At cytological evaluation, in 147 patients with thyroid cancer diagnosis was papillary in 89 (61%), follicular in 44 (30%), and medullary in 5 (3,4%), anaplastic in 9 (6,1%). US criteria and colour flow Doppler examinations of thyroid cancers were: without capsular margin-131 (90,3%), nonhomogeneous echo structure-126 (86,9%), hypoechoic-121 (83,45%), irregular margin-119 (82,1%), non-smooth margin-104 (71,7%), abnormal shape-99 (68,3%), with cystic component-53 (36,55%), microcalcification-36 (24,8%), cervical LN metastasis-15 (10,2%), hypervascular-65%, hypovascular 27,5%, avascular 7,5%. Conclusions: Our study showing that US diagnosis is significant and cost-effective method of diagnosis of thyroid focal lesions. US guided FNAB of thyroid nodules has a high sensitivity (91%).

In early lesions, without accomplished fibrosis, diffuse decreasing of echogenicity can be observed, that is probably caused by inflammatory infiltration. Ultrasonographic image of healthy individual's nail reveals the presence of 3 layers: both external layers (surface and bottom) are hyperechogenic and the inner layer is hypoechogenic. Matrix, in longitudinal section, is triangular shape, anechoic or with few internal echoes. These internal echoes are more intense in toes than in fingers. Nail bed can not be distinguished as a separate layer -it's thickness can be measured indirectly, by assessing the distance from nail plate to the surface of phalanx. The thickness and the vasculature of nail fold can also be easily measured. The ultrasonographic image of psoriatic nails reveals many statistically important differences from image of healthy nails, within nail plate, matrix and nail fold. Above data suggest that ultrasonographic examination may be a valuable dermatologic diagnostic tool that completes classical dermatologic diagnostics and helps to plan the treatment.

Research paper thumbnail of Radiologic-pathologic correlation of 100 consecutive biopsied soft tissue musculoskeletal lesions after multimodality imaging

Before ultrasound (US)-guided core needle biopsy, radiologic imaging diagnosis was done using multimodality imaging. Correlation of histopathologic diagnoses after biopsy and surgical specimen was done. Method: Prospectively, 100 consecutive patients (53 males, 47 females, median age 51 years) with a musculoskeletal soft tissue mass underwent US, computed tomography (CT) and magnetic resonance (MRI). US-guided core needle biopsy was done in all patients. Fifteen MRI parameters and corresponding parameters on US and CT were evaluated for making a radiologic diagnosis. Multivariate logistic regression analysis was used for imaging parameters predictive for malignancy. Histopathologies after biopsy were correlated with radiologic diagnosis in all patients and with the surgical specimen in 92% of the cases. Results: There were 70 benign lesions (53 benign tumors and 17 tumor-like lesions) and 30 malignant lesions(26 malignant tumors and 4 metastases). The overall imaging accuracy for benign and malignant tumors was 92%. Nine cases (9%) were false-positive (4 benign tumors, 1 tumor-like lesion and 4metastases) and 2 (2%) were false-negative. The correlation between imaging staging and histopathology was 63.6% for malignant tumors. MRI had the highest diagnostic accuracy (89%) compared with US (78%) and CT (83%). A diagnostic imaging algorithm was created. Histopathology after biopsy correlated in all surgical specimens. Conclusion: A combination of individual multimodality imaging parameters improved radiologic imaging diagnosis in differentiation between benign and malignant soft tissue musculoskeletal lesions. US-guided core needle biopsy is recommended as the procedure of choice for obtaining representative specimens of soft tissue musculoskeletal tumors for histological examination because of its high diagnostic accuracy and low complication rate.

Methods: This paper is a prospective study conducted over a 3-year period, during which 60 patients with anamnesis for blunt abdominal injuries (BAT) were examined from the total of 60 patients, 39 (65%) were men, 21 (35%) were women, with an average age limit of 2-81 years and an average age of 22,7 years. An US was carried out on all patients immediately after admission, as well as more US exams during the first 24 hours. CT exam was carried out on 44 patients. Results: All patients had abdominal pain. 21 had defans, 17 patients had symptoms from associated injuries to other organs and systems, and 12 patients had a clinical presentation of hemorrhagic shock. The diagnostic methods which are used depend on the hemodycamically stability of the injured person. Out of 60 BAT patients who underwent an US exam, in 56 (93%) of the cases free abdominal fluid was present. In 45 of the cases CT was done, and free fluid was diagnosed in 44 (98%) of the cases. Parenchimal abdominal injury was diagnosed with US in 51 (85%) of the cases, and in 59 (98%) with CT. Conclusions: Negative US finding does not exclude the parenchymal organ injury. In hemodynamically instable patients, an important role plays the application of US, which is fast and simple to perform method which detects the parenchymal abdominal lesion and presence of free fluid (hematoperitoneum), in the abdominal spaces. CT, especially after intravenuos contrast material application, has a leading role in diagnosing of the parenchymal injuries in BAT, in determining the place and the injury expanding. CT diagnosis enables further treatment to be determined.

A 31-year-old male on maintenance treatment with Buprenorphine, came to our Clinic with paralysis of his legs. He also
complained on spinal pains. He couldn’t walk and he was in forced position.
The biochemical analysis showed high values of SE, LDH, globulins, CRP, D-dimer, Tu markers: beta HCG, CYFRA 21-1, presence of paraproteins. The other investigations showed enlarged lymph nodes in the abdomen and small pelvis, thrombosis of v.cava inf and right v. iliaca. The histological finding from the biopsy of the sacrum showed a metastatic process on the skeletal muscles. The patient was treated with buprenorphine, antibiotic therapy, Carbamazepine, anticoagulant therapy and other symtomatic therapy. Despite numerous investigations and treatment, bleeding and
abscesses in the cerebrum occurred. The patient immediately underwent surgery and his recovery was well. He was discharged on his request, but his condition was deteriorated sharply at home and he died in a short time.
There are few studies on malignant diseases found in heroin dependents. It is a challenge to monitor and also to compare the incidence of malignancy between heroin dependents and healthy population
