Results: Serum MIP-1alpha/CCL3 levels were significantly higher in cirrhotic patients with and wi... more Results: Serum MIP-1alpha/CCL3 levels were significantly higher in cirrhotic patients with and without HCC than in healthy subjects and in HCC patients than in patients without HCC (P < 0.001). By plotting receiver-operating characteristic curve, the sensitivity and specificity of serum MIP-1alpha/CCL3 in the diagnosis of HCC were 100% and 93.3% respectively at a cutoff value of 17.5 pg/ml. HCV-related HCCs showed significant increases in MIP-1alpha/CCL3 expression, TAM count and MVD compared with adjacent non-neoplastic liver tissues (P < 0.001). The MIP-1alpha/CCL3 expression in HCCs showed positive correlations with serum MIP-1alpha/CCL3 levels, tumor size, stage and histological grade, TAM count and MVD (P < 0.05). Conclusion: The CC chemokine MIP-1alpha/CCL3 plays an important role in the development and progression of HCV-related HCC possibly, through migration of macrophages to tumor microenvironment and enhancement of angiogenesis. MIP-1alpha/ CCL3 may serve as a potential serum biological marker and a useful therapeutic target for HCC.

A healthy young adult male patient presented with frequent but formed motions and excess of mucou... more A healthy young adult male patient presented with frequent but formed motions and excess of mucous in stool for 3 months. He did not give history of recent diarrhea, blood in stool, straining, weight loss, or pain abdomen. He was reassured regarding benign and prevalent nature of his complaint and advised mild antispasmodic. Although symptoms improved marginally, he continued to pass excessive mucous. Stool examination was negative for ova, cyst, and red blood cell and contained excess of mucous clumps. He was advised to undergo colonoscopy to exclude organic pathology, considering anxiety of the patient. Colonic mucosa was normal up to terminal ileum. Multiple club-shaped organisms, 8-10 mm in size, with a circular body and mobile proboscis were seen in the cecum of colon [Figure 1]. Proboscises which were later identified as suckers were actively stretched more than twice the body diameter. Organisms were collected in saline and formalin separately. It was later identified as Gastrodiscoides hominis, an intestinal fluke. The patient was treated with praziquantel; he did not seem to respond clinically. He refused repeat colonoscopy and was lost to follow-up. Trematodes or flukes as these are commonly called enter human body through food or skin. Trematodes are unsegmented thick leaf-shaped worms that are slightly flattened dorsoventrally; they bear 2 suckers, one in ventral aspect of body and other on the proboscis, which serve as organs of attachment [Figure 2]. Human intestine is infected when metacercariae (encysted form residing inside snail body) is ingested with freshwater aquatic plants. Gastrodiscoides hominis is an intestinal fluke. Cercaria (the free-swimming infective form) may penetrate skin to infect blood vessels (e.g., schistosomiasis), liver (e.g., fascioliasis), pancreas (e.g., eurytrema pancreaticum), and lung (e.g., paragonimiasis). [1] Exact burden of intestinal flukes in human population is not known. [2] A few case reports of human infestation are available outside Southeast Asia. Background & Aim: Nonalcoholic fatty liver disease (NAFLD) is increasing at alarming rates in sou... more Background & Aim: Nonalcoholic fatty liver disease (NAFLD) is increasing at alarming rates in south Indian population, the correlation of BMI with skin fold thickness and total body fat in patients with NAFLD has less literature, we want to see how significantly they correlate. Aims: Correlation of BMI with subcutaneous fat and total body fat in patients with NAFLD. Methods: A total of 118 patients were included in the study. The following anthropometric measurements were collected: body mass (weight and height), waist circumference and skinfold thickness (triceps, biceps, sub scapular, anterior abdominal), total body fat (%) was measured by bio-impedance. Results: The correlation of BMI with skin fold thickness at triceps-0.619, biceps-0.566, sub scapular-0.686, anterior abdominal-0.638, total body fat-0.672. All of which are statistically significant with a P value of <0.01. Conclusion: The correlation of BMI with skin fold thickness and total body fat in patients with NAFLD is statistically significant.

We evaluated the treatment response of this regimen in terms of its impact on the Quality of life of patients during and after the therapy. Methods: This is an Observational, prospective, descriptive study of serologic HCV RNA-positive cases. Data on change in clinical, biochemical profile, and quality of life using 2 standardized questionnaires SF-36 and CLDQ was analyzed at baseline, 12 w from baseline, and 24 w after therapy cessation. The liver disorders are associated with significant maternal and foetal morbidity and mortality. The present study was done with objective to study incidence, clinicopathological correlation of liver disorder during pregnancy and its impact on maternal and foetal outcome.Methods: This prospective observational study was conducted in Department of Obstetrics and Gynaecology, NSCB Medical College Jabalpur. Eighty-five antenatal cases, on the basis of inclusion criteria were studied prospectively. Background & objectives: The effect of vitamin D supplementation on response to antiviral the... more Background & objectives: The effect of vitamin D supplementation on response to antiviral therapy in hepatitis C virus (HCV) genotype 1 and 4 infection still remains unclear, with studies yielding inconsistent results. The aim of the present study was to assess the effect of vitamin D supplementation on treatment outcome in patients with genotype 1/4 chronic hepatitis C (CHC) infection. Methods: Sixty consecutive, treatment-naïve, genotype 1 and 4 chronic HCV patients were included in the study. The patients were randomized into two groups: Vitamin D supplemented group received pegylated (PEG)-interferon α-2a 180 μg per week plus ribavirin (RBV) (1000-1200 mg/d) together with vitamin D3 (2000 IU/d) and control group received identical therapy without vitamin D (32 patients). Results: There were no significant differences between the two groups in terms of age, sex, body mass index and baseline laboratory values. Lower vitamin D levels were associated with higher grades of fibrosis in liver histology (vitamin D >20 ng/ml - 70% vs vitamin D <20 ng/ml - 37%, P<0.05). Vitamin D supplemented group had similar rapid viral response (40 vs 28%, P=0.36), complete early viral response (53.2 vs 40%, P=0.34), end of treatment response (64 vs 46%, P=0.17) and sustained virological response (SVR) (60 vs 44%, P=0.19) as compared to control group. Interleukin 28B polymorphism [odds ratio (OR)-15.37, 95% confidence interval (CI)-2.32-101.76, P=0.04] and baseline serum vitamin D levels (OR-6.36, 95% CI-1.36-29.61 P=0.02) were independent predictors of SVR in genotype 1/4 CHC. Vitamin D supplementation was not found to be predictor of response in genotype 1/4 CHC on multivariate analysis (OR-2.79, 95% CI- 0.63-12.34, P=0.74). Interpretation & conclusions: The present study showed that addition of vitamin D to PEG/RBV combination therapy in treatment-naïve patients who were infected with HCV genotype 1/4 had no effect on the rates of rapid, early and sustained viral responses.

All except 1 had right lobe abscess. Mean duration of disease was 67 days (40-135 days). No patient had feature of SIRS. One patient with left lobe abscess had intra-peritoneal rupture and another had diabetes mellitus at admission. Mean size of abscess was approx. 424cc (267-902cc) before drainage and approximately 50 ml (difficult to assess exactly) at the time of discharge. Mean size of abscess at 1 month was 23cc (0-60cc). No complications were encountered except pain at drainage site. Conclusion: Catheter drainage with indwelling catheter is a feasible treatment for progressively enlarging liver abscess.

The liver disorders are associated with significant maternal and foetal morbidity and mortality. The present study was done with objective to study incidence, clinicopathological correlation of liver disorder during pregnancy and its impact on maternal and foetal outcome.Methods: This prospective observational study was conducted in Department of Obstetrics and Gynaecology, NSCB Medical College Jabalpur. Eighty-five antenatal cases, on the basis of inclusion criteria were studied prospectively. Those subjected to detailed history and examination, clinical symptoms suggestive of liver disorders followed by all available LFTs including LDH along with some more definitive tests to aid identification of underlying cause and followed up till delivery in terms of maternal and foetal outcome. Background & objectives: The effect of vitamin D supplementation on response to antiviral the... more Background & objectives: The effect of vitamin D supplementation on response to antiviral therapy in hepatitis C virus (HCV) genotype 1 and 4 infection still remains unclear, with studies yielding inconsistent results. The aim of the present study was to assess the effect of vitamin D supplementation on treatment outcome in patients with genotype 1/4 chronic hepatitis C (CHC) infection. Methods: Sixty consecutive, treatment-naïve, genotype 1 and 4 chronic HCV patients were included in the study. The patients were randomized into two groups: Vitamin D supplemented group received pegylated (PEG)-interferon α-2a 180 μg per week plus ribavirin (RBV) (1000-1200 mg/d) together with vitamin D3 (2000 IU/d) and control group received identical therapy without vitamin D (32 patients). Results: There were no significant differences between the two groups in terms of age, sex, body mass index and baseline laboratory values. Lower vitamin D levels were associated with higher grades of fibrosis in liver histology (vitamin D >20 ng/ml - 70% vs vitamin D <20 ng/ml - 37%, P<0.05). Vitamin D supplemented group had similar rapid viral response (40 vs 28%, P=0.36), complete early viral response (53.2 vs 40%, P=0.34), end of treatment response (64 vs 46%, P=0.17) and sustained virological response (SVR) (60 vs 44%, P=0.19) as compared to control group. Interleukin 28B polymorphism [odds ratio (OR)-15.37, 95% confidence interval (CI)-2.32-101.76, P=0.04] and baseline serum vitamin D levels (OR-6.36, 95% CI-1.36-29.61 P=0.02) were independent predictors of SVR in genotype 1/4 CHC. Vitamin D supplementation was not found to be predictor of response in genotype 1/4 CHC on multivariate analysis (OR-2.79, 95% CI- 0.63-12.34, P=0.74). Interpretation & conclusions: The present study showed that addition of vitamin D to PEG/RBV combination therapy in treatment-naïve patients who were infected with HCV genotype 1/4 had no effect on the rates of rapid, early and sustained viral responses.

He did not complain of nocturnal motions, recent diarrhea, blood in stool, straining, weight loss, or pain abdomen. Stool test was normal. He was counseled and treated as a case of irritable bowel syndrome. Due to inadequate relief with empirical therapy, colonoscopy was performed in a subsequent visit. Club-shaped small, round organisms with moving proboscis were seen in the cecum. Organism was later identified as a trematode Gastrodiscoides hominis, a rare foodborne trematode. The patient was treated with praziquantel, without complete relief. Trematode infection might not be the cause of symptoms.

Background & Aim: Nonalcoholic fatty liver disease (NAFLD) is increasing at alarming rates in sou... more Background & Aim: Nonalcoholic fatty liver disease (NAFLD) is increasing at alarming rates in south Indian population, the correlation of BMI with skin fold thickness and total body fat in patients with NAFLD has less literature, we want to see how significantly they correlate. Aims: Correlation of BMI with subcutaneous fat and total body fat in patients with NAFLD. Methods: A total of 118 patients were included in the study. The following anthropometric measurements were collected: body mass (weight and height), waist circumference and skinfold thickness (triceps, biceps, sub scapular, anterior abdominal), total body fat (%) was measured by bio-impedance. Results: The correlation of BMI with skin fold thickness at triceps-0.619, biceps-0.566, sub scapular-0.686, anterior abdominal-0.638, total body fat-0.672. All of which are statistically significant with a P value of <0.01. Conclusion: The correlation of BMI with skin fold thickness and total body fat in patients with NAFLD is statistically significant.

Results: Serum MIP-1alpha/CCL3 levels were significantly higher in cirrhotic patients with and wi... more Results: Serum MIP-1alpha/CCL3 levels were significantly higher in cirrhotic patients with and without HCC than in healthy subjects and in HCC patients than in patients without HCC (P < 0.001). By plotting receiver-operating characteristic curve, the sensitivity and specificity of serum MIP-1alpha/CCL3 in the diagnosis of HCC were 100% and 93.3% respectively at a cutoff value of 17.5 pg/ml. HCV-related HCCs showed significant increases in MIP-1alpha/CCL3 expression, TAM count and MVD compared with adjacent non-neoplastic liver tissues (P < 0.001). The MIP-1alpha/CCL3 expression in HCCs showed positive correlations with serum MIP-1alpha/CCL3 levels, tumor size, stage and histological grade, TAM count and MVD (P < 0.05). Conclusion: The CC chemokine MIP-1alpha/CCL3 plays an important role in the development and progression of HCV-related HCC possibly, through migration of macrophages to tumor microenvironment and enhancement of angiogenesis. MIP-1alpha/ CCL3 may serve as a potential serum biological marker and a useful therapeutic target for HCC.

Moreover, IL-33 and Th2 cytokines were markedly decreased in GF-SAMP versus SPF-SAMP, whereas administration of fecal antigens in GF-SAMP mice induced a remarkable increase in IL-33 expression. Conclusions: Together these data show that IL-33 expression/production is highly dependent on the presence of the gut microbiota and may serve as a primary mediator of profibrotic events leading to intestinal fibrosis in IBD.

All except 1 had right lobe abscess. Mean duration of disease was 67 days (40-135 days). No patient had feature of SIRS. One patient with left lobe abscess had intra-peritoneal rupture and another had diabetes mellitus at admission. Mean size of abscess was approx. 424cc (267-902cc) before drainage and approximately 50 ml (difficult to assess exactly) at the time of discharge. Mean size of abscess at 1 month was 23cc (0-60cc). No complications were encountered except pain at drainage site. Conclusion: Catheter drainage with indwelling catheter is a feasible treatment for progressively enlarging liver abscess.
