Purpose: To describe how nurses interpreted health promotion related to older patients in a medic... more Purpose: To describe how nurses interpreted health promotion related to older patients in a medical hospital ward and the presuppositions for this work. Methods: The study had a descriptive and explorative design. Data were created through participant observation of five nurses and informal dialogues with four nurses working in an infection unit in a small hospital in Norway. The nurses where followed during a period of 6 months. The observations and dialogues were interpreted and transformed into text. This text was analysed with qualitative content analysis and interpreted hermeneutically on three levels: (1) reading the text to get an overview of the themes; (2) systematically separating central patterns; and (3) highlighting the patterns with examples. Findings: The nurses' interpretation of health promotion was closely connected to their interpretation of health and holistic nursing. Two main aspects were uncovered in the informal dialogues: a biomedically oriented nursing view and a holistically oriented nursing view. The observations showed that presuppositions, such as waiting for something or somebody, constantly changing situations, complexity and diversity in patient cases, influenced the nurses' work in general and also their focus on health promotion. The nurses balanced between the biomedical and the holistic approach. It seemed to be important for the nurses to be the masters of walking on a tightrope and to have professional clinical competence. Conclusion: It was difficult to find a clear focus on health promotion. The most visible focus was on diagnosis and treatment, while the more complex and comprehensive situations related to health promotion of older patients seemed to be underprioritized. The phenomenon of 'waiting' may be an important issue for further research.

However, still there are many research questions to be raised. Methodology/Approach: Two value chains have been studied within two different sectors: the health sector and the car component industry (mass producer). The research methodology is based on semi-structured interviews with selected persons from different levels within the organizations. The data was transcribed, coded and further analyzed to find enablers or disablers to operational integration in both sectors. Findings: From this study, factors such as management commitment, colocation, and job-rotation can be seen as contributing factors in both organizations. Both experience disablers such as working as functional silos and little alignment of overall goals. Differences are seen in the greater use of job rotation within health care, while the mass producer had more mechanisms to facilitate working in cross functional teams. Practical implication: This paper presents empirical findings of success factors and pitfalls for operational integration within the value chain of two different types of organizations. Based on this mapping, recommendations on how to achieve better operational integration will be presented. Originality/Value of paper: The research initiative provides knowledge experiences from operational integration in two different Norwegian organizations representing two different sectors.

In recent years, digital remote monitoring interventions have emerged to provide enhanced cancer care delivery experiences to patients and clinicians. Background: There is a growing understanding of the importance of having a family perspective in ... more Background: There is a growing understanding of the importance of having a family perspective in treatment and rehabilitation of patients with mental illness. We have carried out several projects showing that information and guidance on mental illness improves communication within the family and reduces conflicts. We did not find any research that describes experiences while participating in parallel groups. Objective: The aim of this study was to describe the opinions and experiences of parents-where one of them is suffering from mental illness participating in parallel group sessions. Method: A qualitative exploratory and descriptive design with a phenomenographic approach to the pairs of parents interviewed has been used. Four pairs of parents were interviewed. The data analyses are based on Alexanderson›s four phases. Results: In the analysis, four description categories emerged. The informants experienced the group sessions as unique, liberating and useful for the whole family. The informants in the study had different experiences with the mental health care, and all wanted more information and guidance. Conclusion: Where one parent has a mental disorder, a parallel group meeting for parents and children has been shown to be a positive experience for the family. The central aspect of our study that parents have emphasised as important is that the group sessions for parents and children were at the same time and place.

Background: Today, the ageing population is larger than ever before, and people who are living lo... more Background: Today, the ageing population is larger than ever before, and people who are living longer with chronic illnesses and multimorbidity need support from multiple healthcare service levels. Similarly, healthcare systems are becoming increasingly specialised and fragmented. The World Health Organization has highlighted novel policies for developing integrated and person-centred services. However, patients, next of kin and health professionals face several challenges in managing healthcare during the care trajectory. Limited literature has addressed the challenges experienced by these groups. Therefore, this study aimed to identify the dilemmas and deliberations faced by patients, next of kin and health professionals during the care trajectory of elderly patients with complex healthcare needs. Method: The study had a qualitative single-case design. The case was taken from a multi-case study exploring the care trajectory of elderly patients. The participants were the patient, their next of kin and the health professionals involved in the patient's care trajectory. Data were obtained via observation and individual interviews conducted during the patient's hospital stay and after the patient returned home. Results: The dilemmas and deliberations in managing the care trajectory were divided into four main themes: the health professionals' pursuit of appropriate and feasible healthcare services, the next of kin's planning horizons, being the person left in limbo and reorganising the home for comprehensive healthcare. Conclusion: The pursuit of a tailored and suitable healthcare service lead to a comprehensive mobilisation of and work by all actors involved. Having a comprehensive understanding of these conditions are of importance in developing an appropriate care trajectory for the elderly patient with complex need.

This prompts a need to focus on and implement a health promoting approach in older people's nursing. Aim: The aim of this exploratory study was to illuminate nurses' reflections about health promotion and empowerment related to older hospital patients. Method: The study had a qualitative descriptive design. The concepts of health, health promotion and empowerment were initially introduced to nurses before participating in multistage focus group interviews. Two groups of nurses participated in three focus group interviews that were conducted by a moderator and an assistant. The interviews were tape-recorded, transcribed verbatim and analysed with qualitative content analysis. Findings: The nurses unfolded the theme «diversity and complexity marks health promotion and empowerment related to older hospital patients» as the latent content in the text. Two categories, «empowerment as balance of power — a twofold relation» and «focusing on the individual person», described the manifest content in the texts. The categories were further supported by sub-categories. Conclusion: The focus group interviews showed the ambiguity and complexity in the interpretation of the concepts — health promotion and empowerment — related to the older hospital patient. The findings also indicated that multistage focus group interviews may be a useful tool to develop nurses' reflections about their clinical practice.

Hospitalized older peoples' views of health and health promotion Older peoples' health varies con... more Hospitalized older peoples' views of health and health promotion Older peoples' health varies considerably and the number of older people who need hospital care will increase in the future. Traditionally, the focus of nursing care in hospitals is oriented towards diagnosis and treatment of disease, although health and health promotion are inherent in theories and definitions of nursing. This is an important issue in health politics, in new laws and regulations in Norway. However, it was difficult to find research-based knowledge about how nurses work with health promotion and health related to older hospitalized patients. This study aimed at describing older hospitalized patients' experiences of health and health promotion. The study used a descriptive and explorative qualitative design, with an interpretative-phenomenological approach. Ten older hospitalized patients, recruited through purposeful sampling, were interviewed during their hospital stay. The informants described health as Being able to be the person I am, to do what I want to do, and feel well and have strength. Health promotion was described as Being enabled through being the person I am, through information and knowledge and through hope and motivation. The findings may be applied to nursing practice in order to focus on promotion of health as an individual process built upon the unique person's life and situation, supported by nursing care and medical treatment.

Health promotion seems to be implicit in many nursing theories, but the theoretical and philosoph... more Health promotion seems to be implicit in many nursing theories, but the theoretical and philosophical basis of health promotion in nursing is not always explicitly stated. The interpretation of health promotion is closely related to the interpretation of man, health, illness and nursing. There is a need to clarify, refine and redefine health promotion in nursing because the concept is partly nonspecific and has not been used to identify a distinctive nursing focus. The aim of this study was to formulate a stipulative definition of health promotive nursing with a holistic‐existential approach. A philosophical frame of reference in combination with conceptual analysis and theoretical synthesis were used as the methodological approach. The philosophical framework served as a basis in selecting the nursing theories and influenced the analysis. Two nursing theories and one nursing model were selected due to their influence on Norwegian nursing and because of their philosophical basis. Through analysis and synthesis of the selected nursing theories, the concepts man, health, illness/disease and nursing were analysed. The paper proposes a stipulative definition of health promotion in nursing based on a holistic‐existential approach, supported by five necessary conditions. The definition and conditions needs to be further investigated by both empirical studies and by comparing with other relevant nursing theories, in order to formulate theoretical statements. The proposed definition may be the first step in a process of developing a theoretical framework of health promotive nursing with a holistic‐existential approach.

Denne rapporten beskriver gjennomføringen av et samarbeidsprosjekt mellom Høgskolen i Gjøvik, sek... more Denne rapporten beskriver gjennomføringen av et samarbeidsprosjekt mellom Høgskolen i Gjøvik, seksjon for sykepleie og Sykehuset Innlandet HF Lillehammer, medisinsk avdeling med tre sengeposter. Samarbeidsprosjektets hensikt er å styrke samarbeidet og praksisundervisningen, og bevisstgjøre studenter og sykepleiere til å jobbe kunnskapsbasert. Prosjektsøknaden ble utarbeidet våren 2010 og det ble innvilget 80.000 kroner i juni 2010. Prosjektgruppen har bestått av Ingrid F. Kleiven høgskolelærer, Solveig Rakvaag fagsykepleier, Sigfrid S. Stendahl assisterende avdelingssjef medisinsk avdeling, og Liv Tone Kveen spesialsykepleier. Prosjektgruppas viktigste rolle har vært å følge vedtatt handlingsplanen se vedlegg, overholder budsjett og koordinere og gjennomføre prosjektet. Referansegruppen har hatt 8 deltakere, Trine Hegvik, Anette Getz Bergmo, og Gro Opsahl Fallingen som er avdelingsledere på de tre postene, Siv Sønsteby Nordhagen praksiskonsulent HIG, Kari Jensen og Else Westeraas hovedveiledere og Geir V. Berg forskningsveileder SIHF Lillehammer. Referansegruppens rolle har vært å komme med konstruktive tilbakemeldinger i forløpet, legge til rette for, og bidra til å implementere prosedyre. Geir V. Berg forskningsveileder ved Sykehuset Innlandet Lillehammer/ førsteamanuensis Høgskolen i Gjøvik har vært veileder. Prosjektet involverte 4 studentgrupper, fra oktober 2010 til oktober 2011. 3 sykepleiere, en fra hver av de tre postene ble frikjøpt for å være med i utarbeidelsen av prosedyren. Studies have uncovered a complex healthcare terrain. Despite a great deal of effort on the part of policy makers and healthcare providers, the work healthcare professionals undertake to develop seamless healthcare is still unclear. Therefore, the aim of this study was to uncover the work that healthcare professionals undertake to achieve coherent and comprehensive healthcare for elderly patients with multiple health problems during their journey through the complex healthcare terrain. Methods: This study has an explorative design with individual interviews. Twenty-five healthcare professionals from primary and specialist care agreed to participate. A thematic analysis method was employed. Results: The analyses revealed three central themes in the healthcare professionals' work to build coherence in the patients' care trajectory: Working to manage a patient's illness trajectory during the course of the patient's life, working to achieve a comprehensive overall picture, and considering multiple options in a "patchwork" terrain. Conclusion: Healthcare professionals have a common understanding that hospital stays are a short part of the elderly person's journey in the healthcare system. In the comprehensive work to obtain the overall picture of the illness trajectory within the patient's life story, healthcare professionals emphasized the importance of working in an interdisciplinary manner. Interprofessional consulting and collaboration should be strengthened to build coherence in the older patient's complex care trajectory.

For lesions with a cystic appearance, the Bosniak classification system has enabled an important separation of benign and (potentially) malignant cysts, giving a practical guide for management. The purpose of this study was to evaluate contrast-enhanced ultrasound (CEUS) as a problem-solving modality for classification of indeterminate renal lesions detected with CT. Materials and methods: In total, 140 consecutive patients with 148 indeterminate renal lesions were examined with ultrasound combined with CEUS (81 men and 59 women with mean age 63.8 years). Results: Altogether, 146 lesions were classified by CEUS in categories according to the Bosniak classification system, or as solid lesions. Mean lesion diameter was 30 mm (range 5-166 mm). Nine lesions were classified as category I, 32 as category II and 59 as category IIF ("non-surgical lesions"). 48 IIF cysts were followed for a minimum of 2 years (mean 4 years and 8 months): three lesions were upgraded to category III (6%) and 45 were stable category. 19 lesions had a more complex cystic appearance: 12 were classified as category III and seven as category IV cysts. 27 lesions were diagnosed as solid, enhancing tumors. In total, 25 patients with lesions of category III, IV and solid ("surgical lesions") underwent renal surgery, with malignancy diagnosed in 18. Conclusion: In the common setting of indeterminate renal lesions incidentally detected with a CT exam, CEUS has the potential to classify lesions into "non-surgical" and "surgical" categories, providing a platform for urological decision making, while avoiding radiation exposure.

Background: ASyMS is a mobile‐phone based remote‐monitoring and alert system that enables real‐ti... more Background: ASyMS is a mobile‐phone based remote‐monitoring and alert system that enables real‐time monitoring of patients' chemotherapy (CT)–related symptoms. Aims: eSMART aims to evaluate the short‐ and long‐term impact of ASyMS, compared with standard care, on patient reported outcomes and the delivery of care. Additionally, eSMART includes economic evaluation and the development of predictive risk models for experiencing CT‐related symptoms to enable personalised and anticipatory care. Methods: Patients newly diagnosed with breast, haematological, or colorectal cancer and scheduled to receive at least 3 cycles of chemotherapy are recruited across 5 countries and 13 clinical sites. A total of 1108 patients will be randomised to either ASyMS or standard care. Background: Today, the ageing population is larger than ever before, and people who are living lo... more Background: Today, the ageing population is larger than ever before, and people who are living longer with chronic illnesses and multimorbidity need support from multiple healthcare service levels. Similarly, healthcare systems are becoming increasingly specialised and fragmented. The World Health Organization has highlighted novel policies for developing integrated and person-centred services. However, patients, next of kin and health professionals face several challenges in managing healthcare during the care trajectory. Limited literature has addressed the challenges experienced by these groups. Therefore, this study aimed to identify the dilemmas and deliberations faced by patients, next of kin and health professionals during the care trajectory of elderly patients with complex healthcare needs. Method: The study had a qualitative single-case design. The case was taken from a multi-case study exploring the care trajectory of elderly patients. The participants were the patient, their next of kin and the health professionals involved in the patient's care trajectory. Data were obtained via observation and individual interviews conducted during the patient's hospital stay and after the patient returned home. Results: The dilemmas and deliberations in managing the care trajectory were divided into four main themes: the health professionals' pursuit of appropriate and feasible healthcare services, the next of kin's planning horizons, being the person left in limbo and reorganising the home for comprehensive healthcare. Conclusion: The pursuit of a tailored and suitable healthcare service lead to a comprehensive mobilisation of and work by all actors involved. Having a comprehensive understanding of these conditions are of importance in developing an appropriate care trajectory for the elderly patient with complex need.

This prompts a need to focus on and implement a health promoting approach in older people's nursing. Aim: The aim of this exploratory study was to illuminate nurses' reflections about health promotion and empowerment related to older hospital patients. Method: The study had a qualitative descriptive design. The concepts of health, health promotion and empowerment were initially introduced to nurses before participating in multistage focus group interviews. Two groups of nurses participated in three focus group interviews that were conducted by a moderator and an assistant. The interviews were tape-recorded, transcribed verbatim and analysed with qualitative content analysis. Findings: The nurses unfolded the theme «diversity and complexity marks health promotion and empowerment related to older hospital patients» as the latent content in the text. Two categories, «empowerment as balance of power — a twofold relation» and «focusing on the individual person», described the manifest content in the texts. The categories were further supported by sub-categories. Conclusion: The focus group interviews showed the ambiguity and complexity in the interpretation of the concepts — health promotion and empowerment — related to the older hospital patient. The findings also indicated that multistage focus group interviews may be a useful tool to develop nurses' reflections about their clinical practice.

Traditionally, the focus of nursing care in hospitals is oriented towards diagnosis and treatment of disease, although health and health promotion are inherent in theories and definitions of nursing. This is an important issue in health politics, in new laws and regulations in Norway. However, it was difficult to find research-based knowledge about how nurses work with health promotion and health related to older hospitalized patients. This study aimed at describing older hospitalized patients' experiences of health and health promotion. The study used a descriptive and explorative qualitative design, with an interpretative-phenomenological approach. Ten older hospitalized patients, recruited through purposeful sampling, were interviewed during their hospital stay. The informants described health as Being able to be the person I am, to do what I want to do, and feel well and have strength. Health promotion was described as Being enabled through being the person I am, through information and knowledge and through hope and motivation. The findings may be applied to nursing practice in order to focus on promotion of health as an individual process built upon the unique person's life and situation, supported by nursing care and medical treatment.

Health promotion seems to be implicit in many nursing theories, but the theoretical and philosoph... more Health promotion seems to be implicit in many nursing theories, but the theoretical and philosophical basis of health promotion in nursing is not always explicitly stated. The interpretation of health promotion is closely related to the interpretation of man, health, illness and nursing. There is a need to clarify, refine and redefine health promotion in nursing because the concept is partly nonspecific and has not been used to identify a distinctive nursing focus. The aim of this study was to formulate a stipulative definition of health promotive nursing with a holistic‐existential approach. A philosophical frame of reference in combination with conceptual analysis and theoretical synthesis were used as the methodological approach. The philosophical framework served as a basis in selecting the nursing theories and influenced the analysis. Two nursing theories and one nursing model were selected due to their influence on Norwegian nursing and because of their philosophical basis. Through analysis and synthesis of the selected nursing theories, the concepts man, health, illness/disease and nursing were analysed. The paper proposes a stipulative definition of health promotion in nursing based on a holistic‐existential approach, supported by five necessary conditions. The definition and conditions needs to be further investigated by both empirical studies and by comparing with other relevant nursing theories, in order to formulate theoretical statements. The proposed definition may be the first step in a process of developing a theoretical framework of health promotive nursing with a holistic‐existential approach.

Denne rapporten beskriver gjennomføringen av et samarbeidsprosjekt mellom Høgskolen i Gjøvik, sek... more Denne rapporten beskriver gjennomføringen av et samarbeidsprosjekt mellom Høgskolen i Gjøvik, seksjon for sykepleie og Sykehuset Innlandet HF Lillehammer, medisinsk avdeling med tre sengeposter. Samarbeidsprosjektets hensikt er å styrke samarbeidet og praksisundervisningen, og bevisstgjøre studenter og sykepleiere til å jobbe kunnskapsbasert. Prosjektsøknaden ble utarbeidet våren 2010 og det ble innvilget 80.000 kroner i juni 2010. Prosjektgruppen har bestått av Ingrid F. Kleiven høgskolelærer, Solveig Rakvaag fagsykepleier, Sigfrid S. Stendahl assisterende avdelingssjef medisinsk avdeling, og Liv Tone Kveen spesialsykepleier. Prosjektgruppas viktigste rolle har vært å følge vedtatt handlingsplanen se vedlegg, overholder budsjett og koordinere og gjennomføre prosjektet. Referansegruppen har hatt 8 deltakere, Trine Hegvik, Anette Getz Bergmo, og Gro Opsahl Fallingen som er avdelingsledere på de tre postene, Siv Sønsteby Nordhagen praksiskonsulent HIG, Kari Jensen og Else Westeraas hovedveiledere og Geir V. Berg forskningsveileder SIHF Lillehammer. Referansegruppens rolle har vært å komme med konstruktive tilbakemeldinger i forløpet, legge til rette for, og bidra til å implementere prosedyre. Geir V. Berg forskningsveileder ved Sykehuset Innlandet Lillehammer/ førsteamanuensis Høgskolen i Gjøvik har vært veileder. Prosjektet involverte 4 studentgrupper, fra oktober 2010 til oktober 2011. 3 sykepleiere, en fra hver av de tre postene ble frikjøpt for å være med i utarbeidelsen av prosedyren. Studies have uncovered a complex healthcare terrain. Despite a great deal of effort on the part of policy makers and healthcare providers, the work healthcare professionals undertake to develop seamless healthcare is still unclear. Therefore, the aim of this study was to uncover the work that healthcare professionals undertake to achieve coherent and comprehensive healthcare for elderly patients with multiple health problems during their journey through the complex healthcare terrain. Methods: This study has an explorative design with individual interviews. Twenty-five healthcare professionals from primary and specialist care agreed to participate. A thematic analysis method was employed. Results: The analyses revealed three central themes in the healthcare professionals' work to build coherence in the patients' care trajectory: Working to manage a patient's illness trajectory during the course of the patient's life, working to achieve a comprehensive overall picture, and considering multiple options in a "patchwork" terrain. Conclusion: Healthcare professionals have a common understanding that hospital stays are a short part of the elderly person's journey in the healthcare system. In the comprehensive work to obtain the overall picture of the illness trajectory within the patient's life story, healthcare professionals emphasized the importance of working in an interdisciplinary manner. Interprofessional consulting and collaboration should be strengthened to build coherence in the older patient's complex care trajectory.

For lesions with a cystic appearance, the Bosniak classification system has enabled an important separation of benign and (potentially) malignant cysts, giving a practical guide for management. The purpose of this study was to evaluate contrast-enhanced ultrasound (CEUS) as a problem-solving modality for classification of indeterminate renal lesions detected with CT. Materials and methods: In total, 140 consecutive patients with 148 indeterminate renal lesions were examined with ultrasound combined with CEUS (81 men and 59 women with mean age 63.8 years). Results: Altogether, 146 lesions were classified by CEUS in categories according to the Bosniak classification system, or as solid lesions. Mean lesion diameter was 30 mm (range 5-166 mm). Nine lesions were classified as category I, 32 as category II and 59 as category IIF ("non-surgical lesions"). 48 IIF cysts were followed for a minimum of 2 years (mean 4 years and 8 months): three lesions were upgraded to category III (6%) and 45 were stable category. 19 lesions had a more complex cystic appearance: 12 were classified as category III and seven as category IV cysts. 27 lesions were diagnosed as solid, enhancing tumors. In total, 25 patients with lesions of category III, IV and solid ("surgical lesions") underwent renal surgery, with malignancy diagnosed in 18. Conclusion: In the common setting of indeterminate renal lesions incidentally detected with a CT exam, CEUS has the potential to classify lesions into "non-surgical" and "surgical" categories, providing a platform for urological decision making, while avoiding radiation exposure.

Background: ASyMS is a mobile‐phone based remote‐monitoring and alert system that enables real‐ti... more Background: ASyMS is a mobile‐phone based remote‐monitoring and alert system that enables real‐time monitoring of patients' chemotherapy (CT)–related symptoms. Aims: eSMART aims to evaluate the short‐ and long‐term impact of ASyMS, compared with standard care, on patient reported outcomes and the delivery of care. Additionally, eSMART includes economic evaluation and the development of predictive risk models for experiencing CT‐related symptoms to enable personalised and anticipatory care. Methods: Patients newly diagnosed with breast, haematological, or colorectal cancer and scheduled to receive at least 3 cycles of chemotherapy are recruited across 5 countries and 13 clinical sites. A total of 1108 patients will be randomised to either ASyMS or standard care.
