Applications of lasers in phototherapy have been the trend for the last few decades. The photodyn... more Applications of lasers in phototherapy have been the trend for the last few decades. The photodynamic therapy process normally depends on photosensitizers and laser beams. Through this study, indocyanine green has been used as a photosensitizer, which is normally activated using laser lines between 750 and 805 nm. The activity of the indocyanine green to do fluorescence by other pulsed laser sources has been tested by fluorescence technique, and it has been proven that the laser lines at 810, 940, and 980nm are able to excite the indocyanine green with different extents. The indocyanine green activation has been tested by several laser lines (810, 940, and 980 nm) commonly used as surgical lasers. The generated oxygen has been measured after irradiating the indocyanine green with the different laser lines. A comparison has been made between laser irradiation as a pinpoint and a broad beam. Osteocalcin is the major noncollagenous calcium-binding single chain protein of bone and dentin matrix and it is a bone marker whose concentration in circulation has been used as a reflector of bone turnover.The present study was conducted to evaluate osteocalcin levels in healthy and diseased periodontium and to correlate the changes in its levels with the changes in the clinical and radiographic parameters before and after treatment. Methods: A total of 30 patients with stage III grade B periodontitis were divided into the following three groups: group I patients, who received propolis in a chitosan polymer gel with non-surgical therapy, group II patients, who received propolis in a polyox polymer gel with non-surgical therapy, and group III patients who served as a control treated with non-surgical therapy only. Clinical parameters were assessed at baseline, one month, and three months. Results: At three months, the mean gingival index (GI) of groups I and II was the same (0.6 ±0.52), and there was no change in the mean GI in group III. Guided tissue regeneration (GTR) is a powerful modality for periodontal regeneration, but it blocks the periosteum and gingival stem cells (GMSCs), from supporting periodontal wound by the nutrients, growth factors, and regenerative cells. The microperforated membrane considered a rewarding solution for this major drawback; GMSCs can migrate through a GTR microperforated membrane toward a chemoattractant, with the blocking of other unfavorable epithelial cells and fibroblasts. In the absence of a sole marker for MSC, a homogeneous population of GMSC is difficult to isolate; using CD146 as confirmatory markers for MSC identification, testing the behaviour of such homogeneous population in migration dynamics was the question to answer in this study. GMSCs were isolated from healthy gingival tissues of patients undergoing crown-lengthening surgical procedures. GMSCs were characterized following the International Society for Cellular Therapy guidelines. After incubation of the GMSCs with titanium discs, osteogenic differentiation was induced for 28 days. Osteogenic lineage was confirmed by means of Alizarin Red S staining. Bone nodule morphology and deposition by GMSCs were characterized by scanning electron microscopy (SEM). An elemental analysis of the bone nodules was done using energy-dispersive x-ray spectroscopy (EDS). The biochemical composition of these nodules was further characterized via Raman spectroscopy, with native alveolar bone used as a control. Results: At three months, the mean gingival index (GI) of groups I and II was the same (0.6 ±0.52), and there was no change in the mean GI in group III. There was a reduction in the mean probing depth (PD) in group I (4.80 ±0.63) and group II (4.90 ±0.74) at the end of the study. The greatest percent gain in clinical attachment level (CAL) was noted in group II (17.26 ±6.71) followed by group I (5.93 ±9.87), whereas the least percent decrease was noted in group III (3.67 ±7.77). Conclusion: The adjunctive use of propolis in a polyox polymer with non-surgical therapy demonstrated superior clinical results over the use of propolis in a chitosan polymer in periodontitis patients.
