When the pandemic reached Spain by the end of February 2020, the scarce data about COVID-19 infection in cancer patients pointed out a higher risk of complications due to cancer diagnosis and also to cancer therapies. These conjectures led to concerns about hospital follow-up and cancer therapies of cancer patients. More recent studies have included a higher number of patients, but heterogeneous according to cancer type and tumor stage, with few melanoma patients recorded. Given that different tumor types are associated with specific comorbidities that have a known impact on COVID-19 evolution, analysis of COVID-19 by cancer types is mandatory. Similarly, analysis by tumor stage is relevant, as advanced cases could have different responses to viral infection due to tumor-related immunosuppression and general condition deterioration. The large-scale implementation of primary and secondary skin cancer prevention strategies in rece... more The large-scale implementation of primary and secondary skin cancer prevention strategies in recent decades has led to an increase in the diagnosis of thin melanomas and a decrease in the mean thickness of tumours diagnosed. The number of newly diagnosed thick melanomas, however, has remained stable. To investigate associations between melanoma thickness, clinical presentation and demographic and phenotypic characteristics. The study is based on a cross-sectional study of 1,459 patients with melanoma from a dermatology department at a tertiary hospital in Spain between 2000 and 2017. We analysed associations between median Breslow thickness and demographic, phenotypic, and clinical characteristics, including the method of melanoma detection. Age ≥ 70 years (regression coefficient [RC] = 1.2, 95% CI: 1.1-1.3; p < 0.001), male sex (RC = 0.9, 95% CI: 0.8-0.9; p < 0.001), symptom-based detection (RC = 1.3, 95% CI: 1.1-1.4; p < 0.001), and a history of sunburn at the melanoma site (RC = 0.9, 95% CI: 0.8-0.9; p = 0.04) were all associated with thicker tumours. Melanomas on the lower extremities, by contrast, were significantly thinner (RC = 0.9, 95% CI: 0.8-0.9; p = 0.04). Thick melanomas occur preferentially in older men and show changes such as bleeding or an increase in volume or colour. This information should be incorporated into health training and education programs to design better prevention strategies and minimize diagnostic delays.

The role of lymphocyte subpopulations in IIT melanoma patients remains to be defined. Our objective was to assess different peripheral blood lymphocyte subpopulations, mainly regulatory T cells (Tregs), in melanoma patients who developed IIT. From 30 melanoma patients receiving high-dose interferon (HDI)-alpha 2b (IFN-α2b) treatment, those who developed IIT (IIT patients) were selected and compared with patients who did not develop IIT (Co-MM) and healthy controls (Co-H). Peripheral blood mononuclear cells were obtained before treatment (BT), mid-treatment (MT), end of treatment (ET), 24 weeks post-treatment and at appearance of IIT (TT). Nine patients developed IIT (30%): four Hashimoto&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;#39;s thyroiditis and five destructive thyroiditis. An increase in Tregs was observed in both melanoma groups during HDI treatment. A decrease in CD3(+) , NKT lymphocyte subpopulations and Bcl2 expression on B cells was also observed in both groups. However, no changes were observed in the percentage of CD4(+) , CD8(+) , CD3(+) γδ(+) , CD19(+) , transitional B cells (CD24(high) CD38(high) CD19(+) CD27(-) ), natural killer (NK), invariant NKT (iNKT) lymphocytes and Th1/Th2 balance when BT was compared with ET. At TT, IIT patients had a higher Tregs percentage than Co-MM (P = 0·012) and Co-H (P = 0·004), a higher iNKT percentage than Co-MM (P = 0·011), a higher transitional B cells percentage than Co-H (P = 0·015), a lower CD3(+) percentage than Co-H (P = 0·001) and a lower Bcl2 expression on B cells than Co-H (P &amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;lt; 0·001). Our results point to the immunomodulatory effects of IFN-α on different lymphocyte subpopulations and a possible role of Tregs in melanoma patients who developed IIT.

Cutaneous melanoma (CM) causes more deaths than any other skin tumor, and incidence and mortality... more Cutaneous melanoma (CM) causes more deaths than any other skin tumor, and incidence and mortality rates have risen in recent years, especially in patients of advanced age. There are differences in the biological behavior of CM tumors in the elderly as well as differential management of the disease, evidently influenced by such factors as limited life expectancy, the high incidence of concomitant conditions in older patients, and issues of quality of life unrelated to CM itself. We review relevant current literature on the epidemiology, etiology, pathogenesis, and immunology of CM as well as research on the clinical features, prevention, and management of these tumors in the elderly.

Resumen El tratamiento de eleccion del melanoma primario localizado es la biopsia escisional segu... more Resumen El tratamiento de eleccion del melanoma primario localizado es la biopsia escisional seguida de una ampliacion de margenes en funcion del Indice de Breslow (IB). Las biopsias parciales pueden ser adecuadas en el estudio inicial de melanomas extensos o en localizaciones complejas. Los margenes recomendados para el melanoma in situ son de 0,5-1 cm, aunque parece ser que especialmente en los de tipo lentiginoso, son mas adecuados cuanto mas cercanos al centimetro. En cuanto a los melanomas invasivos, se recomiendan margenes de 1 cm en IB 2 mm. La exeresis en un solo tiempo se puede plantear cuando se desestime la biopsia selectiva de ganglio centinela, pero actualmente no es una actitud avalada por las guias. En aquellos casos en que se debe maximizar la precision de la exeresis completa de la lesion, fundamentalmente en el lentigo maligno facial, son recomendables las tecnicas de control de margenes. The aim of this study was to compare tumour burden in patients who underwent surgery for melanoma... more The aim of this study was to compare tumour burden in patients who underwent surgery for melanoma and cutaneous squamous cell carcinoma during nationwide lockdown in Spain due to COVID-19 (for the period 14 March to 13 June 2020) and during the same dates in 2019 before the COVID-19 pandemic. In addition, associations between median tumour burden (Breslow thickness for melanoma and maximum clinical diameter for cutaneous squamous cell carcinoma) and demographic, clinical, and medical factors were analysed, building a multivariate linear regression model. During the 3 months of lockdown, there was a significant decrease in skin tumours operated on (41% decrease for melanoma (n = 352 vs n = 207) and 44% decrease for cutaneous squamous cell carcinoma (n = 770 vs n = 429)) compared with the previous year. The proportion of large skin tumours increased. Fear of SARS-CoV-2 infection, and detection of the lesion by the patient or doctor, were related to thicker melanomas; and fear of being diagnosed with cancer, and detection of the lesion by the patient or relatives, were related to greater cutaneous squamous cell carcinoma. In conclusion, lockdown due to COVID-19 has resulted in a reduction in treatment of skin cancer.

Abstract Recent publication of the results of clinical trials in which lymph node dissection was ... more Abstract Recent publication of the results of clinical trials in which lymph node dissection was not associated with any survival benefit in patients with sentinel node metastasis makes it necessary to reconsider the treatment of patients with melanoma. This article provides an update on the available evidence on the diverse factors (routes of metastatic spread, predictors, adjuvant therapy, etc.) that must be considered when treating patients with sentinel node-positive melanoma. The authors propose a decision-making algorithm for use in this clinical setting. The current evidence no longer supports lymph node dissection in patients with low-risk sentinel node metastasis (sentinel node tumor load ≤ 1 mm).

The widespread use of anti-programmed cell death receptor-1 (PD-1) agents has shed light to unusu... more The widespread use of anti-programmed cell death receptor-1 (PD-1) agents has shed light to unusual immune-related adverse effects, especially affecting the skin. We report a case of bullous pemphigoid secondary to nivolumab therapy for metastatic renal carcinoma with a previously unreported complete response to clobetasol ointment alone. The autoimmune blistering disease was successfully treated without oral corticosteroids, and the anti-PD-1 agent could be maintained without recurrence of the skin lesions. Topical therapy remains a good option in selected, mild-to-moderate cases of induced bullous pemphigoid.

Resumen La reciente publicacion de los resultados de ensayos clinicos en los que la diseccion gan... more Resumen La reciente publicacion de los resultados de ensayos clinicos en los que la diseccion ganglionar no ha demostrado beneficio de supervivencia en pacientes con metastasis en el ganglio centinela plantea la necesidad de modificar el tratamiento del paciente con melanoma. El presente trabajo aporta una actualizacion de la evidencia sobre diferentes aspectos necesarios (vias de progresion metastasica, factores predictores, tratamiento adyuvante, etc.) para la toma de decisiones en el paciente con melanoma y metastasis en el ganglio centinela y plantea un algoritmo de toma de decisiones para este escenario clinico. La evidencia actualmente disponible respalda el abandono de la diseccion ganglionar en aquellos pacientes con metastasis de bajo riesgo en el ganglio centinela (carga tumoral en el ganglio centinela inferior o igual a 1 mm).

Ingenol mebutato también mostró una mayor eficacia frente a imiquimod 5%, con 0,535 vs. 0,503 aclaramientos ganados, y unos costes totales de 551,50 D vs. 527,89 D , siendo la relación coste-efectividad incremental resultante de 728,64 D /aclaramiento adicional. Conclusiones: Ingenol mebutato resultó ser una estrategia dominante vs. diclofenaco, y eficiente, es decir, presentó mayor efectividad y mayores costes (relación coste-utilidad incremental inferior a 30.000 D /AVAC) vs. imiquimod.

La evidencia actualmente disponible respalda el abandono de la diseccion ganglionar en aquellos pacientes con metastasis de bajo riesgo en el ganglio centinela (carga tumoral en el ganglio centinela inferior o igual a 1 mm).

Ingenol mebutato también mostró una mayor eficacia frente a imiquimod 5%, con 0,535 vs. 0,503 aclaramientos ganados, y unos costes totales de 551,50 D vs. 527,89 D , siendo la relación coste-efectividad incremental resultante de 728,64 D /aclaramiento adicional. Conclusiones: Ingenol mebutato resultó ser una estrategia dominante vs. diclofenaco, y eficiente, es decir, presentó mayor efectividad y mayores costes (relación coste-utilidad incremental inferior a 30.000 D /AVAC) vs. imiquimod.
