In the contemporary world one can hardly find someone left alone without a smart phone; where his... more In the contemporary world one can hardly find someone left alone without a smart phone; where his fingers would be busy texting online with a person(s) on the other end. It has become a trend nowadays to disclose personal matters and whereabouts not only of the person concerned but whatever is happening around him on these social networking sites; thereby social media is turning to be a personal diary that is shared with thousands across the globe. Family dinner turns outdated and the moment of relaxing after a day’s tough work is met in front of our smart phones or personal computers. In this research, previous studies were analyzed and an online survey was conducted to determine the impact of social media on interpersonal communication among the youngsters in Kochi, India. The study was also conducted to check if social media is turning to be an addiction amongst the present generation and to know if it is creating any communication gap between older generation and the present ones.

The purpose of this article is to present a feasibility study of a recovery system for the engine and engine frame of an existing, expendable heavy launch vehicle and present recommendations for further research. The concept developed is verified based on the Ariane 6. The recovery of the Vulcain Aft Bay (VuAB) is initialised by separation from the first stage at 157.7 km altitude while travelling at 6930 m/s. The study investigates an inflatable aeroshell for protection and deceleration during re-entry, after which it is proposed to further decelerate the VuAB using drogue parachutes. The final part of the concept entails retrieval of the VuAB by a helicopter in mid-air. To enable a controlled gliding flight during retrieval a parafoil is proposed. At launch, the recovery system will weigh 2789 kg with a payload penalty of 720 kg. The system can be integrated into the existing design of the launcher and will not interfere with nominal operations of the launcher. Implementing the recovery system can reduce the cost per launch of an Ariane 6 by 15%

Context: Labor induction with prostaglandin E1 (PGE1) vaginal tablet results in shorter induction... more Context: Labor induction with prostaglandin E1 (PGE1) vaginal tablet results in shorter induction delivery interval and decreased rate of failed induction of labor and reduced caesarean section rate. However, higher doses may be associated with uterine hyper stimulation. It is therefore necessary to determine the safe dose of PGE1 for labor induction. Aims: To assess the maternal and neonatal outcome with use of 25 mg vaginal misoprostol for induction of labor. Settings and Design: A retrospective analysis conducted in an obstetric department of a tertiary care teaching institute. Materials and Methods: The sample consists of women with singleton term pregnancy, with Bishop's score <6 compared with women with spontaneous onset of labor. Statistical Analysis Used: Statistical significance was assessed at 5% level of significance. Chi-square test, with correction for continuity where applicable, was carried out to compare proportions across subgroups or between induction and spontaneous onset groups. Results: The rate of vaginal delivery was higher among spontaneous onset group compared with induction group ( 2 (1) = 30.3, P < 0.001). The induction delivery interval of vaginal delivery was less than 24 h in 91.85% of women. Neonatal intensive care unit admission frequency was similar among both groups ( 2 (1) = 0.14, P = 0.704). The induction group was with less frequency of meconium staining than the control group ( 2 (1) = 8.05, P = 0.0046). Conclusion: Our study showed a higher rate of vaginal delivery with induction delivery interval less than 24 h in a majority of women with better neonatal and maternal outcomes.

Background: The outcome of patients with acute kidney injury (AKI) is highly variable. Patients w... more Background: The outcome of patients with acute kidney injury (AKI) is highly variable. Patients who receive renal replacement therapy (RRT) for similar diseases may recover differently. The factors that operate in each patient may alter the prognosis and outcome. Aims: Our study aims at identification of prognostic factors influencing recovery in patients who required hemodialysis for AKI. Material and Methods: Patients admitted in different ICUs with AKI who underwent hemodialysis in a tertiary care hospital over a three year period were included in the study. Time from day one of disease to first dialysis, hematological and biochemical parameters were noted. Patients were grouped based on the time taken for recovery of renal function following hemodialysis into group A ( 2 weeks). Studied parameters have been statistically analyzed to find any significant association with recovery time. Results: Out of 63 patients, 9 progressed to chronic kidney disease. In the remaining 54, Group A comprised 31 and group B 23. Out of all the factors studied, serum creatinine (7.0±1.3 vs 8.4±3.8; P=0.018), S. bicarbonate (21.7±2.8 vs 19.7±3.8; P=0.03), pH at admission (7.25±0.13 vs 7.1±0.19; P=0.048); number of hemodialysis sessions (3.5 ±1.5 vs 5±2.4; P=0.016) and time lag from day one of disease to first hemodialysis (8.6 ± 3.6 vs 11.5±5.9; P=0.007) showed significant association with recovery time. Conclusion: Recovery following AKI is influenced by factors liked delayed presentation, late initiation of hemodialysis, low pH and low bicarbonate which can predict delayed renal recovery following hemodialysis.

To define the role of the modified anterior cervical approach in anterior reconstruction and instrumentation of the cervicothoracic vertebrae. To analyze the problems encountered during anterior stabilization in these patients. In tuberculosis of cervicothoracic junction, direct anterior visualization is mandatory for optimal decompression of the spinal canal. Anterior reconstruction and stabilization has the advantage of providing immediate stability of vertebral column. The modified anterior cervical approach provides adequate exposure of the upper four thoracic vertebrae. However, radiologic and clinical criteria for anterior instrumentation of the cervicothoracic junction using the modified anterior cervical approach are unclear in literature. A total of 42 patients with tuberculous kyphosis involving the cervicothoracic junction were operated. Based on the association of the sternal notch with the most distal normal vertebra, the patients were divided into 2 surgical groups. The long-neck patients were amenable to anterior instrumentation using the standard supraclavicular approach with a strap-muscle tenotomy. A manubriotomy was mandatory in short-neck patients for optimal visualization and proper placement of the implant. Anterior reconstruction and instrumentation of the cervicothoracic junction offers a distinct advantage of a stable implant-bone construct anteriorly while preserving the posterior osseo-ligamentous tension band. Detailed preoperative assessment based on clinical and radiologic criteria helps in selection of patient for this procedure. Meticulous intraoperative technique helps to minimize the morbidity and complications associated with this procedure.

This survey investigates the cross-sectional association between nighttime road, rail and aircraf... more This survey investigates the cross-sectional association between nighttime road, rail and aircraft noise exposure and the probability to be highly sleep disturbed (%HSD), as measured by self-report in postal and online questionnaires. As part of the Swiss SiRENE study, a total of 5592 survey participants in the entire country were selected based on a stratified random sample of their dwelling. Self-reported sleep disturbance was measured using an ICBEN-style 5-point verbal scale. The survey was carried out in four waves at different times of the year. Source-specific noise exposure was calculated for several façade points for each dwelling. After adjustment for potential confounders, all three noise sources showed a statistically significant association between the nighttime noise level LNight at the most exposed façade point and the probability to report high sleep disturbance, as determined by logistic regression.
