ObjectivesWe sought to determine the immunohistochemistry expression of mismatch repair (MMR) and... more ObjectivesWe sought to determine the immunohistochemistry expression of mismatch repair (MMR) and BRAF V600E proteins in sporadic young-onset colorectal cancer (CRC) and their association with clinicopathological features in the Kelantan population.MethodsThis was a cross-sectional study of sporadic young-onset CRC over 11 years from 1 January 2006 to 31 December 2017 in Kelantan. Formalin-fixed paraffin-embedded tissue blocks were immunohistochemically stained with antibodies for MMR (MLH1, MSH2, MSH6, and PMS2) and BRAF V600E. These expressions were correlated with clinicopathological parameters.ResultsOur patient sample included 31 patients with a mean age of 31.5 years. More than half (61.3%) of the patients were women. The majority presented with abdominal pain (41.9%), and 71.0% had a tumor located on the right side of the colon, with 83.9% being moderately differentiated adenocarcinoma. The majority of patients presented at stage IV (54.8%). The most frequent pattern was all MMR protein expressions, which constituted patients in the microsatellite stable group (64.5%). Nine (29.0%) were microsatellite instability (MSI-high), and two (6.5%) were MSI-low. Positive BRAF V600E expression was observed in 83.9% of patients. Only histopathological subtypes revealed a significant association with BRAF V600E positive expression (p = 0.015).ConclusionsThe majority of sporadic young-onset CRC presented with abdominal pain and advanced cancer stage. Most were microsatellite stable, and most cases showed positive expressions in all MMR markers and BRAF V600E by immunohistochemistry method. This finding will pave the way for further research on this disease.

It arises between the muscle fibres and passes through the intermuscular septa. Due to the infiltrative nature of the tumour, recurrence rate is higher than in other subcutaneous lipomas. Complete surgical excision of lipoma is often regarded as a definitive treatment modality and longterm follow-up is mandatory. Here we report a case of a 49-year-old male presenting with huge left neck mass measuring 12 × 10 cm, which was successfully removed. Computed tomography scan of the neck showed a well-defined septated lesion with a fat component in the right sternocleidomastoid muscle, and the histopathology examination confirmed the diagnosis of well-circumscribed intramuscular sternocleidomastoid lipoma. Follow-up after one year showed no evidence of recurrence.

However, some are located in the deeper plane. Location in the head and neck regions are less common compared to the other parts of the body. Deep seated lipomas are either inter-muscular or intramuscular. We present a case of a simple subcutaneous neck lipoma in a 60-year-old Malay man which turned out to be an intramuscular sternocleidomastoid lipoma.

It generally involves the extremities and the trunk but rarely involves the genital area. We report a case of a large fungating infantile fibrosarcoma of the penis in a 2-year-old Malay boy. Partial recovery of the penile structure was achieved after chemotherapy. The difficulty in managing the social and surgical aspect of this case is discussed in our report. To the best of our knowledge, this is the first case report of infantile fibrosarcoma involving the penis in an Asian region. UROLOGY 80: 931-933, 2012.

The common sites for fibrosarcoma are the limbs and axial area. There are 2 main groups of fibrosarcoma— infantile and adult. Currently, fewer than 300 cases of infantile fibrosarcoma have been reported, with even fewer reports involving the penis in the pediatric age group. Most of the reported cases of fibrosarcoma involving the penis have been of adult patients. We report a case of fibrosarcoma involving the proximal half of the penis. Only 5 case reports have been documented in the Indian subcontinent. None of those cases involved the penile area. The largest case series, which included 29 patients with infantile fibrosarcoma of the penis, did not have a patient as young as our patient. 1
