A modified technique for removable afterloading Iridium-192 implantation is described. Under loca... more A modified technique for removable afterloading Iridium-192 implantation is described. Under local or general anesthesia, a series of flexible tygone tubes are introduced into all the tumor area in the rectum. This technique is especially suitable for palliation in high-risk, elderly patients. No rectal tube is necessary in this technique.

BASED on results of retrospective clinical studies, several features associated with colonic canc... more BASED on results of retrospective clinical studies, several features associated with colonic cancer are generally considered to contribute to a less favorable prognosis. Cancers that occur in the young, or in males, and those located distally usually have poorer prognoses. The probability of obtaining five-year survival is also lessened when the tumor is anapiastic with an infiltrating advancing margin, and when there has been extension of the tumor beyond the bowel wall, into lymphatics or into blood vessels. Few cases have been reported, but it appears that the association of pregnancy with colorectal cancer also lessens the chance for cure. Why this should be is not known. Similarly, cancer superimposed on a colitic bowel has a less favorable prognosis. The two most ominous associated features are perforation and obstruction. Two or more unfavorable conditions may coexist. Perforation with obstruction is an especially grave circumstance-immediately and over the long term. Some of the factors that may influence prognosis are listed in Table 1. There is no unanimity regarding all of them, but it is likely that obstruction does have a real influence on outcome-not only because of the immediate threat due to colonic obstruction itself, but also because of an adverse effect on five-year survival. Some obstruction is a frequent comlication of colorectal cancer, most often as a late manifestation. Symptoms of partial obstruction may be present for many months before the bowel slowly becomes cornpletely obstructed. In other cases, long-standing incomplete obstruction may abruptly convert to total obstruction. This is the so-called "acute on chronic" obstruction. Less often, a patient without apparent prior symptoms will have an acute onset of complete obstruction.

The literature is reviewed, drawing attention to the relationship between anterior sacral meningocele, sacral dysgenesis, and other congenital anomalies. Special consideration is given to the clinical features of this entity, as well as to the techniques and results of surgical management.

Twenty-five per cent of patients with diverticulosis will develop diverticulitis; this percentage rises as the follow-up period lengthens. The probability of developing a complication increases with each attack of diverticulitis and when diabetes coexists. The morbidity and mortality (6.5 per cent) associated with these complications remain high. Consequently, elective operation during a quiescent stage of recurrent diverticulitis appears increasingly preferable to the urgent and often multistaged procedures necessary for complicated diverticulitis. Staged procedures—resecting the diseased segment during the first operation, if possible—remain the safest method for treating these complications.

Frequently, aggressive surgical management is rewarding in terms of patient salvage and lessened morbidity. Welch has written, “Procrastination leads to slow death, and in many instances it is necessary to take what seem to be almost superhuman risks to rescue him.”6 We agree with this philosophy.
