Introduction: Pain and instability following an anterior cruciate ligament (ACL) reconstruction r... more Introduction: Pain and instability following an anterior cruciate ligament (ACL) reconstruction remain a significant issue. Common causes include infection and inflammation, and mechanical issues such as graft failure. Case Presentation: A 36-year-old male with an ACL reconstruction six years prior was admitted due to pain and swelling. The ACL graft that had ruptured was found arthroscopically. Numerous gout crystals and tophus were observed inside. This finding is suggested as a cause of this kind of complication. Discussion: Biological failure is one of the causes of graft failure. Gout arthritis is associated with internal derangement of the joint structures, which is reported in advanced case. Conclusion: Severe gouty arthritis, even though rare, could be an etiology of graft failure following ACL reconstruction and cause significant morbidity.

Several studies have assessed the effectiveness, side-effects, and ease of use of various analgesics. Preemptive analgesia with combined celecoxib and pregabalin has been reported to yield positive outcomes. In this randomized, double-blind controlled clinical trial, 30 subjects underwent surgery for total knee arthroplasty using 15-20mg bupivacaine 5% epidural anesthesia. All subjects were divided into three groups. Group 1 was given celecoxib 400mg and pregabalin 150mg 1 hour before the operation, Group 2 was given celecoxib 200mg and pregabalin 75mg twice daily starting from 3 days before the operation, and Group 3 was given a placebo. Decalcification is a stage that plays an important role in bone histology using various types of solutions. The sample used in this study was lateral condyle from tibia of three garut sheeps (Ovis aries) which had been fixed with 10% Neutral Buffered Formalin (NBF) for 24 hours. Sample were cut into pieces ranging from 1 cm x 1 cm x 1 cm in size, the decalcification using three solutions; 10% nitric acid, 10% EDTA (pH 7.4) and 10% EDTA (pH 7.4) + TBD-1®. The aim of this study was to evaluate the effectiveness of three solutions for the decalcification process of lateral tibial condyle of garut sheep. Most cases of patellar lateral dislocation are due to damage to the medial patellofemoral ligament (MPFL), either rupture of detachment of the patella or femoral attachment. MPFL reconstruction alone was considered adequate for the treatment of this condition. We present a case of a 49-year-old male with chronic posttraumatic lateral patellar dislocation of the right knee of 25 years, which we treated with extensive lateral release and right medial patellofemoral ligament reconstruction with 5-year follow-up data.

Up to now, there is no feasible solution for stopping or reversing the degenerative process of os... more Up to now, there is no feasible solution for stopping or reversing the degenerative process of osteoarthritis (OA). Our study evaluated the effect of intraarticular injection of growth hormone (GH) in OAinduced rabbit knees compared to hyaluronic acid (HA) and placebo. Materials and Methods: A total of 21 male, skeletally mature, New Zealand rabbits received an intraarticular type II collagenase injection for OA induction. Two weeks later, the rabbits were randomized into three groups based on the weekly intraarticular injection to be received: GH, HA, and saline. Injections were done for three consecutive weeks. Evaluation was done at 8 weeks after treatment, clinically using the lameness period, macroscopically using the Yoshimi score and microscopically using the Mankin score. Results: The shortest period of lameness was found in the GH group (15.9±2.12 days), compared to the HA group (19.4±1.72 days) and placebo group (25.0±2.94 days). There was a statistically significant difference in macroscopic scoring between groups (p=0.001) in favor of the GH group. There was also significant difference in the microscopic score between groups (p=0.001) also in favor of the GH group. Conclusions: Intraarticular injection of GH showed better clinical, macroscopic and microscopic results as compared to HA and placebo.

Cartilage defect rarely heals spontaneously since cartilage tissue is poorly vascularized and the... more Cartilage defect rarely heals spontaneously since cartilage tissue is poorly vascularized and the lesion usually does not penetrate to subchondral bone, and hence it does not have access to progenitor cells of bone marrow. Severe cartilage damage may lead to osteoarthritis (OA). Current surgical and non-surgical therapeutic interventions in OA are limited to symptom relief and/or repair of focal lesion, and later a total knee replacement is still necessary. Cell therapy with chondrocyte implantation requires healthy cartilage for donor of the cells. Adult mesenchymal stem cells (MSCs) have the ability to differentiate into chondrogenic lineage. They can readily be isolated from bone marrow as well as many other adult tissues and have an extensive proliferation capacity. Therefore, MSCs may offer a great potential to be developed as an alternative for cell-based articular cartilage therapy.

Cartilage defect rarely heals spontaneously since cartilage tissue is poorly vascularized and the... more Cartilage defect rarely heals spontaneously since cartilage tissue is poorly vascularized and the lesion usually does not penetrate to subchondral bone, and hence it does not have access to progenitor cells of bone marrow. Severe cartilage damage may lead to osteoarthritis (OA). Current surgical and non-surgical therapeutic interventions in OA are limited to symptom relief and/or repair of focal lesion, and later a total knee replacement is still necessary. Cell therapy with chondrocyte implantation requires healthy cartilage for donor of the cells. Adult mesenchymal stem cells (MSCs) have the ability to differentiate into chondrogenic lineage. They can readily be isolated from bone marrow as well as many other adult tissues and have an extensive proliferation capacity. Therefore, MSCs may offer a great potential to be developed as an alternative for cell-based articular cartilage therapy.
