An animal model using joint replacement materials was used to compare the efficacy of cefazolin t... more An animal model using joint replacement materials was used to compare the efficacy of cefazolin to that of vancomycin/ceftazidime combination in preventing infection from Staphylococcus epidermidis. Forty four New Zealand rabbits had a cobalt-chrome with methacrylate bone cement in the distal femoral metaphysis. Deliberate contamination of the wound with one or two strains of Staphylococcus epidermidis was performed with or without the administration of prophylactic antibiotics and the infection rates compared. Cefazolin prevented infection in only 14% of the animal compared to 86% protected by the combination of vancomycin and ceftazidime. The results were compared with both methicillin sensitive and resistant strains. Epidemiological data and the results of our experiments suggest that the combination of vancomycin and ceftazidime should be considered for contemporary prophylaxis of infection in total joint replacement.

Four patients with chronic nonunion of the scaphoid were treated by a vascularized bone graft bas... more Four patients with chronic nonunion of the scaphoid were treated by a vascularized bone graft based on first dorsal metacarpal artery. The mean duration of the nonunion was 28.5 months (range 12–48 months). There was avascular necrosis in all patients confirmed by magnetic resonance imaging (MRI). None of the patients had previous attempts at surgical reconstruction. Two fractures were localized at the waist one in the distal part and one at the proximal pole. Osseous union of the scaphoid was confirmed by X-ray in all patients in an average of 2.1 months. We recommend this technique for the treatment of established scaphoid pseudoarthrosis with avascular necrosis since it is associated with minimal morbidity and predictable good results.

The management of cervical facet dislocations (CFD) has been reported to be complicated and contr... more The management of cervical facet dislocations (CFD) has been reported to be complicated and controversial. Several authors have suggested that the workup and subsequent treatment of these patients may be altered if there is a delay in their initial presentation. The objective of our study was to determine the influence of early versus late CFD presentation on patient management. All patients treated for CFD from January 1988 to December 1992 at The Ben Taub General Hospital Houston, Texas were identified for retrospective review. Patients were divided into two groups based on time of presentation to hospital Group 1 (Early) 72 hours. Charts were reviewed in detail to determine pre and post treatment neurological status and the reduction method (traction versus surgery) used. 34 patients with CFD were identified 22 patients. In Group 1, facet reduction was achieved closed in 64%, whereas in Group 2 facet reduction was achieved closed in only 20% patients. In our patients, 35 percent were late in obtaining treatment. Less neurological deficit was demonstrated in this group of patients and closed reduction was less likely to be successful. The authors conclude that a distinction should exist between early and late presentation in the management algorithm of CFD.

The aim of the study was to develop a suture technique that would be simple and easy to perform, ... more The aim of the study was to develop a suture technique that would be simple and easy to perform, but also strong enough to resist the strength formed during early active exercise protocols. Thirty flexor digitorum profundus muscle tendons were obtained from lambs aged 12 to 16 months. The tendons were assigned to three in situ repair groups, including the modified Kessler technique, six-strand Savage technique, and a strengthened modified Kessler technique, all combined with an epitendinous suture. Each group was subjected to biomechanical tests and the maximum strength of the tendons to rupture and the power exerted to yield a 3-mm separation were recorded. The mean strengths of the tendons repaired with the modified Kessler technique for 3-mm separation and rupture were 29.9+/-2.9 N and 37.0+/-4.0 N, respectively. The corresponding forces were 39.1+/-6.7 N and 51.3+/-6.1 N with the six-strand Savage technique, and 59.9+/-8.3 N and 69.0+/-8.7 N with the strengthened modified Kessle...

Although bone metastases have been reported frequently, the authors are aware of only a single report of intra-articular synovial metastasis. A case of metastatic melanoma, mimicking degenerative medial meniscal tear of the right knee, is presented. Further examination revealed asymptomatic metastases in the liver, subcutaneous tissue, and left trochanter minor.

Between 1990 and 1996, 16 cases of bone defects were treated by vascularised bone grafting by the... more Between 1990 and 1996, 16 cases of bone defects were treated by vascularised bone grafting by the authors. Free vascularised fibula was used in 10 cases and one free iliac crest graft was used for upper extremity bone defects. Four vascular pedicled first metacarpal bone and one radial styloid bone were used for scaphoid nonunion. Average follow-up was 26 months (6-78 months) and success rate was 94%. We recommend vascularised bone grafts in the upper extremity when there is risk of infection; the defect is greater than five centimeters when the forearm rotation is unlimited. The avascularity of the scaphoid pseudarthrosis must be verified radiologically or through magnetic resonance imaging. This technique should only be used when other reconstructive techniques are unlikely to succeed.
