The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. BACKGROUND: Differentiated thyroid cancers with secondaries in sites other than the neck are rela... more BACKGROUND: Differentiated thyroid cancers with secondaries in sites other than the neck are relatively uncommon, and even rarer are the initial atypical presentation i.e. symptoms that are not related to the neck. The objective of this study was to bring to the fore the atypical cases of patients diagnosed with well-differentiated thyroid cancer and the complicating distant metastasis; with specific emphasis on presentation, management and outcome of patients with thyroid malignancy. METHODS: A retrospective cross-sectional descriptive study; in which we conducted a review of casefiles of 126 patients who had thyroid surgeries. There were 48 cases of well-differentiated thyroid cancer with confirmed histopathological diagnosis on the final analysis and we subsequently identified and reviewed 5 patients who initially presented with unusual complaints to the thyroid clinic. RESULTS: During the study period under review, forty-eight consecutive patients, 4 male (8.4%) and 44 females (91.6%) with well-differentiated thyroid cancer, who had more than two follow-up visits to the Thyroid Clinic, over the last two years were included in this study. Subsequently, we described the profile of 5 selected patients with atypical presentations including four patients diagnosed with follicular carcinoma, and one patient with papillary carcinoma. CONCLUSION: It is glaring from this case series that differentiated thyroid cancer can present with atypical manifestations. For the clinicians worldwide, a routine preoperative physical examination of the thyroid patient is mandatory. A high index of clinical suspicion, aggressive work-up and treatment are usually rewarding. We equally suggest and encourage a comprehensive multi-center study for a better understanding of the natural history of the thyroid cancer disease process.

We sought to determine the authenticity of interobserver variability in ultrasound scan in a resource-constrained mission hospital settings, northwestern region of Cameroon. <b>METHODS:</b> In this study, we reviewed the standardized diagnostic approach in acute appendicitis and also performed prospective cross observational qualitative testing using sensitivity, specificity, positive predictive value, negative predictive value, and accuracy to determine the interobserver variability of ultrasonography using the medical database of the two Mission Hospitals, northwestern region of Cameroon from January 2012 to December 2016. A sequential non-randomized convenient sampling was used and data was analyzed using the Statistical Package for the Social Sciences version 22. <b>RESULTS: </b>A total of 103 patients who had pre-operative evaluation with Ultrasound scan and subsequently underwent surgery with histo-pathological examination of the removed appendix were analyzed. Their ages ranged from 15 to 65 years with a mean age of 30.6±18. There were 62 males and 41 females with a ratio of 1.5:1. Of the 103 patients, (n=90; 87.4%) cases were diagnosed as AA by histopathology while (n= 9; 8.7%) cases were negative. Ultrasound was positive in (n=75; 72.8%) cases, equivocal in (n=16; 15.5%), and negative in (n=10; 10.4%) cases. Sensitivity of ultrasound from this study when compared with ultrasound as the gold standard was 90.2% with a specificity of 85.6%; while Overall accuracy was 72.4%. Such reports are expected to provide preventive and improvement guidelines in our settings in the long run. Methods: Forty-two consecutive patients under fifteen with burns injuries managed at our facility were prospectively enrolled in the study. Results: The study population was 42 patients, which consisted of 23 (54%) male and 19 (46%) female, with the median 2.40±3.05 years, interquartile range of 1-14 years. The most frequent timing of burns was about mid-day (median 11:50 am). The majority of injuries were right at the patient's homes (70% cases). Interestingly, 4%, 15%, and 20% applied raw-egg, ice, and nothing as the first intervention for their children burns, respectively. Scald burns were the leading category of wounds (p<0.012). BACKGROUND: Enterocutaneous fistula remains an enigma and constitutes one of the most challenging... more BACKGROUND: Enterocutaneous fistula remains an enigma and constitutes one of the most challenging situations to the patient, the caregiver, and the surgeon. Notwithstanding the conglomerates of recent progress in critical care, therapeutics, as well as nutritional support; there is yet attendant high mortality and morbidity. Interestingly, there is the obscurity of the work of literature on the condition in most African settings. Therefore, this article aimed to provide updated data on this critical postoperative complication of abdominal surgery. METHOD: Published articles discussing enterocutaneous fistula up to April 2020 were obtained for review from Medline search, medical libraries, and Google. The google search was done in the English language. Interestingly, the search was conducted using the words “enterocutaneous fistula,” “intestinal fistula,” “bowel fistula,” “enteric fistula,” “fistula surgery,” and “fistula and surgical diseases burden”. Ultimately, sixteen materials were included in the quantitative analysis. Some were excluded from the subgroup analyses. DISCUSSION: Treatment of enterocutaneous fistula remains a surgical challenge despite the recent improvement of supportive patient care. Once enterocutaneous fistula occurs, adequate stabilization of the patient, a thorough investigation of the fistula anatomy, and nonoperative management should initially be attempted. The majority of the uncomplicated enterocutaneous fistula will close spontaneously when adequately managed. Interestingly, a simple suture of the fistula is not recommended. However, to intervene surgically as per uncomplicated fistula requires careful planning, meticulous dissection, resection, and anastomosis or bypass procedures are the preferred surgical procedures; besides, reconstruction of the abdominal wall is critical. A multidisciplinary team approach with a gastroenterologist, interventional radiologists, dieticians, enterostomal therapy nurses, and surgeons will maximize successful enterocutaneous fistula closure.

Background: The study sought to test the knowledge and practices of breast self-examination (BSE)... more Background: The study sought to test the knowledge and practices of breast self-examination (BSE) amongst women attending a surgical outpatient clinic in a northwestern region of Cameroon. Based on the information, we aimed to undertake an education intervention program focused on dealing with any knowledge and practice gaps discovered. Methods: A cross-sectional descriptive research design was used. A computer-based random sampling technique was adopted to select 200 female patients attending a surgical outpatient clinic in Shisong, the northwestern region of Cameroon. We obtained our data using pre-tested proforma which composed of four sections: I) Socio-demographic characteristics, II) knowledge about BSE and breast cancer, III) practices of BSE and IV) factors affecting BSE. Data obtained was analyzed using the Statistical Package for the Social Sciences (SPSS) version 22 and presented with inferential descriptive statistics. Results: Results showed that women attending a surgical outpatient clinic showed good knowledge of early warning signs and symptoms about breast cancer as well as BSE. About 36% of the women were confident of the timing to start BSE and 78% were conversant with how BSE is done, but were not competent in their practice. The multivariate analysis showed that the single most significant factor affecting the practice of BSE was forgetfulness, odd-ratio (95% CI)=1.55 (1.32–1.82); p=0.001. Conclusions: The female patients attending a surgical outpatient clinic in Shisong were conversant with BSE and they practiced it. Knowledge and practice of BSE may be improved significantly by encouraging an aggressive primary health awareness campaigns nationwide, training the health care providers and setting-up sustained educational framework as well as policy guidelines on BSE skills.

RESULTS: During the study period under review, forty-eight consecutive patients, 4 male (8.4%) and 44 females (91.6%) with well-differentiated thyroid cancer, who had more than two follow-up visits to the Thyroid Clinic, over the last two years were included in this study. Subsequently, we described the profile of 5 selected patients with atypical presentations including four patients diagnosed with follicular carcinoma, and one patient with papillary carcinoma. CONCLUSION: It is glaring from this case series that differentiated thyroid cancer can present with atypical manifestations. For the clinicians worldwide, a routine preoperative physical examination of the thyroid patient is mandatory. A high index of clinical suspicion, aggressive work-up and treatment are usually rewarding. We equally suggest and encourage a comprehensive multi-center study for a better understanding of the natural history of the thyroid cancer disease process.

We sought to determine the authenticity of interobserver variability in ultrasound scan in a resource-constrained mission hospital settings, northwestern region of Cameroon. <b>METHODS:</b> In this study, we reviewed the standardized diagnostic approach in acute appendicitis and also performed prospective cross observational qualitative testing using sensitivity, specificity, positive predictive value, negative predictive value, and accuracy to determine the interobserver variability of ultrasonography using the medical database of the two Mission Hospitals, northwestern region of Cameroon from January 2012 to December 2016. A sequential non-randomized convenient sampling was used and data was analyzed using the Statistical Package for the Social Sciences version 22. <b>RESULTS: </b>A total of 103 patients who had pre-operative evaluation with Ultrasound scan and subsequently underwent surgery with histo-pathological examination of the removed appendix were analyzed. Their ages ranged from 15 to 65 years with a mean age of 30.6±18. There were 62 males and 41 females with a ratio of 1.5:1. Of the 103 patients, (n=90; 87.4%) cases were diagnosed as AA by histopathology while (n= 9; 8.7%) cases were negative. Ultrasound was positive in (n=75; 72.8%) cases, equivocal in (n=16; 15.5%), and negative in (n=10; 10.4%) cases. Sensitivity of ultrasound from this study when compared with ultrasound as the gold standard was 90.2% with a specificity of 85.6%; while Overall accuracy was 72.4%. Age and gender had no significant relationship with the accuracy of ultrasound in this study (p value=0.2 and 0.7 respectively). <b>CONCLUSION:</b> Ultrasound scan is more useful in detecting than in ruling out appendicitis. Such reports are expected to provide preventive and improvement guidelines in our settings in the long run. Methods: Forty-two consecutive patients under fifteen with burns injuries managed at our facility were prospectively enrolled in the study. Results: The study population was 42 patients, which consisted of 23 (54%) male and 19 (46%) female, with the median 2.40±3.05 years, interquartile range of 1-14 years. The most frequent timing of burns was about mid-day (median 11:50 am). The majority of injuries were right at the patient's homes (70% cases). Interestingly, 4%, 15%, and 20% applied raw-egg, ice, and nothing as the first intervention for their children burns, respectively. Scald burns were the leading category of wounds (p<0.012). Notwithstanding the conglomerates of recent progress in critical care, therapeutics, as well as nutritional support; there is yet attendant high mortality and morbidity. Interestingly, there is the obscurity of the work of literature on the condition in most African settings. Therefore, this article aimed to provide updated data on this critical postoperative complication of abdominal surgery. METHOD: Published articles discussing enterocutaneous fistula up to April 2020 were obtained for review from Medline search, medical libraries, and Google. The google search was done in the English language. Interestingly, the search was conducted using the words “enterocutaneous fistula,” “intestinal fistula,” “bowel fistula,” “enteric fistula,” “fistula surgery,” and “fistula and surgical diseases burden”. Ultimately, sixteen materials were included in the quantitative analysis. Some were excluded from the subgroup analyses. DISCUSSION: Treatment of enterocutaneous fistula remains a surgical challenge despite the recent improvement of supportive patient care. Once enterocutaneous fistula occurs, adequate stabilization of the patient, a thorough investigation of the fistula anatomy, and nonoperative management should initially be attempted. The majority of the uncomplicated enterocutaneous fistula will close spontaneously when adequately managed. Interestingly, a simple suture of the fistula is not recommended. However, to intervene surgically as per uncomplicated fistula requires careful planning, meticulous dissection, resection, and anastomosis or bypass procedures are the preferred surgical procedures; besides, reconstruction of the abdominal wall is critical. A multidisciplinary team approach with a gastroenterologist, interventional radiologists, dieticians, enterostomal therapy nurses, and surgeons will maximize successful enterocutaneous fistula closure.

Background: The study sought to test the knowledge and practices of breast self-examination (BSE)... more Background: The study sought to test the knowledge and practices of breast self-examination (BSE) amongst women attending a surgical outpatient clinic in a northwestern region of Cameroon. Based on the information, we aimed to undertake an education intervention program focused on dealing with any knowledge and practice gaps discovered. Methods: A cross-sectional descriptive research design was used. A computer-based random sampling technique was adopted to select 200 female patients attending a surgical outpatient clinic in Shisong, the northwestern region of Cameroon. We obtained our data using pre-tested proforma which composed of four sections: I) Socio-demographic characteristics, II) knowledge about BSE and breast cancer, III) practices of BSE and IV) factors affecting BSE. Data obtained was analyzed using the Statistical Package for the Social Sciences (SPSS) version 22 and presented with inferential descriptive statistics. Results: Results showed that women attending a surgical outpatient clinic showed good knowledge of early warning signs and symptoms about breast cancer as well as BSE. About 36% of the women were confident of the timing to start BSE and 78% were conversant with how BSE is done, but were not competent in their practice. The multivariate analysis showed that the single most significant factor affecting the practice of BSE was forgetfulness, odd-ratio (95% CI)=1.55 (1.32–1.82); p=0.001. Conclusions: The female patients attending a surgical outpatient clinic in Shisong were conversant with BSE and they practiced it. Knowledge and practice of BSE may be improved significantly by encouraging an aggressive primary health awareness campaigns nationwide, training the health care providers and setting-up sustained educational framework as well as policy guidelines on BSE skills.
