Objective: Three clinical cases of Aneurysm in the abdominal Aorta are presented, in order to mak... more Objective: Three clinical cases of Aneurysm in the abdominal Aorta are presented, in order to make a clinical review of aneurysms in the abdominal Aorta (AAA) and underline the importance of their early diagnosis in Primary Care, given the risk factors. Design: A retrospective, descriptive analysis of three cases of AAA from 1991. SITE. The patients studied had attended the hospital emergency department in the Alcoy Health area. Main measurements and results: Case 1: A patient, who attended because of three days of bilateral lumbar pain of a colic type spreading to the mesogastrium, which did not respond to treatment for nephritic colic. The patient died in a few hours, with the diagnosis of hypovolaemic shock caused by a burst AAA. Case 2: A patient with generalised continuous abdominal pain, which over 12 hours had been located in the lower right hemiabdomen, who was admitted with the provisional diagnosis of acute pyelonephritis. Case 3: A patient who, after an emergency surgical intervention where the diagnosis of AAA was confirmed, presented after a month a clinical picture consistent with a cerebral vascular accident and died afterwards because of renal failure. Conclusion: Given the present increase of patients diagnosed in hospital as suffering AAA in our Health Area, we urge that special attention be paid to the early diagnosis of this pathology. It is important to look actively for this pathology in patients with chronic pathologies of high prevalence and include it in Primary Care preventive programmes.

The body mass index (BMI) is the most practical method to measure and compare obesity between ind... more The body mass index (BMI) is the most practical method to measure and compare obesity between individuals. The Percentage of Excess BMI Loss (PEBMIL) is used to present results in operated patients and is based on the premise that a BMI-25 is the final aim, on being the upper limit in normal subjects. It is possible to achieve a BMI-25 in morbid obese (MO) patients with initial low BMIs (&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;lt;50) but it is rare in overweight (OW) patients with a BMI&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;gt;50. Expected BMI (EBMI) would be that which should be reached by all subjects depending on their initial BMI. The objective of this study is to search for, using statistical methods, a formula based on clinical evidence that can identify the EBMI depending on the initial BMI. We analysed the initial and final BMI in a group of 135 MO patients, operated on using the duodenal switch procedure and with a follow up of over 3 years. A linear regression method has been used to obtain a formula that could calculate the EBMI of each patient operated on. We obtained an algorithm in which EBMI=Initial BMIx0.33+14. If we apply the individualised EBMI instead of the BMI-25, the median PEBMIL was 99.48 (range: 76.75-110.46). This result suggests that the application of an individual EBMI is a more reliable estimate of the success or failure of bariatric operations.

Describe the possible complications that may occur after bariatric surgery. They may be immediate or long-term. Some are very serious such as hemorrhage pulmonary embolism, poor ventilation, leaks, poor weight loss, malnutrition, intestinal obstructions, liver failure, calcium metabolism, or vitamin disorders. Prevention, high suspicion, confirmation and specialized treatment should be practiced.
