Common causes include infection and inflammation, and mechanical issues such as graft failure. Case Presentation: A 36-year-old male with an ACL reconstruction six years prior was admitted due to pain and swelling. The ACL graft that had ruptured was found arthroscopically. Numerous gout crystals and tophus were observed inside. This finding is suggested as a cause of this kind of complication. Discussion: Biological failure is one of the causes of graft failure. Gout arthritis is associated with internal derangement of the joint structures, which is reported in advanced case. Conclusion: Severe gouty arthritis, even though rare, could be an etiology of graft failure following ACL reconstruction and cause significant morbidity.

The most-used questionnaire in clinical research for these patients is the International Knee Documentation Committee (IKDC) Subjective Knee Form, as its psychometric properties are considered to be excellent. Purpose: To translate the IKDC into Indonesian and assess its validity for use in Indonesian-speaking patients with ACL injuries. Study Design: Cohort study (diagnosis); Level of evidence, 2. Methods: After a forward-and-backward translation procedure and cross-cultural adaptation, the validity and reliability of the questionnaire were investigated. The responses of ACL injury patients on 3 questionnaires, the Indonesian-IKDC (I-IKDC), 36-Item Short Form Health Survey, and Kujala Anterior Knee Pain Scale, were compared. Following consensus-based standards for the selection of health measurement instruments guidelines, construct val...

Several studies have assessed the effectiveness, side-effects, and ease of use of various analgesics. Preemptive analgesia with combined celecoxib and pregabalin has been reported to yield positive outcomes. In this randomized, double-blind controlled clinical trial, 30 subjects underwent surgery for total knee arthroplasty using 15-20mg bupivacaine 5% epidural anesthesia. All subjects were divided into three groups. Group 1 was given celecoxib 400mg and pregabalin 150mg 1 hour before the operation, Group 2 was given celecoxib 200mg and pregabalin 75mg twice daily starting from 3 days before the operation, and Group 3 was given a placebo. The outcome was measured with Visual Analog Scale, knee range of motion, and postoperative mobilization. There was a significant difference in postoperative morphine usage b...

Decalcification is a stage that plays an important role in bone histology using various types of solutions. The sample used in this study was lateral condyle from tibia of three garut sheeps (Ovis aries) which had been fixed with 10% Neutral Buffered Formalin (NBF) for 24 hours. Sample were cut into pieces ranging from 1 cm x 1 cm x 1 cm in size, the decalcification using three solutions; 10% nitric acid, 10% EDTA (pH 7.4) and 10% EDTA (pH 7.4) + TBD-1®. The aim of this study was to evaluate the effectiveness of three solutions for the decalcification process of lateral tibial condyle of garut sheep. Observation parameter in this study includes: duration of decalcification, sectioning process of ribboning formation, structural integrity and absorption of hematoxylin-eosin (HE) staining. The results shows that 10% EDTA (pH 7.4) solution provides a long duration of ...

Most cases of patellar lateral dislocation are due to damage to the medial patellofemoral ligament (MPFL), either rupture of detachment of the patella or femoral attachment. MPFL reconstruction alone was considered adequate for the treatment of this condition. We present a case of a 49-year-old male with chronic posttraumatic lateral patellar dislocation of the right knee of 25 years, which we treated with extensive lateral release and right medial patellofemoral ligament reconstruction with 5-year follow-up data.

A large tibial internal rotation will result in a patellofemoral pain syndrome. This study aimed to introduce a new technique of modified exorotation graft tension at tibial fixation to minimize endorotation, reduce tibial internal rotation, and prevent patellofemoral pain syndrome. Methods: This study was a randomized double-blind controlled clinical trial. ACL rupture patients underwent ACLR surgery between December 2014 and Februrary 2015. They were randomized to standard endorotation group or modified exorotation group. Hamstring autograft was used and fixed with an EndoButton® and bioabsorbable interference screw. Tibial tubercle to trochlear grove (TTTG) was used to evaluate rotation, whereas cartilage oligomeric matrix protein (COMP) was employed to analyze cartilage breakdown. The functional outcome was measured using the Kujala score ...

Our study evaluated the effect of intraarticular injection of growth hormone (GH) in OAinduced rabbit knees compared to hyaluronic acid (HA) and placebo. Materials and Methods: A total of 21 male, skeletally mature, New Zealand rabbits received an intraarticular type II collagenase injection for OA induction. Two weeks later, the rabbits were randomized into three groups based on the weekly intraarticular injection to be received: GH, HA, and saline. Injections were done for three consecutive weeks. Evaluation was done at 8 weeks after treatment, clinically using the lameness period, macroscopically using the Yoshimi score and microscopically using the Mankin score. Results: The shortest period of lameness was found in the GH group (15.9±2.12 days), compared to the HA group (19.4±1.72 days) and placebo group (25.0±2.94 days). There was a statistically significant difference in macroscopic scoring between groups (p=0.001) in favor of the GH group. There was also significant difference in the microscopic score between groups (p=0.001) also in favor of the GH group. Conclusions: Intraarticular injection of GH showed better clinical, macroscopic and microscopic results as compared to HA and placebo.

However, no studies have confirmed the effects of bench-step exercise (BE) on bone formation and resorption. The goal of this study is to compare the effects of bench-step and walking exercise (WE) on changes in osteocalcin and CTX-1 levels. Methods: Fifty-nine sedentary post-menopausal women with osteopenia (T-score between [Formula: see text]1 and [Formula: see text]2.5) were randomly divided into two groups: WE group ([Formula: see text]) and BE group ([Formula: see text]). Subjects performed 12 weeks of exercise. The osteocalcin and CTX-1 levels were analyzed using enzyme-linked immunosorbent assay. Independent [Formula: see text]-test or Wilcoxon test was used to analyze the osteocalcin and CTX-1 levels. The difference in changes in the osteocalcin and CTX-1 between the groups before and after was analyzed using Mann–Whitney test. Results: The results showed a significant increa...

There are differences in patient demographics among ethnic groups, with Asian populations typically smaller in body size. As a consequence, there may be a perception that celecoxib is less effective, or has poorer tolerability in Asian patients. Methods: This analysis compares data from two multicenter, randomized, double-blind, placebo-controlled, active-comparator trials of celecoxib for the treatment of osteoarthritis of the knee: one study in Asian patients and the

Methylsulfonylmethane (MSM) is a supplement containing organic sulphur and also reported to slow anatomical joint progressivity in the knee OA. The MSM is often combined with glucosamine and chondroitin sulfate. Patients were allocated by permuted block randomization into three groups: GC (n=49), GCM (n=50), or placebo (n=48) groups. GC group received 1500 mg of g...

Severe cartilage damage may lead to osteoarthritis (OA). Current surgical and non-surgical therapeutic interventions in OA are limited to symptom relief and/or repair of focal lesion, and later a total knee replacement is still necessary. Cell therapy with chondrocyte implantation requires healthy cartilage for donor of the cells. Adult mesenchymal stem cells (MSCs) have the ability to differentiate into chondrogenic lineage. They can readily be isolated from bone marrow as well as many other adult tissues and have an extensive proliferation capacity. Therefore, MSCs may offer a great potential to be developed as an alternative for cell-based articular cartilage therapy.
