Parsiyel pulmoner venöz dönüş anomalisi, bir ya da daha fazla pulmoner venin sağ atriuma ya da sa... more Parsiyel pulmoner venöz dönüş anomalisi, bir ya da daha fazla pulmoner venin sağ atriuma ya da sağ boşluklara açılan sistemik dolaşım bölgelerine dökülmesiyle oluşur. Olgu sunumumuzdaki 1 yaş kız hastada sık akciğer enfeksiyonu sebebi ile çekilen BT'de scimitar sendromu düşündüren venöz dönüş anomalisi tespit edilmiş olup hastanın cerrahisi başarı ile gerçekleştirilmiştir. Bu anomali nadir rastlanan bir anomali olup preoperatif, intraoperatif ve postoperatif değerlendirmesi ve yönetimi önem arz etmektedir.

Background/aim: Recovery after coronary artery bypass graft surgery (CABG) can be complicated, le... more Background/aim: Recovery after coronary artery bypass graft surgery (CABG) can be complicated, leading to postoperative morbidity. The roles of hematologic and surgery-related parameters are important. The main purpose of this study is to determine the role of preoperative and postcardiopulmonary bypass neutrophil/lymphocyte ratio (NLR) on postoperative recovery. Materials and methods: Sixty-two patients aged between 41 and 80 years, scheduled for elective CABG surgery with ASA I-II risk and without a history of preoperative blood transfusion, were included in the study. Three patients were excluded due to their need for additional surgical procedures other than CABG. The patients were divided into two groups that were formed depending on preoperative NLR cutoff values below (Group 1, n = 37) and above 4 (Group 2, n = 22). Postoperative data such as length of stay in the hospital and in the intensive care unit (ICU), chest tube drainage, and incidence of atrial fibrillation were recorded for all patients. Results: Preoperative NLR was significantly lower in Group 1 (P < 0.0001), and there was no significant difference between the groups in terms of postoperative NLR (P = 0.217) when the two groups were compared. The patients in Group 2 had a longer length of stay in the ICU (P = 0.035) and in the hospital (P = 0.034). There was a positive correlation between preoperative NLR and length of stay in the ICU (P = 0.017) and the hospital (P = 0.014). No statistically significant differences in postoperative drainage or incidence of postoperative atrial fibrillation were detected between the two groups. Conclusion: The results of our study demonstrate that the postoperative NLR may be useful to predict the length of hospital and ICU stays and help the management of follow-up and treatment processes in patients undergoing CABG surgery.

Bu nedenle, uygulamış olduğumuz kombine yöntemi paylaşmış bulunuyoruz.

Sıklıkla asemptomatik seyreden bu durum, semptomatik olduğunda cerrahi tedavi yapılması gerekmektedir. Seçilecek olan yaklaşım yöntemi ve uygulanacak cerrahi teknik hastadan hastaya farklılık gösterdiğinden dolayı, olumlu sonuç alabilmek için iyi bir durum yönetimi yapılması gerekmektedir. Biz bu olguyu, görece sık görülen bu anomaliyi hatırlatması ve uyguladığımız tekniğin diğer meslektaşlarımıza yol gösterici olması amacıyla paylaşmayı uygun görüyoruz.

Aim/Introduction: Acute ischemia reperfusion (IR) injury observed in the lower extremities occurs... more Aim/Introduction: Acute ischemia reperfusion (IR) injury observed in the lower extremities occurs especially when a temporary cross-clamp is applied to the abdominal aorta during aortic surgery. Preoperative pregabalin has been used as a part of multimodal analgesia in postoperative pain treatment in recent years. Pregabalin has become one of the increasingly common agents in postoperative analgesia. In this study, we aimed to investigate the effect of pregabalin on erythrocyte deformability in rats undergoing IR. Materials and Methods: 24 male Wistar albino rats weighing between 200-250 g were used in the study. Rats were randomly divided into 4 groups of 6 rats each (Control, Ischemia-Reperfusion (IR), IR-Pregabalin 50 mg (50 mg.kg-1), IR-Pregabalin 200 mg (200 mg.kg-1). Pregabalin was administered intraperitoneally 30 min before the procedure. An atraumatic microvascular clamp was placed across the infrarenal abdominal aorta in the IR groups. Following 120 min of ischemia, the clamp was removed and reperfusion was continued for 120 min. All rats were euthanized by intraperitoneal administration of ketamine (100 mg•kg −1) and taking blood from the abdominal aorta. Erythrocytes were seperated from heparinized whole blood samples. Deformability measurements were made in erythrocyte suspensions in phosphate buffered saline. A constant flow filtrometer system was used to measure erythrocyte deformability and relative resistance was calculated. Results: It was found that the formation of ischemia reperfusion increases the relative resistance according to the control group (p < 0.0001). It was determined that application of pregabalin 50 or 200 mg did not change erythrocyte deformability in ischemia reperfusion-induced rats (p = 0.632, p = 0.811). Conclusion: The administration of 50 or 200 mg of pregabalin has no negative effect on the erythrocyte deformability in ischemia reperfusion-induced rats. We think that pregabalin can be safely used for analgesia in the cases of IR. However, these findings should be supported by clinical and experimental studies carried out in more detailed and broader series.

Künt aort travmalı hastalarda parapleji, inme, kalp, böbrek ve solunum yetmezliği riski yüksektir. Torasik endovasküler aort tamiri (TEVAR) kanamanın hızlı kontrolünü sağlar ve uç organlara süratle kan akışının sağlanmasına olanak verir. Bu yazıda, TEVAR işlemi ile başarılı bir şekilde tedavi edilen sol subklavyen arterde distal yırtığı ve arterin intimal ve media tabakası tamamen ayrılmış bir kadın olgu sunuldu.

Ventricular septal defect (VSD) was repaired with a Dacron patch. The infundibular muscle was resected to allow for unobstructed flow of blood to the pulmonary valve and the area above the pulmonary valve was opened with a polytetrafluoroethylene (PTFE) patch. The patient was intubated three times bacause of the atelectasis ocurred in the lungs after the surgery. Atelectasis was treated with nebulised dornase alpha. It avoided the recurrence of atelectasis and the patient was discharged.

Objective: Ischaemia-reperfusion (IR) injury is a chain of events put in place by tissue ischaemi... more Objective: Ischaemia-reperfusion (IR) injury is a chain of events put in place by tissue ischaemia. Reperfusion following the cellular damage causes an active inflammatory response. Erythrocyte deformability and plasma viscosity play a critical role in tissue and organ perfusion. In this study we aimed to evaluate the effect of levosimendan and nigella sativa on erythrocyte deformability during myocardial IR injury in rats. Methods: Twenty-four Wistar albino rats were included in the study. The animals were randomly divided into four experimental groups. The coronary arteries of rats in Group C (control group) were not occluded or reperfused. Myocardial IR was performed by ligating the left anterior descending coronary artery for 30 min, followed by 2 h of reperfusion in the IR (IR), IR-levosimendan (24µg/kg) (IRL) group and IR-nigella sativa (0.2 mL/kg) (IRNS) group. Erythrocyte suspensions formed with a PBS buffer solution containing 5% htc were used to measure the deformability. Results: As a result compared to control group, IR increased the relative resistance, a marker of erythrocyte deformability, significantly (p<0.05). There were significant differences between the groups according to the comparisons with ANOVA test (p<0.0001). The results obtained after corrections with Bonferroni test were as follows: Comparisons of the IRL and IRNS groups revealed similar results (p=0.764). The values of the IR group were significantly higher than those of the control, IRNS and IRL groups (p<0.0001, p=0.001, p=0.003, respectively). Conclusion: Rats with IR injury had decreased erythrocyte deformability. This injury might cause more problems in microcirculation. Levosimendan and nigella sativa may be useful in reducing the adverse effects of this type of injury.

Reperfusion following the damage of cell causes an active inflammatory response. In our research we tried to evaluate the protective effect of Levosimendan and Nigella Sativa on myocardial ischemia-reperfusion injury in rats. Methods: We included twenty-four Wistar albino rats in our research. The rats were randomly divided into four experimental groups. The coronary arteries of rats in Group C (control group) were not occluded or reperfused. Left anterior descending coronary artery was ligated for 30 min to perform myocardial IR and then reperfused for 2 h in the IR (IR), IR-Levosimendan (24µg/kg) (IRL) and IR-Nigella Sativa (0.2 mL/kg) (IRNS) group. Results: Inflammation findings were significantly higher in the IR group compared with the C, IR-NS, and IR-L groups (p=0.001, p=0.019, p=0.019, respectively). Compared with the C, IR-NS, and IR-L groups, the microscopic myocardial disorganization was significantly higher among the IR group (p<0.0001, p=0.007, p=0.001, respectively). The light microscopic myocardial tissue interstitial fibrosis levels were significantly higher in the IR group than in the C, IR-NS, and IR-L groups (p<0.0001, p=0.044, p=0.003, respectively). Conclusıon: Levosimendan and NS administration at the beginning of myocardial ischemia can provide varying degrees of protection against negative effects of variations in light microscopic inflammation findings, myocardial disorganization degrees and myocardial tissue interstitial fibrosis levels.

The patients were divided into two groups that were formed depending on preoperative NLR cutoff values below (Group 1, n = 37) and above 4 (Group 2, n = 22). Postoperative data such as length of stay in the hospital and in the intensive care unit (ICU), chest tube drainage, and incidence of atrial fibrillation were recorded for all patients. Results: Preoperative NLR was significantly lower in Group 1 (P < 0.0001), and there was no significant difference between the groups in terms of postoperative NLR (P = 0.217) when the two groups were compared. The patients in Group 2 had a longer length of stay in the ICU (P = 0.035) and in the hospital (P = 0.034). There was a positive correlation between preoperative NLR and length of stay in the ICU (P = 0.017) and the hospital (P = 0.014). No statistically significant differences in postoperative drainage or incidence of postoperative atrial fibrillation were detected between the two groups. Conclusion: The results of our study demonstrate that the postoperative NLR may be useful to predict the length of hospital and ICU stays and help the management of follow-up and treatment processes in patients undergoing CABG surgery.

Background & Objective: Acute ischemia reperfusion (IR) injury observed in the lower extremities ... more Background & Objective: Acute ischemia reperfusion (IR) injury observed in the lower extremities occurs especially when a temporary cross-clamp is applied to the abdominal aorta during aortic surgery. Preoperative pregabalin has been used as a part of multimodal analgesia in postoperative pain treatment in recent years. Pregabalin has become one of the increasingly common agents in postoperative analgesia. In this study, we aimed to investigate the effect of pregabalin on erythrocyte deformability in rats undergoing IR. Methodology: 24 male Wistar albino rats weighing between 200-250 g were used in the study. Rats were randomly divided into 4 groups of 6 rats each (Control, Ischemia-Reperfusion (IR), IR-Pregabalin 50 mg (50 mg/kg), IR-Pregabalin 200 mg (200 mg/ kg). Pregabalin was administered intraperitoneally 30 min before the procedure. An atraumatic microvascular clamp was placed across the infrarenal abdominal aorta in the IR groups. Following 120 min of ischemia, the clamp was removed and reperfusion was continued for 120 min. All rats were euthanized by intraperitoneal administration of ketamine (100 mg/kg) and taking blood from the abdominal aorta. Erythrocytes were seperated from heparinized whole blood samples. Deformability measurements were made in erythrocyte suspensions in phosphate buffered saline. A constant flow filtrometer system was used to measure erythrocyte deformability and relative resistance was calculated. Results: It was found that the formation of ischemia reperfusion increases the relative resistance according to the control group (p < 0.0001). It was determined that application of pregabalin 50 or 200 mg did not change erythrocyte deformability in ischemia reperfusion-induced rats (p = 0.632, p = 0.811). Conclusion: The administration of 50 or 200 mg of pregabalin has no negative effect on the erythrocyte deformability in ischemia reperfusion-induced rats. We think that pregabalin can be safely used for analgesia in the cases of IR. However, these findings should be supported by clinical and experimental studies carried out in more detailed and broader series.

Ventricular septal defect (VSD) was repaired with a Dacron patch. The infundibular muscle was resected to allow for unobstructed flow of blood to the pulmonary valve and the area above the pulmonary valve was opened with a polytetrafluoroethylene (PTFE) patch. The patient was intubated three times bacause of the atelectasis ocurred in the lungs after the surgery. Atelectasis was treated with nebulised dornase alpha. It avoided the recurrence of atelectasis and the patient was discharged.

Reperfusion following the cellular damage causes an active inflammatory response. Erythrocyte deformability and plasma viscosity play a critical role in tissue and organ perfusion. In this study we aimed to evaluate the effect of levosimendan and nigella sativa on erythrocyte deformability during myocardial IR injury in rats. Methods: Twenty-four Wistar albino rats were included in the study. The animals were randomly divided into four experimental groups. The coronary arteries of rats in Group C (control group) were not occluded or reperfused. Myocardial IR was performed by ligating the left anterior descending coronary artery for 30 min, followed by 2 h of reperfusion in the IR (IR), IR-levosimendan (24µg/kg) (IRL) group and IR-nigella sativa (0.2 mL/kg) (IRNS) group. Erythrocyte suspensions formed with a PBS buffer solution containing 5% htc were used to measure the deformability. Results: As a result compared to control group, IR increased the relative resistance, a marker of erythrocyte deformability, significantly (p<0.05). There were significant differences between the groups according to the comparisons with ANOVA test (p<0.0001). The results obtained after corrections with Bonferroni test were as follows: Comparisons of the IRL and IRNS groups revealed similar results (p=0.764). The values of the IR group were significantly higher than those of the control, IRNS and IRL groups (p<0.0001, p=0.001, p=0.003, respectively). Conclusion: Rats with IR injury had decreased erythrocyte deformability. This injury might cause more problems in microcirculation. Levosimendan and nigella sativa may be useful in reducing the adverse effects of this type of injury.

Reperfusion following the damage of cell causes an active inflammatory response. In our research we tried to evaluate the protective effect of Levosimendan and Nigella Sativa on myocardial ischemia-reperfusion injury in rats. Methods: We included twenty-four Wistar albino rats in our research. The rats were randomly divided into four experimental groups. The coronary arteries of rats in Group C (control group) were not occluded or reperfused. Left anterior descending coronary artery was ligated for 30 min to perform myocardial IR and then reperfused for 2 h in the IR (IR), IR-Levosimendan (24µg/kg) (IRL) and IR-Nigella Sativa (0.2 mL/kg) (IRNS) group. Results: Inflammation findings were significantly higher in the IR group compared with the C, IR-NS, and IR-L groups (p=0.001, p=0.019, p=0.019, respectively). Compared with the C, IR-NS, and IR-L groups, the microscopic myocardial disorganization was significantly higher among the IR group (p<0.0001, p=0.007, p=0.001, respectively). The light microscopic myocardial tissue interstitial fibrosis levels were significantly higher in the IR group than in the C, IR-NS, and IR-L groups (p<0.0001, p=0.044, p=0.003, respectively). Conclusıon: Levosimendan and NS administration at the beginning of myocardial ischemia can provide varying degrees of protection against negative effects of variations in light microscopic inflammation findings, myocardial disorganization degrees and myocardial tissue interstitial fibrosis levels.
