: 40.9–95.2, p = 0.004, respectively) than those with high values. Trypanosoma cruzi calreticulin (TcCalr, formerly known as TcCRT), upon binding to Complement (C) ... more Trypanosoma cruzi calreticulin (TcCalr, formerly known as TcCRT), upon binding to Complement (C) C1 and ficolins, inhibits the classical and lectin pathways and promotes infectivity. This virulence correlates with the expression of TcCalr. The TcCalr C inhibitory capacity was shown in a previous work using a clonal epimastigote cell line from the TCC T. cruzi strain, lacking one TcCalr allele (TcCalr+/-) or over expressing it (TcCalr+). In this work, we detected atypical morphology in TcCalr+/- and in TcCalr+ parasites, as compared to the wild-type (WT) strain. Polyclonal anti-TcCalr antibodies detected TcCalr presence mainly in the parasite nucleus. The number of TcCalr indicator gold particles, detected in electron microscopy and quantified in silico, correlated with the number of TcCalr coding genes. Both TcCalr+ and TcCalr +/- epimastigotes presented morphological alterations.

Four groups of women were assessed as a function of their age (teens vs. adults) and maternal status (mothers vs. nonmothers). Participants completed executive function tests, including Spatial Working Memory (SWM), Intra-Extra-Dimensional-Set-Shift (IED), and Stockings of Cambridge (SOC). Women also completed the Childhood Trauma Questionnaire to assess their experiences of early adversity. Results showed that for the IED-task, there were main effects of age and maternal status and an interaction between the two; adults performed better than teens, mothers performed better than nonmothers, and teen nonmothers performed the least well of all groups. AD affects around 12% of the aged population in the world. Considerable research efforts have pointed to the role of innate immunity as the main culprit in the pathogenesis of AD. In this context, and according to with our neuroimmunomodulation theory, microglial activation modifies the cross-talks between microglia and neurons. We postulated that glial activation triggered by "damage signals" activates a pathological molecular cascade that finally leads to hyperphosphorylation and oligomerization of the tau protein. Interestingly, these modifications correlate with the gradual cognitive impairment of patients with the AD. Microglial activation is determined by the nature and strength of the stimulus. From the records of all of the pediatric hepatologists in the province of Santa Fe, Argentina, we reviewed the clinical charts of patients < 18 years who were diagnosed with AIH (simplified score >6 points) and followed between January 2003 and December 2013. Population data were extracted from the 2010 national census. Values were expressed as percentages and median +/- interquartile range. Mann-Whitney test was used for comparison between groups. 67 patients fulfilled inclusion criteria, from which 11 (16%) were later reclassified as "autoimmune sclerosing cholangitis" (ASC) according to biochemical, histological and radiological findings. This study investigated associations between three types of child maltreatment (exposure to intim... more This study investigated associations between three types of child maltreatment (exposure to intimate partner violence, sexual, and physical abuse) and multimorbidity (chronic physical conditions, pain conditions, and mental disorders) in adults. Multinomial logistic regression was used to analyze weighted data from the 2012 Canadian Community Health Survey (CCHS - MH 2012), a representative population sample (N = 23,846) of respondents ages 18+. All three subtypes of child maltreatment independently predicted increased odds of experiencing multimorbidity as an adult, while adjusting for covariates (adjusted odds ratios ranged from 1.34 (95% CI = 1.00, 1.80) to 4.87 (95% CI = 2.75, 8.63)). A dose-response relationship between the number of child maltreatment subtypes and risk for multimorbidity was also observed (adjusted odds ratios ranged from 1.38 (95% CI = 1.11, 1.73) to 10.96 (95% CI = 6.12, 19.64)). Los espacios curriculares para el abordaje de temas de ESI en la formacion docente se van configurando en un territorio en disputa: legitimados por la Ley de Educacion sexual integral, ocupados estrategicamente por activistas feministas y LGBTTI, y vigilados por grupos conservadores. Las primeras aproximaciones, sistematizaciones y conceptualizaciones que estamos construyendo se articulan alrededor de tres grandes nudos problematicos o discusiones en relacion con el campo investigado: la relacion entre Experiencia e implicacion; Experiencia e interpelacion y Saber- Experiencia- Autoridad, en la (re)configuracion de subjetividades y vinculos pedagogicos. Cumulative risk (CR) models provide some of the most robust findings in the developmental literat... more Cumulative risk (CR) models provide some of the most robust findings in the developmental literature, predicting numerous and varied outcomes. Typically, however, these outcomes are predicted one at a time, across different samples, using concurrent designs, longitudinal designs of short duration, or retrospective designs. We predicted that a single CR index, applied within a single sample, would prospectively predict diverse outcomes, i.e., depression, intelligence, school dropout, arrest, smoking, and physical disease from childhood to adulthood. Further, we predicted that number of risk factors would predict number of adverse outcomes (cumulative outcome; CO). We also predicted that early CR (assessed at age 5/6) explains variance in CO above and beyond that explained by subsequent risk (assessed at ages 12/13 and 19/20). ABSTRACT Introduction: As genetic testing becomes more readily available, it is critical to autom... more ABSTRACT Introduction: As genetic testing becomes more readily available, it is critical to automatically connect these results with the healthcare provider during key moments of patient care, including: display of genetic test results to the physician when ordering a medication, when accessing the patient’s chart for the first time, or when reviewing historical results and clinical notes before a visit. Cerner’s genome-enabled electronic health record allows clinicians and researchers to leverage the benefits of clinical and genomic data for improved patient outcomes. Methods: To provide this type of information at the appropriate time at VCU Health System, genetic test results were stored in the electronic health record in a discrete and standardized manner, which allowed for a more widely-adaptable format in which to deliver this information across providers. Utilizing Cerner Millennium Helix™ and the Clinical Bioinformatics Ontology® (CBO), molecular diagnostic test results were stored in a clinically relevant and machine-readable format. Using computer-based guidelines, the electronic health record and embedded genetic content were aggregated and used to produce logic parameters to alert and make recommendations to the physician at time of care. These rules were used to help with result interpretation and to provide recommendations for dosing changes or alternative therapy. VCUHS and Cerner demonstrated the integration of pharmacogenetics in the Electronic Health Record by focusing on infectious disease (HIV viral load and genotyping), hepatology (HCV viral load and genotyping), and specialty coagulation (CYP450 and VKORC1). Results: Using peer reviewed published clinical guidelines and updated packing for drugs, relevant clinical and laboratory testing including genetic test data were used to provide disease-specific information displays necessary for patient management. In addition, this information was used to provide alerts upon clinical management decisions and placement of medication orders. Half a million patients are hospitalized with severe dengue every year, many of whom would die wi... more Half a million patients are hospitalized with severe dengue every year, many of whom would die without timely, appropriate clinical intervention. The majority of dengue cases are uncomplicated; however, 2-5% progress to severe dengue. Severe dengue cases have been reported with increasing frequency over the last 30 years. To discover biomarkers for severe dengue, we used surface-enhanced laser desorption/ionization time-of-flight mass spectrometry to analyze dengue virus positive serum samples from the acute phase of infection. Using this method, 16 proteins were identified as candidate biomarkers for severe dengue. From these 16 biomarkers, three candidates were selected for confirmation by enzyme-linked immunosorbent assay and Western blot: vitronectin (Vtn, 55.1 kDa), hemopexin (Hx, 52.4 kDa), and serotransferrin (Tf, 79.2 kDa). Vitronectin, Hx, and Tf best differentiated between dengue and severe dengue.

Animal studies investigating mechanisms of transmission have focused on biological factors; whereas research in humans has emphasized cognitive and psychosocial factors. We hypothesized that neuropsychological and physiological factors would act as mediators between maternal retrospective reports of early life experiences (ELE) and current parenting. Method: We recruited a community sample of 89 mothers and their infants (2-6 months of age). Maternal ELE consisted of self-reports of consistency of care and childhood maltreatment. Diurnal salivary cortisol samples were collected as the measure of hypothalamicpituitary-adrenal (HPA) function. Executive function measures included attentional setshifting and spatial working memory. Maternal sensitivity was assessed through videotapes of mothers interacting with their infants. Results: A series of path analyses indicated that maternal ELE was indirectly related to maternal sensitivity via two pathways: one through HPA function, and the other through HPA function and spatial working memory. There was no direct path between maternal ELE and parenting. Conclusion: These findings provide support for the notion that mediators linking early life experiences to parenting in humans may be similar to physiological mechanisms found in animal models. As maternal care is associated with numerous infant outcomes, our findings may have broad relevance to understanding the risk associated with parenting and adverse outcomes in infants. A greater understanding of mechanism is important to informing interventions targeted at disrupting maladaptive trajectories of parenting.

Typically, however, these outcomes are predicted one at a time, across different samples, using concurrent designs, longitudinal designs of short duration, or retrospective designs. We predicted that a single CR index, applied within a single sample, would prospectively predict diverse outcomes, i.e., depression, intelligence, school dropout, arrest, smoking, and physical disease from childhood to adulthood. Further, we predicted that number of risk factors would predict number of adverse outcomes (cumulative outcome; CO). We also predicted that early CR (assessed at age 5/6) explains variance in CO above and beyond that explained by subsequent risk (assessed at ages 12/13 and 19/20). ABSTRACT Introduction: As genetic testing becomes more readily available, it is critical to autom... more ABSTRACT Introduction: As genetic testing becomes more readily available, it is critical to automatically connect these results with the healthcare provider during key moments of patient care, including: display of genetic test results to the physician when ordering a medication, when accessing the patient’s chart for the first time, or when reviewing historical results and clinical notes before a visit. Cerner’s genome-enabled electronic health record allows clinicians and researchers to leverage the benefits of clinical and genomic data for improved patient outcomes. Methods: To provide this type of information at the appropriate time at VCU Health System, genetic test results were stored in the electronic health record in a discrete and standardized manner, which allowed for a more widely-adaptable format in which to deliver this information across providers. Utilizing Cerner Millennium Helix™ and the Clinical Bioinformatics Ontology® (CBO), molecular diagnostic test results were stored in a clinically relevant and machine-readable format. Using computer-based guidelines, the electronic health record and embedded genetic content were aggregated and used to produce logic parameters to alert and make recommendations to the physician at time of care. These rules were used to help with result interpretation and to provide recommendations for dosing changes or alternative therapy. VCUHS and Cerner demonstrated the integration of pharmacogenetics in the Electronic Health Record by focusing on infectious disease (HIV viral load and genotyping), hepatology (HCV viral load and genotyping), and specialty coagulation (CYP450 and VKORC1). Results: Using peer reviewed published clinical guidelines and updated packing for drugs, relevant clinical and laboratory testing including genetic test data were used to provide disease-specific information displays necessary for patient management. In addition, this information was used to provide alerts upon clinical management decisions and placement of medication orders. The majority of dengue cases are uncomplicated; however, 2-5% progress to severe dengue. Severe dengue cases have been reported with increasing frequency over the last 30 years. To discover biomarkers for severe dengue, we used surface-enhanced laser desorption/ionization time-of-flight mass spectrometry to analyze dengue virus positive serum samples from the acute phase of infection. Using this method, 16 proteins were identified as candidate biomarkers for severe dengue. From these 16 biomarkers, three candidates were selected for confirmation by enzyme-linked immunosorbent assay and Western blot: vitronectin (Vtn, 55.1 kDa), hemopexin (Hx, 52.4 kDa), and serotransferrin (Tf, 79.2 kDa). Vitronectin, Hx, and Tf best differentiated between dengue and severe dengue.

Animal studies investigating mechanisms of transmission have focused on biological factors; whereas research in humans has emphasized cognitive and psychosocial factors. We hypothesized that neuropsychological and physiological factors would act as mediators between maternal retrospective reports of early life experiences (ELE) and current parenting. Method: We recruited a community sample of 89 mothers and their infants (2-6 months of age). Maternal ELE consisted of self-reports of consistency of care and childhood maltreatment. Diurnal salivary cortisol samples were collected as the measure of hypothalamicpituitary-adrenal (HPA) function. Executive function measures included attentional setshifting and spatial working memory. Maternal sensitivity was assessed through videotapes of mothers interacting with their infants. Results: A series of path analyses indicated that maternal ELE was indirectly related to maternal sensitivity via two pathways: one through HPA function, and the other through HPA function and spatial working memory. There was no direct path between maternal ELE and parenting. Conclusion: These findings provide support for the notion that mediators linking early life experiences to parenting in humans may be similar to physiological mechanisms found in animal models. As maternal care is associated with numerous infant outcomes, our findings may have broad relevance to understanding the risk associated with parenting and adverse outcomes in infants. A greater understanding of mechanism is important to informing interventions targeted at disrupting maladaptive trajectories of parenting.
