Aggression is a behavior whose primary or sole purpose or function is to injure physically or psychologically. Aim and objectives: To demonstrate that high cortisol level in relation with the comorbidity of substance abuse and the duration of abuse in the conduct group. Patients and methods: This study was conducted on 50 aggressive patients with disruptive aggressive behaviors, attending the adolescent outpatient clinic at Alabbassia Mental hospital, who in turn divided into two groups one of conduct disorder and the other of Oppositional Defiant disorder and 25 resembling the control group during the period from March 2019 to February 2020. Results: Patients with conduct disorder showed statistically significant difference between the cortisol level and the comorbidity of substance abuse and the duration of abuse (by years) (P<0.05). While in the ODD group, there was statistically significant difference between the cortisol level and the gender and the age of the adolescent (P<0.05). Conclusion: Cortisol level is higher in the disruptive aggressive adolescents than that of the control group participating in the current study, moreover it was higher in those with conduct than ODD groups. It was demonstrated that high cortisol level in relation with the comorbidity of substance abuse and the duration of abuse in the conduct group.

Recently, the positive effects of photobiomodulation on the regulation of MSCs proliferation and osteogenic differentiation have gained significant attention. This study aimed to assess the effects of photobiomodulation and vitamin D (as an anabolic factor) on HPDLSCs for bone regeneration. (2) Methods: HPDLSCs were collected, isolated, and characterized and then divided into six groups: groups I and II, control and (10 −7 Mol) vitamin D, respectively; group III, irradiation at 1 J/cm 2 of 808-nm diode laser; group IV, irradiation at 1 J/cm 2 and culture with vitamin D; group V, irradiation at 2 J/cm 2 , and group VI, irradiation at 2 J/cm 2 and culture with vitamin D. Cell viability assay was measured through MTT assay and cell growth curve. Alkaline phosphatase (ALP) enzyme activity and mRNA levels of RUNX2, collagen 1 (Col-1), ALP, and osteonectin were also assessed. (3) Results: Photobiomodulation at 1 and 2 J/cm 2 combined with vitamin D significantly promoted HPDLSC proliferation (in MTT assay and cell growth curve results) and osteogenic differentiation (through the gene expression of RUNX2, Col-1, ALP, and osteonectin levels (p < 0.05). (4) Conclusion: Laser irradiation at 2 J/cm 2 combined with vitamin D3 enhanced osteoblast differentiation and proliferation of cultured HPDLSCs and thus could further substitute bone grafting.

Background: Acute rheumatic fever (ARF) and acute rheumatic carditis (ARC) continue to be a major... more Background: Acute rheumatic fever (ARF) and acute rheumatic carditis (ARC) continue to be a major public health problem in developing countries. Objective: To study the characteristics of children with ARC being treated at a tertiary centre. Methods and Results: We studied 126 children (mean age 10.4 ± 2.3 years, range 5–15 years, 60% males) diagnosed with ARC by treating cardiologists. Most had lower socio-economic status. Fifty of 126 (40%) presented with a first episode of ARC. Joint symptoms were present in 29% and fever in 25%. Only 2.4% had subcutaneous nodules and none had erythema marginatum or chorea. Fifty-one percent presented in NYHA class II and 29% in NYHA class III or IV. Tachycardia and heart failure were present in 53% and 21%, respectively. Idiopathic Sudden Sensorineural Hearing Loss(ISSNHL) is a an irritating problem that continues to... more Idiopathic Sudden Sensorineural Hearing Loss(ISSNHL) is a an irritating problem that continues to pose a diagnostic and therapeutic enigma for the otologist. It is most commonly defined as sensorineural hearing loss of 30dB or greater over at least three contiguous audiometric frequencies occurring within a 72-hr period. It is not one of the more common causes of HL, disproportionate interest in ISSNHL exists most likely because it is one of the few reversible (sensorineural) hearing losses encountered by clinicians. Intratympanic steroid injection is increasingly used in the management of ISSNHL as it offers the potential for directed therapy of a high concentration to the inner ear with avoidance of systemic side effects. It also can improve hearing function in patients with ISSNHL after the failure of systemic steroid treatment.

The present study aimed to micropropagate two date palm cultivars (Zaghlol and Barhy) using immat... more The present study aimed to micropropagate two date palm cultivars (Zaghlol and Barhy) using immature female inflorescence and various growth regulators combinations. This experiment was done in Biotechnology Laboratory, Department of Genetics, Faculty of Agriculture Zagazig University, during three consecutive seasons 2017, 2018, and 2019. Three types of culture media were used for initiation stage, results showed that initiation medium1 (1.0 mg /l 1-Naphthaleneacetic acid (NAA) + 0.1 mg Indole-3-acetic acid (IAA) + 1.0 mg /l 2,4-Dichlorophenoxyacetic acid (2, 4-D)) was the best medium that recorded the highest callus formation rate in both Zaghlol and Barhy cultivars. But the Barhy cultivar showed higher response for callus formation than Zaghlol cultivar, especially for number of days to initiate callus (average mean 125.58 days compare to 279 days for Barhy and Zaghlol, respectively. For plant regeneration stage, it was used three plant regeneration media. Results showed that plant regeneration medium 1 (NAA 0.5 mg/l +benzyl adenine (BA) 2.0 mg/l) showed the best value for each of number of leaves (19.35), leaf length (39.75 cm) and number of days to initiate regeneration (19). Two types of rooting media were used for root formation, it was found that rooting medium 1 (Indole-3-butyric acid (IBA) 2.0 mg/l + BA 2.0 mg/l) gave higher response than rooting medium 2 (NAA 2.0 mg/l +BA 2.0 mg/l) for almost measured criteria except root length. Also, using rooting medium 1 decreased the number of days for root formation to 40 days compared to 110.25 days obtained from rooting medium 2.

Background: Attention-deficit/ hyperactivity disorder (ADHD), characterized by developmentally in... more Background: Attention-deficit/ hyperactivity disorder (ADHD), characterized by developmentally inappropriate inattentiveness, impulsivity, and hyperactivity, and are the most common and thoroughly researched neuropsychiatry disorder affecting children and adolescence. The diagnosis of ADHD requires a comprehensive clinical assessment including a detailed patient history, clinical interview and observation, and a thorough physical examination. Objective: Our study is a prospective study to analyze the socio-demographic data of children with the diagnosis of ADHD attending the O.P.C. of the department of psychiatry at Hamad General Hospital in Qatar, the co morbid psychiatric disorder associated with ADHD, also detecting effect of different treatment modalities on those patients. Results: 29.2% of patients have co morbid Autism, up to 17.6% for organic brain syndrome; oppositional defiant disorders and conduct disorder followed by depression and O.C.D (5.8%) of cases. Conclusion: Clinically, treatment with stimulants result in an immediate, often dramatic, improvement in the core symptoms of ADHD.We concluded that accurate diagnosis and optimal management of this disorder may be the keys to improving long term outcomes of youth with this disorder.

It is essential for early detection and intervention of ID to classify children at high risk of non-genetic ID, which would support both clinical practice and public health. Its etiology has been investigated in several studies, with limited information from developing countries. This study aimed to estimate etiology, sociodemographic characteristics, risk factors, and co-morbidities related to ID. Methods 134 children and adolescents with ID were included in the study fulfilling the diagnostic criteria for ID according to DSM-5. A structured questionnaire focusing on demographic, socioeconomic, pre, and postnatal variables was administered to parents of children and adolescents by trained interviewers. Results The mean age was 8.25 ± 3.37 years, 76.1% males, 39.6% mild ID, 29.9% moderate, 26.1% severe ID, and 4.5% profound ID. Advanced maternal age above 35 years (AOR = 6.79; 95% CI = 2.05 - 22.46; P = 0.001), lack of prenatal care during pregnancy (AOR = 0.131; 95% CI = 0.131; P = 0.007), prematurity (AOR = 8.494; 95% CI = 1.08 - 66.84; P= 0.042), perinatal anoxia (AOR = 43.549; 95% CI = 2.25 - 842.99; P= 0.013) and autism (AOR 9.160; 95% CI = 1.57 - 53.37; P= 0.014) were significantly independent factors associated with increased risk of severe/profound nongenetic ID. Conclusion This study’s findings suggest a complex interaction between prenatal, perinatal, neonatal, and socioeconomic disadvantages and severe form of ID, suggesting that with effective antenatal, perinatal, and neonatal healthcare measures, this incidence can be minimized.

Serum Transforming Growth Factor Beta2 and Feeding Intolerance in Premature Formula-Fed versus Br... more Serum Transforming Growth Factor Beta2 and Feeding Intolerance in Premature Formula-Fed versus Breast Milk-fed Neonates Objectives: to assess occurance of feeding intolerance and NEC in relation to serum level of TGF-b2 in breast milk versus preterm formula fed neonates. Subjects and Methods: a prospective observational study on 80 preterm neonates(36weeks gestational age). They were divided to two groups; breast milk fed group(n ¼ 40) and preterm formula fed group(n ¼ 40). They were assessed clinically for feeding intolerance and NEC and laboratory by measurement of TGF-b2 using ELISA technique. Results:TGF-b2 serum level was significantly lower in preterm formula fed group than breast fed group;median(interquartile) 500(250-3000) pg/ml vs 7750(6500-11250) pg/ml (p < 0.0001). Both primary outcome(feeding intolerance and NEC) and secondary outcome(respiratory support and sepsis) were negatively correlated to TGF-b2 serum level. TGF-b2 serum level below 1250 pg/ ml was found to be a good diagnostic test for feeding intolerance with sensitivity of 75.86% and specificity of 96.08%. Conclusion: low serumTGF-b2 level is a specific marker for feeding intolerance in preterm neonate. Using premature formula in premature neonates is not recommended as it is associated with lower serum TGF-b2 and higher incidence of feeding intolerance compared to breast feeding.

Background: Inadequate adherence to insulin is a major concern, necessitating the use of reliable... more Background: Inadequate adherence to insulin is a major concern, necessitating the use of reliable and valid metrics for assessing adherence. Up to date, there are no Arabic validated tools assessing adherence to insulin therapy among children with type 1 diabetes (T1DM). Thus, the aim of this study is to evaluate the psychometric properties of an Arabic version of the four-item Morisky Green Levine Medication Adherence Scale (MGLS-4) as a self-reported measure of adherence to insulin among a cohort of Egyptian children with T1DM. Methods: The MGLS-4 was translated using forward and backward translation. The Cronbach's alpha was used to assess reliability. Criterion validity of the scale was tested by examining the correlation coefficients between the compliance score (level of adherence) and the HbA1c levels. Results: A total of 400 patients completed the Arabic version of MGLS-4. 26.25% of the studied cohort was found to be non-adherent to insulin therapy; non-adherent patients were significantly older (P=0.001). Decreased maternal education level, decreased frequency of blood glucose monitoring and prolonged disease duration best predicted the occurrence of non-adherence among the studied cohort. The internal consistency of the current version showed good reliability (Cronbach's alpha = 0.857). The adherence score and adherence level showed very strong correlation with HbA1c level (rho = 0.830, P < 0.001 and rho = 0.808, P < 0.001, respectively). Conclusion: The Arabic version of MGLS-4 showed good reliability and validity as a self-administered tool for assessing adherence to insulin in pediatric patients with T1DM.

Methods and Results: We studied 126 children (mean age 10.4 ± 2.3 years, range 5–15 years, 60% males) diagnosed with ARC by treating cardiologists. Most had lower socio-economic status. Fifty of 126 (40%) presented with a first episode of ARC. Joint symptoms were present in 29% and fever in 25%. Only 2.4% had subcutaneous nodules and none had erythema marginatum or chorea. Fifty-one percent presented in NYHA class II and 29% in NYHA class III or IV. Tachycardia and heart failure were present in 53% and 21%, respectively. It is most commonly defined as sensorineural hearing loss of 30dB or greater over at least three contiguous audiometric frequencies occurring within a 72-hr period. It is not one of the more common causes of HL, disproportionate interest in ISSNHL exists most likely because it is one of the few reversible (sensorineural) hearing losses encountered by clinicians. Intratympanic steroid injection is increasingly used in the management of ISSNHL as it offers the potential for directed therapy of a high concentration to the inner ear with avoidance of systemic side effects. It also can improve hearing function in patients with ISSNHL after the failure of systemic steroid treatment.

This experiment was done in Biotechnology Laboratory, Department of Genetics, Faculty of Agriculture Zagazig University, during three consecutive seasons 2017, 2018, and 2019. Three types of culture media were used for initiation stage, results showed that initiation medium1 (1.0 mg /l 1-Naphthaleneacetic acid (NAA) + 0.1 mg Indole-3-acetic acid (IAA) + 1.0 mg /l 2,4-Dichlorophenoxyacetic acid (2, 4-D)) was the best medium that recorded the highest callus formation rate in both Zaghlol and Barhy cultivars. But the Barhy cultivar showed higher response for callus formation than Zaghlol cultivar, especially for number of days to initiate callus (average mean 125.58 days compare to 279 days for Barhy and Zaghlol, respectively. For plant regeneration stage, it was used three plant regeneration media. Results showed that plant regeneration medium 1 (NAA 0.5 mg/l +benzyl adenine (BA) 2.0 mg/l) showed the best value for each of number of leaves (19.35), leaf length (39.75 cm) and number of days to initiate regeneration (19). Two types of rooting media were used for root formation, it was found that rooting medium 1 (Indole-3-butyric acid (IBA) 2.0 mg/l + BA 2.0 mg/l) gave higher response than rooting medium 2 (NAA 2.0 mg/l +BA 2.0 mg/l) for almost measured criteria except root length. Also, using rooting medium 1 decreased the number of days for root formation to 40 days compared to 110.25 days obtained from rooting medium 2.

The diagnosis of ADHD requires a comprehensive clinical assessment including a detailed patient history, clinical interview and observation, and a thorough physical examination. Objective: Our study is a prospective study to analyze the socio-demographic data of children with the diagnosis of ADHD attending the O.P.C. of the department of psychiatry at Hamad General Hospital in Qatar, the co morbid psychiatric disorder associated with ADHD, also detecting effect of different treatment modalities on those patients. Results: 29.2% of patients have co morbid Autism, up to 17.6% for organic brain syndrome; oppositional defiant disorders and conduct disorder followed by depression and O.C.D (5.8%) of cases. Conclusion: Clinically, treatment with stimulants result in an immediate, often dramatic, improvement in the core symptoms of ADHD.We concluded that accurate diagnosis and optimal management of this disorder may be the keys to improving long term outcomes of youth with this disorder.

It is essential for early detection and intervention of ID to classify children at high risk of non-genetic ID, which would support both clinical practice and public health. Its etiology has been investigated in several studies, with limited information from developing countries. This study aimed to estimate etiology, sociodemographic characteristics, risk factors, and co-morbidities related to ID. Methods 134 children and adolescents with ID were included in the study fulfilling the diagnostic criteria for ID according to DSM-5. A structured questionnaire focusing on demographic, socioeconomic, pre, and postnatal variables was administered to parents of children and adolescents by trained interviewers. Results The mean age was 8.25 ± 3.37 years, 76.1% males, 39.6% mild ID, 29.9% moderate, 26.1% severe ID, and 4.5% profound ID. Advanced maternal age above 35 years (AOR = 6.79; 95% CI = 2.05 - 22.46; P = 0.001), lack of prenatal care during pregnancy (AOR = 0.131; 95% CI = 0.131; P = 0.007), prematurity (AOR = 8.494; 95% CI = 1.08 - 66.84; P= 0.042), perinatal anoxia (AOR = 43.549; 95% CI = 2.25 - 842.99; P= 0.013) and autism (AOR 9.160; 95% CI = 1.57 - 53.37; P= 0.014) were significantly independent factors associated with increased risk of severe/profound nongenetic ID. Conclusion This study’s findings suggest a complex interaction between prenatal, perinatal, neonatal, and socioeconomic disadvantages and severe form of ID, suggesting that with effective antenatal, perinatal, and neonatal healthcare measures, this incidence can be minimized.

Subjects and Methods: a prospective observational study on 80 preterm neonates(36weeks gestational age). They were divided to two groups; breast milk fed group(n ¼ 40) and preterm formula fed group(n ¼ 40). They were assessed clinically for feeding intolerance and NEC and laboratory by measurement of TGF-b2 using ELISA technique. Results:TGF-b2 serum level was significantly lower in preterm formula fed group than breast fed group;median(interquartile) 500(250-3000) pg/ml vs 7750(6500-11250) pg/ml (p < 0.0001). Both primary outcome(feeding intolerance and NEC) and secondary outcome(respiratory support and sepsis) were negatively correlated to TGF-b2 serum level. TGF-b2 serum level below 1250 pg/ ml was found to be a good diagnostic test for feeding intolerance with sensitivity of 75.86% and specificity of 96.08%. Conclusion: low serumTGF-b2 level is a specific marker for feeding intolerance in preterm neonate. Using premature formula in premature neonates is not recommended as it is associated with lower serum TGF-b2 and higher incidence of feeding intolerance compared to breast feeding.

Background: Inadequate adherence to insulin is a major concern, necessitating the use of reliable... more Background: Inadequate adherence to insulin is a major concern, necessitating the use of reliable and valid metrics for assessing adherence. Up to date, there are no Arabic validated tools assessing adherence to insulin therapy among children with type 1 diabetes (T1DM). Thus, the aim of this study is to evaluate the psychometric properties of an Arabic version of the four-item Morisky Green Levine Medication Adherence Scale (MGLS-4) as a self-reported measure of adherence to insulin among a cohort of Egyptian children with T1DM. Methods: The MGLS-4 was translated using forward and backward translation. The Cronbach's alpha was used to assess reliability. Criterion validity of the scale was tested by examining the correlation coefficients between the compliance score (level of adherence) and the HbA1c levels. Results: A total of 400 patients completed the Arabic version of MGLS-4. 26.25% of the studied cohort was found to be non-adherent to insulin therapy; non-adherent patients were significantly older (P=0.001). Decreased maternal education level, decreased frequency of blood glucose monitoring and prolonged disease duration best predicted the occurrence of non-adherence among the studied cohort. The internal consistency of the current version showed good reliability (Cronbach's alpha = 0.857). The adherence score and adherence level showed very strong correlation with HbA1c level (rho = 0.830, P < 0.001 and rho = 0.808, P < 0.001, respectively). Conclusion: The Arabic version of MGLS-4 showed good reliability and validity as a self-administered tool for assessing adherence to insulin in pediatric patients with T1DM.
