L'objectif principal de ma thèse était de créer et d'expérimenter un enseignement de médecine narrative auprès d'étudiants volontaires en 4 ème année de médecine à la faculté de Montpellier, après avoir réalisé un état des lieux des enseignements existants en France et dans le monde. Les objectifs secondaires étaient d'évaluer la satisfaction des étudiants et de recueillir leur vécu de l'enseignement. Neuf étudiants ont participé à trois séances, chacune construite sur un modèle pédagogique en trois phases : lecture attentive d'un texte littéraire, écriture réflexive ou créative, et partage des écrits. Ils identifiaient des bénéfices à la médecine narrative : l'acceptation des émotions et le partage avec les pairs, le développement de la réflexivité, l'accès à la perspective du patient et la construction de l'identité professionnelle. Leurs difficultés étaient de se lancer dans l'écriture et de partager leurs écrits. Ils souhaitaient varier les supports, la participation d'intervenants extérieurs et des séances régulières dès le début des stages hospitaliers. Nous proposerons dès la rentrée 2021 un enseignement optionnel aux externes en 3 ème année de médecine à Montpellier. Mots-clés médecine narrative, enseignement, relations médecin-patient, empathie ABSTRACT This article offers a summary of my thesis work for the title of medical Doctor. The main objective of my thesis was to create and experiment a teaching of narrative medicine with 4 th year volunteer medicine students from Montpellier University, after having studied how the subject is already taught in France and around the world. The secondary purpose was to evaluate the students' satisfaction and analyse the way they experienced this teaching. Nine students participated in three sessions, each based on a pedagogical model in three phases : attentive reading of a literary text, reflexive or creative writing, then sharing their writings. They identified benefits from narrative medicine : accepting their emotions, sharing them with their peers, improving their reflexivity, accessing the patient's perspective, and building their professional identity. Their difficulties were to get into writing and to share what they had written. They wanted to diversify the mediums, to have external stakeholders participative, and regular sessions from the beginning of hospital internships. We will offer an optional course for the 3rd year medicine students in Montpellier, from September 2021.

L'intérêt et le bénéfice d'une INM dépend beaucoup du praticien qui la propose ou du chercheur qui l'évalue. Les conditions favorables à l'utilisation des INM tiennent aussia uf ait d'accepter l'incertitude de toute rencontre, en apprenant au fur et à mesure de la rencontre avecl 'autre. Il s'agit d'une posture intellectuelle « compréhensive » guidée à travers une plongéed ans ces mécanismes constitutifs. C'est une volonté de s'immergerd ans le phénomène pour mieux comprendre l'autre, les différentes interventions étant plus des moyens qu'une finalité.

To manage UI, it necessary to improve UI screening. We aimed to analyze and understand the experience of women with UI. Methods We conducted a qualitative study using semi-structured interviews with postmenopausal women (age >50 years), who were recruited from an urban general practice office. The data of the patients were analyzed using the grounded theory method to allow the conceptualization of categories to emerge. Results Data saturation was reached after eight interviews and was confirmed by two additional interviews. There were four conceptualizing categories: UI is a marker of temporality in women and of societal temporality; women’s information about UI is a prerequisite for screening, and the media and information providers have an impact on women’s UI experience; UI has a strong societal taboo for women (women consider UI a minor but pejorative disease and fear stigmatization); and faced with the complexity of implementing personalized screening, women recommend systematic screening by their general practitioner or gynecologist to trivialize UI and optimize its management. Conclusions Women were uninformed about UI. They requested that the provision of information on the female anatomy should be started in childhood.

Resume Introduction De nouveaux comprimes d’immunotherapie allergenique (ITAcp) sont disponibles ... more Resume Introduction De nouveaux comprimes d’immunotherapie allergenique (ITAcp) sont disponibles pour traiter les patients atteints d’allergies respiratoires aux pollens de graminees. Le medecin generaliste (MG) prend en charge ces maladies allergiques frequentes. Objectif Le but de l’etude etait de determiner la part de patients suivis qui pourrait beneficier de ces nouveaux traitements d’ITAcp et d’evaluer la perception, la pratique et l’information qu’en avaient les MG. Methode Une enquete declarative par questionnaire electronique a ete realisee sur 2000 MG du Languedoc-Roussillon d’octobre a decembre 2014. Resultats Au total, 429 medecins generalistes ont repondu avec un taux de reponse de 21,5 %. Soixante-dix pourcent des MG suivaient 10 a 30 % de leur patientele atteints d’allergies respiratoires parmi laquelle moins de 5 % avaient deja beneficie d’une immunotherapie allergenique (ITA). Soixante-six pourcent des MG declaraient que les nouveaux traitements pourraient beneficier a plus de 5 % de ces patients. Meme si certains n’avaient pas d’opinion, la majorite des MG avait une image positive du traitement. Ils l’envisageaient parfois, dans divers contextes, en suivant les recommandations. Ils assuraient un suivi clinique simple avec l’aide de l’allergologue mais se declaraient insuffisamment informes sur l’ITA a 89 %. Conclusion Les nouveaux traitements d’ITAcp n’ont pas une place importante en medecine generale car ils restent peu connus et peu utilises en soins primaires. L’ITA suscite neanmoins l’interet des MG qui en ont une image positive. Une information actualisee et une formation des MG en allergologie en lien avec le specialiste pourraient faire evoluer les pratiques.

Background: In 2020, the number of new cancer cases was estimated at 20 490 862 worldwide up from... more Background: In 2020, the number of new cancer cases was estimated at 20 490 862 worldwide up from 18.1 million in 2018 and 14.1 million in 2012. Since the 2000s, cancer treatments have significantly improved, allowing either a cure or control of the disease. Patients share their experience of the disease and use supportive care solutions through involvement in patient associations and online forums. All the associations were built on the principle of "peer support, " which is based on mutual aid between people who suffer or have suffered from the same somatic or psychological illness or had the same life experience. This experiential knowledge can be explored to understand the role of peers and associations in the appropriation of their cancer. Methods: A qualitative phenomenological study was undertaken through semi-structured interviews with 12 participants. Interviews were audio-recorded, transcribed verbatim, then analyzed by means of triangulation up to the point of theoretical saturation by a semio-pragmatic method. Results: Four categories emerged: (1) "Transforms a painful experience into a positive one. It mobilizes the human values of sharing, love, and humility, which facilitates resilience"; (2) "The characteristics of the association, a non-medical place between people sharing a common destiny, resonates with patients' needs and improves their well-being"; (3) "The association transforms the patients' experiences by facilitating engagement that leads to a patient-expert (empowerment)"; and (4) "Understanding what is happening to them is soothing, reassuring, because patients' concerns need to be heard and their care understood". Conclusions: This study highlights patient associations can serve as the mediator of NPI and facilate the empowerment of breast cancer patients. Practice implications: Educating health professionals in initial and continuing education about non-pharmacological interventions will be a major issue. Teaching the patient-centred approach to health professionals is one of the priorities in initial and continuing medical education.

Abstract Objective To understand the perceptions and attitudes of general practitioners (GPs) reg... more Abstract Objective To understand the perceptions and attitudes of general practitioners (GPs) regarding children with an Autism Spectrum Disorder (ASD). Design Phenomenological qualitative study. Setting Three focus groups, clinical settings. Subjects French GPs. Main outcome measures 22 GPs took part in the study divided among three focus groups. They were volunteers to participate. Data were transcribed verbatim and analysed using a grounded theory data analysis, completed with a semiopragmatic analysis. Results Representing autism as a strange disorder in the doctor–patient relationship, GPs perceive a loss of sensory contact with the child with ASD that prevents the usual professional relationship between doctor and patient. They disengage themselves from monitoring the subject, concentrating on supporting the family. According to them, their role was to refer the patient to a specialist in the case of clinical intuition, but they have several reasons to give themselves time, all the more so because once the diagnosis is made, they lose sight of the patient and their place in the care pathway. GPs expressed the need to acquire skills and strategies to communicate with the autistic child to recover their role and values. Conclusion GPs are disconcerted by the idea of communicating with children with ASD, as it takes them out of their usual professional benchmarks. They need communication tools that enable them to regain their role and relational value of the patient-centred approach. Beyond this, the question of the ‘ethics of care’ of the patient with a joint attention disorder is raised. KEY POINTS GPs are disconcerted with the idea of communicating with children with ASD. GPs need communication tools that enable them to regain their role and relational value of the patient-centred approach. santé et du social travaillant avec des personnes de plus de 60 ans recourent de plus en plus aux... more santé et du social travaillant avec des personnes de plus de 60 ans recourent de plus en plus aux pratiques non pharmacologiques (Ballard et al., 2011; Gow et al., 2012). Ces méthodes ciblées et personnalisées sont nommées par les scientifiques des interventions non médicamenteuses (INM). Elles se distinguent des médecines alternatives, des pratiques socio-culturelles et des messages promotionnels de santé publique (Ninot, 2019). Les INM sont des méthodes psychologiques, corporelles, manuelles, numériques, nutritionnelles ou élémentaires (Fig. 1). Elles sont non invasives et complémentaires des autres approches (Fig. 2). Elles sont fondées sur la science et en particulier sur des études interventionnelles (Fig. 3). Elles ont pour objectifs (1) de soulager des symptômes, (2) de prévenir des maladies, (3) d'augmenter l'autonomie, (4) de renforcer l'état de santé, (5) d'améliorer la qualité de vie, (6) et de prolonger la vie sans perte de qualité de vie (successful aging). De nombreux chercheurs et praticiens signalaient des difficultés à identifier les études comparatives (aussi appelées interventionnelles ou cliniques) pertinentes évaluant les bénéfices et les risques des INM pour mieux vieillir (Hoffmann et al., 2013; Ninot, 2019). Une équipe de la Plateforme universitaire CEPS a pensé un système numérique à cette fin.

Background: In 2020, the number of new cancer cases was estimated at 20 490 862 worldwide up from... more Background: In 2020, the number of new cancer cases was estimated at 20 490 862 worldwide up from 18.1 million in 2018 and 14.1 million in 2012. Since the 2000s, cancer treatments have significantly improved, allowing either a cure or control of the disease. Patients share their experience of the disease and use supportive care solutions through involvement in patient associations and online forums. All the associations were built on the principle of "peer support, " which is based on mutual aid between people who suffer or have suffered from the same somatic or psychological illness or had the same life experience. This experiential knowledge can be explored to understand the role of peers and associations in the appropriation of their cancer. Methods: A qualitative phenomenological study was undertaken through semi-structured interviews with 12 participants. Interviews were audio-recorded, transcribed verbatim, then analyzed by means of triangulation up to the point of theoretical saturation by a semio-pragmatic method. Results: Four categories emerged: (1) "Transforms a painful experience into a positive one. It mobilizes the human values of sharing, love, and humility, which facilitates resilience"; (2) "The characteristics of the association, a non-medical place between people sharing a common destiny, resonates with patients' needs and improves their well-being"; (3) "The association transforms the patients' experiences by facilitating engagement that leads to a patient-expert (empowerment)"; and (4) "Understanding what is happening to them is soothing, reassuring, because patients' concerns need to be heard and their care understood". Conclusions: This study highlights patient associations can serve as the mediator of NPI and facilate the empowerment of breast cancer patients. Practice implications: Educating health professionals in initial and continuing education about non-pharmacological interventions will be a major issue. Teaching the patient-centred approach to health professionals is one of the priorities in initial and continuing medical education.

Abstract Objective To understand the perceptions and attitudes of general practitioners (GPs) reg... more Abstract Objective To understand the perceptions and attitudes of general practitioners (GPs) regarding children with an Autism Spectrum Disorder (ASD). Design Phenomenological qualitative study. Setting Three focus groups, clinical settings. Subjects French GPs. Main outcome measures 22 GPs took part in the study divided among three focus groups. They were volunteers to participate. Data were transcribed verbatim and analysed using a grounded theory data analysis, completed with a semiopragmatic analysis. Results Representing autism as a strange disorder in the doctor–patient relationship, GPs perceive a loss of sensory contact with the child with ASD that prevents the usual professional relationship between doctor and patient. They disengage themselves from monitoring the subject, concentrating on supporting the family. According to them, their role was to refer the patient to a specialist in the case of clinical intuition, but they have several reasons to give themselves time, all the more so because once the diagnosis is made, they lose sight of the patient and their place in the care pathway. GPs expressed the need to acquire skills and strategies to communicate with the autistic child to recover their role and values. Conclusion GPs are disconcerted by the idea of communicating with children with ASD, as it takes them out of their usual professional benchmarks. They need communication tools that enable them to regain their role and relational value of the patient-centred approach. Beyond this, the question of the ‘ethics of care’ of the patient with a joint attention disorder is raised. KEY POINTS GPs are disconcerted with the idea of communicating with children with ASD. santé et du social travaillant avec des personnes de plus de 60 ans recourent de plus en plus aux... more santé et du social travaillant avec des personnes de plus de 60 ans recourent de plus en plus aux pratiques non pharmacologiques (Ballard et al., 2011; Gow et al., 2012). Ces méthodes ciblées et personnalisées sont nommées par les scientifiques des interventions non médicamenteuses (INM). Elles se distinguent des médecines alternatives, des pratiques socio-culturelles et des messages promotionnels de santé publique (Ninot, 2019). Les INM sont des méthodes psychologiques, corporelles, manuelles, numériques, nutritionnelles ou élémentaires (Fig. 1). Elles sont non invasives et complémentaires des autres approches (Fig. 2). Elles sont fondées sur la science et en particulier sur des études interventionnelles (Fig. 3). Elles ont pour objectifs (1) de soulager des symptômes, (2) de prévenir des maladies, (3) d'augmenter l'autonomie, (4) de renforcer l'état de santé, (5) d'améliorer la qualité de vie, (6) et de prolonger la vie sans perte de qualité de vie (successful aging). De nombreux chercheurs et praticiens signalaient des difficultés à identifier les études comparatives (aussi appelées interventionnelles ou cliniques) pertinentes évaluant les bénéfices et les risques des INM pour mieux vieillir (Hoffmann et al., 2013; Ninot, 2019). Une équipe de la Plateforme universitaire CEPS a pensé un système numérique à cette fin.
