Undifferentiated carcinoma of the esophagus (UEC) is an uncommon esophageal malignancy. Patient w... more Undifferentiated carcinoma of the esophagus (UEC) is an uncommon esophageal malignancy. Patient with UEC has a notably poor prognosis because of extensive metastasis despite at the early stage. A standard treatment regimen for UEC has not yet been established. We reported a case of undifferentiated carcinoma in a 46 years old male with a common chief complaint of progressive dysphagia accompanied by chest discomfort after meal and significant weight loss. Base on the history of illness, physical examination, imaging, and histopathology we concluded that the patient was diagnosed with UEC at the gastroesophageal junction. The patient the received chemoradiotherapy of CDDP + 5-FU along with radiation, but declined the temporary gastrostomy as nutritional intervention. Esophageal undifferentiated carcinomas are rare neoplasms and often associated with locoregional recurrence and/or distant metastasis. Chronic suppurative otitis media (CSOM) is an infection of the middle ear cavity both partially a... more Chronic suppurative otitis media (CSOM) is an infection of the middle ear cavity both partially and totally. It is characterized by ear discharge through a tympanic membrane perforation for over a period of 2 to 6 weeks. Hearing loss is the most common complication of CSOM. One of the degrees of hearing loss in tympanic membrane perforation is depending on the site of perforation, but this premise is still debatable because of pros and contras by some researchers. This study aimed to assess the degree of hearing loss in relation to the site of tympanic membrane perforation. A cross-sectional prospective study design was performed involving 43 patients of safe type CSOM who came to the Department of Otolaryngology Head and Neck Surgery from the period January 2016 to November 2018. All subjects were divided into 4 groups based on the site of perforation. There was a perforation in the posteroinferior, the posterosuperior, the anteroinferior, and the anterosuperior. A statistical analysis using Anova along with multivariate analysis was conducted. Our result showed that the most common site of tympanic membrane perforation was at the anteroinferior (30 samples, 59.8%). The highest hearing threshold was seen at posteroinferior with a mean hearing level of 37.7±2.0 dB, anteroinferior with a mean hearing level of 31.7±0.7 dB, anterosuperior with a mean hearing level 30.7±1.4 dB, and posterosuperior mean hearing level 28.9±1.5 dB. The difference was found significant with p=0.004. Posteroinferior tympanic membrane perforation had a higher number of hearing loss compared to the other sites. In conclusion, the tympanic membrane perforation site has an important role in the hearing level of conductive hearing loss in CSOM.

This rarity makes the diagnosis quite difficult and often associated with delayed diagnosis which may lead to advanced loco-regional disease. The annual incidence of NPC in pediatric age group is around 0.5 per million children ranging between the age of 10-14 years old. 1 This incidence is less than 1% of all pediatric malignant lesions. NPC is extremely rare among children regardless of the different regions in the world, also NPC represents 2% of the head and neck malignant neoplasm in pediatric age group. As it is rare in the pediatric group and its non-specific clinical presentation that caused delayed diagnosis on the basis of the initial thinking of benign etiologies or other common inflammatory disease. 1 CASE REPORT This was a case report of two unusual cases of pediatric nasopharyngeal carcinoma that presented to the outpatient clinic with advanced stage and non-specific clinical presentations. The complexity of the medical and family history and clinical symptoms makes the diagnosis difficult, associating with delayed diagnosis and treatment. Here we studied the clinical presentations, risk factor and the diagnostic challenges in the management of pediatric nasopharyngeal cancer.

Nasopharyngeal carcinoma (NPC) is common in adult and elderly population, however, the incidence ... more Nasopharyngeal carcinoma (NPC) is common in adult and elderly population, however, the incidence among children is still very low worldwide. Due to its rarity and non-specific clinical presentation, the diagnosis in children is often delayed and misinterpreted. The difficulty in reaching the final diagnosis was related to its hidden anatomical location, raising challenges in performing endoscopy and biopsy in children to obtain the precise histopathologic feature. We reported two cases on nasopharyngeal carcinoma in children with two different peculiar complaints. The first patient was 12 years old boy who complained of multiple recurrent unilateral masses on the left neck and history of nosebleeds with an unknown history of the previous histopathology features of the neck masses obtained by complete surgical neck mass removal beforehand. The second patient was 15 years old boy with main complaint of chronic headache, accompanied by doubled vision with apparent family history of malignancy, particularly nasopharyngeal carcinoma. Based on the history of illness, physical examination, imaging, and histopathology, we concluded that both patients were diagnosed with advanced stage nasopharyngeal carcinoma with different types of histopathologic feature.
