Aims-Evaluation of dysplasia in long standing ulcerative colitis is a diYcult and often subjectiv... more Aims-Evaluation of dysplasia in long standing ulcerative colitis is a diYcult and often subjective task. Therefore, the aim of this study was to search for a more objective parameter to help distinguish regenerative changes from epithelial dysplasia. Methods-A total of 97 sections from colectomy specimens from 12 patients with ulcerative colitis of more than 10 years duration were stained immunohistochemically with MIB 1 to detect diVerences in the frequency and pattern of nuclei positive for the proliferation marker Ki-67. All patients had epithelial dysplasia in one or more areas (high grade dysplasia, n = 16; low grade dysplasia, n = 15; indefinite for dysplasia, n = 16), and three patients had additional adenocarcinoma (one Dukes's C multifocal, mucinous carcinoma; one Dukes's C adenocarcinoma in the sigmoid; and one Dukes's A adenocarcinoma in the caecum). Two patients had adenomas-one had an 8 cm villous adenoma with intramucosal carcinoma, and the other had a 4 cm tubulovillous adenoma with high grade dysplasia. Results-There were highly significant diVerences between the percentages of Ki-67 immunopositive cells in low grade and high grade dysplasia and carcinoma compared with regenerative epithelium. In high grade dysplasia and carcinoma, the distribution of Ki-67 positive cells was diVuse throughout the full length of the crypt, whereas low grade dysplasia and epithelium indefinite for dysplasia, as well as regenerative epithelium, showed an expanded basal zone. Conclusions-Assessment of the number of Ki-67 immunostained cells is of additional value in deciding whether the mucosa is regenerative or dysplastic, and the MIB 1 staining pattern is characteristic for most lesions with high grade dysplasia and carcinoma. Therefore, this technique could be combined with routine histological evaluation of colorectal epithelium being examined for dysplasia.

Objective: To study the effects of monopolar diathermy on the bile ducts in pigs. Design: Experim... more Objective: To study the effects of monopolar diathermy on the bile ducts in pigs. Design: Experimental study. Setting: University hospital, Norway. Material: 18 pigs. Interventions: Laparotomy, application of diathermy at standard sites along the cystic duct, the bile ducts, and a cystic duct containing a metal clip, 3 to 12 times of 5 seconds' duration at each site. Temperature was subsequently recorded at standard measurement points on the bile ducts. Twelve pigs were killed after three weeks for assessment of the bile ducts at necropsy. Main outcome measurements: Increase in temperature in the bile duct walls and late changes in the bile ducts. Results: Temperature increased by 4-6°C during 6 of 330 diathermy applications along the cystic duct, by 4-18°C in 8 of 126 applications along the common bile duct, and by 4-11°C at the clip in 9 of 54 applications. There were no macroscopic or microscopic changes in the bile ducts. Conclusion: Monopolar diathermy induced unexpected distant increases in the temperature of the bile duct walls and at a clip on the cystic duct probably because diathermy current energy was distributed along channels of high current conductivity.

Forty-four consecutive patients operated on with ileal J-pouch-anal anastomosis (IPAA) and divert... more Forty-four consecutive patients operated on with ileal J-pouch-anal anastomosis (IPAA) and diverting ileostomy were examined with barium contrast medium of the pouch before closure of the ileostomy. CT was performed in 4 of the patients. The anatomy of the ileal reservoir as well as complications were assessed. As normal postoperative anatomy we found a "blind loop" at the upper part of the reservoir in 29 patients and a contrast lucency at the anastomosis between the anal channel and the pouch in 5 patients. Complications were revealed at barium contrast medium examinations in 13 patients, including stenoses at the anastomosis between the pouch and the anal channel in 8 patients and fistulas in 5 patients. CT was especially valuable in the exact diagnosis and location of a pelvic abscess in 3 patients, none of which was shown at pouchography.

Development of resistance against the highly toxic Cd"ion has been described earlier both in anim... more Development of resistance against the highly toxic Cd"ion has been described earlier both in animals, cell cultures and in bacteria. In this paper we have tested if decreased intracellular concentration of Cd plays a role in resistance to Cd in three mammalian cell cultures. The cells originated from different tissue and from two different species. Cd-resistant and corresponding "wild-type" strains were used. The results from all three lines show essentially the same, namely that there is no significant difference in Cd content between the Cd-resistant and the non-resistant cells. The resistant cells from all three lines grow continuously with a cellular Cd-content which is several times higher than that killing all cells of the non-resistant "wild-type".

Characterization of a human epithelial cell line with special reference to its ultrastructure. Ac... more Characterization of a human epithelial cell line with special reference to its ultrastructure. Acta path. microbiol. scand. Sect. A. 89: 73-80, 1981. A commercially available cell line (NCTC 2544) originating from presumably normal human skin was chosen as an in virro model system for subsequent studies of the effects of different agents on human epithelial cells. The cell line. therefore, was ultrastructurally and otherwise characterized at intervals by techniques which allow standardized controls of the model. The cell line was classified as epithelial both by phase contrast microscopy and transmission electron microscopy. The cells were polygonal. fully developed desmosomes were demonstrated and no extracellular filamentous material was observed. Specific epidermal markers like keratohyalin granules or keratinosomes were not demonstrated.

PURPOSE: This study was designed to evaluate the effect of biofeedback and electrostimulation in ... more PURPOSE: This study was designed to evaluate the effect of biofeedback and electrostimulation in a randomized, clinical trial for the treatment of patients with postdelivery anal incontinence. METHODS: Forty-nine females who sustained third-degree or fourth-degree perineal rupture with a mean age of 36 (range, 22-44) years were included in the study. The females were randomized to biofeedback or electrostimulation treatment. Forty females completed the study: 19 in the biofeedback and 21 in the electrostimulation group. Biofeedback or electrostimulation sessions were performed two times daily for eight weeks in each group. Wexner incontinence score, fecal incontinence quality of life scores, and reduced quality of life on visual analog scale were registred before and after treatment. Patients_ selfrating of treatment effect also was registered in both groups. The primary outcome measure was the Wexner incontinence score. RESULTS: There were no differences in treatment effect between groups. Comparing pretreatment status to posttreatment in each group showed no improvement in Wexner score, reduced quality of life, or any of the fecal incontinence quality of life scores. Patients_ self-rating of the treatment effect, however, showed a subjective improvement of symptoms both in the biofeedback and in the electrostimulation group (median, 7 vs. 5.) CONCLU-SIONS: This study shows that there was no difference in effect between biofeedback and electrostimulation. Neither biofeedback nor electrostimulation treatments improved Wexner incontinence score, reduced quality of life, or fecal incontinence quality of life scores. Both treatments resulted in improvement of patients_ subjective perception of incontinence control. [

Cultured cells with elevated levels of cytoplasmic metalloidionein (MT), a small-molecular-weight... more Cultured cells with elevated levels of cytoplasmic metalloidionein (MT), a small-molecular-weight protein extremely rich in cysteine. showed resistance against the alkylating drug chlorambucil. One human epithelial line (HE) and one mouse fibroblast line (CI 1D) were used, as well as substrains of both cell lines (HE10oand CI 1D10o).The substrains contain large amounts of MT. Dose-survival curves obtained from cloning studies with the murine cells after exposure to Chlorambucil for 1 hr revealed that CI 1Dioo was approximately 3-fold more resistant than were CI 1D cells (Do = 32.9 and 11.6 /tg/ml, respectively); this difference was highly significant (p < 0.001 ; f test). In growth rate studies, both MT-containing strains showed resistance against Chloram bucil over the total dose range of 0.5 to 100 u.g/ml compared to the parent strains. After 4 days treatment with 25^g/ml, the growth inhibition of CI 1D was 67%, but only 13% in CI 1D100-For HE and HEioo, 50 u.g/m\ gave growth inhibitions of 83 and 65%, respectively. Following 1-and 24-hr incubations of cells with [14C]chlorambucil and subsequent ultracentrifugation and gel filtration of the cytosols, about 20 to 40% of the 14C-derived radioactivity coeluted with the MT from the resistant strains. The results indicate that intracellular MT sequesters Chlorambucil or its toxic metabolites and that the protein thus contributes to the resistance against this drug.

We report a ten-year series of 18 patients treated for acute acalculous cholecystitis. 12 were ma... more We report a ten-year series of 18 patients treated for acute acalculous cholecystitis. 12 were males and six females, ranging in age from 23 to 77 years, with a mean of 64. 13 underwent major surgical procedures, two for multiple injuries. An additional four patients were also treated for multiple injuries. Clinical knowledge and a critical attitude are essential for the diagnosis. Blood tests are unreliable, since the result may be affected by other complications or associated diseases. It is therefore necessary to use diagnostic imaging. The treatment of choice is percutaneous bile drainage.

Massive upper gastrointestinal haemorrhage may be caused by ulcus simplex Dieulafoy. Of 144 patie... more Massive upper gastrointestinal haemorrhage may be caused by ulcus simplex Dieulafoy. Of 144 patients consecutively operated on for benign gastric ulcer in 1975-1980, 29 were admitted as emergencies because of massive bleeding from the stomach. Six had the Dieulafoy lesion. Identification of the source of bleeding was made by fibreoptic endoscopy in five of these cases, while wide gastrotomy was required in one case. All were successfully treated, one with gastroscopic coagulation and the others surgically. Hitherto there has been no recurrence of bleeding. Diagnostic and therapeutic problems are described. The Dieulafoy lesion may be life-threatening. A century after it was first reported, reminder of the lesion is still necessary, since it probably is more rarely recognized than rare.

The diagnostic efficacy of routine double-contrast barium enema and fiberoptic coloscopy for dete... more The diagnostic efficacy of routine double-contrast barium enema and fiberoptic coloscopy for detection of cancer was retrospectively studied in 303 patients consecutively operated for colorectal adenocarcinoma in a 52-month period from January 1980. Double-contrast barium enema was performed in 180 patients with 184 carcinomas, 157 (85%) of which were revealed by this examination. The detection rate of carcinoma according to site was 89% between cecum and descending colon, 92% in sigmoid colon and 71% in rectum. Fiberoptic colonoscopy was done in 176 patients with 181 carcinomas, 163 (90%) of which were detected by the endoscopical examination. The detection rate of carcinoma according to site was 86% between cecum and descending colon, 90% in sigmoid colon and 95% in rectum. The two methods were equally effective in detecting carcinoma of the colon. Colonoscopy was superior to barium enema in detection of rectal carcinoma.

Open clinical study. University hospital, Norway. 8 consecutive patients with colostomies (n = 2) or urinary stomas (n = 6) who required treatment with liposuction above or around the stoma, mainly for leakage. Syringe-assisted liposuction under local anaesthesia. Change in stoma function, particularly reduction in the incidence of leakage, and by the patients&#39; satisfaction graded according to a four point scale. All patients noted considerable improvements in stoma function, or abdominal contour, or both. Troublesome colostomies and urinary stomas can in selected patients be successfully treated with liposuction above or around the stoma.

Appendectomies can be performed laparoscopically. The diagnosis of appendicitis is established and the inflamed organ removed directly. The laparoscopic technique reduces the incidence of unnecessary appendectomies and corresponding morbidity. Compared with conventional appendectomy, laparoscopy leads to less post-operative pain and inability to work. We discuss the first nine patients we treated in this way.

Diverticuloplasty was performed on five patients, four of whom had biliary or pancreatic duct obstruction and also underwent cholecystectomy and sphincteroplasty. The fifth patient had chronic abdominal pain. Complications occurred in three cases--postoperative diverticular bleeding, retroperitoneal hematoma and peroperative perforation of the common bile duct. The long-term results (3-10 years) were excellent in all cases.

Seventy-five patients with advanced abdominal carcinoid tumors (65 midgut, 10 others) have been e... more Seventy-five patients with advanced abdominal carcinoid tumors (65 midgut, 10 others) have been examined retrospectively to evaluate the role of surgical treatment as a principle, irrespective of stage of disease. Eighteen of 52 patients (35%) exhibited the carcinoid syndrome. Two or more primaries were found in 39% of patients with midgut lesion, 81% of these patients had regional metastases, 5% of these patients had distant lymph node metastases, and 74% of the patients had liver secondaries. All patients underwent operation, an additional 34% of the patients had a further reoperation, 9% of the patients had a second reoperation, 3% of the patients had a third reoperation, and one patient (2%) had a fourth reoperation. Intraoperative debulking (liver excluded) was performed in 33% of the patients, and 48% of the patients had treatment (resection, hepatic artery ligation, embolization) directed at the liver. Twelve patients are presented here. In five patients with confirmed or suspected pancreatic malignancy, pain and diarrhea subsided or disappeared, and two patients had a temporary gain in weight. Four patients with chronic pancreatitis all experienced relief of symptoms. One patient with a pancreatic abscess improved temporarily. A postoperative pancreatic fistula healed within two weeks. One patient with a stone in the pancreatic duct became temporarily free from pain. Two patients had minor complications. Efficiency of pancreatic drainage is difficult to evaluate. However, all our patients seemed to benefit from the treatment.

Cultured cells with elevated levels of cytoplasmic metalloidionein (MT), a small-molecular-weight... more Cultured cells with elevated levels of cytoplasmic metalloidionein (MT), a small-molecular-weight protein extremely rich in cysteine. showed resistance against the alkylating drug chlorambucil. One human epithelial line (HE) and one mouse fibroblast line (CI 1D) were used, as well as substrains of both cell lines (HE10oand CI 1D10o).The substrains contain large amounts of MT. Dose-survival curves obtained from cloning studies with the murine cells after exposure to Chlorambucil for 1 hr revealed that CI 1Dioo was approximately 3-fold more resistant than were CI 1D cells (Do = 32.9 and 11.6 /tg/ml, respectively); this difference was highly significant (p < 0.001 ; f test). In growth rate studies, both MT-containing strains showed resistance against Chloram bucil over the total dose range of 0.5 to 100 u.g/ml compared to the parent strains. After 4 days treatment with 25^g/ml, the growth inhibition of CI 1D was 67%, but only 13% in CI 1D100-For HE and HEioo, 50 u.g/m\ gave growth inhibitions of 83 and 65%, respectively. Following 1-and 24-hr incubations of cells with [14C]chlorambucil and subsequent ultracentrifugation and gel filtration of the cytosols, about 20 to 40% of the 14C-derived radioactivity coeluted with the MT from the resistant strains. The results indicate that intracellular MT sequesters Chlorambucil or its toxic metabolites and that the protein thus contributes to the resistance against this drug.

We report a ten-year series of 18 patients treated for acute acalculous cholecystitis. 12 were ma... more We report a ten-year series of 18 patients treated for acute acalculous cholecystitis. 12 were males and six females, ranging in age from 23 to 77 years, with a mean of 64. 13 underwent major surgical procedures, two for multiple injuries. An additional four patients were also treated for multiple injuries. Clinical knowledge and a critical attitude are essential for the diagnosis. Blood tests are unreliable, since the result may be affected by other complications or associated diseases. It is therefore necessary to use diagnostic imaging. The treatment of choice is percutaneous bile drainage.

Of 144 patients consecutively operated on for benign gastric ulcer in 1975-1980, 29 were admitted as emergencies because of massive bleeding from the stomach. Six had the Dieulafoy lesion. Identification of the source of bleeding was made by fibreoptic endoscopy in five of these cases, while wide gastrotomy was required in one case. All were successfully treated, one with gastroscopic coagulation and the others surgically. Hitherto there has been no recurrence of bleeding. Diagnostic and therapeutic problems are described. The Dieulafoy lesion may be life-threatening. A century after it was first reported, reminder of the lesion is still necessary, since it probably is more rarely recognized than rare.

The diagnostic efficacy of routine double-contrast barium enema and fiberoptic coloscopy for dete... more The diagnostic efficacy of routine double-contrast barium enema and fiberoptic coloscopy for detection of cancer was retrospectively studied in 303 patients consecutively operated for colorectal adenocarcinoma in a 52-month period from January 1980. Double-contrast barium enema was performed in 180 patients with 184 carcinomas, 157 (85%) of which were revealed by this examination. The detection rate of carcinoma according to site was 89% between cecum and descending colon, 92% in sigmoid colon and 71% in rectum. Fiberoptic colonoscopy was done in 176 patients with 181 carcinomas, 163 (90%) of which were detected by the endoscopical examination. The detection rate of carcinoma according to site was 86% between cecum and descending colon, 90% in sigmoid colon and 95% in rectum. The two methods were equally effective in detecting carcinoma of the colon. Colonoscopy was superior to barium enema in detection of rectal carcinoma.

Open clinical study. University hospital, Norway. 8 consecutive patients with colostomies (n = 2) or urinary stomas (n = 6) who required treatment with liposuction above or around the stoma, mainly for leakage. Syringe-assisted liposuction under local anaesthesia. Change in stoma function, particularly reduction in the incidence of leakage, and by the patients&#39; satisfaction graded according to a four point scale. All patients noted considerable improvements in stoma function, or abdominal contour, or both. Troublesome colostomies and urinary stomas can in selected patients be successfully treated with liposuction above or around the stoma.

Appendectomies can be performed laparoscopically. The diagnosis of appendicitis is established and the inflamed organ removed directly. The laparoscopic technique reduces the incidence of unnecessary appendectomies and corresponding morbidity. Compared with conventional appendectomy, laparoscopy leads to less post-operative pain and inability to work. We discuss the first nine patients we treated in this way.

Diverticuloplasty was performed on five patients, four of whom had biliary or pancreatic duct obstruction and also underwent cholecystectomy and sphincteroplasty. The fifth patient had chronic abdominal pain. Complications occurred in three cases--postoperative diverticular bleeding, retroperitoneal hematoma and peroperative perforation of the common bile duct. The long-term results (3-10 years) were excellent in all cases.

Seventy-five patients with advanced abdominal carcinoid tumors (65 midgut, 10 others) have been e... more Seventy-five patients with advanced abdominal carcinoid tumors (65 midgut, 10 others) have been examined retrospectively to evaluate the role of surgical treatment as a principle, irrespective of stage of disease. Eighteen of 52 patients (35%) exhibited the carcinoid syndrome. Two or more primaries were found in 39% of patients with midgut lesion, 81% of these patients had regional metastases, 5% of these patients had distant lymph node metastases, and 74% of the patients had liver secondaries. All patients underwent operation, an additional 34% of the patients had a further reoperation, 9% of the patients had a second reoperation, 3% of the patients had a third reoperation, and one patient (2%) had a fourth reoperation. Intraoperative debulking (liver excluded) was performed in 33% of the patients, and 48% of the patients had treatment (resection, hepatic artery ligation, embolization) directed at the liver. Twelve patients are presented here. In five patients with confirmed or suspected pancreatic malignancy, pain and diarrhea subsided or disappeared, and two patients had a temporary gain in weight. Four patients with chronic pancreatitis all experienced relief of symptoms. One patient with a pancreatic abscess improved temporarily. A postoperative pancreatic fistula healed within two weeks. One patient with a stone in the pancreatic duct became temporarily free from pain. Two patients had minor complications. Efficiency of pancreatic drainage is difficult to evaluate. However, all our patients seemed to benefit from the treatment.
