Early diagnosis of this disease is essential to provide effective preventive and therapeutic measures. This process can be used by a computer-aided methodology to improve accuracy. In this study, a new and optimal method has been utilized for the diagnosis of COVID-19. Here, a method based on fuzzy C-ordered means (FCOM) along with an improved version of the enhanced capsule network (ECN) has been proposed for this purpose. The proposed ECN method is improved based on mayfly optimization (MFO) algorithm. The suggested technique is then implemented on the chest X-ray COVID-19 images from publicly available datasets. Simulation results are assessed by considering a comparison with some state-of-the-art methods, including FOMPA, MID, and 4S-DT. In addition to the common symptoms of the virus, numerous reports of rare symptoms of the virus have also been published. These atypical presentations can result in difficulty in diagnosing the disease. The aim of this work is to summarize “uncommon atypical presentations”, which have not received enough attention in descriptions of the disease presentation to date and the authors specifically discuss the important uncommon atypical presentations of COVID-19.

Methods: In this double-blinded two-arm parallel-group randomized trial, 124 patients undergoing laparotomy with midline incision were assessed from March 2019 to September 2019 at Imam Hossein Medical Center, Tehran, Iran. Its clinical symptoms are heterogenous and depend on the location and size of the tumor. Inflammatory fibroid polyps are one of the rare benign conditions leading to intestinal intussusception in adults.Case presentation: A 76-year-old woman presented to the emergency department with acute abdominal pain, vomiting, nausea and failure in defecation and gas passing. Physical examination revealed abdominal distension. Abdominal X-ray showed dilated small bowel segments with air-fluid levels. Operative finding was an ileo-ileal intussusception due to a mass lesion at 40 cm proximal to the cecum. All the patients between 18 and 65 years of age with complaints of abdominal pain were included into the study. In this study, we have investigated the effect of N-acetylcysteine on neo-intimal hyperplasia after arteriovenous fistula creation in rats. Methods: This study was conducted in 24 rats which were randomly divided into two groups: control and N-acetylcysteine groups. An end-to-side anastomosis was made between the femoral artery and vein. The control group received distilled water intraperitoneally while the animals in N-acetylcysteine group received 300 mg/kg/day of N-acetylcysteine via the same route. This study was performed to compare the efficacy of preoperative magnetic resonance cholangiopanc... more This study was performed to compare the efficacy of preoperative magnetic resonance cholangiopancreatography (MRCP) and intra-operative cholangiography (IOC) methods in patients suspicious to gall stones. Background: According to previous studies, it is recommended that common bile duct investigation should be done in order to rule out choledocholithiasis in all patients with symptomatic cholelithiasis. IOC is an invasive procedure with probable complications, it would seem that MRCP could replace the direct cholangiography. Patients and methods: In a diagnostic clinical trial, Fifty-nine patients with symptomatic biliary stones or cholecystitis were recruited in this study. The included patients had normal size biliary ducts in sonography but high serum alkaline phosphatase level. Preoperative MRCP and IOC were performed for the patients and the obtained results were analyzed and compared. Results: The positive predictive value for IOC was 88% and for MRCP was 43%. The diagnostic accuracy of IOC and MRCP were 98% and 85% respectively, suggesting that IOC is much more diagnostically accurate. There were no significant difference in specificity and sensitivity of these two methods. Conclusion: According to the results, we can conclude that MRCP may not obviate the need for IOC. The suggestion for routine use of MRCP instead of IOC and as a substitution of that procedure needs further investigations on more patients.

The aim of this study was to analyze the long-term outcome of surgical treatment of patients with splenic echinococcosis comparing spleen-preserving surgery with splenectomy. Materials and Methods: During a period of 10 years (1996-2006), 20 patients with splenic echinococcosis entered our study as test and control groups. In our series the spleen was the only organ involved. Splenectomy was performed in the control group and spleen preserving drainage of cyst in test group. Results: There was no significant difference between the splenectomy and spleen-preserving groups concerning median hospital stay, postoperative complication rate and recurrence. ABSTRACT Zusammenfassung GRUNDLAGEN: Patienten an der chronischen Hämodialyse benötigen einen adä... more ABSTRACT Zusammenfassung GRUNDLAGEN: Patienten an der chronischen Hämodialyse benötigen einen adäquaten Dialysezugang durch eine AV-Fistel, arteriovenöse Interponate an den oberen oder unteren Extremitäten oder perkutan eingebrachte Dialysekatheter. Zielsetzung der folgenden Untersuchung war es, autologe Vena saphena magna mit PTFE als Gefäßersatzmaterial bei Oberschenkelshunts zu vergleichen. METHODIK: 70 Patienten wurden im Rahmen einer prospektiv randomisierten Studie in 2 Gruppen untersucht. In beiden Gruppen wurden die Oberschenkelshunts in Schleifenkonfiguration angelegt. In der einen Gruppe wurde autologe Vena saphena magna, in der anderen Gruppe PTFE als Gefäßersatzmaterial verwendet. Im Rahmen des Follow-up wurden in definierten Abständen die Shuntfunktion mittels Dopplersonographie und das Auftreten von Komplikationen nachuntersucht. ERGEBNISSE: In Bezug auf die Shuntfunktion, dem Auftreten von postoperativen Shuntthrombosen und die Durchflussraten konnte kein statistisch signifikanter Unterschied zwischen den beiden Gruppen festgestellt werden (p p &lt; 0,05). SCHLUSSFOLGERUNGEN: Zusammenfassend ist die autologe Vena Saphena magna, aufgrund der signifikant niedrigeren Infektionsrate, eine gute Alternative zu PTFE als Gefäßersatzmaterial für Oberschenkelshunts.

In this study, we have investigated the effect of N-acetylcysteine on neo-intimal hyperplasia after arteriovenous fistula creation in rats. Methods: This study was conducted in 24 rats which were randomly divided into two groups: control and N-acetylcysteine groups. An end-to-side anastomosis was made between the femoral artery and vein. The control group received distilled water intraperitoneally while the animals in N-acetylcysteine group received 300 mg/kg/day of N-acetylcysteine via the same route. After 28 days, the thickness of intima and media was measured using hematoxylin and eosin. This study was performed to compare the efficacy of preoperative magnetic resonance cholangiopanc... more This study was performed to compare the efficacy of preoperative magnetic resonance cholangiopancreatography (MRCP) and intra-operative cholangiography (IOC) methods in patients suspicious to gall stones. Background: According to previous studies, it is recommended that common bile duct investigation should be done in order to rule out choledocholithiasis in all patients with symptomatic cholelithiasis. IOC is an invasive procedure with probable complications, it would seem that MRCP could replace the direct cholangiography. Patients and methods: In a diagnostic clinical trial, Fifty-nine patients with symptomatic biliary stones or cholecystitis were recruited in this study. The included patients had normal size biliary ducts in sonography but high serum alkaline phosphatase level. Preoperative MRCP and IOC were performed for the patients and the obtained results were analyzed and compared. Results: The positive predictive value for IOC was 88% and for MRCP was 43%. The diagnostic accuracy of IOC and MRCP were 98% and 85% respectively, suggesting that IOC is much more diagnostically accurate. There were no significant difference in specificity and sensitivity of these two methods. Conclusion: According to the results, we can conclude that MRCP may not obviate the need for IOC. The suggestion for routine use of MRCP instead of IOC and as a substitution of that procedure needs further investigations on more patients.

The aim of this study was to analyze the long-term outcome of surgical treatment of patients with splenic echinococcosis comparing spleen-preserving surgery with splenectomy. Materials and Methods: During a period of 10 years (1996-2006), 20 patients with splenic echinococcosis entered our study as test and control groups. In our series the spleen was the only organ involved. Splenectomy was performed in the control group and spleen preserving drainage of cyst in test group. Results: There was no significant difference between the splenectomy and spleen-preserving groups concerning median hospital stay, postoperative complication rate and recurrence. (p<0.5) The median follow-up in 15 patients was 52 (range 6-300) months. ABSTRACT Zusammenfassung GRUNDLAGEN: Patienten an der chronischen Hämodialyse benötigen einen adä... more ABSTRACT Zusammenfassung GRUNDLAGEN: Patienten an der chronischen Hämodialyse benötigen einen adäquaten Dialysezugang durch eine AV-Fistel, arteriovenöse Interponate an den oberen oder unteren Extremitäten oder perkutan eingebrachte Dialysekatheter. Zielsetzung der folgenden Untersuchung war es, autologe Vena saphena magna mit PTFE als Gefäßersatzmaterial bei Oberschenkelshunts zu vergleichen. METHODIK: 70 Patienten wurden im Rahmen einer prospektiv randomisierten Studie in 2 Gruppen untersucht. In beiden Gruppen wurden die Oberschenkelshunts in Schleifenkonfiguration angelegt. In der einen Gruppe wurde autologe Vena saphena magna, in der anderen Gruppe PTFE als Gefäßersatzmaterial verwendet. Im Rahmen des Follow-up wurden in definierten Abständen die Shuntfunktion mittels Dopplersonographie und das Auftreten von Komplikationen nachuntersucht. ERGEBNISSE: In Bezug auf die Shuntfunktion, dem Auftreten von postoperativen Shuntthrombosen und die Durchflussraten konnte kein statistisch signifikanter Unterschied zwischen den beiden Gruppen festgestellt werden (p p &lt; 0,05). SCHLUSSFOLGERUNGEN: Zusammenfassend ist die autologe Vena Saphena magna, aufgrund der signifikant niedrigeren Infektionsrate, eine gute Alternative zu PTFE als Gefäßersatzmaterial für Oberschenkelshunts.
