In the era of oral pharmacotherapy for erectile dysfunction, penile prosthesis implantation conti... more In the era of oral pharmacotherapy for erectile dysfunction, penile prosthesis implantation continues to provide a reliable and effective treatment modality with a high satisfaction rate, although it is now regarded as a last line of therapy. The most troublesome complication of prostheses is infection that may result in further multiple surgeries, loss of penile tissue, economic losses, and frustration for both patients and physicians. Adopting a high standard of sterile techniques, administering prophylactic antibiotics, following careful surgical procedures, and choosing devices with an antibiotic coating should significantly decrease the occurrence of infection. When facing infection of a penile prosthesis, early identification and prompt removal of all foreign bodies should produce favorable results. However, reimplantation in infected cases is a difficult task due to scar formation in the corporal bodies and shortening of the penis. Another promising choice is a salvage procedure with aggressive lavage and immediate or delayed (within 72 hours) replacement with a new device. The advantages of the salvage procedure are that most of the length of the penis can be maintained and it has a success rate exceeding 80%.

Satisfaction (EDITS) and its partner version questionnaire were used to assess couples' satisfact... more Satisfaction (EDITS) and its partner version questionnaire were used to assess couples' satisfaction with the use of phosphodiesterase type 5 (PDE5) inhibitors to treat erectile dysfunction (ED) over a 3-month period. Of 161 ED patients, who together with their female partners were invited to answer separate questionnaires at home, 111 patients (68.9%; mean age 61.8 (23-87) years) and female partners (mean age 52.8 (22-77) years) returned completed questionnaires. Patients reported a substantially higher treatment satisfaction score and level of satisfaction with ED treatment than their female partners (Po0.001). Patients with milder severity of ED at baseline and better erectile function after treatment were more likely to be satisfied with the outcome of the treatment. Of the different aspects of satisfaction that patients were asked about, they reported the lowest level of satisfaction about their partners' feeling about continued treatment for ED. Our study shows that more patients than their female partners are more satisfied with medical treatment for ED. To maintain long-term therapy for ED, it is important to include female partners in the assessment and management of the therapy.

Each year, many patients with prostate cancer undergo radical prostatectomy (RP) therapy. One of the most common RP complications is erectile dysfunction (ED). Although consensus guidelines for the management of sexual dysfunction after prostate cancer surgery have been developed for many Western and Asian countries, no such clinical practice guidelines have been developed for Taiwan. The consensus opinions expressed in this article were discussed by numerous experienced physicians in Taiwan, based on both existing international guidelines and their individual experiences with clinical trials and providing advice to clinical physicians on how to inform patients of the risk of ED prior to surgery. Men receiving testosterone therapy often experience resolution or improvement in their sexual symptoms and non-sexual health benefits. Although testosterone has been available since 1930s, the diagnosis and management of TD can be challenging and many controversies still exist. Clinical assessment based on symptoms and signs with a confirmatory testosterone level below normal range are required for diagnosis of TD. Screening for TD in the general population is not recommended. Screening for TD should be taken in men who report symptoms or signs associated with TD, particularly sexual dysfunctions as well as in men with obesity, type 2 diabetes, and metabolic syndrome. The International Society for Sexual Medicine recommended that symptomatic men with total T <350 ng/dL (12 nmol/L) should be treated with T therapy and a trial of T therapy in symptomatic men with total T levels >350 ng/dL can be considered based on clinical presentation. Patients who report no symptomatic improvement after at least 3 to 6 months of testosterone therapy despite adequate serum T levels should discontinue treatment.

It can be distinguished from ischemic priapism by history taking and oxygenated, nonacidotic blood in cavernous blood gas analysis. The diagnosis is further confirmed by elevated cavernosal artery blood flow under color Doppler ultrasound. This article reviews the diagnosis and intervention of two cases with nonischemic priapism. The patients received selective arterial embolization with either microcoil or Gelfoam. Both of them were satisfied with their erectile function 1 week after embolization. Our practice reflects that selective arterial embolization is a safe and effective treatment for patients with nonischemic priapism.

Aim: We investigated the changes of intracavernous pressure (ICP) elicited by cavernous nerve stimulation after a single and repeated dosing of METH in male rats. Methods: Rats were randomly assigned to five treated groups and one control group with each group having 3 rats. Rats in treated group 1, 2, and 3 received a single dose intravenous injection with 0.1, 1.0, and 10.0 μg/kg METH, respectively. Rats in treated group 4 and 5 received an intraperitoneal injection with 1.0 and 5.0 mg/kg METH four times daily for two weeks, respectively. ICP was measured during penile erection elicited by cavernous nerve stimulation. Expression of neural nitric oxide synthase (nNOS) was measured in the cavernous nerve and muscle after single and repeated dosing. Main Outcome Measures: The primary outcome measure was the ΔICP/mean arterial pressure (MAP) and the secondary was the expression of nNOS in the tissue. Results: The ΔICP/MAP increased slightly in group 1 rats and decreased substantially in group 2 and 3 rats compared with the baseline. A single dose of METH within the range of 0.1 to 10.0 μg/kg exhibited an inhibitory effect of ICP (%). The ΔICP/MAP significantly decreased in group 4 and 5 rats after repeated dosing of METH, compared with that in group 3. The expression of nNOS significantly increased in the cavernous muscle after single and repeated dosing of METH compared with the control. Conclusions: The preliminary results suggested that a single dose of METH exhibits an inhibitory effect on ICP and repeated dosing of METH exerted a greater inhibition than a single dosing. However, these results need further study.

Although hair dyes were identified as carcinogenic in animals and increased risk of some cancers among hairdressers, the existing epidemiological data did not support that personal hair dye use increased risk of cancers, even for bladder cancer. Given that Tai et al.’s report of a potential hazard of personal hair dye use on risk of prostate cancer was particular, the methodology of the study was scrutinized and some flaws were found including the issue of external validity.

With the advent of phosphodiesterase type 5 (PDE-5) inhibitors, the treatment of ED has been revolutionized during the last two decades. Although there are great advances in the diagnosis and treatment of ED, there are two common controversial issues in our practice that merit further discussion. First, psychogenic or organic ED? ED is traditionally classified as organic and psychogenic based on the etiology. Many problems arise with this distinction. Psychogenic ED is a misnomer because it is based on an obsolete view of mind-body distinctions, ignores the manifestations of neurobiology of psychological disorders and the fundamental meaning of psychosomatic result, is too often diagnosed by exclusion and may indicate that ED all result from mind problem. The choice of surgical procedure depends on the type of injury, and an avulsion from upper ureter avulsed from upper portion and wrap it with an omentum flap in the case study of a 51-year-old male. A satisfactory outcome of this case and the simplicity of the surgical technique of reposition render it a treatment option for an avulsed ureter.

Although hair dyes were identified as carcinogenic in animals and increased risk of some cancers among hairdressers, the existing epidemiological data did not support that personal hair dye use increased risk of cancers, even for bladder cancer. Given that Tai et al.’s report of a potential hazard of personal hair dye use on risk of prostate cancer was particular, the methodology of the study was scrutinized and some flaws were found including the issue of external validity.

The study was undertaken to explore the effect of CP55,940 ((-)-cis-3-[2-Hydroxy4-(1,1-dimethylhe... more The study was undertaken to explore the effect of CP55,940 ((-)-cis-3-[2-Hydroxy4-(1,1-dimethylheptyl) phenyl]-trans-4-(3-hydroxypropyl)cyclohexanol), a drug commonly used as a CB1/CB2 cannabinoid receptor agonist, on intracellular free Ca2+ levels ([Ca2+]i) in several cell types [Ca2+]i was measured in suspended cells by using the fluorescent dye fura-2 as an indicator. At concentrations between 1-50 microM, CP55,940 increased [Ca2+]i in a concentration-dependent manner with an EC50 of 8 microM. The [Ca2+]i signal comprised an initial rise, a slow decay, and a sustained phase. CP55940 (10 microM)-induced (Ca2+]i signal was not altered by 5 microM of two cannabinoid receptor antagonists (AM-251, N-(Piperidin-1-yl)-5-(4-iodophenyl)-1-(2,4-dichlorophenyl)-4-methyl-1H-pyrazole-3-carboxamide; AM-281, 1-(2,4-Dichlorophenyl)-5-(4-iodophenyl)-4-m3thyl-N-4-morpholinyl-1H-pyrazole-3-carboxamide). Materials and Methods This multi-national cross-sectional survey was administered to clinicians working in major Asia-Pacific cities during the mandatory social lockdown period in June 2020. Clinical demographics and professional data, delivery of Andrology-related healthcare services, and patient distress based on validated questionnaires such as Depression and Anxiety Stress Scales (DASS) and Decisional Engagement Scale (DES) were collected. Results Telehealth medicine was instituted in all the centres with the majority of centres (92.9%) reported a 50% or more reduction in out-patient related services.
