The availability of effective therapeutic options for management of CKD-associated pruritus remains a treatment challenge. The aim of this study was to compare cromolyn sodium cream 4% with placebo for the treatment of renal pruritus. A randomized, double-blind, prospective, 4-week study was designed. 60 patients with ESRD in our dialysis ward were randomly allocated to cromolyn sodium cream 4% or placebo. All of them completed the study period and their pruritus levels were evaluated 5 times (before the start of the study and at the end of each week for 4 weeks) using a Visual Analogue Scale (VAS). The average pruritus score before administration of the drug in cromolyn sodium 4% and placebo group had been 2.5 ± 1.1 and 2.7 ± 1.3, respectively. Methods: This correlational study was conducted from August to September 2016 on 175 eligible diabetic patients (20 to 65 year-old) who referred to the selected centers of diabetes control in Ahvaz City. OBJECTIVE: To evaluate the relationship between DDS and cardiometabolic risk factors in hypertensive patients. METHODS: In this cross-sectional study, 972 hypertensive patients (322 males and 650 females) aged 35–70y participated were recruited. Dietary intake was evaluated using a semi-quantitative food frequency questionnaire and DDS was calculated. Metabolic syndrome was defined according to the IDF/AHA criteria. The anthropometric parameters, fasting blood sugar, lipid profile, and liver enzymes were measured. RESULTS: Male subjects who assigned to the top DDS tertile had 51% lower risk of having low serum HDL-C (OR: 0.49; 95% CI: 0.24–0.96) in the crude model. A similar association was observed for men in the second tertile of DDS after adjusting for covariates (OR: 0.47; 95% CI: 0.23–0.97). Curcumin and zinc have been studied as an antioxidant, antiinflammatory, and antidiabetic agents. In this clinical trial, 84 subjects were randomized into curcumin (500 mg), zinc (30 mg), zinc and curcumin, and placebo groups for 90 days. At the baseline and the end of the study, the outcomes (fasting plasma glucose (FPG), 2‐hour postprandial glucose (2hpp), HbA1C, insulin, insulin sensitivity (IS), insulin resistance (IR), β‐cell function (BCF), weight, body mass index (BMI), dietary intake, and physical activity (PA)) were measured. A hypocaloric diet and PA were recommended for all subjects. In total, 82 subjects completed the study. After the intervention, dietary intake, PA, weight, and BCF% did not show a significant difference among the groups. However, subjects taking only zinc and zinc and curcumin groups experienced decreased BMI compared to the placebo (p = .01 and .007, respectively). The three treated groups had improved FPG (p = .01), 2hpp (p = .003), HbA1C (p = .004), insulin (p = .001), IS% (p = .001), and IR (p < .001) compared to the placebo. Based on these results, zinc and curcumin supplementation exerted a beneficial effect on several key glycemic parameters.

Objective The aim of this study was to investigate the psychological and personality characterist... more Objective The aim of this study was to investigate the psychological and personality characteristics of the applicants for facial cosmetic surgery. Methods This descriptive–analytical study was conducted on 385 facial cosmetic surgery applicants who referred to medical centers in Ahvaz (south Iran). The data collection tool included three questionnaires: (1) demographic questions, (2) the General Health Questionnaire (GHQ), and (3) the NEO Five-Factor Inventory (NEO-FFI). Results The results showed that 45.2% of the participants had abnormal health status. The mean score of the participants on the openness subscale was insignificantly lower than that of the general population ( p > 0.05). On the other hand, the mean scores of the participants on the conscientiousness and agreeableness subscales were significantly lower ( p < 0.001). Finally, the mean score of the samples on the extroversion subscale was significantly higher than that of the general population ( p < 0.001). Conclusion Given the results of the study, it is strongly recommended to address the personality and psychological characteristics of the applicants for cosmetic surgery before operating on them. Based on a prospective cohort study, ADFC (Ahvaz Diabetic Foot Cohort) study, carried out in a university hospital, all of the patients with diabetes were followed up for new diabetic foot ulcer (DFU). The time of ulcer development was final outcome during two years in the present study. To analyze the data, the variables were first evaluated by univariate analysis. Subsequently variables with P value <0.2 were tested in multivariate analysis, using backward elimination multiple Cox regression. From among 605 eligible patients of ADFC study, 566 patients without foot ulcer were included for a 2- years follow-up. Thirty subjects (5.3%) developed DFU during the study course none of whom underwent amputation. The DFU-free survival rate was 0.945 over two years of follow-up. To assess the effect of vitamin D on inflammatory cytokines (IL-17, IL-10, TNF-α), and biomarkers of oxidative stress (total antioxidant capacity (TAC), and malondialdehyde (MDA)) among IBS patients. Background and aims: Patient education as one of the pillars of clinical governance has a special... more Background and aims: Patient education as one of the pillars of clinical governance has a special importance in all patients including burn victims. This study employed a model of organizational culture in order to optimize the institutionalization of patients' education for anxiety and increasing burned patients' satisfaction. Methods: In this clinical trial quasi experimental study, 80 burned hospitalized patients were included in the intervention group and control group. First, the intervention was consisted of sessions based on Organizational Culture Improvement Model of Patient Education in order to institutionalize patient education for nurses and nurse managers in dimensions of the effective communication process, cooperative decision making, goal setting, planning, implementing and recording, supervision and control, and incentive increasing. Results: Findings showed that the means of situational anxiety in the control group and intervention group were 47.12±4.40 and 45.55±4.86, respectively before the intervention which was not be statistically significance. But the mean of situational anxiety were 43.70±5.36 and 38.45±4.08 in the control and intervention groups respectively after the intervention which showed statistical significance (P=0.002). The means of satisfaction in control and intervention groups before the intervention were 18.82±5.76 and 20.75±4.22 respectively which reached to 21.02±6.56 and 26.75±4.51 respectively after the intervention, that showed a significant increase in satisfaction of education in intervention group (P=0.001). Conclusion: This model represents the results of a strategy to improve patient education through the promotion of academic skills and motivation of nurses increasing of the system's commitment on and responsibility for optimized implementation of patient education. So, applying this model will play an important role in the patients' health.

Nurses and administrators can identify and implement mechanisms to manage, change, and improve th... more Nurses and administrators can identify and implement mechanisms to manage, change, and improve the ethical climate in their workplace through an understanding and awareness of this concept as well as how it is perceived by everyone involved. This study aimed to compare the nurses' perception of the actual and ideal organizational ethical climate in hospitals of Ahwaz Juondishapour University of Medical Sciences. This was a cross-sectional descriptive study conducted on 558 nurses working at 9 medical centers of Ahwaz Jondishapour University of Medical Sciences selected by systematic sampling. Data instrument consisted of 2 parts: 1) demographic characteristics, and 2) actual & ideal climate questionnaire based on the Farsi version of Olson's ethical climate questionnaire, used to assess nurses' perception of the hospital ethical climate in 5 categories (managers, colleagues, physicians, patients, and hospitals). Descriptive and inferential statistical tests, independent T-test and One-Way ANOVA were used to analyze the data. Results demonstrated a positive hospital ethical climate (mean = 3.66), while the nurses' perception of the ideal ethical climate was positive as well. A significant difference was found between the mean scores of the nurses' perceptions of actual & ideal ethical climates ( P <0.05). Results demonstrated a significant relationship between nurses' work experience & organizational level, and their perceptions of the hospital ethical climate. A positive ethical climate exists in hospitals, although it is far from the ideal ethical climate, and therefore managers must maintain the actual ethical climate and at the same time implement strategies to improve their hospitals' ethical climate.

To compare the accuracy of IOPen rebound tonometer with Goldmann applanation tonometer (GAT) in i... more To compare the accuracy of IOPen rebound tonometer with Goldmann applanation tonometer (GAT) in individuals with low, normal and high intraocular pressure (IOP) and to evaluate the effect of central corneal thickness (CCT) on IOP measurements. This cross-sectional study consisted of 159 participants. IOP of one eye of each subject was measured consecutively with IOPen and GAT. Then CCT was measured using an ultrasonic pachymeter. Based on GAT IOP readings, participants were divided into low, normal and high IOP groups. Correlation between tonometers and CCT was calculated by spearman's correlation coefficient. Agreement between tonometers was evaluated using Bland-Altman method. Non-significant underestimation of IOP by IOPen was observed in low IOP group (Mean difference: 0.20mmHg; P=0.454) and also in normal IOP group (Mean difference: 0.56mmHg; P=0.065). Purpose: To measure oxygen tension in the aqueous humor of human eyes under different oxygenation... more Purpose: To measure oxygen tension in the aqueous humor of human eyes under different oxygenation conditions. Methods: This prospective comparative interventional case series consisted of two parts. In the first part, 120 consecutive patients scheduled for cataract surgery were randomized into group I (control group) in which surgery was performed under local anesthesia inhaling 21% oxygen; group II in whom general anesthesia using 50% oxygen was employed; and group III receiving general anesthesia with 100% oxygen. After aspirating 0.2 ml aqueous humor under sterile conditions, the aqueous sample and a simultaneously drawn arterial blood sample were immediately analyzed using a blood gas analyzer. In part II the same procedures were performed in 10 patients after fitting a contact lens and patching the eye for 20 minutes (group IV) and in 10 patients after transcorneal delivery of oxygen at a flow rate of 5 L/min (group V). Results: Mean aqueous PO 2 in groups I, II and III was 112.3±6.2, 141.1±20.4, and 170.1±27 mmHg, respectively (P values <0.001) and mean arterial PO 2 was 85.7±7.9, 184.6±46, and 379.1±75.9 mmHg, respectively (P values <0.001). Aqueous PO 2 was 77.2±9.2 mmHg in group IV and 152.3±10.9 mmHg in group V (P values <0.001). There was a significant correlation between aqueous and blood PO 2 (r=0.537, P<0.001). The contribution of atmospheric oxygen to aqueous PO 2 was 23.7%. Conclusion: Aqueous oxygen tension is mostly dependent on the systemic circulation and in part on the atmosphere. Increasing inspiratory oxygen and transcorneal oxygen delivery both increase aqueous PO 2 levels.

We analyzed survival of 185 adult patients on maintenance hemodialysis (9 h/wk to 12 h/wk) at Ema... more We analyzed survival of 185 adult patients on maintenance hemodialysis (9 h/wk to 12 h/wk) at Emam Khomini Hospital in Ahvaz, Iran. Patient survival at 1, 3, and 5 years was 89.2%, 69.2%, and 46.8%, respectively. There was no significant difference between diabetic and nondiabetic patients in 1-year survival (87.1% versus 89.7%, P = .66). But, 3-and 5-year survival rates of diabetic patients were significantly lower than those of nondiabetic patients (52.2% versus 73.8%, P = .04; zero versus 56.9%, P < .001; respectively). Based on our findings, the survival of diabetic patients undergoing hemodialysis was much worse than survival of nondiabetic patients. Thus, prevention of diabetic nephropathy should be more emphasized; and if end-stage renal disease is present, other renal replacement therapies such as kidney transplantation must be considered as soon as possible.

Based on a prospective cohort study, ADFC (Ahvaz Diabetic Foot Cohort) study, carried out in a university hospital, all of the patients with diabetes were followed up for new diabetic foot ulcer (DFU). The time of ulcer development was final outcome during two years in the present study. To analyze the data, the variables were first evaluated by univariate analysis. Subsequently variables with P value <0.2 were tested in multivariate analysis, using backward elimination multiple Cox regression. From among 605 eligible patients of ADFC study, 566 patients without foot ulcer were included for a 2- years follow-up. Background and aims: Patient education as one of the pillars of clinical governance has a special... more Background and aims: Patient education as one of the pillars of clinical governance has a special importance in all patients including burn victims. This study employed a model of organizational culture in order to optimize the institutionalization of patients' education for anxiety and increasing burned patients' satisfaction. Methods: In this clinical trial quasi experimental study, 80 burned hospitalized patients were included in the intervention group and control group. First, the intervention was consisted of sessions based on Organizational Culture Improvement Model of Patient Education in order to institutionalize patient education for nurses and nurse managers in dimensions of the effective communication process, cooperative decision making, goal setting, planning, implementing and recording, supervision and control, and incentive increasing. Results: Findings showed that the means of situational anxiety in the control group and intervention group were 47.12±4.40 and 45.55±4.86, respectively before the intervention which was not be statistically significance. But the mean of situational anxiety were 43.70±5.36 and 38.45±4.08 in the control and intervention groups respectively after the intervention which showed statistical significance (P=0.002). The means of satisfaction in control and intervention groups before the intervention were 18.82±5.76 and 20.75±4.22 respectively which reached to 21.02±6.56 and 26.75±4.51 respectively after the intervention, that showed a significant increase in satisfaction of education in intervention group (P=0.001). Conclusion: This model represents the results of a strategy to improve patient education through the promotion of academic skills and motivation of nurses increasing of the system's commitment on and responsibility for optimized implementation of patient education. So, applying this model will play an important role in the patients' health.

Nurses and administrators can identify and implement mechanisms to manage, change, and improve th... more Nurses and administrators can identify and implement mechanisms to manage, change, and improve the ethical climate in their workplace through an understanding and awareness of this concept as well as how it is perceived by everyone involved. This study aimed to compare the nurses' perception of the actual and ideal organizational ethical climate in hospitals of Ahwaz Juondishapour University of Medical Sciences. This was a cross-sectional descriptive study conducted on 558 nurses working at 9 medical centers of Ahwaz Jondishapour University of Medical Sciences selected by systematic sampling. Data instrument consisted of 2 parts: 1) demographic characteristics, and 2) actual & ideal climate questionnaire based on the Farsi version of Olson's ethical climate questionnaire, used to assess nurses' perception of the hospital ethical climate in 5 categories (managers, colleagues, physicians, patients, and hospitals). Descriptive and inferential statistical tests, independent T-test and One-Way ANOVA were used to analyze the data. Results demonstrated a positive hospital ethical climate (mean = 3.66), while the nurses' perception of the ideal ethical climate was positive as well. A significant difference was found between the mean scores of the nurses' perceptions of actual & ideal ethical climates ( P <0.05). Results demonstrated a significant relationship between nurses' work experience & organizational level, and their perceptions of the hospital ethical climate. A positive ethical climate exists in hospitals, although it is far from the ideal ethical climate, and therefore managers must maintain the actual ethical climate and at the same time implement strategies to improve their hospitals' ethical climate.

This cross-sectional study consisted of 159 participants. IOP of one eye of each subject was measured consecutively with IOPen and GAT. Then CCT was measured using an ultrasonic pachymeter. Based on GAT IOP readings, participants were divided into low, normal and high IOP groups. Correlation between tonometers and CCT was calculated by spearman's correlation coefficient. Agreement between tonometers was evaluated using Bland-Altman method. Purpose: To measure oxygen tension in the aqueous humor of human eyes under different oxygenation... more Purpose: To measure oxygen tension in the aqueous humor of human eyes under different oxygenation conditions. Methods: This prospective comparative interventional case series consisted of two parts. In the first part, 120 consecutive patients scheduled for cataract surgery were randomized into group I (control group) in which surgery was performed under local anesthesia inhaling 21% oxygen; group II in whom general anesthesia using 50% oxygen was employed; and group III receiving general anesthesia with 100% oxygen. After aspirating 0.2 ml aqueous humor under sterile conditions, the aqueous sample and a simultaneously drawn arterial blood sample were immediately analyzed using a blood gas analyzer. In part II the same procedures were performed in 10 patients after fitting a contact lens and patching the eye for 20 minutes (group IV) and in 10 patients after transcorneal delivery of oxygen at a flow rate of 5 L/min (group V). Results: Mean aqueous PO 2 in groups I, II and III was 112.3±6.2, 141.1±20.4, and 170.1±27 mmHg, respectively (P values <0.001) and mean arterial PO 2 was 85.7±7.9, 184.6±46, and 379.1±75.9 mmHg, respectively (P values <0.001). Aqueous PO 2 was 77.2±9.2 mmHg in group IV and 152.3±10.9 mmHg in group V (P values <0.001). There was a significant correlation between aqueous and blood PO 2 (r=0.537, P<0.001). The contribution of atmospheric oxygen to aqueous PO 2 was 23.7%. Conclusion: Aqueous oxygen tension is mostly dependent on the systemic circulation and in part on the atmosphere. Increasing inspiratory oxygen and transcorneal oxygen delivery both increase aqueous PO 2 levels.

We analyzed survival of 185 adult patients on maintenance hemodialysis (9 h/wk to 12 h/wk) at Ema... more We analyzed survival of 185 adult patients on maintenance hemodialysis (9 h/wk to 12 h/wk) at Emam Khomini Hospital in Ahvaz, Iran. Patient survival at 1, 3, and 5 years was 89.2%, 69.2%, and 46.8%, respectively. There was no significant difference between diabetic and nondiabetic patients in 1-year survival (87.1% versus 89.7%, P = .66). But, 3-and 5-year survival rates of diabetic patients were significantly lower than those of nondiabetic patients (52.2% versus 73.8%, P = .04; zero versus 56.9%, P < .001; respectively). Based on our findings, the survival of diabetic patients undergoing hemodialysis was much worse than survival of nondiabetic patients. Thus, prevention of diabetic nephropathy should be more emphasized; and if end-stage renal disease is present, other renal replacement therapies such as kidney transplantation must be considered as soon as possible.
