We read with great interest the letter by Eggert et al , in response to our recently published ar... more We read with great interest the letter by Eggert et al , in response to our recently published article which raised concerns about the better 5-year survival of fibrolamellar hepatocellular carcinoma (fHCC) compared with conventional hepatocellular carcinoma (HCC-NOS).1 ,2 In the letter, the authors pointed out that it cannot be concluded from our meta-analysis that ‘survival in fHCC is similar to conventional HCC in non-cirrhotic liver patients’, given the overlap in the CIs of the subgroup analyses (limited to non-cirrhotic liver patients) and the overall analysis of all cases. In a recent article by Eggert et al examining the epidemiology of fibrolamellar hepatocellular carcinoma (fHCC) in the USA from 2000 to 2010,1 the authors reported better 5-year survival for fHCC compared with conventional HCC similar to previous studies.2–⇓⇓⇓⇓7 Even though several previous studies have advocated that fHCC is less aggressive than conventional HCC, some recent studies have failed to confirm the observation of a better outcome in fHCC.8–⇓⇓11 Cirrhosis is a well …

Henoch-Schönlein purpura (HSP) is a small vessel systemic vasculitis, predominantly affecting chi... more Henoch-Schönlein purpura (HSP) is a small vessel systemic vasculitis, predominantly affecting children, characterized by a tetrad of manifestations, specifically palpable purpura, arthralgia, abdominal pain, and renal disease. HSP in the adult population is rare, and no case has been described of HSP in liver cirrhosis with spontaneous bacterial peritonitis (SBP). We present the case of a 58-year-old male with liver cirrhosis, who was subsequently diagnosed with SBP and later HSP. In this patient, the diagnosis of HSP was demonstrated clinically by his palpable purpura, diarrhea, hematuria, and abdominal pain and confirmed pathologically by his renal and skin biopsies demonstrating leukocytoclastic vasculitis and IgA complexes. We believe that this is an example of altered IgA processing in cirrhosis leading to the development of IgA immune complexes and ultimately HSP. The patient additionally had SBP, which may have increased his risk for developing HSP given antigen processing by mucosa-associated lymphoid tissues leading to immune complex deposition, which may not have been effectively cleared in the context of his liver disease. The patient unfortunately died of gastrointestinal hemorrhage, which is unclear to be due to his underlying cirrhosis or a gastrointestinal manifestation of HSP itself.

Multivariate risk estimates were obtained aft er adjusting for demographics, insurance status, burden of comorbidities, alcoholism, HIV, and diabetes. Results: Th ere were total 32,172 hospital admissions with primary diagnosis of acute pancreatitis in patients with pre-existing history of chronic pancreatitis. Th ere was no signifi cant diff erence in age, sex, race, median house hold income, and insurance status between PCM and non-PCM patients. Patients with PCM were 2.8 fold for pseudocyst, 2.4 fold for pancreatectomy, 2.2 fold for sepsis, 2.8 fold for ARDS and 2.4 fold for overall complications rate. Th e PCM was also associated with 83% longer length of stay and 90% higher total charges than patients without PCM. Although not signifi cant, PCM patients were 2.3 folds at greater risk for in-hospital mortality and 2.8 fold for pseudocyst (see table). Conclusion: PCM is associated with higher risk of pancreatic pseudocyst, pancreatectomy, sepsis, ARDS, greater length of hospital stay, and hospital charges in patients with ACP. Patients with known chronic pancreatitis should be evaluated for PCM and undergo optimal nutrition management. Outpatient optimal nutrition management should be emphasized in patients with chronic pancreatitis.

Although hepatocellular changes in response to lipopolysaccharide (LPS) have been well characterized, less is known about whether and how cholangiocytes contribute to this form of cholestasis. We examined effects of endotoxin on expression and function of the type 3 inositol trisphosphate receptor (ITPR3), because this is the main intracellular Ca2+ release channel in cholangiocytes, and loss of it impairs ductular bicarbonate secretion. Bile duct cells expressed the LPS receptor, Toll‐like receptor 4 (TLR4), which links to activation of nuclear factor‐κB (NF‐κB). Analysis of the human ITPR3 promoter revealed five putative response elements to NF‐κB, and promoter activity was inhibited by p65/p50. They therefore need to be distinguished from hyperplastic polyps, and should be treated similarly to adenomas. Various endoscopic classifications for discriminating SSA/Ps have recently been proposed and validated, including the "Type II-O" pit pattern in magnifying chromoendoscopy and the "varicose microvascular vessel" in narrow-band imaging. However, there is currently no diagnostic consensus on the endoscopic appearance of SSA/Ps. Endocytoscopy (EC) is an emerging modality with diagnostic potential for SSA/Ps. EC is a type of a contact light microscopy, which allows the in vivo visualization of cells and nuclei facilitating precise, real-time pathological prediction. SSA/Ps show oval gland lumens with small round nuclei in EC, indirectly reflecting the pathological features. EC has shown a sensitivity of 83.3% and specificity of 97.8% for the diagnosis of SSA/Ps. ...
