Objective: The objective of the study is to evaluate the skeletal, dentoalveolar and soft tissue ... more Objective: The objective of the study is to evaluate the skeletal, dentoalveolar and soft tissue changes before and after treatment with Alt-RAMEC protocol and protraction headgear in comparison to the controls. Material and Methods: A quasi experimental study was conducted in orthodontic department on 60 patients of cleft lip and palate. These patients were divided into two groups. Group I was the treatment group that underwent Alt-RAMEC protocol followed by facemask therapy while group II was the control group that underwent RME followed by facemask therapy. Total treatment time in both groups was 6-7months. Mean and standard deviation was calculated for all the quantitative variables. Pre and post treatment changes between treatment and control groups were made using paired t-test. Intergroup comparison between treatment and control group was analyzed using independent t-test. Signi cance for all tests was predetermined at a P-value of ≤ 0.05 Results: Treatment group showed signi cant forward movement of maxilla and improvement in the maxillary base. Remarkable improvement in SNA was seen. The overall result was better maxillo-mandibular relationship as shown by positive ANB values and angle of convexity. More effect on maxilla and least effect on mandible was noti ed with Alt-RAMEC protocol and facemask therapy. Improvement in transverse relationship was also evident in the treatment group. Conclusion: Alt-RAMEC protocol in combination with protraction headgear is a better alternative to treat cleft lip and palate patient in comparison to the conventional protocol.

INTRODUCTION The purpose of this study was to observe the release of vascular endothelial growth ... more INTRODUCTION The purpose of this study was to observe the release of vascular endothelial growth factor (VEGF) after conditioning with etidronic acid, 1-hydroxyethylidene-1, 1-bisphosphonate (HEDP) and phytic acid, inositol hexakisphosphate (IP6) in comparison with EDTA on human dentine disks and cylinders. METHODS Human dentine disks were disinfected and prepared by standardized method. Seventeen percent EDTA, 9% HEDP, 1% IP6 and distilled water (5 minutes) were used to immerse dentine disks. The dentine cylinders were prepared by following disinfection and standardized preparation method to achieve a truncated cone shaped canal with 1mm open apex. The dentine cylinder samples were irrigated with sodium hypochlorite (NaOCl) (5 mL/5 minutes) and then rinsed with 17% EDTA, 9% HEDP, 1% IP6 and distilled water (5 mL/5 minutes). Enzyme linked Immunosorbent Essay (ELISA) was performed to measure VEGF release. One way analysis of variance (ANOVA) was used to compare the mean release of VEGF between study groups and controls at a significance level of 0.05. A post hoc Tukey test was used for multiple comparisons between study groups. RESULTS Among conditioners, HEDP released more VEGF from both disks and cylinders. In cylinders, VEGF release by HEDP was significantly greater than other conditioners. Whereas in disks the release of VEGF was similar with all conditioners. CONCLUSIONS The release of VEGF by 9% HEDP and 1% IP6 was comparable to 17% EDTA from dentine disk, however, HEDP demonstrated more release from dentine cylinder than EDTA and IP6.

METHODOLOGY: The pretreatment records of 240 adult subjects aged 15-40 years with bilateral end-on Class II molar relationship were retrospectively selected and categorized under extraction (120) and non-extraction (120) treatment categories. The extraction cases were planned for different combinations of premolar extractions. The independent variables i.e., the cephalometric and orthodontic cast measurements were obtained from the recruited sample. Binary logistic regression analysis was applied using SPSS software. RESULTS: Increased upper and lower incisor inclinations (p < 0.001) and procumbent upper lip (p = 0.004) was statistically significant in the extraction group. OBJECTIVE: Digital radiography has many advantages over conventional radiography. However, the ad... more OBJECTIVE: Digital radiography has many advantages over conventional radiography. However, the adaptation of this new technique is still slow. Moreover, safety measures related to radiography remain an important but neglected area of dental practice. The objective of this study was to assess practices of dentists about digital imaging techniques in dental radiology and radiographic safety. METHODOLOGY: A questionnaire based study was conducted among dental practitioners of Karachi. Convenience sampling technique was used to draw the sample of 164 general practitioners from Karachi. A 4 part questionnaire was used. Part 1 recorded basic demographic data, while part 2,3 and 4 focused on specific aspects of digital radiography and safety. SPSS version 21 was used for statistical analysis. Descriptive statistics was used to calculate frequency and percentage of responses. Chi square test was used to asses associations at a significance level of 0.05. RESULTS: A total of 164 fully filled forms were included in final analysis. Majority of respondents (n=113, 69%) were users of digital radiography. The main reason (n=36, 70.7%) for not using it was cost of equipment. Main reason for prescribing a CBCT was for implants (n=55, 33.5%). Most respondents (n=99, 60.4%) used lead protection for radiographing pregnant patients. A total of 64 respondents (39%) themselves held xray films in patient's mouth. There was significant association (p=0.002) between female gender and not performing radiographs (p=0.005). CONCLUSIONS: Although the adaptation of digital techniques was satisfactory, the radiation safety practices of dental surgeons were inadequate.

Introduction: A change in occlusion during orthodontic treatment can affect muscles and joints su... more Introduction: A change in occlusion during orthodontic treatment can affect muscles and joints surrounding the jaw, resulting in temporomandibular disorders (TMD). The aim of this study was to determine the effect of orthodontic treatment with straight-wire appliance on the incidence of TMD in young adults. Materials and Methods: A cohort study was carried out involving 177 adult patients (91 in the treatment group and 86 in the control group). Fonseca’s questionnaire was used for TMD evaluation for all patients at the time of obtaining orthodontic pretreatment records (T0). TMD assessment was repeated after an average of 3 years of initial assessment for both the treatment and control groups (T1). Results: The number of patients with TMD changed significantly ( P < .001) from T0 to T1 in both groups. All stages of endodontic care delivery are affected by the quality of resultant image. Objective of this study was to observe type and frequency of technical errors reported during obtaining intraoral radiographs in the endodontic Department of a teaching hospital. METHODOLOGY: A total of 600 radiographs were included in this retrospective audit. The radiographs were exposed during routine endodontic procedures. A previously published criteria was used to ascertain the technical quality. SPSS version 21 for windows was used for statistical analysis. Frequency and percentage was calculated as well as chi square test was used to determine associations between variables. P < 0.05 was considered significant. RESULTS: A total of 600 radiographs were included in our study out of which, 185 (30.8%) were found to be technically correct (p=0.183). Radiographs of mandibular molars presented with most errors (n=115, 42.4%, p=0.002). Errors of technique were more prevalent (n=383, 63.8%) followed by errors of processing (n=151, 25.2%, p=0.000). Most common error of technique was positioning error (n=113, 18.8%) (Table no 3). Most common processing error was yellowing of radiographs (n=60, 10%, p=0.003). CONCLUSION: Performance of undergraduate students in obtaining radiographs was poor. Positioning errors were found to be the most common error while radiographs of mandibular molars were most affected.

The retrospective study was conducted at a tertiary care hospital, Karachi, and comprised record of all patients visiting the orthodontic clinics of the hospital from 2005 to 2015. Orthodontic records of 790(79%) subjects, including 189(23.9%) males and 601(76.1%) females, were reviewed. Their mean age was 17 ± 5.06 years. A tooth was considered missing if no evidence of tooth germ was observed on orthopantomograms and dental casts. The total sample was distributed into three groups on the basis of Angle's classification. Chi-square test was applied to determine an association between hypodontia and Angle's malocclusion. However, the reliability of palatal rugae as a forensic marker remains questionable once an individual is subjected to orthodontic treatment. This study aimed at evaluating the changes in the rugae pattern after nonextraction, extraction, and maxillary expansion orthodontic treatment. The lengths and shapes of palatal rugae were evaluated on the pretreatment and post-treatment dental casts of 168 subjects using the Thomas and Kotze classification. Extraction treatment significantly reduced the second and third rugae lengths (p < 0.05), whereas the third rugae length was significantly increased after palatal expansion (p < 0.05). However, the reliability of palatal rugae as a forensic marker remains questionable once an individual is subjected to orthodontic treatment. This study aimed at evaluating the changes in the rugae pattern after nonextraction, extraction, and maxillary expansion orthodontic treatment. The lengths and shapes of palatal rugae were evaluated on the pretreatment and post-treatment dental casts of 168 subjects using the Thomas and Kotze classification. Extraction treatment significantly reduced the second and third rugae lengths (p < 0.05), whereas the third rugae length was significantly increased after palatal expansion (p < 0.05). The shape of rugae remained consistent in all the study groups which may be used as a reliable forensic marker in subjects undergoing orthodontic treatment. Therefore, the aim of the current study was to evaluate the mean changes in the pharyngeal dimensions of children with mandibular deficiency treated with Clark's twin-block appliance (CTB) followed by fixed orthodontic treatment. Orthodontic records comprising standard-quality lateral cephalograms and dental panoramic radiographs were selected.
